    No aplicaEl presente informe de opción de pasantía registra todas las actividades ejercidas durante el periodo práctico del programa de agronomía, entre las actividades ejercidas se realizaron visitas a huertos y cultivos, desarrollo de Escuelas de Campo y apoyo en los circuitos cortos como mercados campesinos cada mes. Estas actividades fueron ejercidas dando cumplimiento al plan de trabajo y al cronograma de actividades, tanto de la propuesta para esta opción de grado como el plan de trabajo y las metas que la corporación Centro Provincial de Gestión Agroempresarial de los Puertos CPGA establecieron. Durante el desarrollo de las actividades se logra un acercamiento con los diferentes productores que solicitaron el servicio de acompañamiento realizando una presentación personal y del convenio de la universidad Abierta y a Distancia con la corporación CPGA. Por medio de estas visitas, se logra realizar la orientación a los diferentes productores a través de transferencia de tecnología y escuelas de campo ECAS con el fin de ejercer un mejoramiento y fortalecer los sistemas agrícolasThis internship option report records all the activities carried out during the practical period of the agronomy program. Among the activities carried out were visits to orchards and crops, development of field schools and support in short circuits such as farmers' markets every month. These activities were carried out in compliance with the work plan and schedule of activities, both the proposal for this degree option and the work plan and goals established by the Provincial Center for Agribusiness Management of Los Puertos CPGA. During the development of the activities, an approach is achieved with the different producers who requested the accompaniment service, making a personal presentation and the agreement of the Open and Distance University with the CPGA corporation. Through these visits, it is possible to provide guidance to the different producers through technology transfer and ECAS field schools in order to improve and strengthen agricultural systems

    Perioperative outcome and cost-effectiveness of spinal versus general anesthesia for lumbar spine surgery

    Background and aim General anesthesia (GA) is the most commonly used anesthetic technique for spinal surgery. This study aimed to compare spinal anesthesia (SA) and GA in patients undergoing spinal surgery, in terms of perioperative outcome and cost effectiveness. Materials and methods The study included 80 patients with ASA (American Society of Anesthesiologists) physical status I–II. The patients were randomized to receive SA (n=40) or GA (n=40). Heart rate (HR), mean arterial blood pressure (MABP), blood loss, duration of surgery, duration of anesthesia, surgeon satisfaction, and duration in the post-anesthesia care unit (PACU) were recorded. Postoperative analgesic requirement, nausea and vomiting (PONV), perioperative hemodynamic variables, and anesthetic costs were determined. Results HR and MABP were significantly higher in the GA group than in the SA group at the end of surgery and at PACU admission. Duration of anesthesia, surgeon satisfaction, postoperative analgesic requirement, and anesthetic costs were significantly higher in the GA group. Mean blood loss was lower in the SA group than in the GA group, but the difference was not significant. Duration of surgery, duration in the PACU, perioperative hemodynamic variables, and complications were similar in both groups. Conclusions SA could be considered a reliable alternative to GA in patients undergoing lumber spine surgery, as it is clinically as effective as GA, but more cost effective

    The effects of volatile induction and maintenance of anesthesia and selective spinal anesthesia on QT interval, QT dispersion, and arrhythmia incidence

    OBJECTIVE: The effects of sevoflurane general anesthesia and bupivacaine selective spinal anesthesia on QT dispersion (QTd) and corrected QT (QTc) interval were investigated. METHODS AND MATERIALS: This prospective, randomized, double-blind study was conducted between July and September 2009 in the Urology and General Surgery operating rooms. Forty ASA I-II patients undergoing noncardiac surgery were randomized into two groups: Group R (n=20) and Group V (n=20). In Group R, 5 mg bupivacaine was administered into the spinal space. Anesthesia induction in Group V was established with sevoflurane + 0.1 mg/kg vecuronium using the maximum vital capacity technique. Anesthesia was maintained with 2-3% sevoflurane + 50% N2O/O2 inhalation. All patients were tested with a 24-hour Holter ECG device. QT, QTc, and QTd intervals were measured using 12-lead ECG records at 1 and 3 minutes during preinduction, postinduction, postincision and postextubation periods. Mean arterial pressure (MAP), heart rate and ECG records were measured simultaneously. RESULTS: None of the patients displayed arrhythmia. There was no significant difference between the groups with regard to QTd values (p>0.05). However, QTc was longer in Group V than in Group R after the induction of anesthesia at 3 minutes, after the intubation at 1 and 3 minutes, and after the incision at 1 and 3 minutes.     Intestinal Perforations in Behçet’s Disease

    Behçet’s disease accompanied by intestinal involvement is called intestinal Behçet’s disease. The intestinal ulcers of Behçet’s disease are usually multiple and scattered and tend to perforate easily, so that many patients require emergency operation. The aim of this study is to determine the extent of surgical resection necessary to prevent reperforation and to point out the findings of concurrent oral and genital ulcers and multiple intestinal perforations in all patients of our series. During a 25-year study period, information of 125 Behçet’s disease cases was gathered. Among the 82 patients who were diagnosed with intestinal Behçet’s disease, 22 cases had intestinal perforations needing emergency laparotomy. We investigated and analyzed these cases according to the patients’ demographic characteristics, clinical presentations, laboratory data, and surgical outcome. There were 14 men and 8 women ranging from 22 to 65 years of age. Nine cases were diagnosed preoperatively, and the diagnoses were confirmed in all 22 cases during the surgical intervention. Surgical resection was performed in every patient, with right hemicolectomy and ileocecal resection in 11 cases, partial ileum resection in 8 cases with two reperforations, and ileocecal resection in 3 cases with one reperforation

    Juvenile ankylosing spondylitis

    Fifteen patients with juvenile ankylosing spondylitis were reviewed. Eight presented with extraspinal arthritis. Certain clinical features were of value early in the disease in distinguishing these patients from those with juvenile rheumatoid arthritis: male sex, remittent oligoarthritis, late onset in childhood, relative sparing of hands and wrists, infrequent cervical spine involvement, and negative tests for rheumatoid factors and antinuclear antibodies.
