    Photon spin-to-orbital angular momentum conversion via an electrically tunable qq-plate

    Exploiting electro-optic effects in liquid crystals, we achieved real-time control of the retardation of liquid- crystal-based qq-plates through an externally applied voltage.     Physics-Informed Extreme Theory of Functional Connections Applied to Data-Driven Parameters Discovery of Epidemiological Compartmental Models

    In this work we apply a novel, accurate, fast, and robust physics-informed neural network framework for data-driven parameters discovery of problems modeled via parametric ordinary differential equations (ODEs) called the Extreme Theory of Functional Connections (X-TFC). The proposed method merges two recently developed frameworks for solving problems involving parametric DEs, 1) the Theory of Functional Connections (TFC) and 2) the Physics-Informed Neural Networks (PINN). In particular, this work focuses on the capability of X-TFC in solving inverse problems to estimate the parameters governing the epidemiological compartmental models via a deterministic approach. The epidemiological compartmental models treated in this work are Susceptible-Infectious-Recovered (SIR), Susceptible-Exposed-Infectious-Recovered (SEIR), and Susceptible-Exposed-Infectious-Recovered-Susceptible (SEIR). The results show the low computational times, the high accuracy and effectiveness of the X-TFC method in performing data-driven parameters discovery of systems modeled via parametric ODEs using unperturbed and perturbed data

    Induction chemotherapy followed by neoadjuvant chemoradiotherapy and surgery in locally advanced rectal cancer: preliminary results of a phase II study

    PURPOSE: To report preliminary results of induction chemotherapy (IC) followed by neoadjuvant chemoradiotherapy (CRT) and surgery in locally advanced rectal cancer (LARC) patients.MATERIALS AND METHODS: This is the preliminary evaluation of a phase II study. Patients with histologically proven rectal adenocarcinoma, stage II-III disease, who met the inclusion criteria, received induction FOLFOXIRI (5-FU, leucovorin, oxaliplatin and irinotecan) regimen in combination with targeted agents followed by CRT and surgery. Analysis of the first 8 patients was required to confirm the treatment feasibility before the accrual of 20 additional patients. RESULTS: The first 8 patients were evaluated. The median follow-up time was 23 months. There were no treatment-related deaths. Trimodality strategy was well tolerated with high compliance and a good level of toxicity. There were no evidence of febrile neutropenia and any grade 4 adverse events were recorded. Three patients had pathologic complete response (pCR) and 1 patient had a nearly pCR (ypT1 ypN0). CONCLUSION: Preliminary results are encouraging. FOLFOXIRI regimen plus targeted agents followed by CRT and surgery seems a safe approach. Longer follow-up and higher number of patients are mandatory to confirm such findings

    Test of mutually unbiased bases for six-dimensional photonic quantum systems

    In quantum information, complementarity of quantum mechanical observables plays a key role. If a system resides in an eigenstate of an observable, the probability distribution for the values of a complementary observable is flat. The eigenstates of these two observables form a pair of mutually unbiased bases (MUBs). More generally, a set of MUBs consists of bases that are all pairwise unbiased. Except for specific dimensions of the Hilbert space, the maximal sets of MUBs are unknown in general. Even for a dimension as low as six, the identification of a maximal set of MUBs remains an open problem, although there is strong numerical evidence that no more than three simultaneous MUBs do exist. Here, by exploiting a newly developed holographic technique, we implement and test different sets of three MUBs for a single photon six-dimensional quantum state (a qusix), encoded either in a hybrid polarization-orbital angular momentum or a pure orbital angular momentum Hilbert space. A close agreement is observed between theory and experiments. Our results can find applications in state tomography, quantitative wave-particle duality, quantum key distribution and tests on complementarity and logical indeterminacy.Comment: 8 pages, 4 figure

    Point prevalence data on antimicrobial usage in Italian acute-care hospitals: Evaluation and comparison of results from two national surveys (2011-2016).

    AbstractObjectives and design:Data from the Italian national point-prevalence survey (PPS) of healthcare-associated infections (HAIs) were used to evaluate antimicrobial usage (AMU) in Italy and to identify targets for future interventions.Methods:The second Italian PPS was conducted in 2016 as part of the European PPS initiated by the ECDC. We compared these results with those of the first national survey, conducted in 2011.Results:An overall AMU prevalence of 44.5% (95% CI, 43.7–45.3) was estimated in 2016. No significant change in AMU prevalence was detected when comparing data with the first survey. In both surveys, the most prevalent indication for AMU was the treatment of infections. Considering all indications, penicillins plus β-lactamase inhibitors (BLIs) were the most commonly prescribed antimicrobial group in 2016; they were used significantly more than in 2011, and piperacillin plus BLI was the most frequently used agent. Broad-spectrum agents accounted for >60% of all antimicrobials for systemic use. No significant increase in the use of carbapenems occurred in 2016. Stable or decreasing carbapenem-resistance levels were identified in this study, although these levels remain alarmingly high for both Klebsiella pneumoniae (50%) and Acinetobacter baumannii (>75%).Conclusions:These results can be used to identify priorities and targets for interventions that promote more prudent use of antimicrobials, improve healthcare quality and patient safety, and combat the emergence and spread of antimicrobial-resistant pathogens

    Timed rise from floor as a predictor of disease progression in Duchenne muscular dystrophy: An observational study

    The role of timed items, and more specifically, of the time to rise from the floor, has been reported as an early prognostic factor for disease progression and loss of ambulation. The aim of our study was to investigate the possible effect of the time to rise from the floor test on the changes observed on the 6MWT over 12 months in a cohort of ambulant Duchenne boys.A total of 487 12-month data points were collected from 215 ambulant Duchenne boys. The age ranged between 5.0 and 20.0 years (mean 8.48 ±2.48 DS).The results of the time to rise from the floor at baseline ranged from 1.2 to 29.4 seconds in the boys who could perform the test. 49 patients were unable to perform the test at baseline and 87 at 12 month The 6MWT values ranged from 82 to 567 meters at baseline. 3 patients lost the ability to perform the 6mwt at 12 months. The correlation between time to rise from the floor and 6MWT at baseline was high (r = 0.6, p<0.01).Both time to rise from the floor and baseline 6MWT were relevant for predicting 6MWT changes in the group above the age of 7 years, with no interaction between the two measures, as the impact of time to rise from the floor on 6MWT change was similar in the patients below and above 350 m. Our results suggest that, time to rise from the floor can be considered an additional important prognostic factor of 12 month changes on the 6MWT and, more generally, of disease progression

    Integrated Antitumor Activities of Cellular Immunotherapy with CIK Lymphocytes and Interferons against KIT/PDGFRA Wild Type GIST

    : Gastrointestinal stromal tumors (GISTs) are rare, mesenchymal tumors of the gastrointestinal tract, characterized by either KIT or PDGFRA mutation in about 85% of cases. KIT/PDGFRA wild type gastrointestinal stromal tumors (wtGIST) account for the remaining 15% of GIST and represent an unmet medical need: their prevalence and potential medical vulnerabilities are not completely defined, and effective therapeutic strategies are still lacking. In this study we set a patient-derived preclinical model of wtGIST to investigate their phenotypic features, along with their susceptibility to cellular immunotherapy with cytokine-induced killer lymphocytes (CIK) and interferons (IFN). We generated 11 wtGIST primary cell lines (wtGISTc). The main CIK ligands (MIC A/B; ULBPs), along with PD-L1/2, were expressed by wtGISTc and the expression of HLA-I molecules was preserved. Patient-derived CIK were capable of intense killing in vitro against wtGISTc resistant to both imatinib and sunitinib. We found that CIK produce a high level of granzyme B, IFNα and IFNγ. CIK-conditioned supernatant was responsible for part of the observed tumoricidal effect, along with positive bystander modulatory activities enhancing the expression of PD-L1/2 and HLA-I molecules. IFNα, but not In, had direct antitumor effects on 50% (4/8) of TKI-resistant wtGISTc, positively correlated with the tumor expression of IFN receptors. wtGIST cells that survived IFNα were still sensitive to CIK immunotherapy. Our data support the exploration of CIK immunotherapy in clinical studies for TKI-resistant wtGIST, proposing reevaluation for IFNα within this challenging setting

    The influence of autistic symptoms on social and non-social cognition and on real-life functioning in people with schizophrenia: Evidence from the Italian Network for Research on Psychoses multicenter study

    BACKGROUND: Autism spectrum disorders (ASDs) and schizophrenia spectrum disorders (SSDs), although conceptualized as separate entities, may share some clinical and neurobiological features. ASD symptoms may have a relevant role in determining a more severe clinical presentation of schizophrenic disorder but their relationships with cognitive aspects and functional outcomes of the disease remain to be addressed in large samples of individuals. AIMS: To investigate the clinical, cognitive, and functional correlates of ASD symptoms in a large sample of people diagnosed with schizophrenia. METHODS: The severity of ASD symptoms was measured with the PANSS Autism Severity Scale (PAUSS) in 921 individuals recruited for the Italian Network for Research on Psychoses multicenter study. Based on the PAUSS scores, three groups of subjects were compared on a wide array of cognitive and functional measures. RESULTS: Subjects with more severe ASD symptoms showed a poorer performance in the processing speed (p\ua0=\ua00.010), attention (p\ua0=\ua00.011), verbal memory (p\ua0=\ua00.035), and social cognition (p\ua0=\ua00.001) domains, and an overall lower global cognitive composite score (p\ua0=\ua00.010). Subjects with more severe ASD symptoms also showed poorer functional capacity (p\ua0=\ua00.004), real-world interpersonal relationships (p\ua0<\ua00.001), and participation in community-living activities (p\ua0<\ua00.001). CONCLUSIONS: These findings strengthen the notion that ASD symptoms may have a relevant impact on different aspects of the disease, crucial to the life of people with schizophrenia. Prominent ASD symptoms may characterize a specific subpopulation of individuals with SSD

    An automatic analysis framework for FDOPA PET neuroimaging

    In this study we evaluate the performance of a fully automated analytical framework for FDOPA PET neuroimaging data, and its sensitivity to demographic and experimental variables and processing parameters. An instance of XNAT imaging platform was used to store the King's College London institutional brain FDOPA PET imaging archive, alongside individual demographics and clinical information. By re-engineering the historical Matlab-based scripts for FDOPA PET analysis, a fully automated analysis pipeline for imaging processing and data quantification was implemented in Python and integrated in XNAT. The final data repository includes 892 FDOPA PET scans organized from 23 different studies. We found good reproducibility of the data analysis by the automated pipeline (in the striatum for the Kicer: for the controls ICC = 0.71, for the psychotic patients ICC = 0.88). From the demographic and experimental variables assessed, gender was found to most influence striatal dopamine synthesis capacity (F = 10.7, p < 0.001), with women showing greater dopamine synthesis capacity than men. Our automated analysis pipeline represents a valid resourse for standardised and robust quantification of dopamine synthesis capacity using FDOPA PET data. Combining information from different neuroimaging studies has allowed us to test it comprehensively and to validate its replicability and reproducibility performances on a large sample size
