Analyse des délais de prise en charge des cancers thoraciques : étude prospective
RésuméIntroductionLe cancer broncho-pulmonaire est la première cause de décès par cancer en France. Son diagnostic est le plus souvent tardif, alors que le délai entre le début des symptômes et la prise en charge est considéré comme un facteur aggravant.Matériel et méthodesNotre étude prospective a recueilli les différentes dates de prise en charge de 139 patients consécutifs bénéficiant d’un traitement primaire pour un cancer thoracique dans notre hôpital entre novembre 2008 et mai 2009. L’objectif de cette étude était d’évaluer différents délais de prise en charge des patients porteurs d’un cancer thoracique quelle que soit sa prise en charge thérapeutique (médicale ou chirurgicale) et de déterminer la cause de ces délais.RésultatsLe délai médian entre la première imagerie pathologique et le traitement est de 9,6 semaines. Les délais étaient significativement plus courts dans les stades tardifs et les carcinomes à petites cellules (p=0,001). Il existait une tendance à des délais plus courts pour les femmes et des délais plus longs pour les classes d’âge les plus élevées.ConclusionL’évaluation des délais de prise en charge, en particulier pour les stades précoces, s’intègre dans le contrôle de la qualité de prise en charge de ces pathologies.SummaryIntroductionLung cancer is the main cause of cancer death in France. The diagnosis is often late and the delay between the onset of symptoms and management is considered an aggravating factor.Material and methodsOur prospective study collected the dates of the start of management of 139 consecutive patients receiving first line treatment for thoracic cancer in our hospital between November 2008 and May 2009. The aim of this study was to evaluate the delays in medical or surgical treatments in patients with thoracic cancer and to determine the cause of these delays.ResultsThe median delay between the first abnormal chest X-ray and treatment was 9.6 weeks. The delays were significantly shorter in the late stages and in small cell cancer (P=0.001). There was a tendency for shorter delays in women and for longer delays in older patients.ConclusionEvaluation of the delays in treatment, particularly in the early stages, is part of the quality control of management of these diseases
Retarded Learning: Rigorous Results from Statistical Mechanics
We study learning of probability distributions characterized by an unknown
symmetry direction. Based on an entropic performance measure and the
variational method of statistical mechanics we develop exact upper and lower
bounds on the scaled critical number of examples below which learning of the
direction is impossible. The asymptotic tightness of the bounds suggests an
asymptotically optimal method for learning nonsmooth distributions.Comment: 8 pages, 1 figur
Inner structure of the Puy de Dˆome volcano: cross-comparison of geophysical models (ERT, gravimetry, muon imaging)
International audienceMuon imaging of volcanoes and of geological structures in general is actively being developed by several groups in the world. It has the potential to provide 3-D density distributions with an accuracy of a few percent. At this stage of development, comparisons with established geophysical methods are useful to validate the method. An experiment has been carried out in 2011 and 2012 on a large trachytic dome, the Puy de Dˆome volcano, to perform such a comparison of muon imaging with gravimetric tomography and 2-D electrical resistivity tomography. Here, we present the preliminary results for the last two methods. North-south and east-west resistivity profiles allow us to model the resistivity distribution down to the base of the dome. The modelling of the Bouguer anomaly provides models for the density distribution within the dome that are directly comparable with the results from the muon imaging. Our ultimate goal is to derive a model of the dome using the joint interpretation of all sets of data
Air shower simulation for background estimation in muon tomography of volcanoes
International audienceOne of the main sources of background for the radiography of volcanoes using atmospheric muons comes from the accidental coincidences produced in the muon telescopes by charged particles belonging to the air shower generated by the primary cosmic ray. In order to quantify this background effect, Monte Carlo simulations of the showers and of the detector are developed by the TOMUVOL collaboration. As a first step, the atmospheric showers were simulated and investigated using two Monte Carlo packages, CORSIKA and GEANT4.We compared the results provided by the two programs for the muonic component of vertical proton-induced showers at three energies: 1, 10 and 100 TeV. We found that the spatial distribution and energy spectrum of the muons were in good agreement for the two codes
Tri-critical behavior in rupture induced by disorder
We discover a qualitatively new behavior for systems where the load transfer
has limiting stress amplification as in real fiber composites. We find that the
disorder is a relevant field leading to tri--criticality, separating a
first-order regime where rupture occurs without significant precursors from a
second-order regime where the macroscopic elastic coefficient exhibit power law
behavior. Our results are based on analytical analysis of fiber bundle models
and numerical simulations of a two-dimensional tensorial spring-block system in

On the thermoelectricity of correlated electrons in the zero-temperature limit
The Seebeck coefficient of a metal is expected to display a linear
temperature-dependence in the zero-temperature limit. To attain this regime, it
is often necessary to cool the system well below 1K. We put under scrutiny the
magnitude of this term in different families of strongly-interacting electronic
systems. For a wide range of compounds (including heavy-fermion, organic and
various oxide families) a remarkable correlation between this term and the
electronic specific heat is found. We argue that a dimensionless ratio relating
these two signatures of mass renormalisation contains interesting information
about the ground state of each system. The absolute value of this ratio remains
close to unity in a wide range of strongly-correlated electron systems.Comment: 15 pages, including two figure
Quantum decision making by social agents
The influence of additional information on the decision making of agents, who
are interacting members of a society, is analyzed within the mathematical
framework based on the use of quantum probabilities. The introduction of social
interactions, which influence the decisions of individual agents, leads to a
generalization of the quantum decision theory developed earlier by the authors
for separate individuals. The generalized approach is free of the standard
paradoxes of classical decision theory. This approach also explains the
error-attenuation effects observed for the paradoxes occurring when decision
makers, who are members of a society, consult with each other, increasing in
this way the available mutual information. A precise correspondence between
quantum decision theory and classical utility theory is formulated via the
introduction of an intermediate probabilistic version of utility theory of a
novel form, which obeys the requirement that zero-utility prospects should have

Demonstration of Universal Parametric Entangling Gates on a Multi-Qubit Lattice
We show that parametric coupling techniques can be used to generate selective
entangling interactions for multi-qubit processors. By inducing coherent
population exchange between adjacent qubits under frequency modulation, we
implement a universal gateset for a linear array of four superconducting
qubits. An average process fidelity of is estimated for
three two-qubit gates via quantum process tomography. We establish the
suitability of these techniques for computation by preparing a four-qubit
maximally entangled state and comparing the estimated state fidelity against
the expected performance of the individual entangling gates. In addition, we
prepare an eight-qubit register in all possible bitstring permutations and
monitor the fidelity of a two-qubit gate across one pair of these qubits.
Across all such permutations, an average fidelity of
is observed. These results thus offer a path to a scalable architecture with
high selectivity and low crosstalk
Acoustic radiation controls friction: Evidence from a spring-block experiment
Brittle failures of materials and earthquakes generate acoustic/seismic waves
which lead to radiation damping feedbacks that should be introduced in the
dynamical equations of crack motion. We present direct experimental evidence of
the importance of this feedback on the acoustic noise spectrum of
well-controlled spring-block sliding experiments performed on a variety of
smooth surfaces. The full noise spectrum is quantitatively explained by a
simple noisy harmonic oscillator equation with a radiation damping force
proportional to the derivative of the acceleration, added to a standard viscous

Cracking in asphalt materials
This chapter provides a comprehensive review of both laboratory characterization and modelling of bulk material fracture in asphalt mixtures. For the purpose of organization, this chapter is divided into a section on laboratory tests and a section on models. The laboratory characterization section is further subdivided on the basis of predominant loading conditions (monotonic vs. cyclic). The section on constitutive models is subdivided into two sections, the first one containing fracture mechanics based models for crack initiation and propagation that do not include material degradation due to cyclic loading conditions. The second section discusses phenomenological models that have been developed for crack growth through the use of dissipated energy and damage accumulation concepts.
