    A hard lesson: Assessing the HTTPS deployment of Italian university websites

    In this paper we carry out a systematic analysis of the state of the HTTPS deployment of the most popular Italian university websites. Our analysis focuses on three different key aspects: HTTPS adoption and activation, HTTPS certificates, and cryptographic TLS implementations. Our investigation shows that the current state of the HTTPS deployment is unsatisfactory, yet it is possible to significantly improve the level of security by working exclusively at the web application layer. We hope this observation will encourage site operators to take actions to improve the current state of protection

    Interval Sentinel Lymph Nodes: An Unusual Localization in Patients with Cutaneous Melanoma

    Background. Recent studies have demonstrated that there exists a great variation in the lymphatic drainage in patients with malignant melanoma. Some patients have drainage to lymph nodes outside of conventional nodal basins. The lymph nodes that exist between a primary melanoma and its regional nodal basin are defined “interval nodes”. Interval node occurs in a small minority of patients with forearm melanoma. We report our experience of the Melanoma Unit of University Hospital Spedali Civili Brescia, Italy. Methods. Lymphatic mapping using cutaneous lymphoscintigraphy (LS) has become a standard preoperative diagnostic procedure to locate the sentinel lymph nodes (SLNs) in cutaneous melanoma. We used LS to identify sentinel lymph nodes biopsy (SLNB) in 480 patients. Results. From over 2100 patients affected by cutaneous melanoma, we identified 2 interval nodes in 480 patients with SLNB . The melanomas were both located in the left forearm. The interval nodes were also both located in the left arm. Conclusion. The combination of preoperative LS and intraoperative hand-held gamma detecting probe plays a remarkable role in identifying these uncommon lymph node locations. Knowledge of the unusual drainage patterns will help to ensure the accuracy and the completeness of sentinel nodes identification

    Clinical and molecular characterization of 40 patients with classic Ehlers--Danlos syndrome: identification of 18 COL5A1 and 2 COL5A2 novel mutations.

    Classic Ehlers-Danlos syndrome (cEDS) is a rare autosomal dominant connective tissue disorder that is primarily characterized by skin hyperextensibility, abnormal wound healing/atrophic scars, and joint hypermobility. A recent study demonstrated that more than 90% of patients who satisfy all of these major criteria harbor a type V collagen (COLLV) defect. This cohort included 40 patients with cEDS who were clinically diagnosed according to the Villefranche nosology. The flowchart that was adopted for mutation detection consisted of sequencing the COL5A1 gene and, if no mutation was detected, COL5A2 analysis. In the negative patients the presence of large genomic rearrangements in COL5A1 was investigated using MLPA, and positive results were confirmed via SNP-array analysis. We report the clinical and molecular characterization of 40 patients from 28 families, consisting of 14 pediatric patients and 26 adults. A family history of cEDS was present in 9 patients. The majority of the patients fulfilled all the major diagnostic criteria for cEDS; atrophic scars were absent in 2 females, skin hyperextensibility was not detected in a male and joint hypermobility was negative in 8 patients (20% of the entire cohort). Wide inter- and intra-familial phenotypic heterogeneity was observed. We identified causal mutations with a detection rate of approximately 93%. In 25/28 probands, COL5A1 or COL5A2 mutations were detected. Twenty-one mutations were in the COL5A1 gene, 18 of which were novel (2 recurrent). Of these, 16 mutations led to nonsense-mediated mRNA decay (NMD) and to COLLV haploinsufficiency and 5 mutations were structural. Two novel COL5A2 splice mutations were detected in patients with the most severe phenotypes. The known p. (Arg312Cys) mutation in the COL1A1 gene was identified in one patient with vascular-like cEDS. Our findings highlight that the three major criteria for cEDS are useful and sufficient for cEDS clinical diagnosis in the large majority of the patients. The borderline patients for whom these criteria fail can be diagnosed when minor signs of connective tissue diseases and family history are present and when genetic testing reveals a defect in COLLV. Our data also confirm that COL5A1 and COL5A2 are the major, if not the only, genes involved in cEDS

    Manejo de açaizeiros (Euterpe oleracea Mart.) cultivados em Latossolo Amarelo na Amazônia.

    Apesar do grande significado socioeconômico que o açaizeiro (Euterpe oleracea Mart.) tem para a região amazônica, como produtor de fruto destinados à alimentação das populações locais e como principal fornecedor de matéria-prima para a indústria de palmito, pouco se conhece sobre as práticas de cultivo que propiciem maior rendimento para a cultura. Com o objetivo de determinar o efeito de diferentes números de estirpes por planta na produtividade de frutos e em algumas características morfológicas de açaizeiros, foi conduzido um experimento em solo de terra firme, no município de Belém, utilizando-se um delineamento estatístico de blocos, com quatro tratamentos (Testemunha - sem desbaste, quatro estirpes por planta, seis estirpes por planta e oito estirpes por planta) com cinco repetições. A produção de frutos, acumulada durante três anos, foi maior no tratamento com quatro estirpes por planta. O diâmetro à altura do peito e o comprimento dos estirpes não foram afetados pelo números de estirpes estudados. Houve redução na quantidade de perfilhos eliminados de um ano para outro , o que sugere que essa prática se torne desnecessária após três anos. O tratamento testemunha (sem desbaste) apresentou 13 estirpes por planta, os quais, na sua maioria, não se desenvolveram

    <p>Abstract</p> <p>Background</p> <p>Despite the high prevalence of sexually transmitted infections (STIs) and HIV infection in US correctional settings, most jails and prisons in the United States prevent inmates from using condoms to prevent STIs/HIV.</p> <p>Discussion</p> <p>This article makes the following arguments to justify a scalable and feasible next step in the prevention of HIV/STIs among inmates: condoms are a basic and essential part of HIV/STI prevention, HIV/STI transmission occurs in the context of corrections, and several model programs show the feasibility of condom distribution in prisons. A lower end estimate for HIV incidence among incarcerated applied to 2,000,000 new inmates annually results in thousands of new HIV infections acquired each year in corrections that could be prevented with condoms in corrections facilities. Programs from parts of the United States, Canada, and much of Europe show how programs distributing condoms in correctional facilities can be safe and effective.</p> <p>Summary</p> <p>Public health and corrections officials must work together to ensure that condoms and broader sexual disease prevention programs are integrated into US jail and prison health systems.</p

    Age and gender influence on HIDRAdisk outcomes in adalimumab-treated hidradenitis suppurativa patients

    Background Hidradenitis suppurativa (HS) is a chronic relapsing inflammatory skin disease characterized by a significant impairment of patients' quality of life (QoL). It has been recently found that clinical severity of HS does not correlate well with QoL. Therefore, it is important to enhance the evaluation of severity considering the disease burden on QoL. Recently, a new graphical tool able to better describe HS burden, the so-called HIDRAdisk, has been introduced. Objective To investigate the utility of HIDRAdisk in clinical practice before and after treatment and to analyse whether specific factors such as age and gender may influence the outcomes in patients with moderate-to-severe HS. Methods A single-centre retrospective study on 24 patients (13F/11M, mean age 38 +/- 15 years) with moderate-to-severe HS was performed. Clinical data (disease severity and quality of life) were collected at baseline and after 12 weeks of adalimumab. Results HIDRAdisk showed significantly better improvements in males (69.8 +/- 6.2-49.6 +/- 10.8) compared to females (80.7 +/- 6.0-72.3 +/- 6.7), P &lt;0.001. A significant correlation was found in the total population between HS severity values measured through the modified Sartorius score (mSS) and QoL measured through HIDRAdisk. As regards the relationship between disease outcomes and age, a trend for better HIDRAdisk outcomes in younger patients (&lt;40 years) compared to older ones was observed. Conclusions HIDRAdisk appears as a complete and informative tool which can easily measure the global burden of HS, guiding treatment choice and evaluating its efficacy
