    Moodivate: A self-help behavioral activation mobile app for utilization in primary care—Development and clinical considerations

    Depressive symptoms are highly prevalent and are associated with considerable functional impairment, significant public health costs, and heightened mortality risk. Individuals experiencing impairment due to depressive symptomatology are most likely to report their symptoms to a primary care provider. As such, national guidelines highlight the need to assess and effectively treat depression via primary care. Despite these guidelines, the dissemination of evidence-based psychotherapy via primary care is limited, likely due to both provider- and patient-level treatment barriers. Mobile health (mHealth) technologies are promising for addressing these barriers and for promoting uptake of evidence-based depression treatment. Among evidence-based psychotherapies for depression, brief Behavioral Activation Treatment for Depression (BATD) has shown great promise and is particularly amenable to mHealth delivery. Herein, we discuss the development of a BATD mobile application, Moodivate, that was developed in order to disseminate BATD via primary care. This paper focuses on description of (1) rationale for Moodivate treatment development, (2) Moodivate treatment components, (3) ongoing clinical trial evaluation of Moodivate, and (4) clinical considerations for incorporating Moodivate into clinical practice

    Meditation smartphone application effects on prehypertensive adults' blood pressure: Dose-response feasibility trial

    OBJECTIVE: Essential hypertension (EH) is the most common chronic disease in the United States and a major cause of morbidity and mortality. Lifestyle interventions (e.g., diet, exercise, stress management) to reduce blood pressure (BP) are often complex with varying effectiveness. Breathing awareness meditation (BAM) is a stress management strategy with encouraging effects on BP, though widespread dissemination is hampered by the lack of an easy-to-use methodology to train and monitor BAM practices. A smartphone application (Tension Tamer [TT]) that implements BAM and tracks adherence has shown promise in addressing these gaps. This 6-month dose-response feasibility trial evaluated effects of the app on BP to further optimize BAM user guidelines. METHODS: Sixty-four adults with prehypertension were randomized to complete TT-guided BAM sessions for 5-, 10-, or 15-min intervals twice daily over 6 months. Continuous heart rate readings derived from the phone's video camera via reflective photoplethysmography were used as feedback and as an index of time-stamped adherence. Outcomes (resting BP, HR) were collected at baseline, 1-, 3-, and 6-months. RESULTS: Mixed modeling results showed a significant time effect for systolic BP (SBP) with a dose-response effect at Months 3 and 6. Adherence declined over time and was lowest in the 15-min dose condition, though SBP reductions were maintained. Generally, adherence was negatively associated with dose as the study progressed.     Interventions to reduce gender-based violence among young people living with or affected by HIV/AIDS in low-income and middle-income countries

    Objective(s): This study explored the effectiveness of gender-based violence (GBV) interventions on young people living with or affected by HIV in low- and middle-income countries (LMICs). Design: Systematic review and meta-analysis. Methods: We pre-registered a protocol, then searched thirteen databases and grey literature. We screened randomised and quasi-experimental studies (n = 2199) of young people (aged 10–24) living with or affected by HIV in LMICs. Outcomes were GBV and/or GBV-related attitudes. We appraised the data for risk of bias and quality of evidence. Narrative syntheses and multi-level random effects meta-analyses were conducted. Results: We included 18 studies evaluating 21 interventions. Intervention arms were categorised as: a) sexual health and social empowerment (SHSE) (n = 7); b) SHSE plus economic strengthening (n = 4); c) self-defence (n = 3); d) safer schools (n = 2); e) economic strengthening only (n = 2); f) GBV sensitisation (n = 2) and g) safer schools plus parenting (n = 1). Risk of bias was moderate/high and quality of evidence low. Narrative syntheses indicated promising effects on GBV exposure, but no or mixed effects on GBV perpetration and attitudes for self-defence and GBV sensitisation interventions. Safer schools interventions showed no effects. For SHSE interventions and SHSE plus economic strengthening, meta-analysis showed a small reduction in GBV exposure but not perpetration. Economic-only interventions had no overall effect. Conclusions: SHSE, SHSE plus and self-defence and gender sensitisation interventions may be effective for GBV exposure and GBV-related attitudes but not for GBV perpetration. However, the quality of evidence is poor. Future intervention research must include both boys and girls, adolescents living with HIV and key populations.</p

    Enacting Hope through Narratives of Indigenous Language and Culture Reclamation

    In globalizing landscapes, Indigenous ways of knowing and being persist in their connectedness to specific geographies, even as they are transformed by migrations, both forced and voluntary, and dynamic exchanges. This paper presents narratives of Indigenous and ally scholars which explore what it means to enact language and culture reclamation from a place of hope—by Indigenous peoples, for Indigenous communities—and in connection with distinct historical, political, and geographic sites. By naming the identities the authors represent—Chickasaw, Nez Perce, Eastern Shoshone/Northern Arapaho, Hopi, San Carlos Apache and Euro-American—we use a framework of hope to counter damaging assumptions of homogeneity of Indigenous communities while also searching for common themes to advance an agenda of decolonization across positionalities. Understanding that Indigenous sovereignties are built on “contingency with the beliefs, and understandings of the past” (Grande 250), we interrupt settler-colonial narratives which portray Indigenous languages and cultures as deficient and vanishing. Further, through narratives, we explore how disciplines such as linguistics, anthropology, education, and cultural studies can be interwoven to highlight experiences of identity reconciliation, spirituality through language revitalization, and storytelling as narrative reclamation. A critical culturally sustaining/revitalizing pedagogy unifies the narratives and provides a framework for attending to “asymmetrical power relations and legacies of colonization” (McCarty and Lee 8). In this way, Indigenous narratives of persistence and optimism find relevance in the global and local here and now while emphasizing the relevancy of hope as a rooted practice of relationality in Indigenous language and cultural education. Sharing narratives of hope acknowledges the experience of colonization, while privileging the hope in Indigenous knowledge as a return to the community and generator of new narratives.Ye

    High infestation of invasive Aedes mosquitoes in used tires along the local transport network of Panama

    Background: The long‑distance dispersal of the invasive disease vectors Aedes aegypti and Aedes albopictus has intro‑ duced arthropod‑borne viruses into new geographical regions, causing a significant medical and economic burden. The used‑tire industry is an effective means of Aedes dispersal, yet studies to determine Aedes occurrence and the factors influencing their distribution along local transport networks are lacking. To assess infestation along the primary transport network of Panama we documented all existing garages that trade used tires on the highway and surveyed a subset for Ae. aegypti and Ae. albopictus. We also assess the ability of a mass spectrometry approach to classify mos‑ quito eggs by comparing our findings to those based on traditional larval surveillance. Results: Both Aedes species had a high infestation rate in garages trading used tires along the highways, providing a conduit for rapid dispersal across Panama. However, generalized linear models revealed that the presence of Ae. aegypti is associated with an increase in road density by a log‑odds of 0.44 (0.73 ± 0.16; P = 0.002), while the presence of Ae. albopictus is associated with a decrease in road density by a log‑odds of 0.36 (0.09 ± 0.63; P = 0.008). Identifica‑ tion of mosquito eggs by mass spectrometry depicted similar occurrence patterns for both Aedes species as that obtained with traditional rearing methods. Conclusions: Garages trading used tires along highways should be targeted for the surveillance and control of Aedes‑mosquitoes and the diseases they transmit.     Planning for Sustainability in Small Municipalities: The Influence of Interest Groups, Growth Patterns, and Institutional Characteristics

    How and why small municipalities promote sustainability through planning efforts is poorly understood. We analyzed ordinances in 451 Maine municipalities and tested theories of policy adoption using regression analysis.We found that smaller communities do adopt programs that contribute to sustainability relevant to their scale and context. In line with the political market theory, we found that municipalities with strong environmental interests, higher growth, and more formal governments were more likely to adopt these policies. Consideration of context and capacity in planning for sustainability will help planners better identify and benefit from collaboration, training, and outreach opportunities

    Dangler for Accelerated Dehydration: A Novel System for Assessing the Impacts of Relative Humidity on Fruit Water Loss During Cold Storage of Blueberries

    Blueberries are prone to dehydration during storage. Firmness is one of the most critical quality attributes associated with this period, with the loss of water from the fruit representing the most significant limitation for the fresh market. Therefore, one of the great challenges is maintaining the quality characteristics of the fruit in shipments by sea, which can take up to 60 days when sent from the southern hemisphere to the northern hemisphere. The random arrangement of each fruit within a packaging unit (different proportions of the stem scar and cuticular surface exposed to the environment) represents an essential source of variation in the prediction of softening during the storage period. A special device, referred to as a dangler for accelerated dehydration (DAD), was designed to expose nearly the entire fruit surface to the environment and determine the impact of factors such as relative humidity and the role of the stem scar and cuticle on fruit water loss. Consequently, to evaluate the ability of DADs to find differences in fruit dehydration, blueberries sampled at early, peak, and late harvest dates were placed in DADs and exposed to three controlled levels of relative humidity (30%, 65%, and 96% relative humidity; 1.2 ± 0.7 °C) for 10 days. Berries within the DADs were untreated, immersed in hexane for 5 seconds to remove bloom, painted with quick-drying nail polish on the pedicel end to seal the stem scar or immersed in hexane for 5 seconds, and painted with quick-drying nail polish on the pedicel end. At each harvest, fruit weight loss was significantly affected by the fruit and RH treatments, as well as the interaction between them. A regression analysis of the control treatment indicated that water loss at lower relative humidities occurred faster in fruit from the first harvest. The results reveal that DADs can be used to characterize preharvest and postharvest stimuli at an individual level and within a short time (10 days)

    Impaired LXRa phosphorylation attenuates progression of fatty liver disease

    Non-alcoholic fatty liver disease (NAFLD) is a very common indication for liver transplantation. How fat-rich diets promote progression from fatty liver to more damaging inflammatory and fibrotic stages is poorly understood. Here, we show that disrupting phosphorylation at Ser196 (S196A) in the liver X receptor alpha (LXRα, NR1H3) retards NAFLD progression in mice on a high-fat-high-cholesterol diet. Mechanistically, this is explained by key histone acetylation (H3K27) and transcriptional changes in pro-fibrotic and pro-inflammatory genes. Furthermore, S196A-LXRα expression reveals the regulation of novel diet-specific LXRα-responsive genes, including the induction of Ces1f, implicated in the breakdown of hepatic lipids. This involves induced H3K27 acetylation and altered LXR and TBLR1 cofactor occupancy at the Ces1f gene in S196A fatty livers. Overall, impaired Ser196-LXRα phosphorylation acts as a novel nutritional molecular sensor that profoundly alters the hepatic H3K27 acetylome and transcriptome during NAFLD progression placing LXRα phosphorylation as an alternative anti-inflammatory or anti-fibrotic therapeutic target
