    Ultrasonic metal sheet thickness measurement without prior wave speed calibration

    Conventional ultrasonic mensuration of sample thickness from one side only requires the bulk wave reverberation time and a calibration speed. This speed changes with temperature, stress, and microstructure, limiting thickness measurement accuracy. Often, only one side of a sample is accessible, making in situ calibration impossible. Non-contact ultrasound can generate multiple shear horizontal guided wave modes on one side of a metal plate. Measuring propagation times of each mode at different transducer separations, allows sheet thickness to be calculated to better than 1% accuracy for sheets of at least 1.5 mm thickness, without any calibration

    Do-not-attempt-cardiopulmonary-resuscitation decisions : an evidence synthesis

    Background: Cardiac arrest is the final common step in the dying process. In the right context, resuscitation can reverse the dying process, yet success rates are low. However, cardiopulmonary resuscitation (CPR) is a highly invasive medical treatment, which, if applied in the wrong setting, can deprive the patient of dignified death. Do-not-attempt-cardiopulmonary-resuscitation (DNACPR) decisions provide a mechanism to withhold CPR. Recent scientific and lay press reports suggest that the implementation of DNACPR decisions in NHS practice is problematic. Aims and objectives: This project sought to identify reasons why conflict and complaints arise, identify inconsistencies in NHS trusts’ implementation of national guidelines, understand health professionals’ experience in relation to DNACPR, its process and ethical challenges, and explore the literature for evidence to improve DNACPR policy and practice. Methods: A systematic review synthesised evidence of processes, barriers and facilitators related to DNACPR decision-making and implementation. Reports from NHS trusts, the National Reporting and Learning System, the Parliamentary and Health Service Ombudsman, the Office of the Chief Coroner, trust resuscitation policies and telephone calls to a patient information line were reviewed. Multiple focus groups explored service-provider perspectives on DNACPR decisions. A stakeholder group discussed the research findings and identified priorities for future research. Results: The literature review found evidence that structured discussions at admission to hospital or following deterioration improved patient involvement and decision-making. Linking DNACPR to overall treatment plans improved clarity about goals of care, aided communication and reduced harms. Standardised documentation improved the frequency and quality of recording decisions. Approximately 1500 DNACPR incidents are reported annually. One-third of these report harms, including some instances of death. Problems with communication and variation in trusts’ implementation of national guidelines were common. Members of the public were concerned that their wishes with regard to resuscitation would not be respected. Clinicians felt that DNACPR decisions should be considered within the overall care of individual patients. Some clinicians avoid raising discussions about CPR for fear of conflict or complaint. A key theme across all focus groups, and reinforced by the literature review, was the negative impact on overall patient care of having a DNACPR decision and the conflation of ‘do not resuscitate’ with ‘do not provide active treatment’. Limitations: The variable quality of some data sources allows potential overstatement or understatement of findings. However, data source triangulation identified common issues. Conclusion: There is evidence of variation and suboptimal practice in relation to DNACPR decisions across health-care settings. There were deficiencies in considering, discussing and implementing the decision, as well as unintended consequences of DNACPR decisions being made on other aspects of patient care. Future work: Recommendations supported by the stakeholder group are standardising NHS policies and forms, ensuring cross-boundary recognition of DNACPR decisions, integrating decisions with overall treatment plans and developing tools and training strategies to support clinician and patient decision-making, including improving communication.     Finishing the euchromatic sequence of the human genome

    The sequence of the human genome encodes the genetic instructions for human physiology, as well as rich information about human evolution. In 2001, the International Human Genome Sequencing Consortium reported a draft sequence of the euchromatic portion of the human genome. Since then, the international collaboration has worked to convert this draft into a genome sequence with high accuracy and nearly complete coverage. Here, we report the result of this finishing process. The current genome sequence (Build 35) contains 2.85 billion nucleotides interrupted by only 341 gaps. It covers ∼99% of the euchromatic genome and is accurate to an error rate of ∼1 event per 100,000 bases. Many of the remaining euchromatic gaps are associated with segmental duplications and will require focused work with new methods. The near-complete sequence, the first for a vertebrate, greatly improves the precision of biological analyses of the human genome including studies of gene number, birth and death. Notably, the human enome seems to encode only 20,000-25,000 protein-coding genes. The genome sequence reported here should serve as a firm foundation for biomedical research in the decades ahead

    Until recently, deer hunting in medieval Scotland has been poorly researched archaeologically. In Hunting and Hunting Reserves in Medieval Scotland Gilbert identified medieval parks at Stirling and Kincardine in Perthshire that William the Lion created, but it is only in recent years that excavations by Hall and Malloy have begun to explore their archaeology. The Royal Commission on the Ancient and Historical Monuments of Scotland recorded another type of hunting feature, a deer trap at Hermitage Castle, in 1996 and then re-recorded the earthwork at Dormount Hope in 2000, originally reported as two separate monuments. Although the earthworks of parks and traps display similarities in the construction of their earthwork boundaries, the individual sites have variations in their topography that beg questions about their function. This paper establishes that the earthwork is indeed a single monument which has an open end allowing deer to be driven into the natural canyon of Dormount Hope. It goes on to discuss its dating in both archaeological and documentary terms and then its function as either a park, trap or hay (haga OE). This last possibility is raised by its apparent mention in a Melrose Abbey charter of the neighbouring estate of Raeshaw dating to the last quarter of the 12th century, made by the lords of Hownam, a family of Anglian origin. This Anglian connection leads to its interpretation as a hay – a kind of deer hunting enclosure or trap known in many parts of England prior to the Norman Conquest, for which ‘hay’ place names, such as Hawick, in the Scottish Borders provide support.<br/

    Reading the pastoral landscape:Palynological and historical evidence for the impacts of long-term grazing on wether hill, ingram, northumberland

    Many upland environments are valued for their openness, which is often actively maintained by extensive pastoral agriculture. Documentary sources indicate the complexity and longevity of regulations designed to protect the hill grazing resource from over-exploitation but these systems leave relatively few traces on the ground. Consequently pollen analysis is an important method for establishing the impact of centuries of grazing on the quality of hill pastures. This is demonstrated at Wether Hill, Northumberland, where a pollen sequence details changes in vegetation composition and diversity over the last c. 1,500 years. These are correlated with historical evidence over the last c. 800years for a more complete understanding of the socioeconomic context that governed the use of hill grazing. Changes in grazing regimes had a profound influence on these hill pastures, contributing to permanent changes in the relative abundance of heather, grasses and herbs, and causing a severe decline in habitat diversity within the last c. 200 years. The results have many regional parallels, indicating extensive reductions in the biodiversity of upland habitats. This has implications for future management and conservation policies and shows the contribution that an understanding of environmental and land-use history can make to debates surrounding current environmental issues.</p
