ESPERIENZA DELLA REUMATOLOGIA PISANA NELLA TERAPIA CON I DIVERSI FARMACI INIBITORI DEL TNF-alfa IN PAZIENTI CON ARTRITE REUMATOIDE VERSO LE SPONDILOARTRITI SIERO NEGATIVE: CONFRONTO DI SICUREZZA ED EFFICACIA
Obbiettivo: lo studio si basa sulla valutazione della sicurezza e dell’ efficacia dei farmaci inibitori del TNF-alfa: nella terapia dell’ artrite reumatoide e delle spondilo-artriti siero negative.
L’obbiettivo è stato valutare se si siano verificati eventi avversi durante il trattamento, nei pazienti in terapia con biologici ( Infliximab, Etanarcept; Adalimubab); se tali eventi siano stati gravi o meno e se abbiano permesso oppure no di proseguire con la terapia. Nel caso in cui l’evento sia stato considerato di media o grave entità il paziente non ha potuto proseguire la terapia e il comportamento è stato: o lo switch ad un altro biologico o la sospensione del trattamento con inibitori del TNF-alfa. In seguito i pazienti che hanno effettuato lo switch sono di nuovo stati analizzati studiando sicurezza ed efficacia.
Inoltre nello studio sono stati valutati tutti quei pazienti che sono andati incontro dopo un iniziale periodo di risposta alla perdita di efficacia. Il comportamento terapeutico è stato: switch o sospensione del biologico e di nuovo abbiamo seguito i pazienti che hanno effettuato lo swtch, valutando ancora sicurezza ed efficacia.
In ultima istanza siamo andati a vedere se vi siano state differenze significative di sicurezza o efficacia tra i pazienti con artrite reumatoide e quelli con spondilo-artriti siero negative.
Materiali e metodi: Totale pazienti trattati fino ad oggi con inibitori del tnf-alfa nella clinica reumatologica di Pisa:363 suddivisi in?138 con INFLIXIMAB, 135 con ETANARCEPT, 90 con ADALIMUBAB
I pazienti trattati con biologici appartengono a 3 categorie di patologie:
228 AR, 15 Of label (12behcet,1takayasu,2hoton), 120 SpondiloArtriti siero negative (SA e AP)
Di questa popolazione generale, di nostro particolare interesse sono stati un gruppo di pazienti che hanno presentato alcuni problemi durante la terapia con biologici, in particolare: eventi avversi, inefficacia del trattamento.
Il nostro campione è composto da: 123 pazienti, di età media 54 anni, composto da 89 femmine e 34 maschi; 78 sono AR, 37 sono Spondiloartriti siero negative (SA e AP), 8 appartengono alla categoria of label.
Lo studio poi si è basato sulla suddivisione della popolazione in 2 categorie: pazienti con AR e pazienti con SS-.
Successivamente ogni categoria è stata suddiviso in 3 gruppi:
GRUPPO N 1 primo biologico: INFLIXIMAB
GRUPPO N 2 primo biologico: ETANARCEPT
GRUPPO N 3 primo biologico: ADALIMUBAB
Infine ogni gruppo è stato analizzato e classificato in 5 sottogruppi:
1. pazienti che nel corso della terapia hanno presento eventi avversi di lieve entità ?prosegue tp
2. pazienti che nel corso della terapia hanno presento eventi avversi di moderata entità ?switch
3. pazienti che nel corso della terapia hanno presento eventi avversi di grave entità ?stop
4. pazienti che nel corso della terapia hanno presentato perdita di efficacia?switch
5. pazienti che nel corso della terapia hanno presentato perdita di efficacia?stop
Risultati:
• 32 pazienti con AR hanno manifestato problemi con la terapia con Infliximab: attualmente 9 effettuano ancora tale trattamento(Infliximab), 11 hanno effettuato 1 o più switch e attualmente sono soggetti ancora a terapia con un biologico; 12 non effettuano più questo tipo di trattamento ( la sospensione può essere avvenuta sia direttamente dal primo biologico, o dopo lo switch ad altri inibitori del TNF-alfa risultati anch’essi inefficacia o causa di gravi eventi avversi).
• 28 pazienti con AR hanno manifestato problemi con la terapia con Etanarcept: attualmente 10 effettuano ancora tale trattamento(Etanarcept), 12 hanno effettuato 1 o più switch e attualmente sono soggetti ancora a terapia con un biologico; 6 non effettuano più questo tipo di ( la sospensione può essere avvenuta sia direttamente dal primo biologico, o dopo lo switch ad altri inibitori del TNF-alfa risultati anch’essi inefficacia o causa di gravi eventi avversi).
• 17 pazienti con AR hanno manifestato problemi con la terapia con Adalimubab: attualmente 12 effettuano ancora tale trattamento(Adalimubab), 5 hanno effettuato 1 o più switch e attualmente sono soggetti ancora a terapia con un biologico; nessuno non effettuano più questo tipo di trattamento.
• 26 pazienti con SS- hanno manifestato problemi con la terapia con Infliximab: attualmente 10 effettuano ancora tale trattamento(Infliximab), 12 hanno effettuato 1 o più switch e attualmente sono soggetti ancora a terapia con un biologico; 4 non effettuano più questo tipo di trattamento ( la sospensione può essere avvenuta sia direttamente dal primo biologico, o dopo lo switch ad altri inibitori del TNF-alfa risultati anch’essi inefficacia o causa di gravi eventi avversi).
• 10 pazienti con SS- hanno manifestato problemi con la terapia con Etanarcept: attualmente 7 effettuano ancora tale trattamento(Etanarcept:), 2 hanno effettuato 1 o più switch e attualmente sono soggetti ancora a terapia con un biologico; 1 non effettuano più questo tipo di trattamento ( la sospensione può essere avvenuta sia direttamente dal primo biologico, o dopo lo switch ad altri inibitori del TNF-alfa risultati anch’essi inefficacia o causa di gravi eventi avversi).
• 1 paziente con SS- ha manifestato problemi con la terapia con Adalimubab: attualmente ha effettuato uno switch e ad un altro biologico.
Conclusione:
Dal nostro studio risulta che in generale la terapia con biologici porta a buoni risultati nel ridurre sintomi e progressione di malattia.
Che il biologico correlato in assoluto ad un maggior numero di eventi avversi è Infliximb; sia nei pazienti affetti da AR che SS- e che non esistono differenze significative dei efficacia o sicurezza tra le 2 categorie.
Inoltre è possibile affermare che in generale il passaggio da un biologico ad un altro è supportato da un buona risposta terapeutica
Regularity of flat free boundaries for two-phase p(x)-Laplacian problems with right hand side
We consider viscosity solutions to two-phase free boundary problems for the -Laplacian with non-zero right hand side. We prove that flat free boundaries are . No assumption on the Lipschitz continuity of solutions is made.
These regularity results are the first ones in literature for two-phase free boundary problems for the -Laplacian and also for two-phase problems for singular/degenerate
operators with non-zero right hand side. They are new even when , i.e., for the -Laplacian.
The fact that our results hold for merely viscosity solutions allows a wide applicability
La delgadez, las dietas hipocalóricas y la salud : un estudio transcultural
La mayorÃa de los estudios transculturales relacionados con cuestiones alimentarias e ima-gen corporal, establecen comparaciones entre nativos occidentales y extranjeros de otras etnias, residentes en el mismo paÃs. A diferencia de los mismos, nos propusimos realizar una comparación parcial entre mujeres argentinas y japonesas que viven en Argentina y Japón, respectivamente, ya que partimos del supuesto de que las pautas culturales están menos contaminadas en el lugar de origen. Dado que la relación peso/altura es un buen indicador del estado nutricional y de salud de una población, y el japonés promedio tiene peso nor-mal, mientras que el argentino tiene sobrepeso, nos resultó de interés analizar qué puede estar influyendo: cuáles son los factores protectores y predisponentes que contribuyen a moldear la conducta alimentaria y la relación con el propio cuerpo. Nuestro objetivo fue in-dagar si en ambos paÃses está presente la tendencia a realizar dietas hipocalóricas y si la misma se fundamenta, entre otras razones, en una percepción sobredimensionada que la delgadez y las dietas hipocalóricas tienen respecto de la salud. Los resultados mostraron que las mujeres argentinas y japonesas consideran que la delgadez es sinónimo de salud. Sin embargo, solo las argentinas perciben como una necesidad permanente el estar a dieta y consideran que los alimentos dietéticos son más saludables. Para esta investigación, cuya metodologÃa es cuantitativa, creamos dos pruebas sencillas que analizan la asociación entre delgadez y salud: Prueba de las fotos y Alimentos preferidos/saludables.Most cross-cultural studies on food behavior and body image, draw comparisons between Western natives and foreigners of other nationalities or cultures, living in the same coun-try. Since the weight/height relation is a good predictor of nutritional status and health of a population, and the average Japanese has normal weight, while Argentina is overweight, we found it interesting to analyze what may be influencing: which are the protective and predisposing factors that shape eating behav-ior and the relationship with one's body. Our aim was to investigate if, in both countries, there is a trend towards low calorie diets and wether it is based, among other reasons, in an oversized perception of the importance that thinness and low-calorie diets have on health. The results showed that Japanese and Argentine women believe that thinness is synonym of health. However, only Argentina's perceived dieting as a permanent need and consider that dietary foods are healthier. For this quantitative research, we created two simple tests that examine the association between thinness and health: Photo Test and Fa-vorite Foods/Healthy Foods

Protective action of ions against cadmium toxicity to young Bufo arenarum tadpoles
Several studies have shown that a large number of amphibian species are capable of tolerating a wide range of external osmolaritles (Bentley 1971; Duellman and Trueb 1986). However, the mechanisms which respond to variations in the osmotic environment are different in larvae and adults. They are also dependent on the way that the osmotic pressure is reached, i.e., through substances which behave as ionic or no-ionic ones In solution (Ballnsky 1981). The eurihalinity of tadpoles was found in species of both Urodela and Anura. The tolerated concentration rate was wide, from distilled water to several hundreds of mOsm. In the particular case of Bufo arenarum, we have demonstrated that its young tadpoles are able to survive when immersed in distilled water, NaCI and mannitol solutions of considerably higher osmolarlties than its natural environment (Ferrari and Sallbián 1967; Ferrarl et al 1988), suggesting the existence of adaptive physiological mechanisms.
We evaluated the impact of Cd(ll) on those mechanisms In the present study. Cadmium is a contaminant widely distributed in freshwater ecosystems (Ravera 1984) and known as an element that interferes in epithelial ionic permeability processes (Hayashi et al. 1977; Hillyard et al. 1979; Takada and Hayashi 1980). We hypothesized that as a consequence of those properties, Cd(II) may alter the already observed ability of Bufo arenarum tadpoles to overcome important changes in the physicochemical characteristics of their incubation media. We evaluated the effect of a sublethal concentration of this metal on the water balance of the animals immersing them in different solutions. Network analysis for complex neurodegenerative diseases
Purpose of Review Biomedicine is witnessing a paradigm shift in the way complex disorders are investigated. In particular, the
need for big data interpretation has led to the development of pipelines that require the cooperation of different fields of expertise,
including medicine, functional biology, informatics, mathematics and systems biology. This review sits at the crossroad of
different disciplines and surveys the recent developments in the use of graph theory (in the form of network analysis) to interpret
large and different datasets in the context of complex neurodegenerative diseases. It aims at a professional audience with different
backgrounds.
Recent Findings Biomedicine has entered the era of big data, and this is actively changing the way we approach and perform
research. The increase in size and power of biomedical studies has led to the establishment of multi-centre, international working
groups coordinating open access platforms for data generation, storage and analysis. Particularly, pipelines for data interpretation
are under development, and network analysis is gaining momentum since it represents a versatile approach to study complex
systems made of interconnected multiple players.
Summary We will describe the era of big data in biomedicine and survey the major freely accessible multi-omics datasets. We
will then introduce the principles of graph theory and provide examples of network analysis applied to the interpretation of
complex neurodegenerative disorders

Monocytes-based in vitro assay for a preliminary biocompatibility assessment of blood-contacting devices
The biological evaluation of biomaterials is currently defined by the ISO-10993 norm in which parts four and five are dedicated to emo-compatibility and cell toxicity, respectively. Our study will provide a novel in vitro experimental approach for the biocompatibility assessment of biomaterials or medical devices using human primary monocytes as cellular model. In these new settings, human monocytes are exposed to a medium containing the extractable compounds derived from materials or devices; subsequently, cell toxicity and pro-inflammatory effects are analysed through MTT assay, flow cytometry and enzyme-linked immunosorbent assay (ELISA) methodologies. These experimental procedures offer the advantage to use a human and primary cell context belonging to the immune system, in order to accurately predict the nature of blood/device interaction occurring during a clinical application. To validate the reliability of this method, we also reported a comparative study between two different membranes showing a different level of biocompatibility. On the bases of these data, it is possible to state that this new experimental model represents a good approach to investigate the effects induced by a biomaterial on cell death and inflammation using human, primary monocytes

Weighted protein interaction network analysis of frontotemporal dementia
The genetic analysis of complex disorders has undoubtedly led to the identification of a wealth of associations between genes and specific traits. However, moving from genetics to biochemistry one gene at a time has, to date, rather proved inefficient and under-powered to comprehensively explain the molecular basis of phenotypes. Here we present a novel approach, weighted protein−protein
interaction network analysis (W-PPI-NA), to highlight key functional players within relevant biological processes associated with a given trait. This is exemplified in the current study by applying W-PPI-NA to frontotemporal dementia (FTD): We first built the state of the art FTD protein network (FTD-PN) and then analyzed both its topological and functional features. The FTD-PN resulted from the sum of the individual interactomes built around FTD-spectrum genes, leading to a total of 4198 nodes. Twenty nine of 4198 nodes, called inter-interactome hubs (IIHs), represented those interactors able to bridge over
60% of the individual interactomes. Functional annotation analysis not only reiterated and reinforced previous findings from single genes and gene-coexpression analyses but also indicated a number of novel potential disease related mechanisms, including DNA damage response, gene expression regulation, and cell waste disposal and potential biomarkers or therapeutic targets including EP300. These processes and targets likely represent the functional core impacted in FTD, reflecting the underlying genetic architecture contributing to disease. The approach presented in this study can be applied to other complex traits for which risk-causative genes are known as it provides a promising tool for setting the foundations for collating genomics and wet laboratory data in a bidirectional manner.
