    Biofortification of selenium in black soldier fly (Hermetia illucens) prepupae reared on seaweed or selenium enriched substrates

    Selenium (Se) is an essential trace element for livestock. The element can be supplemented to feed in inorganic or organic Se forms, where the chemical form in the diet affects the accumulation of Se in animal tissues. Insects are known to be natural bioaccumulators of different nutrients, but no studies have so far looked up on the capacity of insects to be biofortified with Se, as a potential future source for Se in feed. In this study, black soldier fly (BSF, Hermetia illucens) larvae were reared on three different substrates: (1) a control substrate (CTR diet) of plant-ingredients (Gainesville diet); (2) Ascophyllum nodosum diet (AN30%), with 30% substitution of the alfalfa meal with the brown algae; and (3) a Se diet, where the CTR diet was fortified with 0.3 mg/kg of Se. All experiments were carried out under dark condition, at 25 °C with 70% relative humidity for two weeks, and the final BSF prepupae were analysed for Se, metals and minerals, and Se species. The mean total Se content in the prepupae reared on Se dietary group was over five times higher compared to the CTR group, whereas lower Se levels were detected in AN30% dietary group. Se speciation analyses showed that organic selenomethionine (SeMet) was the major Se species present, both in CTR and AN30% group. For the Se group, SeMet was accounting for 54% of the total Se. The levels of SeMet were higher in the Se group compared to the CTR group, indicating that BSF prepupae are able to convert inorganic Se to organic Se.     Modeling vaccination rollouts, SARS-CoV-2 variants and the requirement for non-pharmaceutical interventions in Italy

    Despite progress in clinical care for patients with coronavirus disease 2019 (COVID-19)1, population-wide interventions are still crucial to manage the pandemic, which has been aggravated by the emergence of new, highly transmissible variants. In this study, we combined the SIDARTHE model2, which predicts the spread of SARS-CoV-2 infections, with a new data-based model that projects new cases onto casualties and healthcare system costs. Based on the Italian case study, we outline several scenarios: mass vaccination campaigns with different paces, different transmission rates due to new variants and different enforced countermeasures, including the alternation of opening and closure phases. Our results demonstrate that non-pharmaceutical interventions (NPIs) have a higher effect on the epidemic evolution than vaccination alone, advocating for the need to keep NPIs in place during the first phase of the vaccination campaign. Our model predicts that, from April 2021 to January 2022, in a scenario with no vaccine rollout and weak NPIs (R = 1.27), as many as 298,000 deaths associated with COVID-19 could occur. However, fast vaccination rollouts could reduce mortality to as few as 51,000 deaths. Implementation of restrictive NPIs (R = 0.9) could reduce COVID-19 deaths to 30,000 without vaccinating the population and to 18,000 with a fast rollout of vaccines. We also show that, if intermittent open\u2013close strategies are adopted, implementing a closing phase first could reduce deaths (from 47,000 to 27,000 with slow vaccine rollout) and healthcare system costs, without substantive aggravation of socioeconomic losses

    A phase I, dose-escalation study of volasertib combined with nintedanib in advanced solid tumors

    Background: Volasertib is a potent and selective cell-cycle kinase inhibitor that induces mitotic arrest and apoptosis by targeting Polo-like kinases. This study determined the maximum tolerated dose (MTD) and pharmacokinetics of volasertib combined with nintedanib, a potent and orally bioavailable triple angiokinase inhibitor, in patients with advanced solid tumors. Patients and methods: This open-label, dose-escalation trial recruited patients with advanced metastatic solid tumors following failure of conventional treatment (NCT01022853; Study 1230.7). Volasertib was administered by intravenous infusion over 2 h, starting at 100 mg in the first dose cohort. Nintedanib was administered orally at a dose of 200 mg twice daily. The first treatment cycle comprised 28 days (days 1-7 and days 9-28: nintedanib; day 8: volasertib). From cycle 2 onwards, volasertib was administered on day 1 of a 21-day cycle and nintedanib was administered days 2-21. The primary objective was the MTD of volasertib in combination with nintedanib. Results: Thirty patients were treated. The MTD of volasertib plus fixed-dose nintedanib was 300 mg once every 3 weeks, the same as the recommended single-agent dose of volasertib in solid tumors. Two of 12 assessable patients treated with the MTD experienced dose-limiting toxicities [grade 3 increased alanine aminotransferase (ALT); grade 3 ALT increase and grade 3 increased aspartate aminotransferase]. Disease control [stable disease (SD)/partial response (PR)/complete response (CR)] was achieved in 18 patients (60%): 1 CR (breast cancer), 1 PR (nonsmall-cell lung cancer), and 16 patients with SD. Volasertib showed that multiexponential pharmacokinetic behavior and co-administration of nintedanib had no significant effects on its exposure. Conclusions: Volasertib could be combined with fixed-dose nintedanib at the recommended single-agent dose. At this dose, the combination had a manageable safety profile without unexpected or overlapping adverse events, and showed antitumor activity

    Government policy and healthcare management: proposal of a shared decision-making model

    As a consequence of the current economic crisis many changes have been introduced to health systems on national levels to reduce expenditure and to introduce more cost-effective methods. Then recent developments in medical technology have led to a shift in the traditional framework of physician-patient responsibility in health care decision making. In this context, the health community urges, in any case, decision makers to ensure that all changes introduced are in accordance with Health System Values. This paper presents a new decision making platform, as a tool to facilitate shared decision making, to improve the quality and transparency of tactical and strategic decisions. This new application, based on analytic quantitative method, is a component of a wider distribute web system under development, which aims to inform the new effective health technologies supporting designers in the user need elicitation and early stage health technology assessments. A case study aiming to elicit the user needs affecting the decision of whether to adopt a new magnetic resonance imaging (MRI) is also presented

    Production of α\alpha-particle condensate states in heavy-ion collisions

    The fragmentation of quasi-projectiles from the nuclear reaction 40Ca^{40}Ca + 12C^{12}C at 25 MeV/nucleon was used to produce excited states candidates to α\alpha-particle condensation. The experiment was performed at LNS-Catania using the CHIMERA multidetector. Accepting the emission simultaneity and equality among the α\alpha-particle kinetic energies as experimental criteria for deciding in favor of the condensate nature of an excited state, we analyze the 02+0_2^+ and 22+2_2^+ states of 12^{12}C and the 06+0_6^+ state of 16^{16}O.     Evaluation of the oncogenic risk of diffuse gastric polyposis. A case report

    Benign polyps of the stomach undergo malignant transformation at a rate correlating to the histological type and size of the proliferative lesion. We report a case of a 50-year-old Caucasian woman, affected by a diffuse gastric polyposis of both hyperplastic and adenomatous type. At endoscopy polyps were more than 1,000, scattered over the entire gastric cavity. The patient underwent total gastrectomy. The perilesional gastric mucosa was characterized by the presence of either atrophic or metaplastic areas and by a mild dysplasia. A single tubulo-villous adenomatous polyp was also present in the ascending tract of the colon. The absence of both high-grade dysplastic lesions and outbreaks of neoplastic transformation well correlated with the histochemical and molecular features, confirming the highly proliferative pattern of the polyps in the lack of signs of malignant progression

    Cosmic shear bispectrum from second-order perturbations in General Relativity

    Future lensing surveys will be nearly full-sky and reach an unprecedented depth, probing scales closer and closer to the Hubble radius. This motivates the study of the cosmic shear beyond the small-angle approximation and including general relativistic corrections that are usually suppressed on sub-Hubble scales. The complete expression of the reduced cosmic shear at second order including all relativistic effects was derived in [1]. In the present paper we compute the resulting cosmic shear bispectrum when all these effects are properly taken into account and we compare it to primordial non-Gaussianity of the local type. The new general relativistic effects are generically smaller than the standard non-linear couplings. However, their relative importance increases at small multipoles and for small redshifts of the sources. The dominant effect among these non standard corrections is due to the inhomogeneity of the source redshift. In the squeezed limit, its amplitude can become of the order of the standard couplings when the redshift of the sources is below 0.5. Moreover, while the standard non-linear couplings depend on the angle between the short and long mode, the relativistic corrections do not and overlap almost totally with local type non-Gaussianity.
