    Project implementation have always refers to the construction assumption and forecast while time plans and schedule were made, Therefore, the contruction problem will arise if there is a discrepancy between plan and assumptions comparing to the actual project. Delays of project implementation is generally always lead to adverse consequences both for the owner and contractor. The project the (SKPD) Unit City Government of Pekanbaru in Riau is having a delay of 75 days. The Best construction time acceleration is 45 days. Construction acceleration can be conducted by 3 alternatives solutions such as: overtime alternative, equipment and workers addition alternative, and shift system alternative. The analysis started from the critical path. Network project the analysis, was obtained from the cost due to delay stands at Rp. 4.761.786.975,00. Cost of construction acceleration caused by overtime alternative is Rp. 2.152.360.596,00 with a total 11.136 hours of extra work for workers, cost of construction acceleration by the workers and equipments additionionally for alternative result Rp. 2.434.965.492,00 with a total 14 new workers, with additional of 2 new equipment units. Cost of construction acceleration from shift alternative is Rp. 2.434.965.492,00 with a total of 14 new workers, a total 2 of equipment units. In conclusion, the most effective construction acceleration method is overtime alternative, obtained the minimum financial loss and recommended to use for the construction acceleration

    The overexploitation of groundwater for irrigation purposes is a general problem affecting the management of common resources. The objective of this study was to analyze some of the policy options when facing this issue. A choice experiment was performed in order to elicit farmers’ stated preferences for four policy instruments, namely i) implementation of a compulsory measurement system of groundwater extraction; ii) improved monitoring of rural areas in order to limit illegal access to groundwater; iii) reforming the groundwater tax system; iv) increasing the supply of reclaimed wastewater for irrigation. A survey was conducted in the Puglia region on a representative sample of 187 farmers, selected in the main hot-spot irrigated areas. A Random Parameter Logit Model was estimated using two covariates (i.e. size of irrigable farmland and farmers’ awareness of groundwater shortage). Findings highlighted the farmers’ preference for increasing the supply of water for irrigation (i.e. wastewater supply). Among the measures aiming to handle groundwater demand for irrigation, the respondents positively valued the enhancement of rural area monitoring to prevent illegal access, but they rejected a compulsory groundwater metering system. Finally, the methodological approach proved to be an effective tool to improve policy design, making the decisionmaking process more participatory

    Infected pancreatic necrosis: outcomes and clinical predictors of mortality. A post hoc analysis of the MANCTRA-1 international study

    : The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135-15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359-5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138-5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184-5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598-9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090-6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286-5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p < 0.001; 95% CI 1.912-7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138-0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143-0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    The concept of landscape, promoted by the European Landscape Convention (Florence, 2000), brings significant innovations from both a theoretical point of view and operational actions, methods and policies to be taken, in order to implement the Convention itself. These innovations require integration and disclose new intervention areas for different disciplines among which there is also photography. It may represent a useful tool for not only reading and interpreting the landscape but also for thinking and planning it, discussing about places and approaching inhabitants to their day-to-day landscape. We refer to photography as related to the social sciences. However, it maintains its own specificity for helping to interpret both the material and immaterial component of landscape connected to perception

    Influence of the CAP reform on livestock: Outlook for selected European regions by 2020

    Livestock producers have been widely affected by past reforms of the European Common Agricultural Policy (CAP). This paper analyses the post-2013 CAP influences on farmer decision making in terms of how many livestock units to keep on-farm under the 'with' and 'without' CAP scenarios. The analysis is based on a survey of 1,301 specialized livestock farms across nine European member states carried out in 2009. The changes in trend in the number of animals reared take into account the different livestock typologies. The results show that the member states that have most recently gained accession are most sensitive to CAP reform, along with organic farming and livestock systems located in hilly and mountainous areas. All are expected to show a decline in their numbers of livestock, while specialist dairy units are expected to increase
