    La presente tesis presenta la propuesta de un procedimiento metodológico de aplicación del Sistema de Costos Basado en Actividades ABC en el área de alimentos y bebidas de una empresa hotelera “Hostería Durán S.A.” , de la ciudad de Cuenca, la misma que utiliza un sistema de costeo común donde distribuye los costos indirectos de fabricación directamente hacia los productos finales mediante una tasa predeterminada, siendo nuestro propósito proponer este sistema como un instrumento de gestión empresarial que considere cada una de las actividades para la determinación real de los costos incurridos en el área de estudio. Para aplicar este sistema, se consideró el año 2013, seleccionando los cinco platos de la carta más apetecidos por los clientes de la Hostería. La metodología a emplearse es la aplicación práctica de la propuesta, mediante la investigación documental, la observación directa que nos permita establecer cómo se desarrollan las actividades del personal de la empresa en el área de Alimentos y Bebidas, y el posterior procesamiento, análisis e interpretación de los datos obtenidos durante la investigación Para nuestra investigación aplicaremos los métodos deductivo e inductivo, en donde partimos del conocimiento general al momento de la venta de un producto en el área de estudio, hasta llegar a establecer la propuesta metodológica y aplicación del sistema de Costos ABC, luego de la cual utilizaremos la información obtenida para plantear las conclusiones y recomendaciones a las que se llegaron al culminar el trabajo investigativo que serán de mucha utilidad para Hostería Durán S.A.This thesis presents a proposed methodological procedure of the System Based Cost Activity ABC in the area of food and beverages in a company hotel “Duran Hostry SA", in Cuenca city, which uses the same system common costing which distributes manufacturing overhead costs directly to the final products using a predetermined rate, and our purpose to propose this system as a business management tool that considers each of the activities to determine the actual costs incurred in the study area. To implement this system, is considered the year 2013, selecting the five menu items most desired by customers at Hostry. The methodology used is the practical application of the proposal through documentary literature, direct observation allows us to establish how the activities of personnel company, that are related to Food and Drinks area, and the subsequent processing, analysis and interpretation of the data obtained during the research. For our research, we will apply deductive and inductive methods, where we take a general knowledge at the time of sale the product in the study area up to establish the methodology and system implementation costs ABC. After that, we will use the obtained information to raise the conclusions and recommendations that came to complete the project research, which will be very useful for “Duran Hostry” S.A.Contador Público AuditorCuenc

    Cabbage and fermented vegetables : From death rate heterogeneity in countries to candidates for mitigation strategies of severe COVID-19

    Large differences in COVID-19 death rates exist between countries and between regions of the same country. Some very low death rate countries such as Eastern Asia, Central Europe, or the Balkans have a common feature of eating large quantities of fermented foods. Although biases exist when examining ecological studies, fermented vegetables or cabbage have been associated with low death rates in European countries. SARS-CoV-2 binds to its receptor, the angiotensin-converting enzyme 2 (ACE2). As a result of SARS-CoV-2 binding, ACE2 downregulation enhances the angiotensin II receptor type 1 (AT(1)R) axis associated with oxidative stress. This leads to insulin resistance as well as lung and endothelial damage, two severe outcomes of COVID-19. The nuclear factor (erythroid-derived 2)-like 2 (Nrf2) is the most potent antioxidant in humans and can block in particular the AT(1)R axis. Cabbage contains precursors of sulforaphane, the most active natural activator of Nrf2. Fermented vegetables contain many lactobacilli, which are also potent Nrf2 activators. Three examples are: kimchi in Korea, westernized foods, and the slum paradox. It is proposed that fermented cabbage is a proof-of-concept of dietary manipulations that may enhance Nrf2-associated antioxidant effects, helpful in mitigating COVID-19 severity.Peer reviewe

    Nrf2-interacting nutrients and COVID-19 : time for research to develop adaptation strategies

    There are large between- and within-country variations in COVID-19 death rates. Some very low death rate settings such as Eastern Asia, Central Europe, the Balkans and Africa have a common feature of eating large quantities of fermented foods whose intake is associated with the activation of the Nrf2 (Nuclear factor (erythroid-derived 2)-like 2) anti-oxidant transcription factor. There are many Nrf2-interacting nutrients (berberine, curcumin, epigallocatechin gallate, genistein, quercetin, resveratrol, sulforaphane) that all act similarly to reduce insulin resistance, endothelial damage, lung injury and cytokine storm. They also act on the same mechanisms (mTOR: Mammalian target of rapamycin, PPAR gamma:Peroxisome proliferator-activated receptor, NF kappa B: Nuclear factor kappa B, ERK: Extracellular signal-regulated kinases and eIF2 alpha:Elongation initiation factor 2 alpha). They may as a result be important in mitigating the severity of COVID-19, acting through the endoplasmic reticulum stress or ACE-Angiotensin-II-AT(1)R axis (AT(1)R) pathway. Many Nrf2-interacting nutrients are also interacting with TRPA1 and/or TRPV1. Interestingly, geographical areas with very low COVID-19 mortality are those with the lowest prevalence of obesity (Sub-Saharan Africa and Asia). It is tempting to propose that Nrf2-interacting foods and nutrients can re-balance insulin resistance and have a significant effect on COVID-19 severity. It is therefore possible that the intake of these foods may restore an optimal natural balance for the Nrf2 pathway and may be of interest in the mitigation of COVID-19 severity

    Post-anaesthesia pulmonary complications after use of muscle relaxants (POPULAR): a multicentre, prospective observational study

    Background Results from retrospective studies suggest that use of neuromuscular blocking agents during general anaesthesia might be linked to postoperative pulmonary complications. We therefore aimed to assess whether the use of neuromuscular blocking agents is associated with postoperative pulmonary complications. Methods We did a multicentre, prospective observational cohort study. Patients were recruited from 211 hospitals in 28 European countries. We included patients (aged ≥18 years) who received general anaesthesia for any in-hospital procedure except cardiac surgery. Patient characteristics, surgical and anaesthetic details, and chart review at discharge were prospectively collected over 2 weeks. Additionally, each patient underwent postoperative physical examination within 3 days of surgery to check for adverse pulmonary events. The study outcome was the incidence of postoperative pulmonary complications from the end of surgery up to postoperative day 28. Logistic regression analyses were adjusted for surgical factors and patients’ preoperative physical status, providing adjusted odds ratios (ORadj) and adjusted absolute risk reduction (ARRadj). This study is registered with ClinicalTrials.gov, number NCT01865513. Findings Between June 16, 2014, and April 29, 2015, data from 22803 patients were collected. The use of neuromuscular blocking agents was associated with an increased incidence of postoperative pulmonary complications in patients who had undergone general anaesthesia (1658 [7·6%] of 21694); ORadj 1·86, 95% CI 1·53–2·26; ARRadj –4·4%, 95% CI –5·5 to –3·2). Only 2·3% of high-risk surgical patients and those with adverse respiratory profiles were anaesthetised without neuromuscular blocking agents. The use of neuromuscular monitoring (ORadj 1·31, 95% CI 1·15–1·49; ARRadj –2·6%, 95% CI –3·9 to –1·4) and the administration of reversal agents (1·23, 1·07–1·41; –1·9%, –3·2 to –0·7) were not associated with a decreased risk of postoperative pulmonary complications. Neither the choice of sugammadex instead of neostigmine for reversal (ORadj 1·03, 95% CI 0·85–1·25; ARRadj –0·3%, 95% CI –2·4 to 1·5) nor extubation at a train-of-four ratio of 0·9 or more (1·03, 0·82–1·31; –0·4%, –3·5 to 2·2) was associated with better pulmonary outcomes. Interpretation We showed that the use of neuromuscular blocking drugs in general anaesthesia is associated with an increased risk of postoperative pulmonary complications. Anaesthetists must balance the potential benefits of neuromuscular blockade against the increased risk of postoperative pulmonary complications
