Medium-term health of seniors following exposure to a natural disaster
The article aims to describe the medium-term impacts of a major earthquake event (Chile, February 27, 2010) on 26 seniors. The authors adopted a qualitative study approach. Data obtained using the Impact of Event Scale–Revised (IES-R) show the presence of manifestations of posttraumatic stress in the majority of respondents. In addition, data collected in interviews demonstrated a progressive deterioration of the health of respondents over a period of 4 years following the disaster. Seniors are particularly vulnerable to the effects of material loss, emotional stress, and postdisaster health complications. These impacts are exacerbated by low economic status. Furthermore, broader research is necessary involving elderly living in poverty who have survived natural disasters and others without such experiences, in order to better identify and differentiate between health complications associated with exposure to disaster events and those linked more strictly with natural aging processes
An extensive spectroscopic time-series of three Wolf-Rayet stars. I. The lifetime of large-scale structures in the wind of WR 134
During the summer of 2013, a 4-month spectroscopic campaign took place to
observe the variabilities in three Wolf-Rayet stars. The spectroscopic data
have been analyzed for WR 134 (WN6b), to better understand its behaviour and
long-term periodicity, which we interpret as arising from corotating
interaction regions (CIRs) in the wind. By analyzing the variability of the He
II 5411 emission line, the previously identified period was refined to
P = 2.255 0.008 (s.d.) days. The coherency time of the variability, which
we associate with the lifetime of the CIRs in the wind, was deduced to be 40
6 days, or 18 cycles, by cross-correlating the variability
patterns as a function of time. When comparing the phased observational
grayscale difference images with theoretical grayscales previously calculated
from models including CIRs in an optically thin stellar wind, we find that two
CIRs were likely present. A separation in longitude of
90 was determined between the two CIRs and we suggest that the
different maximum velocities that they reach indicate that they emerge from
different latitudes. We have also been able to detect observational signatures
of the CIRs in other spectral lines (C IV 5802,5812 and He I
5876). Furthermore, a DAC was found to be present simultaneously with
the CIR signatures detected in the He I 5876 emission line which is
consistent with the proposed geometry of the large-scale structures in the
wind. Small-scale structures also show a presence in the wind, simultaneously

Randomised controlled trial of adjunctive inspiratory muscle training for patients with COPD.
BACKGROUND: This study aimed to investigate whether adjunctive inspiratory muscle training (IMT) can enhance the well-established benefits of pulmonary rehabilitation (PR) in patients with COPD. METHODS: 219 patients with COPD (FEV1: 42%±16% predicted) with inspiratory muscle weakness (PImax: 51±15 cm H2O) were randomised into an intervention group (IMT+PR; n=110) or a control group (Sham-IMT+PR; n=109) in this double-blind, multicentre randomised controlled trial between February 2012 and October 2016 (ClinicalTrials.gov NCT01397396). Improvement in 6 min walking distance (6MWD) was a priori defined as the primary outcome. Prespecified secondary outcomes included respiratory muscle function and endurance cycling time. FINDINGS: No significant differences between the intervention group (n=89) and the control group (n=85) in improvements in 6MWD were observed (0.3 m, 95% CI -13 to 14, p=0.967). Patients who completed assessments in the intervention group achieved larger gains in inspiratory muscle strength (effect size: 1.07, p<0.001) and endurance (effect size: 0.79, p<0.001) than patients in the control group. 75 s additional improvement in endurance cycling time (95% CI 1 to 149, p=0.048) and significant reductions in Borg dyspnoea score at isotime during the cycling test (95% CI -1.5 to -0.01, p=0.049) were observed in the intervention group. INTERPRETATION: Improvements in respiratory muscle function after adjunctive IMT did not translate into additional improvements in 6MWD (primary outcome). Spectroscopy, MOST Photometry, and Interferometry of MWC 314: Is it an LBV or an interacting binary?
MWC 314 is a bright candidate luminous blue variable that resides in a fairly
close binary system, with an orbital period of 60.7530.003 d. We observed
MWC 314 with a combination of optical spectroscopy, broad-band ground- and
space-based photometry, as well as with long baseline, near-infrared
interferometry. We have revised the single-lined spectroscopic orbit and
explored the photometric variability. The orbital light curve displays two
minima each orbit that can be partially explained in terms of the tidal
distortion of the primary that occurs around the time of periastron. The
emission lines in the system are often double-peaked and stationary in their
kinematics, indicative of a circumbinary disc. We find that the stellar wind or
circumbinary disc is partially resolved in the K\prime-band with the longest
baselines of the CHARA Array. From this analysis, we provide a simple,
qualitative model in an attempt to explain the observations. From the
assumption of Roche Lobe overflow and tidal synchronisation at periastron, we
estimate the component masses to be M1 M and M2
M, which indicates a mass of the LBV that is extremely low. In addition
to the orbital modulation, we discovered two pulsational modes with the MOST
satellite. These modes are easily supported by a low-mass hydrogen-poor star,
but cannot be easily supported by a star with the parameters of an LBV. The
combination of these results provides evidence that the primary star was likely
never a normal LBV, but rather is the product of binary interactions. As such,
this system presents opportunities for studying mass-transfer and binary

Multiple carbon accounting to support just and effective climate policies
Negotiating reductions in greenhouse gas emission involves the allocation of emissions and of emission reductions to specific agents, and notably, within the current UN framework, to associated countries. As production takes place in supply chains,increasingly extending over several countries, there are various options available in which emissions originating from one and the same activity may be attributed to different agents along the supply chain and thus to different countries. In this way, several distinct types of national carbon accounts can be constructed. We argue that these accounts will typically differ in the information they provide to individual countries on the effects their actions have on global emissions; and they may also, to varying degrees, prove useful in supporting the pursuit of an effective and just climate policy. None of the accounting systems, however, prove 'best' in achieving these aims under real-world circumstances; we thus suggest compiling reliable data to aid in the consistent calculation of multiple carbon accounts on a global level
Powder Compaction: Compression Properties of Cellulose Ethers
Effective development of matrix tablets requires a comprehensive understanding of different raw material attributes and their impact on process parameters. Cellulose ethers (CE) are the most commonly used pharmaceutical excipients in the fabrication of hydrophilic matrices. The innate good compression and binding properties of CE enable matrices to be prepared using economical direct compression (DC) techniques. However, DC is sensitive to raw material attributes, thus, impacting the compaction process. This article critically reviews prior knowledge on the mechanism of powder compaction and the compression properties of cellulose ethers, giving timely insight into new developments in this field
Design of bio-nanosystems for oral delivery of functional compounds
Nanotechnology has been referred to as one of the most interesting topics in food technology due to the potentialities of its use by food industry. This calls for studying the behavior of nanosystems as carriers of biological and functional compounds aiming at their utilization for delivery, controlled release and protection of such compounds during food processing and oral ingestion. This review highlights the principles of design and production of bio-nanosystems for oral delivery and their behavior within the human gastrointestinal (GI) tract, while providing an insight into the application of reverse engineering approach to the design of those bio-nanosystems. Nanocapsules, nanohydrogels, lipid-based and multilayer nanosystems are discussed (in terms of their main ingredients, production techniques, predominant forces and properties) and some examples of possible food applications are given. Phenomena occurring in in vitro digestion models are presented, mainly using examples related to the utilization of lipid-based nanosystems and their physicochemical behavior throughout the GI tract. Furthermore, it is shown how a reverse engineering approach, through two main steps, can be used to design bio-nanosystems for food applications, and finally a last section is presented to discuss future trends and consumer perception on food nanotechnology.Miguel A. A novel, highly discriminatory risk model predicting acute severe right ventricular failure in patients undergoing continuous‐flow left ventricular assist device implant
Various risk models with differing discriminatory power and predictive accuracy have been used to predict right ventricular failure (RVF) after left ventricular assist device (LVAD) placement. There remains an unmet need for a contemporary risk score for continuous flow (CF)‐LVADs. We sought to independently validate and compare existing risk models in a large cohort of patients and develop a simple, yet highly predictive risk score for acute, severe RVF. Data from the Mechanical Circulatory Support Research Network (MCSRN) registry, consisting of patients who underwent CF‐LVAD implantation, were randomly divided into equal‐sized derivation and validation samples. RVF scores were calculated for the entire sample, and the need for a right ventricular assist device (RVAD) was the primary endpoint. Candidate predictors from the derivation sample were subjected to backward stepwise logistic regression until the model with lowest Akaike information criterion value was identified. A risk score was developed based on the identified variables and their respective regression coefficients. Between May 2004 and September 2014, 734 patients underwent implantation of CF‐LVADs [HeartMate II LVAD, 76% (n = 560), HeartWare HVAD, 24% (n = 174)]. A RVAD was required in 4.5% (n = 33) of the patients [Derivation cohort, n = 15 (4.3%); Validation cohort, n = 18 (5.2%); P = 0.68)]. 19.5% of the patients (n = 143) were female, median age at implant was 59 years (IQR, 49.4–65.3), and median INTERMACS profile was 3 (IQR, 2–3). RVAD was required in 4.5% (n = 33) of the patients. Correlates of acute, severe RVF in the final model included heart rate, albumin, BUN, WBC, cardiac index, and TR severity. Areas under the curves (AUC) for most commonly used risk predictors ranged from 0.61 to 0.78. The AUC for the new model was 0.89 in the derivation and 0.92 in the validation cohort. Global warming and the cosmopolitan political conception of justice
Within the literature in green political theory on global environmental threats one can often find dissatisfaction with liberal theories of justice. This is true even though liberal cosmopolitans regularly point to global environmental problems as one reason for expanding the scope of justice beyond the territorial limits of the state. One of the causes for scepticism towards liberal approaches is that many of the most notable anti-cosmopolitan theories are also advanced by liberals. In this paper, I first explain why one of the strongest expressions of liberal anti-cosmopolitanism cannot simply be dismissed because it may fail to support desired environmental ends. The political conception of justice represents one of the most important challenges to cosmopolitanism generally and is thus a serious challenge to viewing global environmental problems in terms of cosmopolitan justice. Second, I will show through the case of anthropogenic global warming that the political conception of justice under current conditions does have clear cosmopolitan implications despite its proponents' claims
