    4.1 Background Modelling is essential for enhancing our understanding of the functioning of tropical wetland ecosystems, and for simulating future trajectories and testing for system thresholds. Anthropogenic activities such as drainage and land-use change can be integrated in models and their impacts on fluxes of greenhouse gas concentrations simulated. Models can also be used to test the response of peatlands and mangroves to climate extremes, variability and change, and to estimate reference levels and greenhouse gas emissions scenarios in the framework of climate change mitigation projects such as REDD+. In coastal settings, models are used to explore wetland resilience to sea-level rise. Finally, models can also be developed to support the decision making process by providing policyrelevant information on the consequences and trade-offs of adopting different management and climate scenarios

    Long-term impact of sewage sludge application on soil microbial biomass: An evaluation using meta-analysis

    The Long-Term Sludge Experiments (LTSE) began in 1994 as part of continuing research into the effects of sludge-borne heavy metals on soil fertility. The long-term effects of Zn, Cu, and Cd on soil microbial biomass carbon (Cmic) were monitored for 8 years (1997-2005) in sludge amended soils at nine UK field sites. To assess the statutory limits set by the UK Sludge (Use in Agriculture) Regulations the experimental data has been reviewed using the statistical methods of meta-analysis. Previous LTSE studies have focused predominantly on statistical significance rather than effect size, whereas meta-analysis focuses on the magnitude and direction of an effect, i.e. the practical significance, rather than its statistical significance. The results presented here show that significant decreases in Cmic have occurred in soils where the total concentrations of Zn and Cu fall below the current UK statutory limits. For soils receiving sewage sludge predominantly contaminated with Zn, decreases of approximately 7ā€“11% were observed at concentrations below the UK statutory limit. The effect of Zn appeared to increase over time, with increasingly greater decreases in Cmic observed over a period of 8 years. This may be due to an interactive effect between Zn and confounding Cu contamination which has augmented the bioavailability of these metals over time. Similar decreases (7ā€“12%) in Cmic were observed in soils receiving sewage sludge predominantly contaminated with Cu; however, Cmic appeared to show of recovery after a period of 6 years. Application of sewage sludge predominantly contaminated with Cd appeared to have no effect on Cmic at concentrations below the current UK statutory limit

    Examining the relationship between inflammatory biomarkers during COVIDā€19 hospitalization and subsequent longā€COVID symptoms: A longitudinal and retrospective study

    IntroductionLong-COVID is a heterogeneous condition with a litany of physical and neuropsychiatric presentations and its pathophysiology remains unclear. Little is known about the association between inflammatory biomarkers, such as interleukin-6 (IL-6) and C-reactive protein (CRP) in the acute phase, and persistent symptoms after hospitalization in COVID-19 patients.MethodsIL-6, CRP, troponin-T, and ferritin were analyzed at admission for all patients with COVID-19 between September 1, 2020 to January 10, 2021. Survivors were followed up 3-months following hospital discharge and were asked to report persistent symptoms they experienced. Admission data were retrospectively collected. Independent t-tests and Mannā€“Whitney U tests were performed.ResultsIn a sample of 144 patients (62.5% male, mean Age 62 years [SDā€‰=ā€‰13.6]) followed up 3 months after hospital discharge, the commonest symptoms reported were fatigue (54.2%), breathlessness (52.8%), and sleep disturbance (37.5%). In this sample, admission levels of IL-6, CRP and ferritin were elevated. However, those reporting myalgia, low mood, and anxiety at follow-up had lower admission levels of IL-6 (34.9 vs. 52.0ā€‰pg/mL, pā€‰=ā€‰.043), CRP (83 vs. 105ā€‰mg/L, pā€‰=ā€‰.048), and ferritin (357 vs. 568ā€‰ug/L, pā€‰=ā€‰.01) respectively, compared with those who did not report these symptoms. Multivariate regression analysis showed that these associations were confounded by gender, as female patients had significantly lower levels of IL-6 and ferritin on admission (29.5 vs. 56.1, pā€‰=ā€‰.03 and 421.5 vs. 589, pā€‰=ā€‰.001, respectively) and were more likely to report myalgia, low mood and anxiety, when compared to males.ConclusionsOur data demonstrate that female patients present more often with lower levels of inflammatory biomarkers on admission which are subsequently associated with long-term post-COVID symptoms, such as myalgia and anxiety, in those discharged from hospital with severe COVID-19. Further research is needed into the role of serum biomarkers in post-COVID prognostication

    Three birds with one stone: Tidal wetland restoration, carbon sequestration, and enhancing resilience to rising sea levels in the Snohomish River Estuary, Washington

    Recent attention has focused on exploring the carbon storage and sequestration values of tidal wetlands to mitigate greenhouse gas emissions. Efforts are now underway to develop the tools and refine the science needed to bring carbon markets to bear on tidal wetland restoration activities. Effective restoration not only maximizes carbon storage in former tidal wetlands but also, through the accumulation of organic and mineral matter, enhances these systemsā€™ resilience to rising sea levels. To this end, this project focuses on the Snohomish River estuary of the Puget Sound, Washington, which offers a continuum of diked and un-diked wetlands including seasonal floodplains, open mudflats, mature and tidal forests, and salt marsh habitats. In addition, there is strong restoration potential in a suite of ongoing and proposed projects. We report here on the carbon storage pools, long-term sediment accretion rates (100 years), and estimated rates of carbon storage, derived from sediment cores collected at representative sites within the Snohomish estuary during the spring and summer of 2013. We found that natural wetlands (open to tidal exchange and riverine inputs) were accreting at rates that equaled or exceeded current rates of eustatic sea level rise, while formerly, or currently diked wetlands (closed to such exchanges and inputs) revealed marked evidence of subsidence. Restored sites showed evidence of both high rates of sediment accretion (1.61 cm/year) and carbon storage (352 g C/m2/year)

    Inverse association between blood pressure and pulse oximetry accuracy: an observational study in patients with suspected or confirmed COVID-19 infection

    BackgroundPulse oximeters are a standard non-invasive tool to measure blood oxygen levels, and are used in multiple healthcare settings. It is important to understand the factors affecting their accuracy to be able to use them optimally and safely. This analysis aimed to explore the association of the measurement error of pulse oximeters with systolic BP, diastolic BP and heart rate (HR) within ranges of values commonly observed in clinical practice.MethodsThe study design was a retrospective observational study of all patients admitted to a large teaching hospital with suspected or confirmed COVID-19 infection from February 2020 to December 2021. Data on systolic and diastolic BPs and HR levels were available from the same time period as the pulse oximetry measurements.ResultsData were available for 3420 patients with 5927 observations of blood oxygen saturations as measured by pulse oximetry and ABG sampling within 30 min. The difference in oxygen saturation using the paired pulse oximetry and arterial oxygen saturation difference measurements was inversely associated with systolic BP, increasing by 0.02% with each mm Hg decrease in systolic BP (95% CI 0.00% to 0.03%) over a range of 80ā€“180 mm Hg. Inverse associations were also observed between the error for oxygen saturation as measured by pulse oximetry and with both diastolic BP (+0.03%; 95% CI 0.00% to 0.05%) and HR (+0.04%; 95% CI 0.02% to 0.06% for each unit decrease in the HR).ConclusionsCare needs to be taken in interpreting pulse oximetry measurements in patients with lower systolic and diastolic BPs, and HRs, as oxygen saturation is overestimated as BP and HR decrease. Confirmation of the oxygen saturation with an ABG may be appropriate in some clinical scenarios

The light wave transmission that this technology uses is modified by skin pigmentation and thus may vary by skin colour. A recent study of paired measures of oxygen saturation from pulse oximetry and arterial blood gas reported differing outputs in patients with black skin compared to patients with white skin that has the potential to adversely impact on patient care [2]

    ObjectiveTo explore the associations between arterial pO2, pCO2 and pH and how these are modified by age.MethodsAn analysis of 2598 patients admitted with a diagnosis of Covid-19 infection to a large UK teaching hospital.ResultsThere were inverse associations for arterial pO2, pCO2 and pH with respiratory rate. The effects of pCO2 and pH on respiratory rate were modified by age; older patients had higher respiratory rates at higher pCO2 (p = 0.004) and lower pH (p = 0.007) values.ConclusionsThis suggests that ageing is associated with complex changes in the physiological feedback loops that control respiratory rate. As well as having clinical relevance, this may also impact on the use of respiratory rate in early warning scores across the age range
