    Osteoporosis Epidemiology Among Adults With Cerebral Palsy: Findings From Private and Public Administrative Claims Data

    Individuals with cerebral palsy (CP) have an increased risk for the early development of osteoporosis; however, little is known about the epidemiology of osteoporosis for adults with CP, which is vital to inform clinical practice for osteoporosis prevention, treatment, and management. The purpose of this cross‐sectional study was to determine sex‐stratified prevalence of osteoporosis among adults with CP, as compared with adults without CP. Data from 2016 were extracted from Optum Clinformatics Data Mart (private insurance administrative claims data) and a random 20% sample from the fee‐for‐service Medicare (public insurance administrative claims data). Diagnostic codes were used to identify CP and osteoporosis diagnoses. Sex‐stratified prevalence of osteoporosis was compared between adults with and without CP for the following age groups: 18 to 30, 31 to 40, 41 to 50, 51 to 60, 61 to 70, and >70 years of age. The overall prevalence of osteoporosis was 4.8% for adults without CP (n = 8.7 million), 8.4% for privately insured adults with CP (n = 7,348), and 14.3% for publicly insured adults with CP (n = 21,907). Women and men with CP had a higher prevalence of osteoporosis compared with women and men without CP for all age groups. Finally, publicly insured women and men with CP had a higher prevalence of osteoporosis compared with privately insured women and men with CP for all age groups, except for the similar prevalence among the 18‐ to 30‐year age group. These findings suggest that osteoporosis is more prevalent among adults with CP compared with adults without CP. Study findings highlight the need for earlier screening and preventive medical services for osteoporosis management among adults with CP. © 2019 The Authors.     Coastal saltmarsh managed realignment drives rapid breach inlet and external creek evolution, Freiston Shore (UK)

    The creation of saltmarsh through the managed realignment of sea defences, implemented in NW Europe as a sustainable coastal defence option, represents a substantial hydrodynamic perturbation to the local coastal system. The impact of a significantly increased tidal prism on hydromorphological features was investigated at Freiston Shore, Lincolnshire UK. Local tidal conditions and inadequate drainage at this realignment trial contributed to significant channel erosion due to the establishment of water surface slopes and pooling between the newly realigned site and the adjacent intertidal zone. Very high spatial resolution aerial photography and blimp photography were used to monitor inlet evolution from breaching in August 2002 to March 2008, showing a highly non-linear response with breach channels increasing in width by up to 960% within 2.5. months. Airborne laser scanning/LiDAR and terrestrial laser scanning quantified breach channel volume increases, showing a similar pattern. Breach channel evolution did not follow established tidal prism-channel width/cross-sectional area relationships that are often used to guide realignment design. Pre- and post-breach rates of external creek morphology change between 1999 and 2006 were also quantified, with intertidal creeks attached to the breach channels increasing significantly after realignment in both width and depth. This study highlights the physical processes affected by managed realignment, and the importance of understanding the causes of complex water surface slopes at multiple scales. © 2013 Elsevier B.V

    OSIRIS-REx Orbit Determination Performance During the Navigation Campaign

    The OSIRIS-REx mission Navigation Campaign consists of three sub-phases: Approach,Preliminary Survey, and Orbital A. Approach was designed for initial characterization ofBennu while matching Bennu's heliocentric velocity. Preliminary Survey provided the firstspacecraft-based estimate of Bennu's mass. This phase consisted of five target flybys witha close approach distance of about 7 km. Orbital A was a two-month phase devoted to theNavigation Team learning the close proximity operations dynamics and environment aroundBennu and transitioning from center-finding optical navigation to landmark feature-basednavigation.     Calculation of the Electron Self Energy for Low Nuclear Charge

    We present a nonperturbative numerical evaluation of the one-photon electron self energy for hydrogenlike ions with low nuclear charge numbers Z=1 to 5. Our calculation for the 1S state has a numerical uncertainty of 0.8 Hz for hydrogen and 13 Hz for singly-ionized helium. Resummation and convergence acceleration techniques that reduce the computer time by about three orders of magnitude were employed in the calculation. The numerical results are compared to results based on known terms in the expansion of the self energy in powers of (Z alpha).Comment: 10 pages, RevTeX, 2 figure

    Factors underlying Ebola virus infection among health workers, Kenema, Sierra Leone, 2014-2015

    Background. Ebola virus disease (EVD) in health workers (HWs) has been a major challenge during the 2014-2015 outbreak. We examined factors associated with Ebola virus exposure and mortality in HWs in Kenema District, Sierra Leone. Methods. We analyzed data from the Sierra Leone National Viral Hemorrhagic Fever Database, contact tracing records, Kenema Government Hospital (KGH) staff and Ebola Treatment Unit (ETU) rosters, and burial logs.Results. From May 2014 through January 2015, 600 cases of EVD originated in Kenema District, including 92 (15%) HWs, 66 (72%) of whom worked at KGH. Among KGH medical staff and international volunteers, 18 of 62 (29%) who worked in the ETU developed EVD, compared with 48 of 83 (58%) who worked elsewhere in the hospital. Thirteen percent of HWs with EVD reported contact with EVD patients, while 27% reported contact with other infected HWs. The number of HW EVD cases at KGH declined roughly 1 month after implementation of a new triage system at KGH and the opening of a second ETU within the district. The case fatality ratio for HWs and non-HWs with EVD was 69% and 74%, respectively.Conclusions. The cluster of HW EVD cases in Kenema District is one of the largest ever reported. Most HWs with EVD had potential virus exposure both inside and outside of hospitals. Prevention measures for HWs must address a spectrum of infection risks in both formal and informal care settings as well as in the community.Publisher PDFPeer reviewe

    The First Hour of Extra-galactic Data of the Sloan Digital Sky Survey Spectroscopic Commissioning: The Coma Cluster

    On 26 May 1999, one of the Sloan Digital Sky Survey (SDSS) fiber-fed spectrographs saw astronomical first light. This was followed by the first spectroscopic commissioning run during the dark period of June 1999. We present here the first hour of extra-galactic spectroscopy taken during these early commissioning stages: an observation of the Coma cluster of galaxies. Our data samples the Southern part of this cluster, out to a radius of 1.5degrees and thus fully covers the NGC 4839 group. We outline in this paper the main characteristics of the SDSS spectroscopic systems and provide redshifts and spectral classifications for 196 Coma galaxies, of which 45 redshifts are new. For the 151 galaxies in common with the literature, we find excellent agreement between our redshift determinations and the published values. As part of our analysis, we have investigated four different spectral classification algorithms: spectral line strengths, a principal component decomposition, a wavelet analysis and the fitting of spectral synthesis models to the data. We find that a significant fraction (25%) of our observed Coma galaxies show signs of recent star-formation activity and that the velocity dispersion of these active galaxies (emission-line and post-starburst galaxies) is 30% larger than the absorption-line galaxies. We also find no active galaxies within the central (projected) 200 h-1 Kpc of the cluster. The spatial distribution of our Coma active galaxies is consistent with that found at higher redshift for the CNOC1 cluster survey. Beyond the core region, the fraction of bright active galaxies appears to rise slowly out to the virial radius and are randomly distributed within the cluster with no apparent correlation with the potential merger of the NGC 4839 group.
