    A patient with sustained monomorphic ventricular tachycardia developed a second tachycardia of incessant character with alternating RR-interval and alternating QRS-complexes after treatment with amiodarone. A retrograde His bundle deflection was recorded during the latter arrhythmia, that was induced as well with atrial as with ventricular stimulation. The "alternating" tachycardia could be explained by alternating bundle branch reentry and alternating conduction in the myocardium. This view is supported by the finding of an alternating mid-diastolic potential. This case report introduces a new electrocardiographic entity in the spectrum of wide QRS-tachycardias

    The effect of perceived quality, perceived value, trust and marketing on purchase intention of organic products in Malaysia

    The organic products industry is facing several challenges in Malaysia although the demand for such product in this country is growing. One of the challenges is that the supply of local organic product is not keeping up with the increased demand. The Jack of organic products in the market is one of the main barriers for these products to reach consumers. Hence, this study was conducted to investigate the effect of perceived quality, perceived value, and green trust on green purchase intention for an organic product in Malaysia. The study also considered marketing as a moderating variable. To meet the objective, a quantitative approach was employed involving a survey. A total of 532 questionnaires Were distributed conveniently to consumers who visited 38 organic food stores in Kuala Lumpur. Of the questionnaires distributed, 400 usable responses were obtained for data analysis, yielding a response rate of 87 percent. A structural equation modelling was applied to analyse the data using the PLS-SEM software. The study applies Theory Planned Behaviour as the underpinning theory. Theory Planned discussed about the underlying factors that influence consumer intention and behaviour. The results show significant relationships between perceived quality, perceived value, and trust on green purchase intention. Moreover, marketing was found to moderate the relationship between independent variables and green purchase intention. This study serves as an important foundation research on green food consumption patterns among Malaysia consumers and provides potential green food marketers in Malaysia with consumer insights into green awareness of organic food products and also in green trust element in organic food products

    1014-92 Impaired Microcirculation in Chronic Heart Failure

    The microcirculation plays a crucial role in the exchange of oxygen, energy rich substrates and metabolites. The effects on the microcirculation of impaired heart function and disturbed peripheral skin circulation in chronic heart failure (CHF) are unknown. Nailfold capillary morphology and dynamics in CHF were studied in relation to parameters of left ventricular (LV) structure and function. Twenty patients (13 male, 7 female, age 64±2 years) with CHF NYHA class II underwent a capillaroscopic examination at the finger nailfold using a computerised videophotometric system (Capiflow) at rest and after 1 min arterial occlusion. Patients were treated with diuretics, ACE inhibitors and digoxin and mean duration of symptoms was 65±9 months. Study parameters were number, length, and diameter of the capillaries as well as capillary blood velocity (CBV). Further experiments included echocardiography and determination of LV ejection fraction by Tc-scintigraphy. Nailfold capillaries in established CHF are enlarged (afferent diameter 6.6±0.4 μm, efferent diameter 8.2±0.4 μm, ref. resp. <6 and <8 μm) and CBV is dramatically decreased (351±64 μ/s, ref. >600 μm/s at 23°C). The reactive hyperemic response to one minute arterial occlusion is attenuated (peak CBV 879±158 μm/s, ref >2000 μm/s). CBV correlates positively with LV ejection fraction (r=0.61, p=001) and inversely with LV end-diastolic (r=-0.56, P=0.04) and end-systolic (r=-0.69, P=0.01) diameters. The time-to-peak flow (35±5 s, ref. 6–10 s) after one min arterial occlusion is positively related (r=0.68, P<0.05) to the duration of CHF. Our data indicate that microcirculation in chronic heart failure deteriorates in function of the severity and duration of heart failure

    In 2014, the debate on the indication of revascularization in case of asymptomatic carotid disease continued, while another one regarding the use of surgery vs. stenting addressed some new issues regarding the long-term cardiac risk of these patients. Renal arteries interventions trials were disappointing, as neither renal denervation nor renal artery stenting was found associated with better blood pressure management or outcome. In contrast, in lower-extremities artery disease, the endovascular techniques represent in 2014 major alternatives to surgery, even in distal arteries, with new insights regarding the interest of drug-eluting balloons. Regarding the aorta, the ESC published its first guidelines document on the entire vessel, emphasizing on the role of every cardiologist for screening abdominal aorta aneurysm during echocardiography. Among vascular wall biomarkers, the aorta stiffness is of increasing interest with new data and meta-analysis confirming its ability to stratify risk, whereas carotid intima-media thickness showed poor performances in terms of reclassifying patients into risk categories beyond risk scores. Regarding the veins, new data suggest the interest of D-dimers and residual venous thrombosis to help the decision of anti-coagulation prolongation or discontinuation after the initial period of treatment for deep vein thrombosi

    Responses of the marine diatom Thalassiosira pseudonana to changes in CO2 concentration: a proteomic approach

    The concentration of CO2 in many aquatic systems is variable, often lower than the KM of the primary carboxylating enzyme Rubisco, and in order to photosynthesize efficiently, many algae operate a facultative CO2 concentrating mechanism (CCM). Here we measured the responses of a marine diatom, Thalassiosira pseudonana, to high and low concentrations of CO2 at the level of transcripts, proteins and enzyme activity. Low CO2 caused many metabolic pathways to be remodeled. Carbon acquisition enzymes, primarily carbonic anhydrase, stress, degradation and signaling proteins were more abundant while proteins associated with nitrogen metabolism, energy production and chaperones were less abundant. A protein with similarities to the Ca2+/ calmodulin dependent protein kinase II_association domain, having a chloroplast targeting sequence, was only present at low CO2. This protein might be a specific response to CO2 limitation since a previous study showed that other stresses caused its reduction. The protein sequence was found in other marine diatoms and may play an important role in their response to low CO2 concentration

    Perceptions on Liking and Disliking Public Agricultural Research Institutes in Uganda: A Case of Abi, Bulindi, Ngetta, Mbarara and Mukono ZARDIs

    A ZARDI wide study project ‘Harnessing the domino effect for transformative technology promotion and outreach’ was carried in three different phases starting from 2018/19, 2019/20, 2020/2021. A multi stage qualitative studies were done through Focus Group Discussions, Key Informant Interviews, and observations in addition to employing the use of Social Resource Maps. Quantitative study carried from the five ZARDIs drew a total of N=1093 respondents. The study was subjected to two different tests carried to confirm; the Kaiser-Meyer-Olkin (KMO) Measure of Sampling Adequacy (MSA) and Bartlett’s test of Sphericity (df) for factors explaining the liking and disliking of the PARIs. The study findings revealed two important factors for liking (Positive PARIs image); reliable and trusted source of technology and information, and exhibits good community relations.  Factors explaining disliking (Negative PARIs image) include; Limited access to information, technologies and markets and in-adequate community outreach programs. This study concludes that, Public Agricultural Research Institutes are not just perceived by their managerial practices nor just defined by its programs and mandates, but they are functions of at least interrelated positive and negative factors.     A bridged low band gap A-D-A quaterthiophene as efficient donor for organic solar cells

    International audience[small alpha],[small omega]-Bis(dicyanovinyl)quaterthiophene 1 with a median 4,4-diethyl-4H-cyclopenta[2,1-b:3,4-b[prime or minute]]dithiophene has been synthesized. UV-Vis absorption data show that the covalent bridging of the inner 2,2[prime or minute]-bithiophene leads to a significant reduction of the HOMO-LUMO gap essentially due to an increase of the HOMO level as confirmed by electrochemical and theoretical results. X-ray diffraction analysis of a single crystal of 1 shows that except for the out-of-plane ethyl groups, the conjugated system displays a quasi-planar geometry while the molecular packing exhibits strong [small pi]-stacking interactions and multiple short intermolecular contacts. Quaterthiophene 1 has been used as active donor material in organic solar cells of various architectures including bi-layer planar hetero-junctions and hybrid co-evaporated bulk hetero-junctions with C60 as electron acceptor material. A maximum conversion efficiency of 4.30% is obtained with a hybrid co-evaporated device. These results are discussed in terms of structure-properties relationships with reference to the open-chain parent [small alpha],[small omega]-bis(dicyanovinyl)quaterthiophene 2
