    Polycystic ovary syndrome, metabolic syndrome, cardiovascular risk and the role of insulin sensitizing agents

    The Polycystic Ovary Syndrome (PCOS) affects 6 to 10% of women of childbearing age. Insulin resistance and hyperinsulinemia are present in nearly all PCOS patients and play a central role in the development of both hyperandrogenism and metabolic syndrome (MS). MS occurs in approximately 43% of PCOS patients, raising the cardiovascular risk to up seven fold in these patients. Several serum, functional and structural markers of endothelial dysfunction and subclinical atherosclerosis were described in PCOS patients, even those young and non-obese. However, despite the fact that PCOS adversely affects the cardiovascular profile, long-term studies did not demonstrate a consistent raise in cardiovascular mortality, which seems to be more observed in the post-menopausal period. Recently, oral contraceptives are being substituted for insulin sensitizing agents (metformin and glitazones) in the PCOS treatment, due to their effects on insulin resistance and cardiovascular risk.A Síndrome dos Ovários Policísticos (SOP) afeta de 6 a 10% das mulheres em idade reprodutiva. Resistência à insulina e hiperinsulinemia estão presentes em praticamente todas as pacientes com SOP e desempenham papel central no desenvolvimento tanto do hiperandrogenismo como da síndrome metabólica (SM). SM ocorre em aproximadamente 43% das pacientes com SOP, elevando em até sete vezes o risco de doença cardiovascular nestas pacientes. Vários marcadores séricos, funcionais e estruturais de disfunção endotelial e de aterosclerose subclínica foram descritos em pacientes com SOP, mesmo nas jovens e não-obesas. Entretanto, embora a SOP afete adversamente o perfil cardiovascular, estudos a longo prazo não demonstraram consistentemente aumento da mortalidade cardiovascular, a qual parece ser mais observada no período da pós-menopausa. Recentemente, os anticoncepcionais orais estão sendo substituídos pelos agentes sensibilizadores de insulina (metformina e glitazonas) no tratamento da SOP, devido aos seus efeitos sobre a resistência à insulina e o risco cardiovascular.UNIFESP-EPMUNIFESP, EPMSciEL

    Hormonal therapy of menopause: 2004 position of the Department of Female Endocrinology and Andrology of the Brazilian Society of Endocrinology and Metabolism

    After randomized clinical trials produced impact and questions at the medical community on menopause hormonal therapy (MHT), the Department of Female Endocrinology and Andrology of the Brazilian Society of Endocrinology convened a group of specialists to produce an informative, critical and position paper, offering guidelines for those who practice or are called to express their opinion on menopause therapy. MHT is indicated for relief of vasomotor symptoms, maintenance of vaginal trophism, bone mass and collagen preservation, sexual and general well-being. Studies on primary cardiovascular prevention are not conclusive, and thus insufficient to indicate or not MHT for this purpose. Schemes and forms of association should de individualized. Whenever possible, one should use 17-beta estradiol, associated to natural progesterone or its derivatives in women with an intact uterus, at the lowest effective doses. Duration of therapy depends on its aims, and should be periodically re-evaluated through an individual indication/contra-indication balance. Orientation toward a healthier lifestyle, with smoking cessation, adequate calcium intake and a low fat diet, together with regular physical activity is fundamental during menopause.Após os estudos clínicos randomizados produzirem impacto e questionamentos sobre terapia hormonal da menopausa (THM) na comunidade médica, o Departamento de Endocrinologia Feminina e Andrologia da SBEM reuniu um grupo de especialistas para formular um documento informativo, crítico e de posicionamento que pudesse servir de orientação aos que praticam ou opinam sobre tratamento da menopausa. THM está indicada para alívio dos sintomas motores, conservação do trofismo vaginal, preservação de massa óssea e colágeno, melhora do bem estar e sexualidade. Os estudos sobre prevenção cardiovascular primária não são conclusivos, portanto insuficientes para indicar ou deixar de indicar THM com este objetivo. Esquemas e tipos de associação devem ser individualizados. Sempre que possível deve-se optar pelas menores doses eficazes de 17-beta estradiol, associado a progesterona natural ou seus derivados nas mulheres com útero. A duração do tratamento é vinculada a seus objetivos, devendo ser reavaliada periodicamente através de um balanço individual entre indicação e contra-indicação.     Välttelevän kiintymyssuhteen merkitys lapsen hyvinvoinnille

    Tarkoituksena tässä opinnäytetyössä on koota tutkimustietoa välttelevästä kiintymyssuhteesta ja sen merkityksestä lapsen hyvinvoinnille. Tavoitteena oli kehittää ammatillista osaamista teoriapohjan osalta ja käsitellä tutkittavaa aihetta psykiatrisen hoitotyön näkökulmasta, mikä on suuntautumiseni. Tutkimuskysymys on jaettu kahteen osaan: vanhempien ja lapsen välinen kiintymyssuhde ja lapsen kehitys ja hyvinvointi. Näiden osien ulottuvilta on haettu tutkimusartikkeleita, tutkimusraportteja ja asiantuntijakirjoituksia, joista on koottu tiivistelmiä kirjallisuuskatsauksen tapaan. Pohdinnassa on eritelty aineistoa tarkemmin tutkimuskysymyksen osalta. Kiintymyssuhde kehittyy lapsella varhaisen vuorovaikutuksen kautta 0–2-vuotiaana. Jos vanhemmat laiminlyövät kommunikoinnin lapsen kanssa, emotionaalinen kehitys jää vajaaksi eikä lapsi opi ilmaisemaan omia tarpeitaan ja tunteitaan. Välttelevän kiintymyssuhteen seuraukset voivat olla laajat epäsosiaalisuudesta kognitiivisiin häiriöihin. Epäsosiaalisuuteen kytkeytyviin häiriöihin kuuluu puhumattomuus, eristäytyminen ja laajemmin kroonistunut masennus, mihin taas liittyy vahva itsetuhoisuuden riski

    NEOTROPICAL ALIEN MAMMALS: a data set of occurrence and abundance of alien mammals in the Neotropics

    Biological invasion is one of the main threats to native biodiversity. For a species to become invasive, it must be voluntarily or involuntarily introduced by humans into a nonnative habitat. Mammals were among first taxa to be introduced worldwide for game, meat, and labor, yet the number of species introduced in the Neotropics remains unknown. In this data set, we make available occurrence and abundance data on mammal species that (1) transposed a geographical barrier and (2) were voluntarily or involuntarily introduced by humans into the Neotropics. Our data set is composed of 73,738 historical and current georeferenced records on alien mammal species of which around 96% correspond to occurrence data on 77 species belonging to eight orders and 26 families. Data cover 26 continental countries in the Neotropics, ranging from Mexico and its frontier regions (southern Florida and coastal-central Florida in the southeast United States) to Argentina, Paraguay, Chile, and Uruguay, and the 13 countries of Caribbean islands. Our data set also includes neotropical species (e.g., Callithrix sp., Myocastor coypus, Nasua nasua) considered alien in particular areas of Neotropics. The most numerous species in terms of records are from Bos sp. (n = 37,782), Sus scrofa (n = 6,730), and Canis familiaris (n = 10,084); 17 species were represented by only one record (e.g., Syncerus caffer, Cervus timorensis, Cervus unicolor, Canis latrans). Primates have the highest number of species in the data set (n = 20 species), partly because of uncertainties regarding taxonomic identification of the genera Callithrix, which includes the species Callithrix aurita, Callithrix flaviceps, Callithrix geoffroyi, Callithrix jacchus, Callithrix kuhlii, Callithrix penicillata, and their hybrids. This unique data set will be a valuable source of information on invasion risk assessments, biodiversity redistribution and conservation-related research. There are no copyright restrictions.
