    Cesagen response to Nuffield Council on bioethics consultation on novel neurotechnologies:intervening in the brain

    In what follows, we do not answer every question [by the Nuffield Council on Bioethics). We first proceed with our comments, referring to the numbered questions as appropriate. Thereafter, we give a case study from recent studies within Cesagen to illustrate more general insights for public policy. Case study 1 illustrates some of the complications that arise in public consultation about human enhancement, in particular, with reference to idealistic perceptions which are strongly influenced by long-term popular imaginations about the future of humans and their societies. As we said in a response to a previous consultation, our position is that attention needs to be paid to how the technologies and the associated issues are framed – ethically, politically, scientifically, and by whom. This includes how a given technology is itself described (typically well before it actually exists, if it comes to do so); the claims made for its purported benefits; how stakeholders are conceptualised; how social-cultural aspects will evolve. Such framing is not exclusively a scientific and technological matter but involves cultural and social imaginations as well as artistic ones

    The physical activity experiences of men with serious mental illness: Three short stories

    Objectives: Although a considerable amount of research has explored the effects of physical activity on mental health, the voices of people with mental illness have been largely excluded from published reports. Through this study we aim to foreground service users' voices in order to shed light on the personal and subjective nature of the relationship between physical activity and serious mental illness (SMI). Methods: An interpretive case study approach was used to explore in depth the physical activity experiences of three men with SMI. Creative analytic practice was used to write three creative non-fictions which, as first-person narratives, foreground the participants' voices. Results: We present three short stories in an effort to communicate participants' personal and subjective experiences of physical activity in an accessible, engaging, and evocative manner. We hope to: (i) provide potentially motivating physical activity success stories for others who live with SMI; (ii) increase awareness among mental health professionals of the possibilities of physical activity; and (iii) provide an empathetic understanding of possibilities and problems of living with SMI which may help challenge the stigma surrounding mental illness. Conclusions: For us, the stories communicate the diversity and difference inherent in the ways men with SMI experience physical activity. We reflect on how the short story form allows these differences to be preserved and respected. We resist making further interpretations of the stories preferring instead to encourage the reader to form her or his own conclusions. © 2007 Elsevier Ltd. All rights reserved

    The Effect of Age of Acquisition in Visual Word Processing: Further Evidence for the Semantic Hypothesis

    The authors investigated whether the meaning of visually presented words is activated faster for early-acquired words than for late-acquired words. They addressed the issue using the semantic Simon paradigm. In this paradigm, participants are instructed to decide whether a stimulus word is printed in uppercase or lowercase letters. However, they have to respond with a verbal label ("living" or "nonliving") that is either congruent with the meaning of the word (e.g., saying "living" to the stimulus DOG) or incongruent (e.g., saying "nonliving" to the stimulus dog). Results showed a significant congruency effect that was stronger for early-acquired words than for late-acquired words. The authors conclude that the age of acquisition is an important variable in the activation of the meaning of visually presented words. In the past decade, researchers have revived the question regarding the extent to which the frequency effect in visual word recognition is a confound of the age at which words have been acquired (hence called age of acquisition One hypothesis is that part of the AoA effect originates from the semantic system. According to this explanation, the order of acquisition has a lasting effect on the time needed to activate the meanings of words. Empirical evidence for this idea comes from the word-associate generation task (Brysbaert, Theoretical support for a semantic involvement in the AoA effect comes from simulations with models based on both distributed and localist representations. The distributed account attributes the AoA effect to differences in the connection weights between the units of the orthographic and the semantic layers; the localist account attributes it to the organization of the semantic system

    Shame, social deprivation, and the quality of the voice-hearing relationship.

    OBJECTIVES: Many individuals hold different beliefs about the voices that they hear and have distinct relationships with them, the nature of which may determine the distress experienced. Understanding what factors contribute to these beliefs and relationships and consequently the resulting distress is important. The current research examined whether shame and social deprivation, in a sample of adult voice-hearers, were related to the relationships that individuals had with their voices or the beliefs that they held about them. DESIGN: The study utilized a cross-sectional, Internet-based design. METHODS: Eighty-seven adult voice-hearers from England were recruited to the online survey. Participants completed measures regarding shame, beliefs about voices, and relationships with voices and provided demographic information and postcodes that were used to refer to Index of Multiple Deprivation data. RESULTS: Social deprivation and shame were not associated. Shame was positively associated with variables describing negative voice-hearing beliefs/relationships but not positive voice-hearing beliefs/relationships. Principal component analysis on the eight voice-hearing variables yielded two components related to positive and negative voice-hearing qualities. A multiple regression conducted on the two components identified that shame was only associated with negative voice-hearing qualities. CONCLUSIONS: The results suggest that therapies that target shame may be helpful when working with negative voice-hearing beliefs and relationships. Future research should utilize experimental or longitudinal designs to examine the direction of the relationship. PRACTITIONER POINTS: The results contribute to the limited research evidence available regarding the relationship between shame and voice-hearing. The results suggest the utility of psychological therapies that focus on shame such as compassion-focused therapy and that conceptualize voices interpersonally such as cognitive analytic therapy. No conclusions can be made regarding causation. The sample size was relatively small, and results cannot be generalized to other areas of the United Kingdom. Future research should utilize experimental and longitudinal designs to examine the impact of shame on voice-hearing experiences and to examine other factors that may predict shame

    Impact of a quadrivalent meningococcal ACWY glycoconjugate or a serogroup B meningococcal vaccine on meningococcal carriage: an observer-blind, phase 3 randomised clinical trial

    Background: Meningococcal conjugate vaccines protect individuals directly, but also confer herd protection by interrupting carriage transmission. This Phase III observer-blind, randomised, controlled study evaluated the effects of meningococcal quadrivalent (ACWY) glycoconjugate (MenACWY-CRM) or serogroup B (4CMenB) vaccination on meningococcal carriage rates in young adults. Methods: University students (aged 18–24 years) from ten sites in England were randomised to receive two vaccinations one month apart: two doses of Japanese Encephalitis vaccine (controls), two doses of 4CMenB (4CMenB), or one dose of MenACWY-CRM then placebo (MenACWY-CRM). Meningococci were isolated from oropharyngeal swabs collected before vaccination and at five scheduled intervals over one year. Primary analysis was cross-sectional carriage one month after the vaccine course; secondary analyses included comparison of carriage at any time point after primary analysis until study termination. Findings: 2954 subjects were randomised (control, n=987; 4CMenB, n=988; MenACWY-CRM, n=979); approximately one-third of each group was positive for meningococcal carriage at study entry. By one month, there was no significant difference in carriage between controls and 4CMenB (Odds Ratios (OR) [95% CI]; 1·2 [0·8−1·7]) or MenACWY-CRM (OR [95% CI], 0·9 [0·6–1·3]) groups. From three months after dose two, 4CMenB vaccination resulted in significantly lower carriage of any meningococcal strain (calculated efficacy 18·2% [95% CI: 3·4–30·8]) and capsular groups BCWY (calculated efficacy 26·6% [95% CI: 10·5–39·9]) compared to control vaccination. Significantly lower carriage rates were also observed in the MenACWY-CRM group compared with controls: calculated efficacies 39·0% [95%CI: 17·3-55·0] and 36.2% [95%CI: 15·6-51·7] for serogroups Y and CWY, respectively. Interpretation: MenACWY-CRM and 4CMenB vaccines reduced meningococcal carriage rates over 12 months post-vaccination and, therefore, may affect transmission where widely implemented
