The digital revolution has greatly changed the approach to culture, work, leisure time, the way we interact and communicate with people: the digitization of information has taken root and has become essential in all areas of social life as a phenomenon that guides and monitors the transformation of society in all its various forms, bringing great changes in almost all areas and especially in the world of work and industry. Industry 4.0 refers to a fourth industrial revolution (following water/steam power, mass production and automation through IT and robotics) and introduces the concept of "cyber-physical systems" to differentiate this new evolutionary phase from the electronic automation that has gone before. The term Industry 4.0 refers to the next phase in the digitization of the manufacturing sector, driven by four disruptions: 1) the astonishing rise in data volumes, computational power, and connectivity, especially new low-power wide-area networks; 2) the emergence of analytics and business-intelligence capabilities; 3) new forms of human-machine interaction such as touch interfaces and augmented-reality systems; 4) improvements in transferring digital instructions to the physical world, such as advanced robotics and 3-D printing. One of the core elements of the fourth industrial revolution is the systematic application of IoT technologies (Internet of Things) and IoM (Internet of Me) to production processes on a global scale. The aim of the paper is primarily an analysis of what are the possible future benefits of IoT and IoM, trying to understand the effort and their opportunities for a consumer oriented business. Then to analyze the competitive advantage that companies are pursuing when they implement a business which is IoT / IoM oriented. The project research question relates to the possible extrapolation of a standardized behavioural pattern, in order to draw guidelines for companies who want to start a similar strategic management, through the direct experiences of an Italian start-up first mover group in the sectors involved in IoT and IoM revolution, defining risks, opportunities and threats. Theresearch presents two phases methodologically joinedbetween them: from a theoretical point of view it is defined the change from the Business Model towards the “ecosystem” of Business Model in the strategy IoT/IoM oriented, and from a empirical point of view the business development of some start-up companies using IoT and IoM technologies is studied, through the direct interview qualitative tool

Fully Polarizable QM/Fluctuating Charge Approach to Two-Photon Absorption of Aqueous Solutions

We present the extension of the quantum/classical polarizable fluctuating charge model to the calculation of single residues of quadratic response functions, as required for the computational modeling of two-photon absorption cross-sections. By virtue of a variational formulation of the quantum/classical polarizable coupling, we are able to exploit an atomic orbital-based quasienergy formalism to derive the additional coupling terms in the response equations. Our formalism can be extended to the calculation of arbitrary order response functions and their residues. The approach has been applied to the challenging problem of one- and two-photon spectra of rhodamine 6G (R6G) in aqueous solution. Solvent effects on one- and two-photon spectra of R6G in aqueous solution have been analyzed by considering three different approaches, from a continuum (QM/PCM) to two QM/MM models (non-polarizable QM/TIP3P and polarizable QM/FQ). Both QM/TIP3P and QM/FQ simulated OPA and TPA spectra show that the inclusion of discrete water solvent molecules is essential to increase the agreement between theory and experiment. QM/FQ has been shown to give the best agreement with experiments

Mechanical prophylaxis of venous thromboembolism in ill hospitalized medical patients: evidence and guidelines

Venous thromboembolism (VTE) represents one of the leading causes of mortality and morbidity in ill medical patients. Avoiding VTE is therefore of utmost importance in clinical practice. VTE prophylaxis can be assured by pharmacological strategies, such as heparinoids, unfractioned heparin, low molecular weight heparins, fondaparinux or oral anticoagulants and, when these are contraindicated, by mechanical measures, such as graduated compression stockings (GCS) and/ or intermittent pneumatic compression (ICP). However, due to the lack of solid literature evidence, VTE mechanical prophylaxis is not standardized in hospitalized ill medical patients. Much recently, findings from randomized clinical trials on VTE prophylaxis in ill medical patients, such as CLOTS I in patients with stroke and LIFENOX in patients with other kind of medical diseases, seem to increase doubts and reduce certainness in this context and recommendations from guidelines don't help in reducing confusion. Therefore the aim of this review is to focus on mechanical prophylaxis of VTE in hospitalized ill medical patients.http://dx.doi.org/10.7175/rhc.v3i3.20

The Risk-based Treatment of Acute Pulmonary Embolism

Risk evaluation and prognostic stratification based upon clinical and radiological findings and new cardiac biomarkers, such as natriuretic peptides (NP) and troponins, represent key points in modern management of acute pulmonary embolism (PE). Literature evidence shows that normotensive PE with right heart dysfunction (RHD), defined as submassive PE, has poorer prognosis when compared to normotensive PE without RHD, defined as non-massive PE; thus whether submassive PE should be managed more aggressively and with closer monitoring represents the crucial question about acute PE treatment. Although the answer is yet unclear, the most recent guidelines address to thrombolysis as treatment choice in selected high risk patients with submassive PE. Guidelines also clarify the indications for unfractioned and low molecular weight heparins and fondaparinux. Therefore, in the present article, the authors focus on modern risk-based therapeutic guidelines of acute PE
