Nutrición enteral domiciliaria en España: registro Nadya del año 2011-2012
Objective: To describe the results of the home enteral
nutrition (HEN) registry of the NADYA-SENPE group in
2011 and 2012.
Material and methods: We retrieved the data of the patients
recorded from January 1st 2011 to December 31st 2012.
Results: There were 3021 patients in the registry
during the period from 29 hospitals, which gives 65.39 per
million inhabitants. 97.95% were adults, 51.4% male.
Mean age was 67.64 ± 19.1, median age was 72 years for
adults and 7 months for children. Median duration with
HEN was 351 days and for 97.5% was their first event
with HEN. Most patients had HEN because of neurological
disease (57.8%). Access route was nasogastric tube
for 43.5% and gastrostomy for 33.5%. Most patients had
limited activity level and, concerning autonomy, 54.8%
needed total help. Nutritional formula was supplied from
chemist’s office to 73.8% of patients and disposables,
when necessary, was supplied from hospitals to 53.8% of
patients. HEN was finished for 1,031 patients (34.1%)
during the period of study, 56.6% due to decease and
22.2% due to recovery of oral intake.
Conclusions: Data from NADYA-SENPE registry
must be explained cautiously because it is a non-compulsory
registry. In spite of the change in the methodology of
the registry in 2010, tendencies regarding HEN have been
maintained, other than oral routeObjetivos: Describir los resultados del registro de nutrición

Material y métodos: Se recopilaron los datos introducidos
en el registro desde el 1 de enero de 2011 al 31 de diciembre
de 2012.
Resultados: Hubo 3021 pacientes en el registro durante
el periodo, procedentes de 29 hospitales, lo que da una
prevalencia de 65,39 casos por millón de habitantes.
97.95% fueron adultos, 51,4% varones. La edad media
fue 67,64 ± 19,1 años y la mediana 72 años para los adultos
y 7 meses para los niños. La duración media de la NED
fue 351 días y para el 97,5% fue el primer episodio con
NED. La mayoría de pacientes tenían NED por una enfermedad
neurológica (57,8%). La vía de acceso fue sonda
nasogástrica para el 43,5% y gastrostomía para el 33,5%.
La mayoría de pacientes tuvieron un nivel de actividad física
limitado y, respecto a la autonomía, 54,8% necesitaba
ayuda total. La fórmula de nutrición se suministró desde
las oficinas de farmacia para el 73,8% y los fungibles,
cuando fueron necesarios, desde los hospitales para el
53,8%. La NED se suspendió en 1.031 pacientes (34,1%)
durante el periodo de estudio, 56,6% debido a fallecimiento
y 22,2% debido a recuperación de la vía oral.
Conclusiones: Los datos del registro NADYA-SENPE
deben ser interpretados con precaución ya que se trata de
un registro voluntario. A pesar del cambio de metodología
del registro en 2010, las tendencias en NED se han mantenido,
salvo la importancia cuantitativa de la vía ora
Global respiratory syncytial virus–related infant community deaths
Background
Respiratory syncytial virus (RSV) is a leading cause of pediatric death, with >99% of mortality occurring in low- and lower middle-income countries. At least half of RSV-related deaths are estimated to occur in the community, but clinical characteristics of this group of children remain poorly characterized.
Methods
The RSV Global Online Mortality Database (RSV GOLD), a global registry of under-5 children who have died with RSV-related illness, describes clinical characteristics of children dying of RSV through global data sharing. RSV GOLD acts as a collaborative platform for global deaths, including community mortality studies described in this supplement. We aimed to compare the age distribution of infant deaths <6 months occurring in the community with in-hospital.
Results
We studied 829 RSV-related deaths <1 year of age from 38 developing countries, including 166 community deaths from 12 countries. There were 629 deaths that occurred <6 months, of which 156 (25%) occurred in the community. Among infants who died before 6 months of age, median age at death in the community (1.5 months; IQR: 0.8−3.3) was lower than in-hospital (2.4 months; IQR: 1.5−4.0; P < .0001). The proportion of neonatal deaths was higher in the community (29%, 46/156) than in-hospital (12%, 57/473, P < 0.0001).
Conclusions
We observed that children in the community die at a younger age. We expect that maternal vaccination or immunoprophylaxis against RSV will have a larger impact on RSV-related mortality in the community than in-hospital. This case series of RSV-related community deaths, made possible through global data sharing, allowed us to assess the potential impact of future RSV vaccines
Testing a global standard for quantifying species recovery and assessing conservation impact.
Recognizing the imperative to evaluate species recovery and conservation impact, in 2012 the International Union for Conservation of Nature (IUCN) called for development of a "Green List of Species" (now the IUCN Green Status of Species). A draft Green Status framework for assessing species' progress toward recovery, published in 2018, proposed 2 separate but interlinked components: a standardized method (i.e., measurement against benchmarks of species' viability, functionality, and preimpact distribution) to determine current species recovery status (herein species recovery score) and application of that method to estimate past and potential future impacts of conservation based on 4 metrics (conservation legacy, conservation dependence, conservation gain, and recovery potential). We tested the framework with 181 species representing diverse taxa, life histories, biomes, and IUCN Red List categories (extinction risk). Based on the observed distribution of species' recovery scores, we propose the following species recovery categories: fully recovered, slightly depleted, moderately depleted, largely depleted, critically depleted, extinct in the wild, and indeterminate. Fifty-nine percent of tested species were considered largely or critically depleted. Although there was a negative relationship between extinction risk and species recovery score, variation was considerable. Some species in lower risk categories were assessed as farther from recovery than those at higher risk. This emphasizes that species recovery is conceptually different from extinction risk and reinforces the utility of the IUCN Green Status of Species to more fully understand species conservation status. Although extinction risk did not predict conservation legacy, conservation dependence, or conservation gain, it was positively correlated with recovery potential. Only 1.7% of tested species were categorized as zero across all 4 of these conservation impact metrics, indicating that conservation has, or will, play a role in improving or maintaining species status for the vast majority of these species. Based on our results, we devised an updated assessment framework that introduces the option of using a dynamic baseline to assess future impacts of conservation over the short term to avoid misleading results which were generated in a small number of cases, and redefines short term as 10 years to better align with conservation planning. El Acuerdo de paz en Colombia : entre la perfidia y la potencia transformadora
La lucha por la implementación del Acuerdo de Paz transcurre de manera simultánea en dos sentidos. Por una parte, como necesidad de superar y derrotar en forma definitiva las resistencias de sectores que liderados por las expresiones más extremistas de la derecha política buscan desconocer la realidad histórica y concreta del Acuerdo de Paz, en cuanto posibilidad de tramitar el conflicto social y d clase por la vía exclusivamente política. Pero, al mismo tiempo, por la otra, como necesidad también de una implementación integral, de construir la legitimidad de una concepción de la paz que trascienda la visión meramente pacificadora o gatopardista, es decir, que apunte a una paz que efectivamente posea los contenidos propios de la democracia verdadera y la justicia social en camino hacias las transformaciones más profundas de la sociedad colombiana. Y, por qué no decirlo, que se encauce hacia la superación definitiva del orden social vigente; aspiración que también se encontraba tanto en los orígenes como en la larga historia de la lucha guerrillera en Colombia
Abacavir/Lamivudine plus Rilpivirine Is an Effective and Safe Strategy for HIV-1 Suppressed Patients: 48 Week Results of the SIMRIKI Retrospective Study.
OBJECTIVES: Based on data from clinical practice, we evaluated the effectiveness and safety of switching to abacavir/lamivudine plus rilpivirine (ABC/3TC+RPV) treatment in virologically suppressed HIV-1-infected patients. METHODS: We performed a multicenter, non-controlled, retrospective study of HIV-1-infected patients who switched treatment to ABC/3TC+RPV. Patients had an HIV-RNA <50 copies/mL for at least 24 weeks prior to changing treatments. The primary objective was HIV-1 RNA <50 copies/mL at week 48. Effectiveness was analyzed by intention-to-treat (ITT), missing = failure and on-treatment (OT) analyses. The secondary objectives analyzed were adverse effects changes in renal, hepatic or lipid profiles, changes in CD4+ cell count and treatment discontinuations. RESULTS: Of the 205 patients included, 75.6% were men and the median age was 49. At baseline, before switching to ABC/3TC+RPV, median time since HIV diagnosis was 13.1 years, median time with undetectable HIV-1 RNA was 6.2 years and median time of previous antiretroviral regimen was 3.1 years (48.3% patients were taking efavirenz and ABC/3TC was the most frequent backbone coformulation in 69.7% of patients). The main reasons for switching were drug toxicity/poor tolerability (60.5%) and simplification (20%). At week 48, the primary objective was achieved by 187 out of 205 (91.2%) patients by ITT analysis, and 187 out of 192 (97.4%) patients by OT analysis. The CD4+ lymphocyte count and CD4+ percentage increased significantly from baseline to week 48 by a median of 48 cells/μL (-50 to 189) and 1.2% (-1.3% to 4.1%), respectively, P<0.001. Thirty-eight adverse events (AE) were detected in 32 patients. Of these, 25 had no clear association with treatment. Three patients interrupted therapy due to AE. We observed a decrease in all lipid parameters, P<0.001, and a slight improvement in the glomerular filtration rate, P<0.01. Therapy was considered to have failed in 18 patients owing to virological failure (5 [2.4%]), toxicity/poor tolerability (4 [2%]), clinical decision (3 [1.5%]), loss to follow-up (3 [1.5%]), death (1 [0.5%]), and no clinical data (2 [1%]). CONCLUSIONS: The results of this study confirms that ABC/3TC+RPV is an effective, safe, and cost-effective option for the treatment of patients with virologically stable HIV-1 infection.The study was supported by ViiV Healthcare and SEIMC-GESIDA Foundation (grant number Gesida-8314). Quorum sensing network in clinical strains of A. baumannii : AidA is a new quorum quenching enzyme
Acinetobacter baumannii is an important pathogen that causes nosocomial infections generally associated with high mortality and morbidity in Intensive Care Units (ICUs). Currently, little is known about the Quorum Sensing (QS)/Quorum Quenching (QQ) systems of this pathogen. We analyzed these mechanisms in seven clinical isolates of A. baumannii. Microarray analysis of one of these clinical isolates, Ab1 (A. baumannii ST-2-clon-2010), previously cultured in the presence of 3-oxo-C12-HSL (a QS signalling molecule) revealed a putative QQ enzyme (α/β hydrolase gene, AidA). This QQ enzyme was present in all nonmotile clinical isolates (67% of which were isolated from the respiratory tract) cultured in nutrient depleted LB medium. Interestingly, this gene was not located in the genome of the only motile clinical strain growing in this medium (A. baumannii strain Ab421-GEIH-2010 [Ab7], isolated from a blood sample). The AidA protein expressed in E. coli showed QQ activity. Finally, we observed downregulation of the AidA protein (QQ system attenuation) in the presence of HO (ROS stress). In conclusion, most of the A. baumannii clinical strains were not surface motile (84%) and were of respiratory origin (67%). Only the pilT gene was involved in surface motility and related to the QS system. Finally, a new QQ enzyme (α/β hydrolase gene, AidA protein) was detected in these strains
