    The Value of Large Scale Tourism in Samoa

    During the mid to late seventies serious questions were raised regarding the value of mass tourism. This lead to the creation of the alternative development paradigm with its preference towards small scale, local and community based and controlled projects which have gained wide acceptance within the field of development studies. This research looks to reopen and examine the case against mass tourism as a development tool under the following arguments: · The initial assessment was done in the late seventies and a combination of changes in business practices and consumer demands for ethical behaviour has potentially changed the development outcomes for host nations · Governments have a greater and more balanced awareness of the range of development issues and enterprises need to respond to this · Alternative development suffers from similar criticisms to those that have been directed at mass tourism as well as some unique issues · Labour force training has been largely overlooked as critical step in maximising potential development outcomes for host nations · Two major critiques around power inequality (Britton 1983) and empowerment (Sofield 2003) are external subjective judgements about development outcomes where feedback and conclusions from within host populations might add additional insight. A survey questioning the impact a large scale development had on people’s lives was central to the research. However, due to difficulties getting survey data from a large scale development in Samoa the research has focused on providing the arguments above from literature and on secondary research aims of seeking the governments views and strategies to deal with tourism and linking these strategies back to literature

    Self-assembled nanocomplex between polymerized phenylboronic acid and doxorubicin for efficient tumor-targeted chemotherapy

    Since the discovery that nano-scaled particulates can easily be incorporated into tumors via the enhanced permeability and retention (EPR) effect, such nanostructures have been exploited as therapeutic small molecule delivery systems. However, the convoluted synthetic process of conventional nanostructures has impeded their feasibility and reproducibility in clinical applications. Herein, we report an easily prepared formulation of self-assembled nanostructures for systemic delivery of the anti-cancer drug doxorubicin (DOX). Phenylboronic acid (PBA) was grafted onto the polymeric backbone of poly(maleic anhydride). pPBA-DOX nanocomplexes were prepared by simple mixing, on the basis of the strong interaction between the 1,3-diol of DOX and the PBA moiety on pPBA. Three nanocomplexes (1, 2, 4) were designed on the basis of [PBA]:[DOX] molar ratios of 1: 1, 2: 1, and 4: 1, respectively, to investigate the function of the residual PBA moiety as a targeting ligand. An acid-labile drug release profile was observed, owing to the intrinsic properties of the phenylboronic ester. Moreover, the tumor-targeting ability of the nanocomplexes was demonstrated, both in vitro by confocal microscopy and in vivo by fluorescence imaging, to be driven by an inherent property of the residual PBA. Ligand competition assays with free PBA pre-treatment demonstrated the targeting effect of the residual PBA from the nanocomplexes 2 and 4. Finally, the nanocomplexes 2 and 4, compared with the free DOX, exhibited significantly greater anti-cancer effects in vitro and even in vivo.     Effects of improved sanitation on diarrheal reduction for children under five in Idiofa, DR Congo: a cluster randomized trial.

    BACKGROUND: The lack of safe water and sanitation contributes to the rampancy of diarrhea in many developing countries. METHODS: This study describes the design of a cluster-randomized trial in Idiofa, the Democratic Republic of the Congo, seeking evidence of the impact of improved sanitation on diarrhea for children under four. Of the 276 quartiers, 18 quartiers were randomly allocated to the intervention or control arm. Seven hundred and-twenty households were sampled and the youngest under-four child in each household was registered for this study. The primary endpoint of the study is diarrheal incidence, prevalence and duration in children under five. DISCUSSION: Material subsidies will be provided only to the households who complete pit digging plus superstructure and roof construction, regardless of their income level. This study employs a Sanitation Calendar so that the mother of each household can record the diarrheal episodes of her under-four child on a daily basis. The diary enables examination of the effect of the sanitation intervention on diarrhea duration and also resolves the limitation of the small number of clusters in the trial. In addition, the project will be monitored through the 'Sanitation Map', on which all households in the study area, including both the control and intervention arms, are registered. To avoid information bias or courtesy bias, photos will be taken of the latrine during the household visit, and a supervisor will determine well-equipped latrine uptake based on the photos. This reduces the possibility of recall bias and under- or over-estimation of diarrhea, which was the main limitation of previous studies.     Cutaneous Metastasis of Choriocarcinoma : A Case Report

    Choriocarcinoma is one of the malignant tumors of trophoblastic cells characterized by the secretion of human chorionic gonadotrophin (hCG) (1-3). Cutaneous metastasis is a rare presentation of choriocarcinoma but a poor prognostic sign because it is associated only with widespread disease (3-5). A 52-yr-old female complaining of dyspnea for 2 months, presented with fingertip sized erythematous nodules on the left side of the neck and the right side of the upper back of 1 month duraton. She has suffered from Behçet's disease since 1999. Microscopic examination of a nodule of upper back demonstrated biphasic pattern of cytotrophoblasts and hCG-positive syncytiotrophoblasts, and the typical histologic features of choriocarcinoma. She was referred to the gynecological oncology department. After 17 cycles of combination chemotherapy, the serum hCG level has fallen from 700,000 to under 2.0 mIU/mL and the skin lesions have almost disappeared. However, after 3 months, total abdominal hysterectomy and bilateral salpingo-oophorectomy were performed due to elevated serum hCG level (4,447.7 mIU/mL), and she is scheduled to receive post-operative adjuvant chemotherapy

    Association of TNFA Promoter Region Haplotype in Behçet's Disease

    Although the etiology of Behçet's Disease (BD; MIM 109650) remains to be clearly elucidated, levels of tumor necrosis factor alpha (TNF-α) have been reported to be significantly elevated in BD patients, and TNF-α blockers have been demonstrated to exhibit some degree of therapeutic efficacy for a certain subset of BD sufferers. In this study, we have conducted an analysis of the TNFA haplotypes in the promoter response element that affect the binding affinity of specific transcription factors, in order to characterize their association with the clinical features of BD. Six polymorphisms in the promoter region of TNFA were genotyped in 254 BD patients and 344 control subjects, via the PCR-RFLP technique. TNFA -1031*C, -863*A and -308*G alleles were associated with an increased risk of BD (p=0.030, OR=1.4; p=0.008, OR=1.5; p=0.010, OR=1.8, respectively). The sole TNFA haplotype -1031C-863A-857C-376G-308G-238G, was associated with a 1.6 fold increase in the risk of BD, whereas the TNFA haplotype -1031T-863C-857C-376G-308A-238G was associated with a 0.6 decreased risk of BD. The TNFA -1031*C, -863*A, -857*C and -308*G alleles were significantly associated with BD. The findings of this study, collectively, indicate that TNFA haplotypes in the promoter response elements may exert significant influence on susceptibility to BD

    Discovery of Q203, a potent clinical candidate for the treatment of tuberculosis

    New therapeutic strategies are needed to combat the tuberculosis pandemic and the spread of multidrug-resistant (MDR) and extensively drug-resistant (XDR) forms of the disease, which remain a serious public health challenge worldwide1, 2. The most urgent clinical need is to discover potent agents capable of reducing the duration of MDR and XDR tuberculosis therapy with a success rate comparable to that of current therapies for drug-susceptible tuberculosis. The last decade has seen the discovery of new agent classes for the management of tuberculosis3, 4, 5, several of which are currently in clinical trials6, 7, 8. However, given the high attrition rate of drug candidates during clinical development and the emergence of drug resistance, the discovery of additional clinical candidates is clearly needed. Here, we report on a promising class of imidazopyridine amide (IPA) compounds that block Mycobacterium tuberculosis growth by targeting the respiratory cytochrome bc1 complex. The optimized IPA compound Q203 inhibited the growth of MDR and XDR M. tuberculosis clinical isolates in culture broth medium in the low nanomolar range and was efficacious in a mouse model of tuberculosis at a dose less than 1 mg per kg body weight, which highlights the potency of this compound. In addition, Q203 displays pharmacokinetic and safety profiles compatible with once-daily dosing. Together, our data indicate that Q203 is a promising new clinical candidate for the treatment of tuberculosis

    Effects of Substrate-Blocks Interactions on the Phase Behaviors of Cylinder-Forming Block Copolymers

    Self-assembly of block copolymers greatly facilitates production of controlled periodic nanostructures for nanotechnological applications. Orientation control in such structures with defect-free ordering on larger length scales still remain a major research challenge in many cases. In addition to their pure block forms, blends of copolymers with other polymers offer productive research areas in relation to nanostructure self-assembly. We prepared well-aligned nanocylinders in block copolymers over an enhanced sample area and height scale without using any external field applications. Self-assembled 3-dimensional perpendicular cylinder orientation was achieved mainly by blending of minority homopolymer into the block copolymer. In this work, we illustrate that the mixed cylinder nanodomain orientation exits in block copolymer mixtures even with a relatively strong homologous polymer pair interaction and a decreased domain-domain excess free energy, when the interfacial interaction between the majority component and the substrate is high enough to restrict the perpendicular nanodomain orientation. In case of overall perpendicular cylinder-forming block copolymer mixtures, on the other hand, the strong homologous PS pair interaction overcomes the limitation from the relatively high domain-substrate interfacial interactions.     Laser Plasma Sintering for Fabricating Indium Tin Oxide Thin Films

    This paper introduces laser plasma sintering (LPS), and uses it to fabricate dense indiumtin oxide (ITO) thin films. LPS uses a nanosecond pulsed laser to generate an airborne laser-induced plasma (LIP) of high temperature and high pressure, which sinters the particles on the substrate. ITO thin films were fabricated by spin-coating ITO nanoparticle (NP) ink onto a substrate then using the LIP to sinter the NPs. The deposited NPs are sintered by the combined effect of the thermal radiation from the LIP and the impact of the shockwave. This LPS process could generate a polycrystalline structure with an average grain size of 280 nm. The electrical resistivity of the film was decreased to similar to 1/1000 of the initial value without losing transparency of the ITO film. The minimum resistivity of 1.6 x 10(-5) Omega.m was obtained significantly reducing the optical transparency of the ITO film. The thickness of the sintered layer and electrical resistivity could be controlled by adjusting the process parameters. LPS also strengthened the adhesion between the sintered film and the substrate significantly, compared with the conventional thermal or laser sintering process. This work suggests that LPS process can be an effective tool to fabricate various thin films with enhanced crystallinity and adhesion characteristics. (C) 2016 Elsevier B.V.
