Exploring fraud and abuse in National Health Insurance Scheme (NHIS) using data mining technique as a statistical model
This study explored patterns of fraud and abuse that exist in the National Health Insurance Scheme (NHIS) claims in the Awutu-Effutu-Senya District using data mining techniques, with a specific focus on malaria-related claims. The study employed quantitative research approach with survey design as a strategy of enquiry. This survey explores the utility of various data mining techniques such as data collection, data cleaning/extraction, data integration, data selection, data transformation and pattern evaluation in the health domain. Samples of 720 clients diagnosed with malaria in the years 2013, 2014 and 2015 from 4 NHIS service providers in the districts were randomly selected for this study. Results from two-way between-subjects Analysis of Variance (ANOVA) revealed that Hospital B Private and Hospital A Private recorded the highest and lowest mean cost of malaria treatment respectively. The study further revealed that repetition of NHIS registration number, overbilling of drugs, drug mismatch, excessive prescription of drugs for malaria treatment and duplication of clients records were some of the fraud and abuse at the facility
Primary school mathematics teachers’ ideas, beliefs, and practices of constructivist instructional strategies
The study explored Ghanaian primary school mathematics teachers’ ideas, beliefs and practices of constructivist instructional strategies (CIS). The design for the study was a sequential exploratory design, comprising two hundred and fifty-two (252) mathematics teachers (126 lower primary teachers and 126 upper primary teachers), who were purposively selected from school districts in the Upper East region. The qualitative data consisted of interview responses and lesson observations. The quantitative data consisting mainly of teachers’ responses to 3-point Likert scale questionnaire items, helped to investigate relationship in two quantitative variables. The results indicated that through CIS pupils were able to construct their own understanding, and were willing to follow a learner-centred method of instruction. Additionally, teachers became aware of social interaction and authentic learning tasks, two aspects of CIS. It was also determined that as teachers’ perceptions of CIS increased, their frequency of use of selected CIS correspondingly increased. The implications of this study are that mathematics teachers should be provided with resources that would enable them to teach using CIS. To forestall this, in-service and professional development programmes should continually be organized for teachers to keep them abreast of CIS
The Impact of Irrigation on the Nutritional Status of Children in the Sissala West District of Ghana
Abstract: According to the Food and Agriculture Organization (FAO, 2006) the most sustainable way to reduce hunger effectively is to improve agriculture and rural development simultaneously. The study investigated the impact of irrigation dam projects on child nutritional status. A total of 397 mother-child pair from three communities (control, 1 year and 2 year intervention with introduction of irrigation dam) with and without irrigation dams participated in the study. Nutritional status was assessed using anthropometric indicators (height/length, weight and MUAC) and haemoglobin levels. For haemoglobin assessment a sub-sample of 200 children was used. Twenty-four h dietary recall was used to assess dietary intake. The results showed significant differences (p<0.001) in wasting rates of 11.0, 21.2 and 12.3%, respectively, among children in the control, 1 year intervention and 2-year intervention communities, respectively. Haemoglobin assessment revealed that a greater proportion of children (70.1%) were anaemic. The 1 year intervention had a significantly higher number of children (p<0.001) who had haemoglobin in the normal range. Most (95.9%) of the children did not meet their energy needs according to their dietary intake records. Child feeding practices continue to be a challenge in these communities. Currently, the impact of the irrigation dam on the livelihoods and nutritional status of the children is not obvious but more time is needed to realize the full potential. Nutrition education should target the incorporation of fish from the dam into young child feeding in these communities
Anaemia prevalence and nutrient intake among women in peri-urban settlements in Accra, Ghana
Anaemia among women is a major public health concern globally. In developing countries, nutritional anaemia may be due to poor bioavailability of dietary iron, haemoglobinopathies, or intestinal parasites. The study objectives were to determine the prevalence of anaemia and current nutrient intakes essential to erythropoiesis among women of reproductive age (WRA) in peri-urban settlements in the Ga-East Municipality, Accra, Ghana. This cross-sectional study assessed the nutrient intake, sickling and haemoglobin levels of 134 women aged 15-49 years enrolled in a peanut-based field trial at baseline. A pretested semi-quantitative food frequency questionnaire was used to assess energy and nutrient intake using the Food Processor (ESHA) software. Whole blood was used to determine full blood count using the haematology automated analyser. Sickling was determined by blood staining. Anaemia was classified based on recommended cut-offs. Chi-square analysis was used as a test of independence between anaemia and age groups. Linear regression was used to determine predictors of haemoglobin concentration. The mean age of the women was 29 ± 8 years. The mean total caloric intake was 2315 ± 915 kcal, whilst protein and fat intakes were 67 ± 27g and 68± 30g, respectively. Almost three-quarters of the women met the recommended dietary allowance (RDA) for iron and vitamin C. However, only a third met the RDA for fat, and about two-thirds met their needs for energy and protein. None of the women met the RDA for folate which is integral for haem formation. Also 17% (23/134) of the women were sickle cell anaemia positive. Mean haemoglobin concentration was 12.1 ± 1.8 g/dL. Mild, moderate and severe anaemia due to iron deficiency was present in 35.8%, 6.7% and 1.5%, of the women respectively. All categories of anaemia was present in 44% of the women. Anaemia, prevalence increased during the early stages of the reproductive age (15-29 years) and declined towards the end of the reproductive age period (40-49 years). An increase in age of one year was significantly associated with a 0.056 g/dL rise in haemoglobin level (p=0.014). Anaemia due to iron deficiency exists as a public health problem among women in peri-urban settlements in the Ga-East Municipality, Accra, Ghana. Dietary diversity to include green leafy vegetables which are rich in folate and pro-vitamin A may reduce the current level of prevalence.Key words: Anaemia, Nutrients, Peri-urban, Women, RDA, sickling test, Food Processor softwar

Agronomic potentials of quality protein maize hybrids developed in Ghana
A quality protein maize (QPM) hybrid programme was started in 1991 to develop and promote high and stableyielding QPM hybrids to increase production of
nutritionally superior maize varieties in Ghana. Six 3- way QPM hybrids developed from inbred lines originating from germplasm of the International Centre for Maize
and Wheat Improvement (CIMMYT) were evaluated on research stations and in farmers\' fields in Ghana from 1995 to 1996. In the on-station evaluations, grain yields across 10 sites in both years averaged 6.0 ton ha-1 for the three hybrids (GH132-28, GH110-5 and
GH2328-88), 5.22 ton ha-1 for Obatanpa, and 3.60 ton ha-1 for the local maize variety. In farmers\' fields, data from over 50 farm sites in 1995 and 1996 showed mean yields of 4.95 ton ha-1 for the three hybrids, and 4.28 ton ha-1 for Obatanpa compared to 3.59 ton ha-1 for farmers\' varieties. On the average, the hybrids were similar to Obatanpa in days to 50 per cent silking, but were shorter in plant height and ear placement. Consumer
preference tests showed that the three hybrids were rated similar to the local variety in popular traditional food preparations such as ‘kenkey\' and ‘tuo zafi\'. In 1997,
the National Variety Release Committee approved the release of GH132-28, GH110-5, and GH2328-88 under the local names Dadaba, Mamaba, and CIDA-ba, respectively. These hybrids are recommended for planting in all the major agro-ecologies to boost maize production in Ghana.
Ghana Journal of Agricultural Science Vol. 40 (1) 2007: pp. 81-8
Rapid and sensitive detection of mycobacterium ulcerans by use of a loop-mediated isothermal amplification test
This work reports the design and evaluation of a rapid loop-mediated isothermal amplification test for detecting Mycobacterium ulcerans DNA based on the multicopy insertion sequence IS2404. The test is robust and specific with a detection limit equivalent to 20 copies of the target sequence (0.01 to 0.1 genome). The test has potential for the diagnosis of Buruli ulcer under field conditions
Combined Inflammatory and Metabolic Defects Reflected by Reduced Serum Protein Levels in Patients with Buruli Ulcer Disease
Buruli ulcer is a skin disease caused by Mycobacterium ulcerans that is spreading in tropical countries, with major public health and economic implications in West Africa. Multi-analyte profiling of serum proteins in patients and endemic controls revealed that Buruli ulcer disease down-regulates the circulating levels of a large array of inflammatory mediators, without impacting on the leukocyte composition of peripheral blood. Notably, several proteins contributing to acute phase reaction, lipid metabolism, coagulation and tissue remodelling were also impacted. Their down-regulation was selective and
persisted after the elimination of bacteria with antibiotic therapy. It involved proteins with various functions and origins, suggesting that M. ulcerans infection causes global and chronic defects in the host’s protein metabolism. Accordingly, patients had reduced levels of total serum proteins and blood urea, in the absence of signs of malnutrition, or functional failure of liver or kidney. Interestingly, slow healers had deeper metabolic and coagulation defects at the start of antibiotic therapy. In addition to providing novel insight into Buruli ulcer pathogenesis, our study therefore identifies a unique
proteomic signature for this disease

High degree of prostate related LUTS in a prospective cross-sectional community study in Ghana (Mamprobi)
BACKGROUND: Changing voiding patterns, volume and frequency, may sometimes be mistaken for anxiety, stress or increase in fluid consumption. In the aging male population, the commencement of lower urinary tract symptoms (LUTS) may be silent and perceived as "normal" and unrelated to Benign prostatic enlargement (BPE). The purpose of the study was to determine the prevalence of apparently "silent LUTS" (perceived asymptomatic LUTS) in men in a Ghanaian Community as well as its underlying risk factors.
METHODS: One hundred and eleven (111) men (40-70 years) were recruited from a community in Ghana. The International Prostate Symptoms Score (IPSS) questionnaire (administered in the local language and English) and ultrasonographic imaging of the prostate volume (PV) were utlized to collect data. IPSS score >7 plus PV > 30 cm3 was definitive of lower urinary tract symptoms. Eighty-one (81) participants were classified "LUTS Negative" (LN) and 30, "LUTS Positive" (LP). Risk factors i.e., cholesterol (CHOL), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), very low-density lipoprotein (VLDL), coronary risk (CR), BMI and Blood Pressure were also determined.
RESULTS: The prevalence of LUTS using only IPSS definition alone was 42.3%. However, IPSS in combination with Prostate Volume gave a prevalence of 27.0%. LN subjects had enlarged prostate (41.98%) and LP, 100%. Quality of life (QoL) was better in the LUTS Negative than LUTS Positive group (p < 0.001). In the univariant analysis coronary risk, triglyceride and VLDL contributed to LUTS (p = 0.023, 0.22, 0.22, respectively). In a multivariant analysis HDL-C (p = 0.027), BMI (p = 0.047) and triglyceride (p = 0.019) significantly contributed to LUTS.
CONCLUSIONS: The prevalence of LUTS (42.3%) is high. Components of Metabolic Syndrome- HDL-C, BMI, and coronary risk were associated with LUTS. This emphasizes the need for community education
Sero-Epidemiology as a Tool to Screen Populations for Exposure to Mycobacterium ulcerans
Sero-epidemiological analyses revealed that a higher proportion of sera from individuals living in the Buruli ulcer (BU) endemic Densu River Valley of Ghana contain Mycobacterium ulcerans 18 kDa small heat shock protein (shsp)-specific IgG than sera from inhabitants of the Volta Region, which was regarded so far as BU non-endemic. However, follow-up studies in the Volta Region showed that the individual with the highest anti-18 kDa shsp-specific serum IgG titer of all participants from the Volta Region had a BU lesion. Identification of more BU patients in the Volta Region by subsequent active case search demonstrated that sero-epidemiology can help identify low endemicity areas. Endemic and non-endemic communities along the Densu River Valley differed neither in sero-prevalence nor in positivity of environmental samples in PCR targeting M. ulcerans genomic and plasmid DNA sequences. A lower risk of developing M. ulcerans disease in the non-endemic communities may either be related to host factors or a lower virulence of local M. ulcerans strains
