    Pediatric thyroid disease: when is surgery necessary, and who should be operating on our children?

    Surgical diseases of the thyroid in the pediatric population represent a diverse set of both benign and malignant conditions. Overall, incidence is rare. Benign conditions include Graves' disease, toxic adenomas, congenital hyperthyroidism, and goiter. Differentiated thyroid cancer (DTC) and medullary thyroid carcinoma (MTC), with its related familial cancer syndromes, are the most common malignancies. Near-total or total thyroidectomy is the appropriate surgery for thyroid cancer, with/out central lymph node dissection. Emerging practice guidelines from professional societies are helpful, although they generally have not addressed surgical management of the pediatric patient. Thyroidectomy in children is associated with a higher rate of complications, such as recurrent laryngeal nerve injury and hypoparathyroidism, as compared to the surgery in adults. Therefore, it is essential that pediatric thyroidectomy be performed by high-volume thyroid surgeons, regardless of specialty. Case volume to support surgical expertise usually must be borrowed from the adult experience, given the relative paucity of pediatric thyroidectomies at an institutional level. These surgeons should work as part of a multidisciplinary team that includes pediatric endocrinologists and anesthesiologists, pediatricians, nuclear medicine physicians, and pathologists to afford children the best clinical outcomes

    Global Epidemiology of Lung Cancer.

    While lung cancer has been the leading cause of cancer-related deaths for many years in the United States, incidence and mortality statistics - among other measures - vary widely worldwide. The aim of this study was to review the evidence on lung cancer epidemiology, including data of international scope with comparisons of economically, socially, and biologically different patient groups. In industrialized nations, evolving social and cultural smoking patterns have led to rising or plateauing rates of lung cancer in women, lagging the long-declining smoking and cancer incidence rates in men. In contrast, emerging economies vary widely in smoking practices and cancer incidence but commonly also harbor risks from environmental exposures, particularly widespread air pollution. Recent research has also revealed clinical, radiologic, and pathologic correlates, leading to greater knowledge in molecular profiling and targeted therapeutics, as well as an emphasis on the rising incidence of adenocarcinoma histology. Furthermore, emergent evidence about the benefits of lung cancer screening has led to efforts to identify high-risk smokers and development of prediction tools. This review also includes a discussion on the epidemiologic characteristics of special groups including women and nonsmokers. Varying trends in smoking largely dictate international patterns in lung cancer incidence and mortality. With declining smoking rates in developed countries and knowledge gains made through molecular profiling of tumors, the emergence of new risk factors and disease features will lead to changes in the landscape of lung cancer epidemiology

    Integer and fractional charge Lorentzian voltage pulses analyzed in the frame of Photon-assisted Shot Noise

    The periodic injection nn of electrons in a quantum conductor using periodic voltage pulses applied on a contact is studied in the energy and time-domain using shot noise computation in order to make comparison with experiments. We particularly consider the case of periodic Lorentzian voltage pulses. When carrying integer charge, they are known to provide electronic states with a minimal number of excitations, while other type of pulses are all accompanied by an extra neutral cloud of electron and hole excitations. This paper focuses on the low frequency shot noise which arises when the pulse excitations are partitioned by a single scatterer in the framework of the Photo Assisted Shot Noise (PASN) theory. As a unique tool to count the number of excitations carried per pulse, shot noise reveals that pulses of arbitrary shape and arbitrary charge show a marked minimum when the charge is integer. Shot noise spectroscopy is also considered to perform energy-domain characterization of the charge pulses. In particular it reveals the striking asymmetrical spectrum of Lorentzian pulses. Finally, time-domain information is obtained from Hong Ou Mandel like noise correlations when two trains of pulses generated on opposite contacts collide on the scatterer. As a function of the time delay between pulse trains, the noise is shown to measure the electron wavepacket autocorrelation function for integer Lorentzian thanks to electron antibunching.     Seawater acidification more than warming presents a challenge for two Antarctic macroalgal-associated amphipods

    Elevated atmospheric pCO2 concentrations are triggering seawater pH reductions and seawater temperature increases along the western Antarctic Peninsula (WAP). These factors in combination have the potential to influence organisms in an antagonistic, additive, or synergistic manner. The amphipods Gondogeneia antarctica and Paradexamine fissicauda represent prominent members of macroalgal-associated mesograzer assemblages of the WAP. Our primary objective was to investigate amphipod behavioral and physiological responses to reduced seawater pH and elevated temperature to evaluate potential cascading ecological impacts. For 90 d, amphipods were exposed to combinations of seawater conditions based on present ambient (pH 8.0, 1.5°C) and predicted end-of-century conditions (pH 7.6, 3.5°C). We recorded survival, molt frequency, and macroalgal consumption rates as well as change in wet mass and proximate body composition (protein and lipid). Survival for both species declined significantly at reduced pH and co-varied with molt frequency. Consumption rates in G. antarctica were significantly higher at reduced pH and there was an additive pH-temperature effect on consumption rates in P. fissicauda. Body mass was reduced for G. antarctica at elevated temperature, but there was no significant effect of pH or temperature on body mass in P. fissicauda. Exposure to the pH or temperature levels tested did not induce significant changes in whole body biochemical composition of G. antarctica, but exposure to elevated temperature resulted in a significant increase in whole body protein content of P. fissicauda. Our study indicates that while elevated temperature causes sub-lethal impacts on both species of amphipods, reduced pH causes significant mortality
