    'Managing scarcity'- a qualitative study on volunteer-based healthcare for chronically ill, uninsured migrants in Berlin, Germany

    OBJECTIVES: In Germany, healthcare for people lacking legal residency status and European Union citizens without health insurance is often provided by non-governmental organisations. Scientific studies assessing the situation of the patients with chronic diseases in this context are scarce. We aimed to characterise medical care for chronically ill migrants without health insurance and outline its possibilities and limitations from the treating physicians' perspective. DESIGN: Qualitative semi-structured interviews; qualitative content analysis. SETTING: Organisations and facilities providing healthcare for uninsured migrants: free clinics, medical practices and public health services. PARTICIPANTS: 14 physicians working regularly in healthcare for uninsured migrants. RESULTS: Delayed contact to the healthcare system was frequently addressed in the interviews. Care was described as constrained by a scarcity of resources that often impedes adequate treatment for many conditions, most pronounced in the case of oncological diseases or chronic viral infections (HIV, hepatitis). For other chronic conditions such as cardiovascular diseases or diabetes, some diagnostics and basic medications were described as partially available, while management of complications or rehabilitative measures are frequently unfeasible. For the patients with mental health problems, attainability of psychotherapeutic treatment is reported as severely limited. Care is predominantly described as fragmented with limitations to information flow and continuity. Which level of care a patient receives appears to depend markedly on the respective non-governmental organisation and the individual commitment, subjective decisions and personal connections of the treating physician. CONCLUSIONS: Restrictions in medical care for uninsured migrants have even more impact on chronically ill patients. Volunteer-based care often constitutes an inadequate compensation for regular access to the healthcare system, as it is strongly influenced by the limitation of its resources and its arbitrariness

    Robust discretization methods for (nearly-incompressible) linear elasticity are free of volume-locking and gradient-robust. While volume-locking is a well-known problem that can be dealt with in many different discretization approaches, the concept of gradient-robustness for linear elasticity is new. We discuss both aspects and propose novel Hybrid Discontinuous Galerkin (HDG) methods for linear elasticity. The starting point for these methods is a divergence-conforming discretization. As a consequence of its well-behaved Stokes limit the method is gradient-robust and free of volume-locking. To improve computational efficiency, we additionally consider discretizations with relaxed divergence-conformity and a modification which re-enables gradient-robustness, yielding a robust and quasi-optimal discretization also in the sense of HDG superconvergence

    Towards computable flows and robust estimates for inf-sup stable FEM applied to the time-dependent incompressible Navier--Stokes equations

    Inf-sup stable FEM applied to time-dependent incompressible Navier--Stokes flows are considered. The focus lies on robust estimates for the kinetic and dissipation energies in a twofold sense. Firstly, pressure-robustness ensures the fulfilment of a fundamental invariance principle and velocity error estimates are not corrupted by the pressure approximability. Secondly, Re-semi-robustness means that constants appearing on the right-hand side of kinetic and dissipation energy error estimates (including Gronwall constants) do not explicitly depend on the Reynolds number. Such estimates rely on an essential regularity assumption for the gradient of the velocity, which is discussed in detail. In the sense of best practice, we review and establish pressure- and Re-semi-robust estimates for pointwise divergence-free H1-conforming FEM (like Scott--Vogelius pairs or certain isogeometric based FEM) and pointwise divergence-free H(div)-conforming discontinuous Galerkin FEM. For convection-dominated problems, the latter naturally includes an upwind stabilisation for the velocity which is not gradient-based

    Common miRNA patterns of Alzheimer’s disease and Parkinson’s disease and their putative impact on commensal gut microbiota

    With the rise of Next-Generation-Sequencing (NGS) methods, Micro-RNAs (miRNAs) have achieved an important position in the research landscape and have been found to present valuable diagnostic tools in various diseases such as multiple sclerosis or lung cancer. There is also emerging evidence that miRNAs play an important role in the pathogenesis of neurodegenerative diseases such as Alzheimer’s disease (AD) or Parkinson’s disease (PD). Apparently, these diseases come along with changes in miRNA expression patterns which led to attempts from researchers to use these small RNA species from several body fluids for a better diagnosis and in order to observe disease progression. Additionally, it became evident that microbial commensals might play an important role for pathology development and were shown to have a significantly different composition in patients suffering from neurodegeneration compared with healthy controls. As it could recently be shown that secreted miRNAs are able to enter microbial organisms, it is conceivable that the host’s miRNA might affect the gut microbial ecosystem. As such, miRNAs may inherit a central role in shaping the “diseased microbiome” and thereby mutually act on the characteristics of these neurodegenerative diseases. We have therefore 1) compiled a list of miRNAs known to be associated with AD and/or PD, 2) performed an in silico target screen for binding sites of these miRNA on human gut metagenome sequences and 3) evaluated the hit list for interesting matches potentially relevant to the etiology of AD and or PD. The examination of protein identifiers connected to bacterial secretion system, lipopolysaccharide biosynthesis and biofilm formation revealed an overlap of 37 bacterial proteins that were targeted by human miRNAs. The identified links of miRNAs to the biological processes of bacteria connected to AD and PD have yet to be validated via in vivo experiments. However, our results show a promising new approach for understanding aspects of these neurodegenerative diseases in light of the regulation of the microbiome

    Mithilfe des vom BMBF geförderten Forschungsprojekts AktVis soll durch die Entwicklung und Erprobung neuer praxisnaher Entscheidungshilfen sowie interaktiver Planungsinstrumente eine aktive Innenentwicklung unterstützt werden. Denn trotz des breiten Konsens, angefangen bei der Wissenschaft bis hin zur kommunalen Praxis, hinsichtlich der ökologischen, ökonomischen und sozialen Notwendigkeit einer Reduzierung der Flächeninanspruchnahme und Fokussierung städtebaulicher Entwicklungen auf den Siedlungskörper, bestehen weiterhin Defizite in der faktischen Umsetzung. Insbesondere der Zugriff auf Grundstücke, die im Privateigentum stehen, erweist sich als schwierig. Für eine erfolgreiche Aktivierung von Innenentwicklungspotenzialen werden daher die Stakeholder und Schlüsselakteure, also die Eigentümer, Bürger und lokalen Gruppen, in den Kommunen als außerordentlich relevant angesehen. Diese sollen durch eine intensive Einbindung und die aktive Mitgestaltung der Ortskerne für Projekte mobilisiert werden. Mithilfe eines Multitouch-Tisches sowie eines WebGIS werden die Möglichkeiten des selbstbestimmten Editierens von Inhalten und der Veränderungen der Ortschaften gegeben. Die dreidimensionale Visualisierung mit unterschiedlichen Tools soll zur Aktivierung von Potenzialen beitragen

    A Unified Research Data Infrastructure for Catalysis Research – Challenges and Concepts

    Modern research methods produce large amounts of scientifically valuable data. Tools to process and analyze such data have advanced rapidly. Yet, access to large amounts of high‐quality data remains limited in many fields, including catalysis research. Implementing the concept of FAIR data (Findable, Accessible, Interoperable, Reusable) in the catalysis community would improve this situation dramatically. The German NFDI initiative (National Research Data Infrastructure) aims to create a unique research data infrastructure covering all scientific disciplines. One of the consortia, NFDI4Cat, proposes a concept that serves all aspects and fields of catalysis research. We present a perspective on the challenging path ahead. Starting out from the current state, research needs are identified. A vision for a integrating all research data along the catalysis value chain, from molecule to chemical process, is developed. Respective core development topics are discussed, including ontologies, metadata, required infrastructure, IP, and the embedding into research community.
