    A parallel multigrid solver for multi-patch Isogeometric Analysis

    Isogeometric Analysis (IgA) is a framework for setting up spline-based discretizations of partial differential equations, which has been introduced around a decade ago and has gained much attention since then. If large spline degrees are considered, one obtains the approximation power of a high-order method, but the number of degrees of freedom behaves like for a low-order method. One important ingredient to use a discretization with large spline degree, is a robust and preferably parallelizable solver. While numerical evidence shows that multigrid solvers with standard smoothers (like Gauss Seidel) does not perform well if the spline degree is increased, the multigrid solvers proposed by the authors and their co-workers proved to behave optimal both in the grid size and the spline degree.     Precision medicine in distinct heart failure phenotypes: Focus on clinical epigenetics

    Heart failure (HF) management is challenging due to high clinical heterogeneity of this disease which makes patients responding differently to evidence-based standard therapy established by the current reductionist approach. Better understanding of the genetic and epigenetic interactions may clarify molecular signatures underlying maladaptive responses in HF, including metabolic shift, myocardial injury, fibrosis, and mitochondrial dysfunction. DNA methylation, histone modifications and micro-RNA (miRNAs) may be major epigenetic players in the pathogenesis of HF. DNA hypermethylation of the kruppel-like factor 15 (KLF1.5) gene plays a key role in switching the failing heart from oxidative to glycolytic metabolism. Moreover, hypomethylation at H3K9 promoter level of atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP) genes also leads to reactivation of fetal genes in man. The role of miRNAs has been investigated in HF patients undergoing heart transplantation, for whom miR-10a, miR-155, miR-31, and miR-92 may be putative useful prognostic biomarkers. Recently, higher RNA methylation levels have been observed in ischemic human hearts, opening the era of "epitranscriptome" in the pathogenesis of HF. Currently, hydralazine, statins, apabetalone, and omega-3 polyunsatured fatty acids (PUFA) are being tested in clinical trials to provide epigenetic-driven therapeutic interventions. Moreover, network-oriented analysis could advance current medical practice by focusing on protein-protein interactions (PPIs) perturbing the "cardiac" interactome. In this review, we provide an epigenetic map of maladaptive responses in HF patients. Furthermore, we propose the "EPi-transgeneratlonal network mOdeling for STratificatiOn of heaRt Morbidity" (EPIKO-STORM), a clinical research strategy offering novel opportunities to stratify the natural history of HF

    Estimating the trace of matrix functions with application to complex networks

    The approximation of trace(f(Ω)), where f is a function of a symmetric matrix Ω, can be challenging when Ω is exceedingly large. In such a case even the partial Lanczos decomposition of Ω is computationally demanding and the stochastic method investigated by Bai et al. (J. Comput. Appl. Math. 74:71–89, 1996) is preferred. Moreover, in the last years, a partial global Lanczos method has been shown to reduce CPU time with respect to partial Lanczos decomposition. In this paper we review these techniques, treating them under the unifying theory of measure theory and Gaussian integration. This allows generalizing the stochastic approach, proposing a block version that collects a set of random vectors in a rectangular matrix, in a similar fashion to the partial global Lanczos method. We show that the results of this technique converge quickly to the same approximation provided by Bai et al. (J. Comput. Appl. Math. 74:71–89, 1996), while the block approach can leverage the same computational advantages as the partial global Lanczos. Numerical results for the computation of the Von Neumann entropy of complex networks prove the robustness and efficiency of the proposed block stochastic method

    A Novel Sequence-Based Antigenic Distance Measure for H1N1, with Application to Vaccine Effectiveness and the Selection of Vaccine Strains

    H1N1 influenza causes substantial seasonal illness and was the subtype of the 2009 influenza pandemic. Precise measures of antigenic distance between the vaccine and circulating virus strains help researchers design influenza vaccines with high vaccine effectiveness. We here introduce a sequence-based method to predict vaccine effectiveness in humans. Historical epidemiological data show that this sequence-based method is as predictive of vaccine effectiveness as hemagglutination inhibition (HI) assay data from ferret animal model studies. Interestingly, the expected vaccine effectiveness is greater against H1N1 than H3N2, suggesting a stronger immune response against H1N1 than H3N2. The evolution rate of hemagglutinin in H1N1 is also shown to be greater than that in H3N2, presumably due to greater immune selection pressure.Comment: 26 pages, 7 figures, 2 tables, supplemen

    Endoscopic management of gastrointestinal leaks and fistulae: What option do we have?

    Gastrointestinal leaks and fistulae are serious, potentially life threatening conditions that may occur with a wide variety of clinical presentations. Leaks are mostly related to post-operative anastomotic defects and are responsible for an important share of surgical morbidity and mortality. Chronic leaks and long standing post-operative collections may evolve in a fistula between two epithelialized structures. Endoscopy has earned a pivotal role in the management of gastrointestinal defects both as first line and as rescue treatment. Endotherapy is a minimally invasive, effective approach with lower morbidity and mortality compared to revisional surgery. Clips and luminal stents are the pioneer of gastrointestinal (GI) defect endotherapy, whereas innovative endoscopic closure devices and techniques, such as endoscopic internal drainage, suturing system and vacuum therapy, has broadened the indications of endoscopy for the management of GI wall defect. Although several endoscopic options are currently used, a standardized evidence-based algorithm for management of GI defect is not available. Successful management of gastrointestinal leaks and fistulae requires a tailored and multidisciplinary approach based on clinical presentation, defect features (size, location and onset time), local expertise and the availability of devices. In this review, we analyze different endoscopic approaches, which we selected on the basis of the available literature and our own experience. Then, we evaluate the overall efficacy and procedural-specific strengths and weaknesses of each approach

    Epigenetic therapies for heart failure: Current insights and future potential

    Despite the current reductionist approach providing an optimal indication for diagnosis and treatment of patients with heart failure with reduced ejection fraction (HFrEF), there are no standard pharmacological therapies for heart failure with preserved ejection fraction (HFpEF). Although in its infancy in cardiovascular diseases, the epigenetic-based therapy (“epidrugs”) is capturing the interest of physician community. In fact, an increasing number of controlled clinical trials is evaluating the putative beneficial effects of: 1) direct epigenetic-oriented drugs, eg, apabetalone, and 2) repurposed drugs with a possible indirect epigenetic interference, eg, metformin, statins, sodium glucose transporter inhibitors 2 (SGLT2i), and omega 3 polyunsaturated fatty acids (PUFAs) in both HFrEF and HFpEF, separately. Apabetalone is the first and unique direct epidrug tested in cardiovascular patients to date, and the BETonMACE trial has reported a reduction in first HF hospitalization (any EF value) and cardiovascular death in patients with type 2 diabetes and recent acute coronary syndrome, suggesting a possible role in secondary prevention. Patients with HFpEF seem to benefit from supplementation to the standard therapy with statins, metformin, and SGLT2i owing to their ability in reducing mortality. In contrast, the vasodilator hydralazine, with or without isosorbide dinitrate, did not provide beneficial effects. In HFrEF, metformin and SGLT2i could reduce the risk of incident HF and mortality in affected patients whereas clinical trials based on statins provided mixed results. Furthermore, PUFAs diet supplementation was significantly associated with reduced cardiovascular risk in both HFpEF and HFrEF. Future large trials will reveal whether direct and indirect epitherapy will remain a work in progress or become a useful way to customize the therapy in the real-world management of HFpEF and HFrEF. Our goal is to discuss the recent advancement in the epitherapy as a possible way to improve personalized therapy of HF

    DEVELOPMENT OF OFF-DESIGN TURBOCHARGER MODELLING COMBINED WITH 1-D ENGINE MODEL

    The present work aims to carry out an off-design turbocharger modellingpowered by exhaust gases from a Wärtsilä 20V34SG engine. First of all, 1-D engine model was already developed in GT-Power software whileconsidering a thermodynamic turbocharger modelling with constantisentropic efficiencies. Secondly, by using the results from 1-D enginemodel, the off-design turbocharger modelling is calibrated separately inEES software, taking into account compressible assumption, trianglevelocities and geometric dimensions. The case study is derived from a R&Dproject (ANEEL PD-06483-0318/2018) that targets to cool and dehumidifythe intake air at compressor’s upstream through a cooling coil, therebyallowing engine’s operation at reduced knocking conditions. The brakemean effective pressure (BMEP) is varied in the range of 20 to 23.45 bar,corresponding to brake power from 8.7 to 10.2 MW, respectively. With theoff-design turbocharger modelling it is possible to analyze its operationalbehavior under higher BMEP, hence, allowing to predict some importantparameters. The results showed that the turbocharger is operating within themanufacturer’s limit for BMEP of 23.45 bar, presenting total-to-staticisentropic efficiencies of 0.81 and 0.784 for compressor and turbine,respectively, rotational speed around 28135 RPM, pressure ratio atcompressor of 4.567 and maintaining control on waste-gate valve

    Guarded Autonomous Transitions Increase Conciseness and Expressiveness of Timed Automata

    International audienceTimed Automata (TA) are an appropriate model for specifying timed requirements for Continuous Time Markov Chains (CTMC). However in order to keep tractable the model checking of a TA over a CTMC, temporal logics based on TA, like CSL TA , restrict TA to have a single clock and to be deterministic (DTA). Different variants of DTAs have been proposed to address the issue of their expressiveness and conciseness. Here we study the effect of two possible features: (1) autonomous transitions which are triggered by time elapsing in addition to synchronized transitions and (2) transitions guarded by propositional formulas instead of propositional formulas guarding locations. We first show that autonomous guarded transitions increase the expressiveness of DTAs (as already shown for guarded locations). Then we identify a hierarchy of DTAs subclasses all equivalent to DTAs without guarded autonomous transitions and we analyze their respective conciseness. In particular we show that eliminating resets in autonomous transitions implies an exponential blow-up, while eliminating autonomous transitions without reset can be performed in polynomial time if decision diagrams are used. Finally we compare TA with guarded transitions to TA with guarded locations showing that the former model is exponentially more concise than the latter one
