Management of acute diverticulitis with pericolic free gas (ADIFAS). an international multicenter observational study

    Get PDF
Background: There are no specific recommendations regarding the optimal management of this group of patients. The World Society of Emergency Surgery suggested a nonoperative strategy with antibiotic therapy, but this was a weak recommendation. This study aims to identify the optimal management of patients with acute diverticulitis (AD) presenting with pericolic free air with or without pericolic fluid. Methods: A multicenter, prospective, international study of patients diagnosed with AD and pericolic-free air with or without pericolic free fluid at a computed tomography (CT) scan between May 2020 and June 2021 was included. Patients were excluded if they had intra-abdominal distant free air, an abscess, generalized peritonitis, or less than a 1-year follow-up. The primary outcome was the rate of failure of nonoperative management within the index admission. Secondary outcomes included the rate of failure of nonoperative management within the first year and risk factors for failure. Results: A total of 810 patients were recruited across 69 European and South American centers; 744 patients (92%) were treated nonoperatively, and 66 (8%) underwent immediate surgery. Baseline characteristics were similar between groups. Hinchey II-IV on diagnostic imaging was the only independent risk factor for surgical intervention during index admission (odds ratios: 12.5, 95% CI: 2.4-64, P =0.003). Among patients treated nonoperatively, at index admission, 697 (94%) patients were discharged without any complications, 35 (4.7%) required emergency surgery, and 12 (1.6%) percutaneous drainage. Free pericolic fluid on CT scan was associated with a higher risk of failure of nonoperative management (odds ratios: 4.9, 95% CI: 1.2-19.9, P =0.023), with 88% of success compared to 96% without free fluid ( P <0.001). The rate of treatment failure with nonoperative management during the first year of follow-up was 16.5%. Conclusion: Patients with AD presenting with pericolic free gas can be successfully managed nonoperatively in the vast majority of cases. Patients with both free pericolic gas and free pericolic fluid on a CT scan are at a higher risk of failing nonoperative management and require closer observation

The common imaginary in the research environment is that scientific dissemination is outside the real task of science and this is only the generation of new knowledge, validated by the importance given to aspects such as publication in top journals or in generally indexed by large indices or publishers.

Knowledge about suicidal behavior is beginning to take on great relevance due to the number of lives that are lost daily as a result of this phenomenon. Thus, there are more and more neurocognitive theories that seek to explain suicide, specifically those that study inhibitory control that generate more controversy as to whether or not there is a direct relationship. Thus, the objective of this narrative review is to explore the current state of knowledge regarding the link between inhibitory control and suicidal behavior. Throughout this investigation, discrepancies were found in which various authors consider that there is a relationship between suicidal behavior and inhibitory control based on the results obtained from the instruments applied in relation to the executive functions applied. In the same way, it can also be observed that the debate arises when low scores are evidenced that indicate a deterioration of cognitive inhibition in suicidal behavior, which fluctuate from cases with slight differences in subjects with ideation or previous attempts, to cases in which a high connection between the two is estimated.

Cytokine-based Predictive Models to Estimate the Probability of Chronic Periodontitis: Development of Diagnostic Nomograms

    Get PDF
Although a distinct cytokine profile has been described in the gingival crevicular fluid (GCF) of patients with chronic periodontitis, there is no evidence of GCF cytokine-based predictive models being used to diagnose the disease. Our objectives were: to obtain GCF cytokine-based predictive models; and develop nomograms derived from them. A sample of 150 participants was recruited: 75 periodontally healthy controls and 75 subjects affected by chronic periodontitis. Sixteen mediators were measured in GCF using the Luminex 100™ instrument: GMCSF, IFNgamma, IL1alpha, IL1beta, IL2, IL3, IL4, IL5, IL6, IL10, IL12p40, IL12p70, IL13, IL17A, IL17F and TNFalpha. Cytokine-based models were obtained using multivariate binary logistic regression. Models were selected for their ability to predict chronic periodontitis, considering the different role of the cytokines involved in the inflammatory process. The outstanding predictive accuracy of the resulting smoking-adjusted models showed that IL1alpha, IL1beta and IL17A in GCF are very good biomarkers for distinguishing patients with chronic periodontitis from periodontally healthy individuals. The predictive ability of these pro-inflammatory cytokines was increased by incorporating IFN gamma and IL10. The nomograms revealed the amount of periodontitis-associated imbalances between these cytokines with pro-inflammatory and anti-inflammatory effects in terms of a particular probability of having chronic periodontitisThis work was supported by project EM2014/025 from Regional Ministry of Culture, Education and University (regional government of Galicia, Spain), which is integrated in Regional Plan of Research, Innovation and Development 2011–2015S

The “Joker” laugh: Social judgments towards affective deviants in a sample of young offenders with callous unemotional traits

    No full text
We assessed negative bias in Colombian young offenders towards affective deviants (those who violate emotional norms). Postulating that affective deviants elicit an “uncanny/creepy” feeling resembling that produced by psychopaths, we explored social judgments of affective deviants in individuals with callous-unemotional (CU) traits. 188 young offenders evaluated a target displaying congruent or incongruent affective displays in response to pictures eliciting positive/negative affect, depending on the condition. Affective deviants were rated as creepier and less trustworthy, and more likely to have bad moral character, than targets who displayed normative affect. Further, affective deviants who displayed positive affect in response to negative stimuli were rated as having worse moral character than those who displayed negative affect in response to positive stimuli. CU traits predicted lower trustworthiness judgments of targets in congruent conditions, but higher trustworthiness judgments of targets in incongruent conditions. CU traits also predicted higher desired social distance and creepiness judgments in congruent conditions. Creepiness ratings correlated with judgments of bad moral character, suggesting that this emotional response may be involved in moral evaluations of strangers. These findings indicate that deviant affective displays produce a variety of negative social judgments, with CU traits playing a role in these social cognitive heuristics.E.G.C. acknowledges the predoctoral contract FPU18/00365 granted by the Ministerio de Ciencia, Innovación y Universidades
