    Impact of adding a cognitive task while performing physical fitness tests in women with fibromyalgia. A cross-sectional descriptive study

    Los síntomas de la fibromialgia causan una reducción significativa en la capacidad de realizar las actividades de la vida diaria. Estas actividades a menudo requieren la capacidad de realizar más de una tarea al mismo tiempo. El objetivo era investigar cómo la adición de una tarea cognitiva modifica el rendimiento en las pruebas de aptitud física en los controles de fibromialgia y salud. Un total de 61 mujeres participaron en este estudio, 31 de ellas diagnosticadas con fibromialgia por un reumatólogo. Realizaron 3 pruebas de aptitud física (rizo de brazo, agarre y pruebas de 10 pasos de escalera) en 2 condiciones: A) regular (una sola tarea [ST]) y B) pensando en 3 palabras que se dieron antes de cada prueba y que tuvieron que ser recordadas y verbalizadas después de la ejecución de cada prueba (tarea dual). El coste de la doble tarea se calculó como la diferencia entre las actuaciones en las condiciones normales y las de doble tarea (DT). Los controles sanos obtuvieron resultados significativamente mejores que la fibromialgia en ambas condiciones, de doble tarea y de una sola tarea. Las mujeres con fibromialgia disminuyeron significativamente el rendimiento en la prueba de los 10 pasos cuando se añadió una tarea cognitiva. No se encontraron diferencias entre los grupos en los costos de la doble tarea (DTC). Las mujeres con fibromialgia mostraron un menor rendimiento físico que los controles sanos en ambas condiciones de tareas simples y dobles. Además, se observaron diferencias entre las condiciones de tareas simples y dobles en la prueba de los 10 pasos en las mujeres con fibromialgia. Esto podría estar relacionado con una reducción en la capacidad de realizar actividades de la vida diaria. Sin embargo, los resultados relativos al DTC indican que ambos grupos pueden estar influenciados de manera similar por la adición de un cognitivo secundario. Por lo tanto, es necesario realizar más investigaciones con diferentes niveles de dificultad de las condiciones del DT en la fibromialgia.Fibromyalgia symptoms cause a significant reduction in the ability to perform daily life activities. The aim was to investigate how the addition of a cognitive task modifies the performance in physical fitness tests in fibromialgia and healthy controls. A total of 61 women participated in this study, 31 of them diagnosed with fibromyalgia by a rheumatologist. They performed 3 physical fitness tests (arm curl, handgrip, and 10-steps stair tests) in 2 conditions: A) regular (single task [ST]) and B) while thinking in 3 words that were given before each test and had to be recalled and verbalized after the execution of each test (dual task). The dual task cost was calculated as the difference between the performances in the regular and dual-task (DT) conditions. Healthy controls obtained significantly better results than fibromyalgia in both, dual and single-task conditions. Women with fibromyalgia significantly decreased the performance in the 10-steps stair test when a cognitive task was added. Between-group differences in the dual-task costs (DTC) were not found. Women with fibromyalgia showed lower physical performance than healthy controls in both, single and dual task conditions. In addition, differences between single and dual task conditions were observed in the 10-steps stair test in women with fibromyalgia. This could be related with a reduction in the ability to perform daily life activities. However, results regarding DTC indicate that both groups may be similarly influenced by the addition of a secondary cognitive. Thus, further research with different difficulty levels of DT conditions is needed in fibromyalgia.peerReviewe

    Computational Determination of Air Valves Capacity Using CFD Techniques

    [EN] The analysis of transient flow is necessary to design adequate protection systems that support the oscillations of pressure produced in the operation of motor elements and regulation. Air valves are generally used in pressurized water pipes to manage the air inside them. Under certain circumstances, they can be used as an indirect control mechanism of the hydraulic transient. Unfortunately, one of the major limitations is the reliability of information provided by manufacturers and vendors, which is why experimental trials are usually used to characterize such devices. The realization of these tests is not simple since they require an enormous volume of previously stored air to be used in such experiments. Additionally, the costs are expensive. Consequently, it is necessary to develop models that represent the behaviour of these devices. Although computational fluid dynamics (CFD) techniques cannot completely replace measurements, the amount of experimentation and the overall cost can be reduced significantly. This work approaches the characterization of air valves using CFD techniques, including some experimental tests to calibrate and validate the results. A mesh convergence analysis was made.     Phase stability of lanthanum orthovanadate at high-pressure

    When monoclinic monazite-type LaVO4 (space group P21/n) is squeezed up to 12 GPa at room temperature, a phase transition to another monoclinic phase has been found. The structure of the high-pressure phase of LaVO4 is indexed with the same space group (P21/n), but with a larger unit-cell in which the number of atoms is doubled. The transition leads to an 8% increase in the density of LaVO4. The occurrence of such a transition has been determined by x-ray diffraction, Raman spectroscopy, and ab initio calculations. The combination of the three techniques allows us to also characterize accurately the pressure evolution of unit-cell parameters and the Raman (and IR)-active phonons of the low- and high-pressure phase. In particular, room-temperature equations of state have been determined. The changes driven by pressure in the crystal structure induce sharp modifications in the color of LaVO4 crystals, suggesting that behind the monoclinic-to-monoclinic transition there are important changes of the electronic properties of LaVO4.Comment: 39 pages, 6 tables, 7 figure

    Fibromyalgia and quality of life: mapping the revised fibromyalgia impact questionnaire to the preference-based instruments

    ANTECEDENTES La versión revisada del Cuestionario de Impacto de Fibromialgia (FIQR) es uno de los cuestionarios específicos más utilizados en estudios de FM. Sin embargo, este cuestionario no permite el cálculo de los AVAC, ya que no es una medida basada en preferencias. El objetivo de este estudio fue desarrollar un algoritmo de mapeo que permita que los puntajes de FIQR se transformen en puntajes de utilidad que se pueden usar en los análisis de costo de utilidad. METODOS Se realizó una encuesta transversal. Se pidió a ciento 92 mujeres españolas con fibromialgia que completaran cuatro cuestionarios generales de calidad de vida, es decir, EQ-5D-5 L, 15D, AQoL-8D y SF-12, y un instrumento específico para la enfermedad, el FIQR. Se adoptó un enfoque de mapeo directo para derivar algoritmos de mapeo entre el FIQR y cada uno de los cuatro instrumentos de utilidad de atributos múltiples (MAU). La utilidad del estado de salud se trató como la variable dependiente en el análisis de regresión, mientras que la puntuación de FIQR y la edad fueron factores predictivos. RESULTADOS Las puntuaciones medias de utilidad oscilaron entre 0,47 (AQoL-8D) y 0,69 (15D). Todas las correlaciones entre la puntuación total de FIQR y las puntuaciones de utilidad de los instrumentos de MAU fueron altamente significativas (p <0,0001) con magnitudes mayores que 0,5. Aunque se encontraron diferencias muy leves en el error absoluto promedio entre el estimador de mínimos cuadrados ordinarios (OLS) y el modelo lineal generalizado (GLM), los modelos basados en GLM fueron mejores para EQ-5D-5 L, AQoL-8D y 15D. CONCLUSIÓN Los algoritmos de mapeo desarrollados en este estudio permiten la estimación de valores de utilidad a partir de puntajes en un cuestionario específico de fibromialgia.BACKGROUND The revised version of the Fibromyalgia Impact Questionnaire (FIQR) is one of the most widely used specific questionnaires in FM studies. However, this questionnaire does not allow calculation of QALYs as it is not a preference-based measure. The aim of this study was to develop mapping algorithm which enable FIQR scores to be transformed into utility scores that can be used in the cost utility analyses. METHODS A cross-sectional survey was conducted. One hundred and 92 Spanish women with Fibromyalgia were asked to complete four general quality of life questionnaires, i.e. EQ-5D-5 L, 15D, AQoL-8D and SF-12, and one specific disease instrument, the FIQR. A direct mapping approach was adopted to derive mapping algorithms between the FIQR and each of the four multi-attribute utility (MAU) instruments. Health state utility was treated as the dependent variable in the regression analysis, whilst the FIQR score and age were predictors. RESULTS The mean utility scores ranged from 0.47 (AQoL-8D) to 0.69 (15D). All correlations between the FIQR total score and MAU instruments utility scores were highly significant (p < 0.0001) with magnitudes larger than 0.5. Although very slight differences in the mean absolute error were found between ordinary least squares (OLS) estimator and generalized linear model (GLM), models based on GLM were better for EQ-5D-5 L, AQoL-8D and 15D. CONCLUSION Mapping algorithms developed in this study enable the estimation of utility values from scores in a fibromyalgia specific questionnaire.• Ministerio de Economía y Competitividad.     Supramolecular fullerene sponges as catalytic masks for regioselective functionalization of C60

    Isomer-pure poly-functionalized fullerenes are required to boost the development of fullerene chemistry in all fields. On a general basis, multi-adduct mixtures with uncontrolled regioselectivity are obtained, and the use of chromatographic purification is prohibitively costly and time consuming, especially in the production of solar cells. Single-isomer poly-functionalized fullerenes are only accessible via stoichiometric, multistep paths entailing protecting-unprotecting sequences. Herein, a nanocapsule is used as a supramolecular tetragonal prismatic mask to exert full control on the reactivity and the equatorial regioselectivity of Bingel-Hirsch cyclopropanation reactions of a confined C guest. Thus, equatorial bis-, tris-, and tetrakis-C homo-adducts are exclusively obtained in a stepwise manner. Furthermore, isomer-pure equatorial hetero-tetrakis-adducts or hetero-Th-hexakis-adducts are synthesized at will in one-pot synthesis for the first time. This work provides a synthetically valuable path to produce a plethora of new pure-isomer poly-functionalized C-based compounds as candidates for testing in solar cell devices and biomedical applications

    Translation and cultural adaptation of the composite physical function for its use in Chile

    El envejecimiento está directamente relacionado con la pérdida de la independencia física. El cuestionario compuesto de función física (CPF) evalúa una gama de 12 elementos y de actividades de la vida diaria con el fin de determinar los niveles de dependencia de un ancianos. Sin embargo, no existe una versión en español de este instrumento. Objetivo: traducir y adaptar culturalmente la ACB al español para su uso en ancianos chilenos. Método: Se utilizó una metodología internacional normalizado en este estudio, que consistió en doble traducción directa al español, la armonización de las versiones y traducciones al inglés. La aceptabilidad y la familiaridad de la versión obtenida fue analizada mediante el sondaje y parafraseando a métodos utilizando una muestra de 20 adultos mayores, de edades comprendidas entre los 65 y 80 años. Resultados: Todos los artículos fueron claras y comprensibles, aunque pequeñas adaptaciones necesarias para hacerse con el fin de mejorar la comprensibilidad de dos elementos. Estas adaptaciones consistieron en añadir información en corchetes al final de la frase. Conclusión: La versión en español del cuestionario de la ACB fue obtenida para su uso en Chile. Este cuestionario ha demostrado ser comprensible y adaptable para su uso en los adultos mayores chilenos. Su facilidad de uso hace que este cuestionario sea potencialmente útil en futuras investigaciones y encuestas.Aging is directly related with loss of physical independency. Composite Physical Function questionnaire (CPF) assess, throw 12 items, a range of daily life activities in order to determine dependency levels in elderly. However, there is not a Spanish version of this instrument. Aim: To translate and culturally adapt the CPF to Spanish for its use in Chilean elderly. Method: Standardized international methodology was used in this study, which consisted in double direct translation to Spanish, harmonization of versions and back-translation to English. Acceptability and familiarity of the obtained version was analyzed using probing and paraphrasing methods using a sample of 20 older adults aged from 65 to 80 years old. Results: All items were clear and understandable, although minor adaptations needed to be done in order to improve the understandability of two items. These adaptations consisted in adding information in brackets at the end of the sentence. Conclusion: Spanish version of the CPF questionnaire was obtained to its use in Chile. This questionnaire has been proved to be understandable and adapted to its use in Chilean older adults.     A GIS-supported Multidisciplinary Database for the Management of UNESCO Global Geoparks: the Courel Mountains Geopark (Spain)

    [Abstract] The management of a UNESCO Global Geopark (UGGp) requires a vast wealth of miscellaneous scientific knowledge that can be successfully organised using a Geographical Information System (GIS). This paper presents a pragmatic GIS database to assist in the suitable governance of the Courel Mountains UGGp (2017) in Northwest Spain. The database is structured in 66 coverages compiled from public sources and previous works or produced through traditional mapping (combining fieldwork and photointerpretation) and GIS tools. The acquired data was later homogenised and validated by a multidisciplinary team and archived in independent coverages. Forty thematic maps illustrate the broad range of cartographic information included in the GIS database. Among them, 25 basic maps provide an overview of the UGGp and 15 new maps focus on crosscutting and technical issues. All maps illustrate the huge potential of GIS to create new resources combining coverages and adapting the legend according to their purpose and audience. The database facilitates the suitable publishing of consistent outputs (e.g., brochures, books, panels, webpages, web serves), as well as the elaboration of technical data to assist the park management. The database furnishes information on the design of education actions, touristic routes, activities and Geopark facilities. The GIS database is also a supportive tool for scientific research and provides the necessary knowledge to conduct geoconservation actions based on land use, geological hazards and the occurrence of natural and cultural heritages. Altogether, the GIS database constitutes a powerful instrument for policy-making, facilitating the identification and evaluation of alternative strategy plans.This work was developed in the framework of the Scientific Program of the Courel Mountains UGGp with the cooperation of tourism agents (A.M.     Pressure-induced amorphization of YVO4:Eu3+ nanoboxes

The Version of Record is available online at http://dx.doi.org/10.1088/0957-4484/27/2/025701A structural transformation from the zircon-type structure to an amorphous phase has been found in YVO4:Eu3+ nanoboxes at high pressures above 12.7 GPa by means of x-ray diffraction measurements. However, the pair distribution function of the high-pressure phase shows that the local structure of the amorphous phase is similar to the scheelite-type YVO4. These results are confirmed both by Raman spectroscopy and Eu3+ photoluminescence which detect the phase transition to a scheelite-type structure at 10.1 and 9.1 GPa, respectively. The irreversibility of the phase transition is observed with the three techniques after a maximum pressure in the upstroke of around 20 GPa. The existence of two D-5(0)-> F-7(0) photoluminescence peaks confirms the existence of two local environments for Eu3+, at least for the low-pressure phase.     Skin manifestations in COVID-19: prevalence and relationship with disease severity

    Background: Data on the clinical patterns and histopathology of SARS-CoV-2 related skin lesions, as well as on their relationship with the severity of COVID-19 are limited. Methods and Materials: Retrospective analysis of a prospectively collected cohort of patients with SARS-CoV-2 infection in a teaching hospital in Barcelona, Spain, from 1 April to 1 May 2020. Clinical, microbiological and therapeutic characteristics, clinicopathological patterns of skin lesions, and direct immunofluorescence and immunohistochemical findings in skin biopsies were analyzed. Results: Fifty-eight out of the 2761 patients (2.1%) either consulting to the emergency room or admitted to the hospital for COVID-19 suspicion during the study period presented COVID-19 related skin lesions. Cutaneous lesions could be categorized into six patterns represented by the acronym "GROUCH": Generalized maculo-papular (20.7%), Grover's disease and other papulo-vesicular eruptions (13.8%), livedo Reticularis (6.9%), Other eruptions (22.4%), Urticarial (6.9%), and CHilblain-like (29.3%). Skin biopsies were performed in 72.4%, including direct immunofluorescence in 71.4% and immunohistochemistry in 28.6%. Patients with chilblain-like lesions exhibited a characteristic histology and were significantly younger and presented lower rates of systemic symptoms, radiological lung infiltrates and analytical abnormalities, and hospital and ICU admission compared to the rest of patients. Conclusion: Cutaneous lesions in patients with COVID-19 appear to be relatively rare and varied. Patients with chilblain-like lesions have a characteristic clinicopathological pattern and a less severe presentation of COVID-19

    A three-shell supramolecular complex enables the symmetry-mismatched chemo- and regioselective bis-functionalization of C60

    Molecular Russian dolls (matryoshkas) have proven useful for testing the limits of preparative supramolecular chemistry but applications of these architectures to problems in other fields are elusive. Here we report a three-shell, matryoshka-like complex—in which C60 sits inside a cycloparaphenylene nanohoop, which in turn is encapsulated inside a self-assembled nanocapsule—that can be used to address a long-standing challenge in fullerene chemistry, namely the selective formation of a particular fullerene bis-adduct. Spectroscopic evidence indicates that the ternary complex is sufficiently stable in solution for the two outer shells to affect the addition chemistry of the fullerene guest. When the complex is subjected to Bingel cyclopropanation conditions, the exclusive formation of a single trans-3 fullerene bis-adduct was observed in a reaction that typically yields more than a dozen products.
