    Mitral Valve Myxomas: an Unusual Entity

    Primary tumours of the heart are uncommon entities, cardiac myxomas being the most frequent. However, mitral valve myxomas are exceptionally rare. In the last 12 years, there have been 25 myxomas diagnosed at our institution, with only two of them originating from the mitral valve. Both patients were female, the first, 25, and the second, 72 years old. The younger patient was very symptomatic with a large mass, 4 cm long, which involved both leaflets causing significant obstruction to the left ventricular inflow. The second one had a smaller mass located at the atrial side of the posterior leaflet that only produced some flow divergence. Neither of them had constitutional nor embolic symptoms. Both patients were submitted to emergent surgical resection that in the first case involved the mitral valve and replacement with mechanical prosthesis. The macroscopic appearance of these tumours suggested a malignant aetiology which may represent somewhat different features of the myxomas when originating from the cardiac valves. Both patients are well reflecting the good prognosis of this illness after resection, although the younger patient was re-operated because of prosthetic valve obstruction and suspicion of recurrence that was not confirmed.     Area Rastreio Neonatal de Hemoglobinopatias: A Experiência de um Hospital de Nível II na Área Metropolitana de Lisboa

    Introdução: As hemoglobinopatias são doenças genéticas, causadas por alterações da produção de hemoglobina, das quais se destacam, pela gravidade e prevalência, a doença falciforme e as síndromes talassémicas. O diagnóstico precoce das hemoglo - binopatias é essencial, permitindo a implementação de medidas que reduzem a morbilidade e a mortalidade nestes doentes. Este estudo caracteriza uma amostra de recém-nascidos incluídos num programa de rastreio de hemoglobinopatias no ano de 2013, num hospital de nível II na área metropolitana de Lisboa. Métodos: Estudo retrospetivo descritivo baseado em dados colhidos nos registos clínicos e laboratoriais hospitalares de crian - ças nascidas entre 1 de janeiro e 31 de dezembro de 2013 com origem familiar em áreas de risco indicadas pela Direção Geral da Saúde ou com história familiar de hemoglobinopatia. Aos recém-nascidos incluídos foi realizado hemograma e eletroforese de hemoglobinas entre o primeiro e o terceiro dia de vida. Resultados: Realizados 739 rastreios (27,7% das crianças nascidas neste período). Foram detetadas 86 (12%) variantes do padrão normal de hemoglobinas, incluindo três casos de doença falciforme (dois casos de hemoglobinopatia SS e um caso de hemoglobinopatia SC) e 83 casos de portadores de hemoglobinopatias. Todos os recém-nascidos com variantes de hemoglo - bina foram observados em consulta de hematologia pediátrica.     FISH in analysis of gamma ray-induced micronuclei formation in barley

    A micronucleus test in combination with fluorescent in situ hybridization (FISH) using telomere-, centromere-specific probes and 5S and 25S rDNA was used for a detailed analysis of the effects of gamma ray irradiation on the root tip meristem cells of barley, Hordeum vulgare (2n = 14). FISH with four DNA probes was used to examine the involvement of specific chromosomes or chromosome fragments in gamma ray-induced micronuclei formation and then to explain their origin. Additionally, a comparison of the possible origin of the micronuclei induced by physical and chemical treatment: maleic hydrazide (MH) and N-nitroso-N-methylurea (MNU) was done. The micronuclei induced by gamma ray could originate from acentric fragments after chromosome breakage or from whole lagging chromosomes as a result of a dysfunction of the mitotic apparatus. No micronuclei containing only centromeric signals were found. An application of rDNA as probes allowed it to be stated that 5S rDNA–bearing chromosomes are involved in micronuclei formation more often than NOR chromosomes. This work allowed the origin of physically- and chemically-induced micronuclei in barley cells to be compared: the origin of micronuclei was most often from terminal fragments. FISH confirmed its usefulness in the characterization of micronuclei content, as well as in understanding and comparing the mechanisms of the actions of mutagens applied in plant genotoxicity

    Monovarietal extra-virgin olive oil classification: a fusion of human sensory attributes and an electronic tongue

    Olive oil quality grading is traditionally assessed by human sensory evaluation of positive and negative attributes (olfactory, gustatory, and final olfactorygustatory sensations). However, it is not guaranteed that trained panelist can correctly classify monovarietal extra-virgin olive oils according to olive cultivar. In this work, the potential application of human (sensory panelists) and artificial (electronic tongue) sensory evaluation of olive oils was studied aiming to discriminate eight single-cultivar extra-virgin olive oils. Linear discriminant, partial least square discriminant, and sparse partial least square discriminant analyses were evaluated. The best predictive classification was obtained using linear discriminant analysis with simulated annealing selection algorithm. A low-level data fusion approach (18 electronic tongue signals and nine sensory attributes) enabled 100 % leave-one-out cross-validation correct classification, improving the discrimination capability of the individual use of sensor profiles or sensory attributes (70 and 57 % leave-one-out correct classifications, respectively).     The Effectiveness of a Home Care Program for Supporting Caregivers of Persons with Dementia in Developing Countries: A Randomised Controlled Trial from Goa, India

    OBJECTIVES: To develop and evaluate the effectiveness of a home based intervention in reducing caregiver burden, promoting caregiver mental health and reducing behavioural problems in elderly persons with dementia. METHODOLOGY AND PRINCIPAL FINDINGS: This was a randomised controlled trial in which the person with dementia-caregiver dyad was randomly allocated either to receive the intervention immediately or to a waiting list group which received the intervention after 6 months. It was carried out in communities based in two talukas (administrative blocks) in Goa, India. Mild to moderate cases with dementia (diagnosed using the DSM IV criteria and graded using the Clinical Dementia Rating scale) and their caregivers were included in the trial. Community based intervention provided by a team consisting of Home Care Advisors who were supervised by a counselor and a psychiatrist, focusing on supporting the caregiver through information on dementia, guidance on behaviour management, a single psychiatric assessment and psychotropic medication if needed. We measured caregiver mental health (General Health Questionnaire), caregiver burden (Zarit Burden Score), distress due to behavioural disturbances (NPI-D), behavioural problems in the subject (NPI-S) and activities of daily living in the elder with dementia (EASI). Outcome evaluations were masked to the allocation status. We analysed each outcome with a mixed effects model. 81 families enrolled in the trial; 41 were randomly allocated to the intervention. 59 completed the trial and 18 died during the trial. The intervention led to a significant reduction of GHQ (-1.12, 95% CI -2.07 to -0.17) and NPI-D scores (-1.96, 95%CI -3.51 to -0.41) and non-significant reductions in the ZBS, EASI and NPI-S scores. We also observed a non-significant reduction in the total number of deaths in people with dementia in the intervention arm (OR 0.34, 95% CI 0.01 to 1.03). CONCLUSION: Home based support for caregivers of persons with dementia, which emphasizes the use of locally available, low-cost human resources, is feasible, acceptable and leads to significant improvements in caregiver mental health and burden of caring.     Ginseng and ginkgo biloba effects on cognition as modulated by cardiovascular reactivity: a randomised trial

    Background There is some evidence to suggest that ginseng and Ginkgo biloba can improve cognitive performance, however, very little is known about the mechanisms associated with such improvement. Here, we tested whether cardiovascular reactivity to a task is associated with cognitive improvement. Methodology/Principal findings Using a double-blind, placebo controlled, crossover design, participants (N = 24) received two doses of Panax Ginseng (500, 1000 mg) or Ginkgo Biloba (120, 240 mg) (N = 24), and underwent a series of cognitive tests while systolic, diastolic, and heart rate readings were taken. Ginkgo Biloba improved aspects of executive functioning (Stroop and Berg tasks) in females but not in males. Ginseng had no effect on cognition. Ginkgo biloba in females reversed the initial (i.e. placebo) increase in cardiovascular reactivity (systolic and diastolic readings increased compared to baseline) to cognitive tasks. This effect (reversal) was most notable after those tasks (Stroop and Iowa) that elicited the greatest cardiovascular reactivity during placebo. In males, although ginkgo also decreased cardiovascular readings, it did so from an initial (placebo) blunted response (i.e. decrease or no change from baseline) to cognitive tasks. Ginseng, on the contrary, increased cardiovascular readings compared to placebo. Conclusions/Significance These results suggest that cardiovascular reactivity may be a mechanism by which ginkgo but not ginseng, in females is associated with certain forms of cognitive improvement

    Performance of Anaerobic Co‑digestion of Pig Slurry with Pineapple (Ananas comosus) Bio‑waste Residues

    Agro-food industries produce large amounts of bio-waste, challenging innovative valorisation strategies in the framework of circular economy principles. Anaerobic digestion technology is an interesting route to stabilise organic matter and produce biogas as a renewable energy source. This paper aimed to study the optimal performance conditions for anaerobic co-digestion (AcoD) of pig slurry with pineapple (Ananas comosus) peel bio-waste. The anaerobic digestion (AD) trials were performed at lab scale, in a continuous stirred reactor, for 16 days’ hydraulic retention time in mesophilic conditions (37 ± 1 °C). Three hydraulic retention time were performed, one for the reference scenario ( T0) and two for AcoD trials ( T1, T2). Feeding mixtures (20:80; v:v) of pineapple peel liquor and pig slurry, with an OLR of 1.46 ± 0.04 g TVS L− 1 reactor day− 1 were used during AD/AcoD trials, presenting high values for soluble chemical oxygen demand and C/N ratio. This operational conditions highlight bioenergy recovery up to 0.58 L CH4 g TVSadded −1, in comparison with that obtained with pig slurry substrate (0.31 L CH4 g VSadded −1). The AD performance showed a total volatile solids and chemical oxygen demand removal efficiency of 23% to 47% and 26% to 48%, comparing T0 with the average of T1 and T2, respectively. The digester stability, evaluated by specific energetic loading rate, was below the limit (0.4 day−1) throughout the trials. Pig slurry co-digestion with pineapple peel liquor seems to be a promising approach for potential bioenergy recovery.info:eu-repo/semantics/publishedVersio
