Can medical students identify a potentially serious acetaminophen dosing error in a simulated encounter? a case control study
Evaluation of Population-Level Changes Associated With the 2021 US Preventive Services Task Force Lung Cancer Screening Recommendations in Community-Based Health Care Systems
Importance: The US Preventive Services Task Force (USPSTF) released updated lung cancer screening recommendations in 2021, lowering the screening age from 55 to 50 years and smoking history from 30 to 20 pack-years. These changes are expected to expand screening access to women and racial and ethnic minority groups.
Objective: To estimate the population-level changes associated with the 2021 USPSTF expansion of lung cancer screening eligibility by sex, race and ethnicity, sociodemographic factors, and comorbidities in 5 community-based health care systems.
Design, Setting, and Participants: This cohort study analyzed data of patients who received care from any of 5 community-based health care systems (which are members of the Population-based Research to Optimize the Screening Process Lung Consortium, a collaboration that conducts research to better understand how to improve the cancer screening processes in community health care settings) from January 1, 2010, through September 30, 2019. Individuals who had complete smoking history and were engaged with the health care system for 12 or more continuous months were included. Those who had never smoked or who had unknown smoking history were excluded.
Exposures: Electronic health record-derived age, sex, race and ethnicity, socioeconomic status (SES), comorbidities, and smoking history.
Main Outcomes and Measures: Differences in the proportion of the newly eligible population by age, sex, race and ethnicity, Charlson Comorbidity Index, chronic obstructive pulmonary disease diagnosis, and SES as well as lung cancer diagnoses under the 2013 recommendations vs the expected cases under the 2021 recommendations were evaluated using Ï2 tests.
Results: As of September 2019, there were 341âŻ163 individuals aged 50 to 80 years who currently or previously smoked. Among these, 34âŻ528 had electronic health record data that captured pack-year and quit-date information and were eligible for lung cancer screening according to the 2013 USPSTF recommendations. The 2021 USPSTF recommendations expanded screening eligibility to 18 533 individuals, representing a 53.7% increase. Compared with the 2013 cohort, the newly eligible 2021 population included 5833 individuals (31.5%) aged 50 to 54 years, a larger proportion of women (52.0% [nâ=â9631]), and more racial or ethnic minority groups. The relative increases in the proportion of newly eligible individuals were 60.6% for Asian, Native Hawaiian, or Pacific Islander; 67.4% for Hispanic; 69.7% for non-Hispanic Black; and 49.0% for non-Hispanic White groups. The relative increase for women was 13.8% higher than for men (61.2% vs 47.4%), and those with a lower comorbidity burden and lower SES had higher relative increases (eg, 68.7% for a Charlson Comorbidity Index score of 0; 61.1% for lowest SES). The 2021 recommendations were associated with an estimated 30% increase in incident lung cancer diagnoses compared with the 2013 recommendations.
Conclusions and Relevance: This cohort study suggests that, in diverse health care systems, adopting the 2021 USPSTF recommendations will increase the number of women, racial and ethnic minority groups, and individuals with lower SES who are eligible for lung cancer screening, thus helping to minimize the barriers to screening access for individuals with high risk for lung cancer
Moduli Stabilization with Long Winding Strings
Stabilizing all of the modulus fields coming from compactifications of string
theory on internal manifolds is one of the outstanding challenges for string
cosmology. Here, in a simple example of toroidal compactification, we study the
dynamics of the moduli fields corresponding to the size and shape of the torus
along with the ambient flux and long strings winding both internal directions.
It is known that a string gas containing states with non-vanishing winding and
momentum number in one internal direction can stabilize the radius of this
internal circle to be at self-dual radius. We show that a gas of long strings
winding all internal directions can stabilize all moduli, except the dilaton

Supersymmetric Models with Higher Dimensional Operators
In 4D renormalisable theories, integrating out massive states generates in
the low energy effective action higher dimensional operators (derivative or
otherwise). Using a superfield language it is shown that a 4D N=1
supersymmetric theory with higher derivative operators in either the Kahler or
the superpotential part of the Lagrangian and with an otherwise arbitrary
superpotential, is equivalent to a 4D N=1 theory of second order (i.e. without
higher derivatives) with additional superfields and renormalised interactions.
We provide examples where a free theory with trivial supersymmetry breaking
provided by a linear superpotential becomes, in the presence of higher
derivatives terms and in the second order version, a non-trivial interactive
one with spontaneous supersymmetry breaking. The couplings of the equivalent
theory acquire a threshold correction through their dependence on the scale of
the higher dimensional operator(s). The scalar potential in the second order
theory is not necessarily positive definite, and one can in principle have a
vanishing potential with broken supersymmetry. We provide an application to
MSSM and argue that at tree-level and for a mass scale associated to a higher
derivative term in the TeV range, the Higgs mass can be lifted above the

On stable higher spin states in Heterotic String Theories
We study properties of 1/2 BPS Higher Spin states in heterotic
compactifications with extended supersymmetry. We also analyze non BPS Higher
Spin states and give explicit expressions for physical vertex operators of the
first two massive levels. We then study on-shell tri-linear couplings of these
Higher Spin states and confirm that BPS states with arbitrary spin cannot decay
into lower spin states in perturbation theory. Finally, we consider scattering
of vector bosons off higher spin BPS states and extract form factors and
polarization effects in various limits.Comment: 38 page
Community-based in situ simulation: bringing simulation to the masses
Simulation-based methods are regularly used to train inter-professional groups of healthcare providers at academic medical centers (AMC). These techniques are used less frequently in community hospitals. Bringing in-situ simulation (ISS) from AMCs to community sites is an approach that holds promise for addressing this disparity. This type of programming allows academic center faculty to freely share their expertise with community site providers. By creating meaningful partnerships community-based ISS facilitates the communication of best practices, distribution of up to date policies, and education/training. It also provides an opportunity for system testing at the community sites. In this article, we illustrate the process of implementing an outreach ISS program at community sites by presenting four exemplar programs. Using these exemplars as a springboard for discussion, we outline key lessons learned discuss barriers we encountered, and provide a framework that can be used to create similar simulation programs and partnerships. It is our hope that this discussion will serve as a foundation for those wishing to implement community-based, outreach ISS

Genomic surveillance elucidates Ebola virus origin and transmission during the 2014 outbreak
In its largest outbreak, Ebola virus disease is spreading through Guinea, Liberia, Sierra Leone, and Nigeria. We sequenced 99 Ebola virus genomes from 78 patients in Sierra Leone to ~2000Ă coverage. We observed a rapid accumulation of interhost and intrahost genetic variation, allowing us to characterize patterns of viral transmission over the initial weeks of the epidemic. This West African variant likely diverged from central African lineages around 2004, crossed from Guinea to Sierra Leone in May 2014, and has exhibited sustained human-to-human transmission subsequently, with no evidence of additional zoonotic sources. Because many of the mutations alter protein sequences and other biologically meaningful targets, they should be monitored for impact on diagnostics, vaccines, and therapies critical to outbreak response.Organismic and Evolutionary Biolog
Periodontal Ehlers-Danlos Syndrome Is Caused by Mutations in C1R and C1S, which Encode Subcomponents C1r and C1s of Complement
Periodontal Ehlers-Danlos syndrome (pEDS) is an autosomal-dominant disorder characterized by early-onset periodontitis leading to premature loss of teeth, joint hypermobility, and mild skin findings. A locus was mapped to an approximately 5.8 Mb region at 12p13.1 but no candidate gene was identified. In an international consortium we recruited 19 independent families comprising 107 individuals with pEDS to identify the locus, characterize the clinical details in those with defined genetic causes, and try to understand the physiological basis of the condition. In 17 of these families, we identified heterozygous missense or in-frame insertion/deletion mutations in C1R (15 families) or C1S (2 families), contiguous genes in the mapped locus that encode subunits C1r and C1s of the first component of the classical complement pathway. These two proteins form a heterotetramer that then combines with six C1q subunits. Pathogenic variants involve the subunit interfaces or inter-domain hinges of C1r and C1s and are associated with intracellular retention and mild endoplasmic reticulum enlargement. Clinical features of affected individuals in these families include rapidly progressing periodontitis with onset in the teens or childhood, a previously unrecognized lack of attached gingiva, pretibial hyperpigmentation, skin and vascular fragility, easy bruising, and variable musculoskeletal symptoms.
