    A rational use of laboratory tests in the diagnosis and management of hepatitis C virus infection

    The prevalence of HCV infection is very diversified according to geographical areas and ranges from 1% in the Northern regions of the world to more than 20% as we move South. Due to the presence of HCVassociated liver diseases and the development of effective treatments, the diagnosis of HCV infection is a growing medical need. Several tests are available, from simple screening to identify the presence of antiHCV antibodies to the more sophisticated quantification of viral load and genotyping. However, these tests are to be used in a logical, consequential and cost-effective manner. This review article will report on the protocol in use in the North-Eastern part of Italy for the screening and diagnosis of HCV infection. The protocol is based on a consensus among several experts and may be the basis for a more rational approach in this rapidly growing field

    Molecular characterization of ‘Candidatus Phytoplasma mali’ strains in outbreaks of apple proliferation in north eastern Italy, Hungary, and Serbia

    During 2005-2008 apple plants of different varieties showing proliferation symptoms were observed in diverse areas of north eastern Italy, Hungary and Serbia. PCR/RFLP analyses showed that all the samples were infected with ‘Candidatus Phytoplasma mali’. In the 16S plus spacer region two phytoplasma profiles (P-I and P-II) were distinguished. P-I profile was detected in reference strains AP, AT1, AT2, in samples from Serbia, and in the majority of samples from Trentino; the P-II profile was prevalent in samples from Veneto; both profiles were identified in samples from Hungary, in some cases together in single samples. The analyses of rpl22-s3 genes allow the identification, in all the samples showing a P-I profile, the presence of phytoplasmas belonging to rpX-A subgroup, while in the samples showing a P-II profile it was possible to distinguish the other three reported rpX subgroups. In the majority of samples from the Veneto region phytoplasmas belonging to rpX-D subgroup were identified, while rpX-B and rpX-C subgroups were identified only in a few samples from Trentino and Veneto regions, respectively. Further RFLP analyses on AP13/AP10 amplicons differentiate among strains belonging to the rpX-A subgroup: the samples from Serbia show AP profiles, while those from Trentino show AT-2 profiles.     Communication with the public in the health-care system: a descriptive study of the use of social media in Local Health Authorities and public hospitals in Italy

    Introduction. In 2010 the Italian Ministry of Health set out recommendations for the use of social technology and Web 2.0, inviting organisations within the Italian national health service (Servizio Sanitario Nazionale, SSN) to equip themselves with instruments. Objectives. 1. to ascertain how many local health authorities (Aziende Sanitarie Locali, ASL) and public hospitals have a presence on the most widely used social media websites in Italy: Facebook, Twitter and YouTube; 2. to find out how well the Facebook, Twitter and YouTube pages of ASLs and public hospitals are known among the general population; 3. to find out how ASLs and public hospitals engage with the general public on social media sites. Materials and methods. The websites of all ASLs and public hospitals across the country were visited to look for the icons of the social media sites under examination. The data considered were publicly available upon access. Results. A total of 245 websites were analysed. 7.34% ASLs and hospitals had social media accounts. 8 organisations had an account on all three of the social media sites considered in the study. Conclusions. The results show a low presence of ASLs and hospitals on social media. Other studies are needed in this field

    Epidemiological and molecular assessment of a measles outbreak in a highly vaccinated population of northeast Italy.

    SUMMARYTwo distinct measles outbreaks, unrelated from the epidemiological point of view but caused by genetically related strains, occurred in the Friuli Venezia Giulia region of northeastern Italy. Forty-two cases were reported during the period April–May 2008. In the first outbreak the index case was a teacher who introduced the virus into the Pordenone area, involving eight adolescents and young adults. The other concomitant outbreak occurred in the city of Trieste with 33 cases. The containment of the epidemics can be explained by the high MMR vaccine coverage in an area where the first dose was delivered to 93·4% and the second dose to 88·3% of the target children. Phylogenetic analysis of 14 measles virus strains showed that they belonged to a unique D4 genotype indistinguishable from the MVs/Enfield.GBR/14.07 strain, probably introduced from areas (i.e. Piedmont and Germany) where this genotype was present or had recently caused a large epidemic

    Tuberculous meningitis in HIV-infected patients in Brazil: clinical and laboratory characteristics and factors associated with mortality

    Tuberculous meningitis (TBM) is a growing problem in HIV-infected patients in developing countries, where there is scarce data about this co-infection. Our objectives were to analyze the main features and outcomes of HIV-infected patients with TBM. Methods This was a retrospective study of HIV-infected Brazilian patients admitted consecutively for TBM. All patients had Mycobacterium tuberculosis isolated from the cerebrospinal fluid (CSF). Presenting clinical and laboratory features were studied. Multivariate analysis was used to identify variables associated with death during hospitalization and at 9 months after diagnosis. Survival was estimated using the Kaplan–Meier method. Results We included 108 cases (median age 36 years, 72% male). Only 15% had fever, headache, and meningeal signs simultaneously. Forty-eight percent had extrameningeal tuberculosis. The median CD4+ cell count was 65 cells/μl. Among 90 cases, 7% had primary resistance to isoniazid and 9% presented multidrug-resistant strains. The overall mortality during hospitalization was 29% and at 9 months was 41%. Tachycardia and prior highly active antiretroviral therapy (HAART) were associated with 9-month mortality. The 9-month survival rate was 22% (95% confidence interval 12–43%). Conclusions Clinical and laboratory manifestations were unspecific. Disseminated tuberculosis and severe immunosuppression were common. Mortality was high and the 9-month survival rate was low.     The effect of local thermal fluctuations on the folding kinetics: a study from the perspective of the nonextensive statistical mechanics

    Protein folding is a universal process, very fast and accurate, which works consistently (as it should be) in a wide range of physiological conditions. The present work is based on three premises, namely: (ii) folding reaction is a process with two consecutive and independent stages, namely the search mechanism and the overall productive stabilization; (iiii) the folding kinetics results from a mechanism as fast as can be; and (iiiiii) at nanoscale dimensions, local thermal fluctuations may have important role on the folding kinetics. Here the first stage of folding process (search mechanism) is focused exclusively. The effects and consequences of local thermal fluctuations on the configurational kinetics, treated here in the context of non extensive statistical mechanics, is analyzed in detail through the dependence of the characteristic time of folding (τ\tau) on the temperature TT and on the nonextensive parameter qq.The model used consists of effective residues forming a chain of 27 beads, which occupy different sites of a 3−3-D infinite lattice, representing a single protein chain in solution.     New native host plant records for frugivorous flies (Diptera: Tephritidae, Lonchaeidae) in Northeastern Brazil

    The fruit flies Anastrepha fraterculus (Wiedemann, 1830), Anastrepha sororcula Zucchi, 1979, Ceratitis capitata (Wiedemann, 1824) (Diptera: Tephritidae) and Neosilba pendula (Bezzi, 1989) (Diptera: Lonchaeidae) are recorded for the first time emerging from Campomanesia dichotoma (O.Berg) Mattos and Campomanesia ilhoensis Mattos (Myrtaceae), two native species occurring in fragments of Atlantic Forest over dunes, in the northernmost limit of this biome in the state of Rio Grande do Norte, Brazil. The flies emerged from fruit samples obtained at the conservation area at Parque Estadual Dunas do Natal. The parasitoids that emerged from the puparia were also recorded, with Aganaspis pelleranoi (Brèthes) being recorded for the first time for the state. These records are of importance for monitoring and further understanding of the cycles on alternative hosts especially where these species are considered agricultural pests.
