    Bloodstream Infection Due to Piperacillin/Tazobactam Non-Susceptible, Cephalosporin Susceptible Escherichia Coli: A Missed Opportunity for DE-Escalation of Therapy

    An increasing number of reports describing Escherichia coli isolates with piperacillin/tazobactam resistance, despite retained cephalosporin susceptibility, suggest further emergence of this phenotypic resistance pattern. In this report, a patient with metastatic breast cancer presented to medical care after two days of chills, nausea, vomiting, reduced oral intake, and generalized weakness. Blood and urine cultures grew E. coli as identified by rapid diagnostics multiplex PCR and MALDI-TOF, respectively. The patient continued to manifest signs of sepsis with hypotension and tachypnea during the first three days of hospitalization despite empirical antimicrobial therapy with intravenous piperacillin/tazobactam. After in vitro antimicrobial susceptibility testing demonstrated a piperacillin/tazobactam minimal inhibitory concentration (MIC) of 64 and a ceftriaxone MIC of ≤1 mcg/mL, antimicrobial therapy was switched from intravenous piperacillin/tazobactam to ceftriaxone. All symptoms and signs of infection resolved within 48 h of starting ceftriaxone therapy. This report describes the clinical failure of piperacillin/tazobactam in the treatment of a bloodstream infection due to E. coli harboring a phenotypic resistance pattern of isolated piperacillin/tazobactam non-susceptibility. The case demonstrates the role of cephalosporins as potential treatment options and highlights the value of early de-escalation of antimicrobial therapy based on rapid diagnostic testing for microbial identification

    Memory usage verification using Hip/Sleek.

    Embedded systems often come with constrained memory footprints. It is therefore essential to ensure that software running on such platforms fulfils memory usage specifications at compile-time, to prevent memory-related software failure after deployment. Previous proposals on memory usage verification are not satisfactory as they usually can only handle restricted subsets of programs, especially when shared mutable data structures are involved. In this paper, we propose a simple but novel solution. We instrument programs with explicit memory operations so that memory usage verification can be done along with the verification of other properties, using an automated verification system Hip/Sleek developed recently by Chin et al.[10,19]. The instrumentation can be done automatically and is proven sound with respect to an underlying semantics. One immediate benefit is that we do not need to develop from scratch a specific system for memory usage verification. Another benefit is that we can verify more programs, especially those involving shared mutable data structures, which previous systems failed to handle, as evidenced by our experimental results

    D-Branes and Fluxes in Supersymmetric Quantum Mechanics

    Type 0A string theory in the (2,4k) superconformal minimal model backgrounds, with background ZZ D-branes or R-R fluxes can be formulated non-perturbatively. The branes and fluxes have a description as threshold bound states in an associated one-dimensional quantum mechanics which has a supersymmetric structure, familiar from studies of the generalized KdV system. The relevant bound state wavefunctions in this problem have unusual asymptotics (they are not normalizable in general, and break supersymmetry) which are consistent with the underlying description in terms of open and closed string sectors. The overall organization of the physics is very pleasing: The physics of the closed strings in the background of branes or fluxes is captured by the generalized KdV system and non-perturbative string equations obtained by reduction of that system (the hierarchy of equations found by Dalley, Johnson, Morris and Watterstam). Meanwhile, the bound states wavefunctions, which describe the physics of the ZZ D-brane (or flux) background in interaction with probe FZZT D-branes, are captured by the generalized mKdV system, and non-perturbative string equations obtained by reduction of that system (the Painleve II hierachy found by Periwal and Shevitz in this context).Comment: 41 pages, LaTe

    Prostate cancer is a sizeable threat to the health and well-being of men living in the United States, and African American men suffer at a disproportionately higher rate compared with their Caucasian counterparts (American Cancer Society, 2014). Prostate cancer occurs most frequently in older men, but it occurs at an earlier age in African Americans; the differences in tumor type and disease aggressiveness or progression between Caucasian and African American men may drive the disparity (Powell, Bock, Ruterbusch, & Sakr, 2010; Roberts, 2014). Education is important in bringing people into the cancer care continuum, which begins with prevention and screening. Participatory approaches to educating individuals and communities about prostate cancer and informed decision making (IDM) about screening may be an important step in addressing cancer disparities

The definitive version is available at http://hea.sagepub.comArea-based initiatives (ABIs) have formed an important part of public policy towards more socio-economically deprived areas in many countries. Co-ordinating service provision within and across sectors has been a common feature of these initiatives. Despite sustained policy interest in ABIs, little empirical work has explored relations between ABI providers and partnership development within this context remains under-theorised. This paper addresses both of these gaps by exploring partnerships as a social and developmental process, drawing on concepts from figurational sociology to explain how provider relations develop within an ABI. Qualitative methods were used to explore, prospectively, the development of an ABI targeted at a town in the north west of England. A central finding was that, although effective delivery of ABIs is premised on a high level of coordination between service providers, the pattern of interdependencies between providers limits the frequency and effectiveness of cooperation. In particular, the interdependency of ABI providers with others in their organisation (what is termed here ‘organisational pull’) constrained the ways in which they worked with providers outside of their own organisations. ‘Local’ status, which could be earned over time, enabled some providers to exert greater control over the way in which provider relations developed during the course of the initiative. These findings demonstrate how historically constituted social networks, within which all providers are embedded, shape partnership development. The theoretical insight developed here suggests a need for more realistic expectations among policy makers about how and to what extent provider partnerships can be managed.     Annulus Amplitudes in the Minimal Superstring

    We study the annulus amplitudes in the (2,4) minimal superstring theory using the continuum worldsheet approach. Our results reproduce the semiclassical behavior of the wavefunctions of FZZT-branes recently studied in hep-th/0412315 using the dual matrix model. We also study the multi-point functions of neutral FZZT-branes and find the agreement between their semiclassical limit and the worldsheet annulus calculation.Comment: 15 pages, lanlma
