    Management of chondroblastoma of the femoral head by controlled hip dislocation technique and use of allograft : case report

    Epiphyseal Chondroblastoma (EC) is known to be a very rare tumor with highly aggressive local capacity despite its benign nature. We report a case of a female patient treated for long lasting right hip pain diagnosed with EC of the femoral head. The lesion was treated with complete local resection by means of controlled surgical hip dislocation, “trap door” technique and use of allograft. Functional results in the short run where adequate with no local recurrence and no metastatic lung lesions.     Bone Marrow-Derived Cells and Peripheral Nerve Injury: Translational Implications for Pain and Regeneration Treatments

    Traumatic injury of peripheral nerves is a serious concern for both patients and clinicians, and is commonly associated to neuropathicpain and complete or partial loss of functionality of the affected limb. Stem cell therapy has emerged as a promising tool to improvethe outcome of patients with peripheral nerve injury, and an increasing number of pre-clinical and clinical studies are adding supporttowards their use in humans. In the present review, we will address specifically the participation of bone marrow stromal cells (BMSC;including a group of multipotent adult progenitor cells (MAP)) and bone marrow mononuclear cells (BMMC, a heterogeneousfraction that contains BMSC populations, among others), both of endogenous origin or exogenously transplanted, for the control ofpain and the improvement of regeneration. We will describe the state-of-the-art knowledge on the cellular and molecularmechanisms involved in the action of BMSC and BMMC during traumatic injury of nerves.     Engineering oral and parenteral amorphous amphotericin B formulations against experimental Trypanosoma cruzi infections

    (Figure Presented) Chagas disease (CD) is a parasitic zoonosis endemic in most mainland countries of Central and South America affecting nearly 10 million people, with 100 million people at high risk of contracting the disease. Treatment is only effective if received at the early stages of the disease. Only two drugs (benznidazole and nifurtimox) have so far been marketed, and both share various limitations such as variable efficacy, many side effects, and long duration of treatment, thus reducing compliance. The in vitro and in vivo efficacy of poly-aggregated amphotericin B (AmB), encapsulated poly-aggregated AmB in albumin microspheres (AmB-AME), and dimeric AmB-sodium deoxycholate micelles (AmB-NaDC) was evaluated. Dimeric AmB-NaDC exhibited a promising selectivity index (SI = 3164) against amastigotes, which was much higher than those obtained for licensed drugs (benznidazole and nifurtimox). AmB-AME, but not AmB-NaDC, significantly reduced the parasitemia levels (3.6-fold) in comparison to the control group after parenteral administration at day 7 postinfection. However, the oral administration of AmB-NaDC (10-15 mg/kg/day for 10 days) resulted in a 75% reduction of parasitemia levels and prolonged the survival rate in 100% of the tested animals. Thus, the results presented here illustrate for the first time the oral efficacy of AmB in the treatment of trypanosomiasis. AmB-NaDC is an easily scalable, affordable formulation prepared from GRAS excipients, enabling treatment access worldwide, and therefore it can be regarded as a promising therapy for trypanosomiasis

    The Changing Landscape for Stroke\ua0Prevention in AF: Findings From the GLORIA-AF Registry Phase 2

    Background GLORIA-AF (Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation) is a prospective, global registry program describing antithrombotic treatment patterns in patients with newly diagnosed nonvalvular atrial fibrillation at risk of stroke. Phase 2 began when dabigatran, the first non\u2013vitamin K antagonist oral anticoagulant (NOAC), became available. Objectives This study sought to describe phase 2 baseline data and compare these with the pre-NOAC era collected during phase 1. Methods During phase 2, 15,641 consenting patients were enrolled (November 2011 to December 2014); 15,092 were eligible. This pre-specified cross-sectional analysis describes eligible patients\u2019 baseline characteristics. Atrial fibrillation disease characteristics, medical outcomes, and concomitant diseases and medications were collected. Data were analyzed using descriptive statistics. Results Of the total patients, 45.5% were female; median age was 71 (interquartile range: 64, 78) years. Patients were from Europe (47.1%), North America (22.5%), Asia (20.3%), Latin America (6.0%), and the Middle East/Africa (4.0%). Most had high stroke risk (CHA2DS2-VASc [Congestive heart failure, Hypertension, Age  6575 years, Diabetes mellitus, previous Stroke, Vascular disease, Age 65 to 74 years, Sex category] score  652; 86.1%); 13.9% had moderate risk (CHA2DS2-VASc = 1). Overall, 79.9% received oral anticoagulants, of whom 47.6% received NOAC and 32.3% vitamin K antagonists (VKA); 12.1% received antiplatelet agents; 7.8% received no antithrombotic treatment. For comparison, the proportion of phase 1 patients (of N = 1,063 all eligible) prescribed VKA was 32.8%, acetylsalicylic acid 41.7%, and no therapy 20.2%. In Europe in phase 2, treatment with NOAC was more common than VKA (52.3% and 37.8%, respectively); 6.0% of patients received antiplatelet treatment; and 3.8% received no antithrombotic treatment. In North America, 52.1%, 26.2%, and 14.0% of patients received NOAC, VKA, and antiplatelet drugs, respectively; 7.5% received no antithrombotic treatment. NOAC use was less common in Asia (27.7%), where 27.5% of patients received VKA, 25.0% antiplatelet drugs, and 19.8% no antithrombotic treatment. Conclusions The baseline data from GLORIA-AF phase 2 demonstrate that in newly diagnosed nonvalvular atrial fibrillation patients, NOAC have been highly adopted into practice, becoming more frequently prescribed than VKA in Europe and North America. Worldwide, however, a large proportion of patients remain undertreated, particularly in Asia and North America.     Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.     Spinal neuropeptide expression and neuropathic behavior in the acute and chronic phases after spinal cord injury: effects of progesterone administration

    Patients with spinal cord injury (SCI) develop chronic pain that severely compromises their quality of life. We have previously reported that progesterone (PG), a neuroprotective steroid, could offer a promising therapeutic strategy for neuropathic pain. In the present study, we explored temporal changes in the expression of the neuropeptides galanin and tyrosine (NPY) and their receptors (GalR1 and GalR2; Y1R and Y2R, respectively) in the injured spinal cord and evaluated the impact of PG administration on both neuropeptide systems and neuropathic behavior. Male rats were subjected to spinal cord hemisection at T13 level, received daily subcutaneous injections of PG or vehicle, and were evaluated for signs of mechanical and thermal allodynia. Real time PCR was used to determine relative mRNA levels of neuropeptides and receptors, both in the acute (1 day) and chronic (28 days) phases after injury. A significant increase in Y1R and Y2R expression, as well as a significant downregulation in GalR2 mRNA levels, was observed 1 day after SCI. Interestingly, PG early treatment prevented Y1R upregulation and resulted in lower NPY, Y2R and GalR1 mRNA levels. In the chronic phase, injured rats showed well-established mechanical and cold allodynia and significant increases in galanin, NPY, GalR1 and Y1R mRNAs, while maintaining reduced GalR2 expression. Animals receiving PG treatment showed basal expression levels of galanin, NPY, GalR1 and Y1R, and reduced Y2R mRNA levels. Also, and in line with previously published observations, PG-treated animals did not develop mechanical allodynia and showed reduced sensitivity to cold stimulation. Altogether, we show that SCI leads to considerable changes in the spinal expression of galanin, NPY and their associated receptors, and that early and sustained PG administration prevents them. Moreover, our data suggest the participation of galaninergic and NPYergic systems in the plastic changes associated with SCI-induced neuropathic pain, and further supports the therapeutic potential of PG- or neuropeptide-based therapies to prevent and/or treat chronic pain after central injuries.Fil: Coronel, Maria Florencia.     Long-lasting ameliorating effects of the oligodeoxynucleotide IMT504 on mechanical allodynia and hindpaw edema in rats with chronic hindpaw inflammation

    Purpose Previously we showed that systemic administration of IMT504 prevents or ameliorates mechanical and thermal allodynia in rats with sciatic nerve crush. Here we analyzed if IMT504 is also effective in reducing mechanical allodynia and inflammation in rats undergoing hindpaw inflammation. Materials and methods: Male Sprague-Dawley rats received unilateral intraplantar injection of complete Freund́s adjuvant (CFA), and were grouped into: 1) untreated CFA, 2) vehicle-treated CFA, 3) IMT504-treated CFA (5 daily (5*) doses of 20, 2 or 0.2 mg/kg, or 3*2 mg/kg). Naïve groups were also included. Finally, early (immediately after intraplantar CFA) and late (7 days after intraplantar CFA) IMT504 treatment protocols were also tested. Hindpaw mechanical allodynia, dorsoventral thickness, edema and cellular infiltration of ipsilateral hindpaws were evaluated in all groups. Results: Untreated CFA rats exhibited mechanical allodynia of quick onset (day 1) and long duration (7 weeks inclusive). Early and late treatments with 5*20 mg/kg IMT504 to CFA rats resulted in both quick and long-lasting antiallodynic effects, as compared to untreated CFA rats. This was also the case in CFA rats undergoing late IMT504 treatment at lower doses (3* and 5*2 mg/kg). Very low doses of IMT504 (5*0.2 mg/kg) only showed a mild improvement in withdrawal threshold, never reaching basal levels. Finally, rats treated with 3* or 5*2 mg/kg or 5*0.2 mg/kg exhibited significant decreases in dorsoventral thickness, edema, and inflammatory cell infiltration of the inflamed hindpaw. Conclusion: Early and late administration of IMT504 results in quick and long-lasting reductions in mechanical allodynia and hindpaw edema. While the mechanisms behind these effects remain to be established, data suggests that IMT504 administration could be a promising strategy in the control of inflammatory pain.Fil: Leiguarda, Candelaria.     Sex-related differences in oxaliplatin-induced changes in the expression of transient receptor potential channels and their contribution to cold hypersensitivity

    Transient receptor potential (TRP) channels are involved in the development of oxaliplatin-induced neuropathic pain, a frequent and debilitating side effect of cancer therapy. Here we explored whether oxaliplatin-induced changes in the expression of TRP channels, as well as the development of pain-related behaviours, differed between male and female animals. Adult rats were injected with oxaliplatin or saline and mechanical and cold allodynia were evaluated using Von Frey and Choi Tests. The mRNA levels of TRPV1, TRPM8 and TRPA1 were assessed in lumbar ganglia and spinal cord by using real time RT-PCR. Oxaliplatin administration induced mechanical and cold hypersensitivity and allodynia in both sexes, with more severe responses to cold stimulation detected in females. Oxaliplatin also induced a significant increase in the expression of TRPV1, TRPM8 and TRPA1 in lumbar dorsal root ganglia. Interestingly, while TRPV1 and TRPA1 upregulation showed no sex difference, the increase in TRPM8 mRNA levels was more pronounced in female ganglia, correlating with the increased sensitivity to innocuous cold stimuli observed in females. TRPV1 and TRPM8 were also found to be upregulated in the spinal cord of animals of both sexes. Our results reveal previously undescribed changes in the expression of TRP channels occurring in peripheral ganglia and spinal cord of both male and female oxaliplatin-treated animals, with some of these changes exhibiting sex-related differences that could underlie the development of sex-specific patterns of pain-related behaviours.Fil: Miguel, Constanza Agata.     Efecto de aceite de orégano adicionado en la dieta sobre la cantidad de mesófilos aerobios detectados en pechuga fresca y congelada de pollo

    Chicken meat is an excellent quality food but it is easily contaminated with aerobic mesophilic. The aim of this research was to determine the effect of dietary oregano oil over the amount of aerobic mesophilic detected on fresh and frozen breast from broilers 35 and 42 days old. A total of 504 broilers were randomly assigned to 4 treatments with 3 replications of 42 broilers each, broilers were fed with starter diet and grower-finisher diet. At 35 and 42 days, 5 broilers were slaughtered, breast meat samples were collected and aerobic mesophilic count was conducted on fresh breast and 30 days frozen breast (-18 °C). Data of the amount of mesophilic CFU per gram of meat obtained were analyzed using a completely randomized design; the comparison of means was made with the Tukey test. In the fresh and frozen breast, the CFU on meat decreased, and the effect was greater in both, fresh and frozen breast meat (P ≤ 0.05) when increasing dietary amount of oregano oil to 400 mg per kg on feed. In conclusion, oregano oil added to the diet decreased the amount of aerobic mesophilic CFU on fresh and frozen broiler breast of 42 days of age, but 400 mg of oregano oil per kg of feed, improved the decrease in the number of aerobic mesophilic.La carne de pollo es un alimento de excelente calidad pero se contamina de mesófilos aerobios con facilidad. El objetivo fue determinar el efecto del aceite de orégano adicionado en la dieta sobre la cantidad de mesófilos aerobios detectados en pechuga fresca y congelada de pollos de engorda de 35 y 42 días de edad. Un total de 504 pollos Ross se dividieron en 4 tratamientos con 3 repeticiones de 42 aves, las aves recibieron una dieta de inicio y una dieta de finalización. A los 35 y 42 días cinco pollos fueron sacrificados, se realizó un muestreo de carne de pechuga para el recuento de mesófilos aerobios en carne fresca y en pechugas congeladas después de 30 días (-18 °C). Los datos de la cantidad de unidades formadoras de colonias por g de carne obtenidos se analizaron por un Diseño Completamente al Azar, la comparación de medias fue realizada con la prueba Tukey. En carne fresca y congelada de aves de 35 y 42 días disminuyó los mesófilos aerobios y el efecto fue mayor al incrementar la cantidad de aceite de orégano en la dieta a 400 mg de aceite de orégano por kg de alimento, tanto en carne fresca como en carne congelada durante 30 días (P ≤ 0.05). En conclusión, el aceite de orégano adicionado en la dieta disminuyó la cantidad de mesófilos aerobios en carne fresca y congelada de pollos de engorda de 35 y 42 días de edad y 400 mg de aceite de orégano por kg de alimento disminuyó mejor la cantidad de mesófilos aerobios en la carne

    The objective was to validate the efficiency of a commercial vaccine tetravalent (E. acervulina, E. maxima, E. tenella, and E.preacox) to control avian coccidiosis in a farm of broiler chickens under conditions of natural challenge. The chickens were divided into two groups of 100 animals each. Group 1 was vaccinated against coccidia (E. acervulina, E. maxima, E. tenella, E. preacox) on the first day of age and animals were fed food without anticoccidiostatos. On the other hand group 2 received an anti coccidiostat in food on the first day of age. Anticoccidial vaccine was applied by ocular spray. The parasite challenge was natural. In group 1 there were found 430 to 16,550 oocytes’ in 100 gr. of feces with chocolaty diarrhea and no mortality by coccidiosis. In group 2 there were found 110 oocytes in 100 gr. of feces with chocolaty diarrhea, nestled position, loss of appetite, bloody diarrhea and feather bristling, finding 8% of mortality by coccidiosis. The use of the quadrivalent vaccine (E. acervulina, E. maxima, E. tenella, E. preacox) against coccidiosis reduces mortality.El objetivo fue validar la eficacia de una vacuna comercial tetravalente (E. acervulina, E. máxima, E. tenella, E. preacox) para el control de coccidiosis aviar en una granja de pollos de engorda bajo condiciones de desafió natural. Los pollos se dividieron en dos grupos de 100 animales. El grupo 1 se vacunó contra coccidia (E. acervulina, E. máxima, E. tenella, E. preacox) al primer día de edad y los animales fueron alimentados con alimento sin anticoccidiostatos. Mientras que el grupo 2 recibió un anticoccidiostato en el alimento, desde el primer día de edad. La vacuna anticoccidial se aplico por aerosol vía ocular. El desafió o reto parasitario fue natural. En el grupo 1 se encontraron de 430 a 16550 de ooquistes en/100 gr.de heces con diarrea chocolatosa, sin mortalidad por coccidiosis. En el grupo 2 se encontraron 110 de ooquistes en/100 gr. de heces fueron con diarrea chocolatosa, posición acurrucada, pérdida de apetito, pluma erizada y diarrea sanguinolenta, encontrando 8% de mortalidad por coccidiosis. EL uso de la vacuna tetravalente, E. acervulina, E. máxima, E. tenella, E. preacox contra la coccidiosis disminuye la mortalidad
