    Hepatocellular Carcinoma: An Unusual Presentation of this Rare Clinical Entity in Children

    Malignant hepatic tumors are rare in children and hepatocellular carcinomas only represent 20% of cases. A previously healthy 10 year-old male was admitted for sudden abdominal pain. Ultrasound imaging showed an ileo-ileal intussusception with spontaneous resolution, but in the face of worsening pain, fever, and a palpable epigastric mass, abdominal magnetic resonance imaging was performed, showing a liver lesion. Laboratory tests presented elevated liver enzymes and C-reactive protein, so a liver abscess was considered and treated with metronidazole plus ceftriaxone. All of the microbiology tests as well as tumoral markers were negative. Despite clinical and laboratory improvement, the lesion persisted in the imaging. A liver biopsy confirmed a hepatocellular carcinoma, and the patient was submitted to surgical resection and chemotherapy. Contrarily to adults in whom most cases are secondary to chronic liver disease, children may not have risk factors for the disease, which makes it harder to make a prompt diagnosis.info:eu-repo/semantics/publishedVersio

    The growth of typical star-forming galaxies and their supermassive black holes across cosmic time since z~2

    Understanding galaxy formation and evolution requires studying the interplay between the growth of galaxies and the growth of their black holes across cosmic time. Here, we explore a sample of Hα-selected star-forming galaxies from the High Redshift Emission Line Survey and use the wealth of multiwavelength data in the Cosmic Evolution Survey field (X-rays, far-infrared and radio) to study the relative growth rates between typical galaxies and their central supermassive black holes, from z = 2.23 to z = 0. Typical star-forming galaxies at z ∼ 1–2 have black hole accretion rates (M˙BHM˙BH) of 0.001–0.01 M⊙ yr−1 and star formation rates (SFRs) of ∼10–40 M⊙ yr−1, and thus grow their stellar mass much quicker than their black hole mass (3.3±0.2 orders of magnitude faster). However, ∼3 per cent of the sample (the sources detected directly in the X-rays) show a significantly quicker growth of the black hole mass (up to 1.5 orders of magnitude quicker growth than the typical sources). M˙BHM˙BH falls from z = 2.23 to z = 0, with the decline resembling that of SFR density or the typical SFR (SFR*). We find that the average black hole to galaxy growth (M˙BHM˙BH/SFR) is approximately constant for star-forming galaxies in the last 11 Gyr. The relatively constant M˙BHM˙BH/SFR suggests that these two quantities evolve equivalently through cosmic time and with practically no delay between the two

Recent studies focus on the role of obesity and metabolic syndrome in inducing or worsening joint damage in hand OA patients, suggesting that chronic low-grade systemic inflammation may represent a possible linking factor. The gut microbiome has a crucial metabolic role which is fundamental for immune system development, among other important functions. Intestinal microbiota dysbiosis may favour metabolic syndrome and low-grade inflammation-two important components of hand OA onset and evolution. The aim of this narrative is to review the recent literature concerning the possible contribution of dysbiosis to hand OA onset and progression, and to discuss the importance of gut dysbiosis on general health and disease.publishersversionpublishe

    A Pilot Study on the Metabolic Impact of Mediterranean Diet in Type 2 Diabetes: Is Gut Microbiota the Key?

    The Mediterranean diet (MD) has been recommended for type 2 diabetes (T2D) treatment. The impact of diet in shaping the gut microbiota is well known, particularly for MD. However, the link between MD and diabetes outcome improvement is not completely clear. This study aims to evaluate the role of microbiota modulation by a nonpharmacological intervention in patients with T2D. In this 12-week single-arm pilot study, nine participants received individual nutritional counseling sessions promoting MD. Gut microbiota, biochemical parameters, body composition, and blood pressure were assessed at baseline, 4 weeks, and 12 weeks after the intervention. Adherence to MD [assessed by Mediterranean Diet Adherence Screener (MEDAS) score] increased after the intervention. Bacterial richness increased after 4 weeks of intervention and was negatively correlated with fasting glucose levels and Homeostatic Model Assessment for Insulin Resistance (HOMA-IR). Prevotella to Bacteroides ratio also increased after 4 weeks. In contrast, glycated haemoglobin (HbA1c) and HOMA-IR were only decreased at the end of study. Alkaline phosphatase activity was assessed in fecal samples and was negatively correlated with HbA1c and positively correlated with bacterial diversity. The results of this study reinforce that MD adherence results in a better glycemic control in subjects with T2D. Changes in gut bacterial richness caused by MD adherence may be relevant in mediating the metabolic impact of this dietary intervention.info:eu-repo/semantics/publishedVersio

    The tailings dam failure of 5 November 2015 in SE Brazil and its preceding seismic sequence

    The collapse of a mine tailings dam and subsequent flood in SE Brazil on 5 November 2015 was preceded by a small-magnitude seismic sequence. In this report, we explore the spatiotemporal associations between the seismic events and the accident and discuss their possible connection. We also analyze the signals generated by the turbulent mudflow, as recorded by the Brazilian Seismographic Network (RSBR). In light of our observations, we propose as possible contributing factor for the dam collapse either ground shaking and/or soil liquefaction triggered by the earthquakes. The possibility of such a small-magnitude earthquake contributing to the collapse of a tailings dam raises important concerns regarding safety and related legislation of dams in Brazil and the world. ©2016.     The PAU survey: classifying low-z SEDs using Machine Learning clustering

The clustering algorithm is implemented and optimized to get the relevant groups in the data SEDs. We find 12 groups from a total number of 5234 targets in the survey at 0.01 < z < 0.28. Among the groups, 3545 galaxies (68 per cent) show emission lines in the SEDs. These groups also include 1689 old galaxies with no active star formation. We have fitted the SED to every single galaxy in each group with CIGALE. The mass, age, and specific star formation rates (sSFR) of the galaxies range from 0.15 < age/Gyr <11; 6 < log (M/M⊙) <11.26, and -14.67 < log (sSFR/yr-1) <-8. The groups are well-defined in their properties with galaxies having clear emission lines also having lower mass, are younger and have higher sSFR than those with elliptical like patterns. The characteristic values of galaxies showing clear emission lines are in agreement with the literature for starburst galaxies in COSMOS and GOODS-N fields at low redshift. The star-forming main sequence, sSFR versus stellar mass and UVJ diagram show clearly that different groups fall into different regions with some overlap among groups. Our main result is that the joint of low- resolution (R ∼50) photometric spectra provided by the PAU survey together with the unsupervised classification provides an excellent way to classify galaxies.     The evolution of the UV luminosity and stellar mass functions of Lyman-α emitters from z ~2 to z ~6

    We measure the evolution of the rest-frame UV luminosity function (LF) and the stellar mass function (SMF) of Lyman-α (Ly α) emitters (LAEs) from z ~ 2 to z ~ 6 by exploring ~4000 LAEs from the SC4K sample. We find a correlation between Ly α luminosity (LLy α) and rest-frame UV (MUV), with best fit MUV = -1.6+0.2-0.3 log10(LLy α/erg s-1) + 47+12-11 and a shallower relation between LLy α and stellar mass (M∗), with best fit log10(M∗/M·) = 0.9+0.1-0.1 log10(LLy α/erg s-1) - 28+4.0-3.8. An increasing LLy α cut predominantly lowers the number density of faint MUV and low M∗ LAEs. We estimate a proxy for the full UV LFs and SMFs of LAEs with simple assumptions of the faint end slope. For the UV LF, we find a brightening of the characteristic UV luminosity (M∗UV) with increasing redshift and a decrease of the characteristic number density (Φ∗). For the SMF, we measure a characteristic stellar mass (M∗∗/M·) increase with increasing redshift, and a Φ∗ decline. However, if we apply a uniform luminosity cut of log10(LLy α/erg s-1) ≥ 43.0, we find much milder to no evolution in the UV and SMF of LAEs. The UV luminosity density (ρUV) of the full sample of LAEs shows moderate evolution and the stellar mass density (ρM) decreases, with both being always lower than the total ρUV and ρM of more typical galaxies but slowly approaching them with increasing redshift. Overall, our results indicate that both ρUV and ρM of LAEs slowly approach the measurements of continuum-selected galaxies at z > 6, which suggests a key role of LAEs in the epoch of reionization

    The X-SHOOTER Lyman-αα survey at z=2 (XLS-z2) I:the panchromatic spectrum of typical Lyman-αα emitters

    We present the first results from the X-SHOOTER Lyman-α\alpha survey at z=2z=2 (XLS-z2z2). XLS-z2z2 is a deep spectroscopic survey of 35 Lyman-α\alpha emitters (LAEs) utilising 90\approx90 hours of exposure time with VLT/X-SHOOTER and covers rest-frame Lyα\alpha to Hα\alpha emission with R4000\approx4000. We present the sample selection, the observations and the data reduction. Systemic redshifts are measured from rest-frame optical lines for 33/35 sources. In the stacked spectrum, our LAEs are characterised by an interstellar medium with little dust, a low metallicity and a high ionisation state. The ionising sources are young hot stars that power strong emission-lines in the optical and high ionisation lines in the UV. The LAEs exhibit clumpy UV morphologies and have outflowing kinematics with blue-shifted SiII absorption, a broad [OIII] component and a red-skewed Lyα\alpha line. Typically 30 % of the Lyα\alpha photons escape, of which one quarter on the blue side of the systemic velocity. A fraction of Lyα\alpha photons escapes directly at the systemic suggesting clear channels enabling a 10\approx10 % escape of ionising photons, consistent with an inference based on MgII. A combination of a low effective HI column density, a low dust content and young star-burst determine whether a star forming galaxy is observed as a LAE. The first is possibly related to outflows and/or a fortunate viewing angle, while we find that the latter two in LAEs are typical for their stellar mass of 109^9 M_{\odot}
