    Sentidos subjetivos emergentes en una víctima del conflicto armado colombiano

    The present research is of qualitative approach of interpretive cohort that allowed to assume the narrative testimony of a victim of the Colombian armed conflict in the corregimiento de Santa Isabel, César; to dig in and build from him, the subjective senses that are his own. For this, an in-depth interview was developed with one of the community leaders of the San Isabel corregimiento, he assumed this role in a scenario of resistance and in a period of high waves of social violence directed towards the civilian population of part of armed actors. Four subjective configurations are evident with their respective subjective senses: death (murder of neighbors and community members, death of the father), distrust and fear (towards institutions), social practices (family, leadership in the Communal Action Board) zozobra (in the territory).     Mechatronic design, experimental setup, and control architecture design of a novel 4 DoF parallel manipulator

    "This is an Author's Accepted Manuscript of an article published in [include the complete citation information for the final versíon of the article as published in the Mechanics Based Design of Structures and Machines 2018 [copyright Taylor & Francis], available online at: https://www.tandfonline.com/doi/10.1080/15397734.2017.1355249."[EN] Although parallel manipulators started with the introduction of architectures with six degrees of freedom, a vast number of applications require less than six degrees of freedom. Consequently, scholars have proposed architectures with three and four degrees of freedom, but relatively few four degrees of freedom parallel manipulators have become prototypes, especially of the two rotation and two translation motion types. In this article, we explain the mechatronics design, prototype, and control architecture design of a four degrees of freedom parallel manipulators with two rotation and two translation motions. We chose to design a four degrees of freedom manipulator based on the motion needed to complete the tasks of lower limb rehabilitation. To the author's best knowledge, parallel manipulators between three and six degrees of freedom for rehabilitation of lower limb have not been proposed to date. The developed architecture enhances the three minimum degrees of freedom required by adding a four degrees of freedom, which allows combinations of normal or tangential efforts in the joints, or torque acting on the knee. We put forward the inverse and forward displacement equations, describe the prototype, perform the experimental setup, and develop the hardware and control architecture.     Oxidative Stress, Telomere Shortening, and Apoptosis Associated to Sarcopenia and Frailty in Patients with Multimorbidity

    The presence of oxidative stress, telomere shortening, and apoptosis in polypathological patients (PP) with sarcopenia and frailty remains unknown. Methods: Multicentric prospective observational study in order to assess oxidative stress markers (catalase, glutathione reductase (GR), total antioxidant capacity to reactive oxygen species (TAC-ROS), and superoxide dismutase (SOD)), absolute telomere length (aTL), and apoptosis (DNA fragmentation) in peripheral blood samples of a hospital-based population of PP. Associations of these biomarkers to sarcopenia, frailty, functional status, and 12-month mortality were analyzed. Results: Of the 444 recruited patients, 97 (21.8%), 278 (62.6%), and 80 (18%) were sarcopenic, frail, or both, respectively. Oxidative stress markers (lower TAC-ROS and higher SOD) were significantly enhanced and aTL significantly shortened in patients with sarcopenia, frailty or both syndromes. No evidence of apoptosis was detected in blood leukocytes of any of the patients. Both oxidative stress markers (GR, p = 0.04) and telomere shortening (p = 0.001) were associated to death risk and to less survival days. Conclusions: Oxidative stress markers and telomere length were enhanced and shortened, respectively, in blood samples of polypathological patients with sarcopenia and/or frailty. Both were associated to decreased survival. They could be useful in the clinical practice to assess vulnerable populations with multimorbidity and of potential interest as therapeutic targets

    Development and Validation of an Early Mortality Risk Score for Older Patients Treated with Chemotherapy for Cancer

    Background: Estimation of life expectancy in older patients is relevant to select the best treatment strategy. We aimed to develop and validate a score to predict early mortality in older patients with cancer. Patients and Methods: A total of 749 patients over 70 years starting new chemotherapy regimens were prospectively included. A prechemotherapy assessment that included sociodemographic variables, tumor/treatment variables, and geriatric assessment variables was performed. Association between these factors and early death was examined using multivariable logistic regression. Score points were assigned to each risk factor. External validation was performed on an independent cohort. Results: In the training cohort, the independent predictors of 6-month mortality were metastatic stage (OR 4.8, 95% CI [2.4-9.6]), ECOG-PS 2 (OR 2.3, 95% CI [1.1-5.2]), ADL ≤ 5 (OR 1.7, 95% CI [1.1-3.5]), serum albumin levels ≤ 3.5 g/dL (OR 3.4, 95% CI [1.7-6.6]), BMI < 23 kg/m2 (OR 2.5, 95% CI [1.3-4.9]), and hemoglobin levels < 11 g/dL (OR 2.4, 95% CI (1.2-4.7)). With these results, we built a prognostic score. The area under the ROC curve was 0.78 (95% CI, 0.73 to 0.84), and in the validation set, it was 0.73 (95% CI: 0.67-0.79). Conclusions: This simple and highly accurate tool can help physicians making decisions in elderly patients with cancer who are planned to initiate chemotherapy treatment

    El presente trabajo se propone articular una reflexión sobre algunos retos que enfrenta la investigación psicosociológica de cara al acompañamiento, soporte e inclusive orientación de los procesos y dinámicas psicosociales de construcción de paz. Se inspira en el conjunto de trabajos aquí incluidos, pero no se basa en ellos de manera exclusiva ni prioritaria; busca, más bien, ofrecer un conjunto de criterios para la reflexión crítica en un momento histórico único por las oportunidades de incidencia que se le abren, para el mejoramiento de la investigación psicosociológica. Se espera animar a su vez nuevas prácticas reflexivas que contribuyan a incrementar la relevancia y la pertinencia de este tipo de investigación, a elegir caminos fecundos tanto para la definición de programas de investigación como de decisiones curriculares cruciales para la formación avanzada de nuevas generaciones de profesionales que contribuyan en los próximos años a este fundamental reto de construcción de la paz profunda en Colombia. El libro es una contribución a las reflexiones epistemológicas, metodológicas y éticas en un momento histórico y político crucial para el país, en el que desafortunadamente no vemos aún materializarse el deseo de paz en la Colombia profunda.
