    O PORTUGUÊS NA MODALIDADE ESCRITA COMO SEGUNDA LÍNGUA PARA SURDOS: UM ESTUDO SOBRE O USO DOS CONECTIVOS

    O presente trabalho procede de uma pesquisa realizada em uma escola pública na cidade de Bagé (RS), cujo objetivo foi analisar o processo de ensino e aprendizagem dos conectivos do português como segunda língua para surdos. Os surdos participantes da pesquisa possuem a Língua Brasileira de Sinais (Libras) como primeira língua e, como segunda língua, a Língua Portuguesa. As bases teóricas desta pesquisa têm como referências principais Quadros (1997; 2004; 2006; 2008) e Ferreira-Brito (1993; 1995). Optou-se por uma perspectiva qualitativa em que os dados foram obtidos por meio de observação de algumas aulas de português e de produções textuais. Este trabalho traz para a discussão a escrita dos surdos e quer contribuir para o direcionamento de um novo olhar avaliativo dessa produção, demonstrando que se deve respeitar a Libras como primeira língua do surdo e o português sua segunda língua

    Processos de letramentos com aluno surdo: o caso Murilo

    This study arose from the need to understand the process of acquisition of Portuguese writing through the practices of literacy for deaf students which have Pounds as their first language and second, the Portuguese. The main objective of this study was to analyze social practices of Literacy contributes in building student writing deaf from the genus letter. The survey was conducted with a deaf student in the 8th grade, elementary school, located in the municipality of Bagé-RS. With this research, we stress the importance of support and encouragement, family and school teaching so that the student can be literate and not only decode or encode the code of the Portuguese language. Stands out, however, that the level of literacy of the deaf student is the result mainly of your willpower and dedication to learn Portuguese and does not influence the teacher. In this sense, though Murilo cannot write using the gender card, the data collected by means of a written work showed evidence of the profile marked by truths revealed, dreams and desires. The results presented here show that there is an explicit awareness of the student as to value pounds, your L1. Also indicate your desire to want to exercise a profession, from actions already implemented, especially when reveals how reads and want to continue reading, seeking information.Esta pesquisa surgiu da necessidade de entender o processo de aquisição da escrita do português através das práticas de letramento por alunos surdos os quais possuem Libras como primeira língua e como segunda, o português. O objetivo principal do presente estudo foi analisar como as práticas sociais de Letramento contribuem na construção escrita do aluno surdo a partir do gênero carta. A pesquisa foi realizada com um aluno surdo, inserido numa turma do 8º ano, de uma escola de ensino fundamental, localizada no município de Bagé- RS. Com esta pesquisa, salientamos a importância do apoio e incentivo pedagógico, familiar e escolar para que o aluno consiga ser letrado e não apenas decodificar ou codificar o código da língua portuguesa. Destaca-se, no entanto, que o nível de letramento do aluno surdo é fruto, principalmente, da sua força de vontade e dedicação em aprender o português e não dá influência do professor. Nesse sentido, a pesar de Murilo não conseguir escrever usando o gênero carta, os dados coletados por meio de uma produção escrita mostraram evidências do perfil marcado por verdades reveladas, sonhos e desejos. Os resultados aqui apresentados mostram que existe uma consciência explícita do aluno quanto a valorizar Libras, sua L1. Também apontam seu desejo de querer exercer uma profissão, a partir de ações já concretizadas, especialmente, quando revela o quanto lê e quer continuar lendo, buscando informações

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.     OBJECTIVE This randomized clinical trial aimed to compare the outcomes of the mandibular overdenture retained by one (1-IOD) or two (2-IOD) implants. MATERIALS AND METHODS Participants received new complete dentures, were assessed at baseline and randomly assigned to groups. Early loaded single midline implant (1-IOD) or two implants in the canine regions bilaterally (2-IOD). Ball attachments were used for overdenture retention. Post-treatment outcomes (6- and 12-month follow-ups) included patient satisfaction, oral health-related quality of life measures, and chewing function. Data analysis included within- and between-group comparisons, and Generalized Estimating Equations. Both superiority and non-inferiority hypotheses were tested. RESULTS Forty-seven participants were included (1-IOD = 23; 2-IOD = 24). Significant improvements in OHIP-Edent were observed after insertion of new dentures and at the 1-year follow-up compared with baseline. No differences were found between the 1- and 2-IOD groups for the OHIP-Edent and QoLFAST scores. Patient satisfaction with the mandibular denture improved significantly from baseline to the 6-month follow-up and remained unaltered until 1 year, with no differences between groups. The magnitudes of treatment effect sizes were moderate to large for patient-reported outcomes. Progressive improvement in chewing function occurred in both groups. Non-inferiority testing based on a 15% non-inferiority margin showed inconclusive results for patient-reported outcomes, whilst chewing function in the 1-IOD group was concluded to be not inferior to the 2-IOD group. CONCLUSIONS Results support the use of both 1- and 2-IOD. The 1-IOD was also an acceptable alternative to the 2-IOD as a secondary option for the treatment of the edentulous mandible
