    Prevalência de casos de hanseníase no município de Votuporanga (SP) no período de 2014 a 2018

    Leprosy is caused by Hansen's bacillus, which affects the skin and peripheral nervous system. Brazil has a detection rate of 12.2 / 100 thousand inhabitants. The objective of the research was to analyze the cases of leprosy in the municipality of Votuporanga / SP, between 2014 to 2018. Data were collected from Datasus at the end of 2018. Of the 33 cases, 51, 5% were in males and 48, 5% in females, the majority between the ages of 30 and 59 (58%). The mode of entry was the predominance of new cases (76%). Routing was the most commonly used detection mode (52%); the spontaneous demand, 18%, examination of contacts, 18% and others,  12%. As to the clinical form, 34% were virchowiana, 24% dimorphic, 27% tuberculoid and 15% undetermined, thus, the Multibacillary form (58%) and the Paucibacilar form (42%) were highlighted The therapeutic regimen confirms the data already mentioned, with more prevalent MDT / MB / 12 doses (58%). The affected skin lesions and nerves were mostly ignored. The degree of disability was found to be more significant (79%), 15% had disability grade 1 and 6% were already in grade 2. The type of output with the highest percentage was the cure (70%), transfers reached  6%  and 21% don’t appear in the records. The municipality studied did not reach the goal of elimination and presents many contagious forms. It is extremely important to identify and treat cases; and to invest in disability preventions.A Hanseníase também conhecida como mal de Hansen é causada pelo bacilo denominado Mycobacterium Leprae, que afeta pele e sistema nervoso periférico. Agravo de transmissão respiratória que afeta 30% da humanidade que entra em contato com o microrganismo. No Brasil a taxa de detecção é de 12,2/100 mil hab. O objetivo da pesquisa foi analisar os casos de hanseníase no município de Votuporanga/SP, no período entre 2014 a 2018. Os dados foram coletados do Datasus no final de 2018. Dos 33 casos, 51,5% foram no sexo masculino e 48,5% no sexo feminino, sendo a maioria entre a faixa etária de 30 a 59 anos (58%). O modo de entrada houve predomínio de casos novos (76%). O encaminhamento foi o modo de detecção mais utilizado (52%); já a demanda espontânea ficou com 18%, exame de contatos 18% e outros ficaram com 12%. Quanto à forma clínica 34% foram virchowiana, 24% dimorfa, 27% tuberculóide e 15% indeterminada, sendo assim destacou-se a forma Multibacilar (58%) e a forma Paucibacilar ficou com 42%. O esquema terapêutico corrobora com os dados já citados, sendo mais prevalente PQT/MB/12 doses (58%).  As lesões cutâneas e os nervos afetados foram ignorados na maior parte dos casos. O grau 0 de incapacidade demonstrou-se mais significativo (79%), 15% teve grau 1 de incapacidade e 6% já estão no grau 2. Em relação ao tipo de saída com maior porcentagem foi a cura com 70%, 6% foi transferência e 21% não consta nos registros. O município estudado não atingiu a meta de eliminação e apresenta muitas formas contagiantes. É de extrema importância identificar e tratar os casos; e investir na prevenção das incapacidades

    Meckel’s diverticulum, a congenital malformation of the gastrointestinal tract, is asymptomatic in the majority of patients but can be associated with some complications. Gastrointestinal bleeding is one such complication and is more common in children than in adults. Despite the variety of examinations available, diagnosis can be difficult, especially in older patients, because the sensitivity of examinations decreases with patient age.     Remdesivir and corticosteroids in the treatment of hospitalized COVID-19 patients

    okCoronavirus disease 2019 (COVID-19) is a pandemic infection caused by the newly discovered severe acute respiratory syndrome coronavirus 2. Remdesivir (RDV) and corticosteroids are used mainly in COVID-19 patients with acute respiratory failure. The main objective of the study was to assess the effectiveness of remdesivir with and without corticosteroids in the treatment of COVID-19 patients. We conducted a prospective observational study, including adult patients consecutively hospitalized with confirmed COVID-19 and acute respiratory failure. Patients were divided according to treatment strategy: RDV alone versus RDV with corticosteroids. The primary outcome was the time to recovery in both treatment groups. We included 374 COVID-19 adult patients, 184 were treated with RDV, and 190 were treated with RDV and corticosteroid. Patients in the RDV group had a shorter time to recovery in comparison with patients in the RDV plus corticosteroids group at 28 days after admission [11 vs. 16 days (95% confidence Interval 9.7-12.8; 14.9-17.1; p = .016)]. Patients treated with RDV alone had a shorter length of hospital stay. The use of corticosteroids as adjunctive therapy of RDV was not associated with improvement in mortality of COVID-19 patients.publishersversionpublishe

Published by BMJ.OBJECTIVES: Since the outbreak of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), the pressure to minimise its impact on public health has led to the implementation of different therapeutic strategies, the efficacy of which for the treatment of coronavirus disease 2019 (COVID-19) was unknown at the time. Remdesivir (REM) was granted its first conditional marketing authorisation in the EU in June 2020. The European Medicines Agency (EMA) and local health authorities all across the EU have since strongly recommended the implementation of pharmacovigilance activities aimed at further evaluating the safety of this new drug. The objective of this study was to evaluate adverse drug reactions (ADRs) attributed to either REM or hydroxychloroquine (HCQ) in patients hospitalised for COVID-19 in Centro Hospitalar de Lisboa Ocidental, a Portuguese hospital centre based in Lisbon. We present the preliminary results reporting plausible adverse effects of either HCQ or REM. METHODS: An observational cohort study was carried out between 16 March and 15 August 2020. Participants were divided into two cohorts: those prescribed an HCQ regimen, and those prescribed REM. Suspected ADRs were identified using an active monitoring model and reported to the Portuguese Pharmacovigilance System through its online notification tool. The ADR cumulative incidence was compared between the two cohorts. RESULTS: The study included 149 patients, of whom 101 were treated with HCQ and the remaining 48 with REM. The baseline characteristics were similar between the two cohorts. A total of 102 ADRs were identified during the study period, with a greater incidence in the HCQ cohort compared with the REM cohort (47.5% vs 12.5%; p<0.001). Causality was assessed in 81 ADRs, all of which were considered possible. CONCLUSIONS: Real-world data are crucial to further establish the safety profile for REM.     Complicações necrosantes e intercorrências na rinomodelação com ácido hialurônico

    Objetivo: Descrever os efeitos adversos, intercorrências e complicações do preenchimento com ácido hialurônico na região de nariz, indicando possíveis causas para os problemas apresentados após o procedimento. Metodologia: Trata-se de uma revisão integrativa realizada nas seguintes bases de dados: Medline via PubMed, SciELO e Lilacs, com artigos incluídos no período 2013 a 2023, utilizando os descritores, Ácido hialurônico; Necrose; Preenchedores dérmicos; Nariz. A busca integrada foi realizada unindo os descritores com o operador booleano “AND”. Foram analisadas 9 publicações. Resultados: Foi realizada a identificação dos trabalhos selecionados, elencando-se os dados relacionados à título, autor, ano de publicação, tipo de estudo, metodologia, principais intercorrências encontradas e a conclusão do estudo. Após análise dos estudos, observou-se que a ocorrência de complicações após a aplicação de ácido hialurônico não é frequente. Para que não ocorra o agravamento, é necessário que os primeiros sinais e sintomas sejam analisados para assim seguir com o protocolo mais adequado a fim de prevenir ou reverter. Conclusão: Seguindo passos simples de segurança e tendo um conhecimento anatômico refinado, os preenchedores podem ser uma boa ferramenta para um aprimoramento seguro e abrangente da rinomodelação

    Synthesis, biological evaluation, and molecular modeling of nitrile-containing compounds : exploring multiple activities as anti-Alzheimer agents

Eighty‐three nitrile‐containing compounds, 13 of which are new, were synthesized and evaluated. in vitro screening revealed that 31 , a new compound, presented the best lead for trifunctional inhibition against MAO A (0.34 μM), MAO B (0.26 μM), and AChE (52 μM), while 32 exhibited a lead for selective MAO A (0.12 μM) inhibition coupled to AChE (48 μM) inhibition. Computational analysis revealed that the malononitrile group can find an advantageous position with the aromatic cleft and FAD of MAO A or MAO B. However, the total binding energy can be handicapped by an internal penalty caused by twisting of the ligand molecule and subsequent disruption of the conjugation ( 32 in MAO B compared to the conjugated 31 ). Conjugation is also important for AChE as well as the hydrophilic character of malononitrile that allows this group to be in close contact with the aqueous environment as seen for 83 . Although the effect of 31 and 32 against Aβ1–42, was very weak, the effect of 63 and 65 , and of the new compound 75 , indicated that these compounds were able to disaggregate Aβ1–42 fibrils. The most effective was 63 , a (phenylhydrazinylidene)propanedinitrile derivative that also inhibited MAO A (1.65 μM), making it a potential lead for Alzheimer's disease application.PostprintPeer reviewe

    Conhecimento e uso de plantas medicinais pelo município de São João do Paraíso – Maranhão / Knowledge and use of medicinal plants by the municipality of São João do Paraíso - Maranhão

    As plantas medicinais têm sido amplamente utilizadas por diferentes comunidades e culturas para diversos propósitos desde os tempos antigos. Portanto, o objetivo deste estudo foi determinar o uso e o conhecimento de plantas medicinais no município de São João do Paraíso, Maranhão. A pesquisa foi realizada por meio de um questionário online no mês de setembro para investigar as plantas mais conhecidas, formas de uso, partes utilizadas, finalidades terapêuticas, descendentes do conhecimento popular e fontes de onde essas plantas foram obtidas. A pesquisa recebeu um total de 90 respostas, sendo citadas mais de 30 plantas medicinais, as folhas foram consideradas como a parte mais utilizada da planta, além de 93% dos entrevistados considerarem o uso de plantas medicinais eficazes. Isso indica que as plantas medicinais são amplamente utilizadas e valorizadas pela população como uma alternativa econômica para o tratamento de doenças e desconfortos nestes anos. Portanto, é necessário apoiar pesquisas futuras para comprovar a eficiência e métodos do uso das plantas medicinais
