    The differential impact of two anesthetic techniques on cortisol levels in Nigerian surgical patients

    Background: Surgical procedures are associated with a complexity of stress response characterized by neurohumoral, immulogic, and metabolic alterations. Aim: The aim was to compare the effects on the stress response by isoflurane‑based intratracheal general anesthesia (ITGA) and bupivacaine‑based epidural anesthesia (EA), using cortisol as a biochemical marker. Materials and Methods: Following the approval of the Hospital Ethical Board, informed written consent from patients recruited into this study was obtained. One group received general anesthesia with relaxant technique (group A) while the other group had bupicaine epidural anesthesia with catheter placement for top‑ups (group B) for their surgeries. Both groups were assessed for plasma cortisol levels – baseline, 30 minutes after skin the start of surgery and at skin closure. Results: There was no statistically significant difference in the baseline mean heart rate, mean arterial pressure (mean MAP) and the mean duration of surgery between the two groups; the baseline mean plasma cortisol level was 88.70 ± 3.85 ng/ml for group A and 85.55 ± 2.29 ng/ml for group B, P=0.148. At 30 minutes after the start of surgery the plasma cortisol level in the GA group was 361.60 ± 31.27 ng/ml while it was 147.45 ± 22.36 ng/ml in the EA group, showing a significant difference, P=0.001. At skin closure the mean plasma cortisol value of 384.65 ± 48.04 ng/ml recorded in the GA group was found to be significantly higher than the value of 140.20 ± 10.74 ng/ml in the GA group, P<0.002. Conclusion: Using plasma cortisol as a measure, bupivacaine‑based epidural anesthesia significantly reduces the stress response to surgical stimuli when compared with isoflurane‑based tracheal general anesthesia.Keywords: Cortisol levels, epidural anesthesia, general anesthesia, surgeryNigerian Journal of Clinical Practice •Jan-Mar 2012 • Vol 15 • Issue

    Comparison of maternal satisfaction following epidulral and general anaesthesia for repeat caesarean section

    Objective: To assess maternal satisfaction following two anaesthetic techniques (general and epidural anaesthesia) for repeat Caesarean section.Design: Prospective study.Setting: University of Port Harcourt Teaching Hospital, Nigeria.Subjects: One hundred and twenty parturients scheduled for elective Caesarean section.Results: The demographic data and mean satisfaction scores obtained from questionnaire and visual analogue scale (VAS) were subjected to analysis using the Student’s t-test with the computerised statistical calculator SPSS Windows 10.0. The mean satisfaction score of the parturients in the epidural anaesthesia population using the questionnaire was 149.0±10.65 while that in the general anaesthesia population was 105.3±12.42. The mean satisfaction score from the VAS was 9.0±1.50 for those who had epidural anaesthesia while it was 2.6±0.70 in the general anaesthesia population.     Overview of homocysteine and folate metabolism. With special references to cardiovascular disease and neural tube defects

    This overview addresses homocysteine and folate metabolism. Its functions and complexity are described, leading to explanations why disturbed homocysteine and folate metabolism is implicated in many different diseases, including congenital birth defects like congenital heart disease, cleft lip and palate, late pregnancy complications, different kinds of neurodegenerative and psychiatric diseases, osteoporosis and cancer. In addition, the inborn errors leading to hyperhomocysteinemia and homocystinuria are described. These extreme human hyperhomocysteinemia models provide knowledge about which part of the homocysteine and folate pathways are linked to which disease. For example, the very high risk for arterial and venous occlusive disease in patients with severe hyperhomocysteinemia irrespective of the location of the defect in remethylation or transsulphuration indicates that homocysteine itself or one of its “direct” derivatives is considered toxic for the cardiovascular system. Finally, common diseases associated with elevated homocysteine are discussed with the focus on cardiovascular disease and neural tube defects

    Searches for neutral Higgs bosons in e+ee^{+}e^{-} collisions at centre-of-mass energies from 192 to 202 GeV

    Searches for neutral Higgs bosons are performed with the 237 pb^-1 of data collected in 1999 by the ALEPH detector at LEP, for centre-of-mass energies between 191.6 and 201.6 GeV. These searches apply to Higgs bosons within the context of the Standard Model and its minimal supersymmetric extension (MSSM) as well as to invisibly decaying Higgs bosons. No evidence of a signal is seen. A lower limit on the mass of the Standard Model Higgs boson of 107.7 GeV/c^2 at 95% confidence level is set. In the MSSM, lower limits of 91.2 and 91.6 GeV/c^2 are derived for the masses of the neutral Higgs bosons h and A, respectively. For a Higgs boson decaying invisibly and produced with the Standard Model cross section, masses below 106.4 GeV/c^2 are excluded

    Measurement of W-pair production in e+ee^+ e^- collisions at 189 GeV

    The production of W-pairs is analysed in a data samplecollected by ALEPH at a mean centre-of-mass energy of 188.6 GeV,corresponding to an integrated luminosity of 174.2 pb^-1. Crosssections are given for different topologies of W decays intoleptons or hadrons. Combining all final states and assumingStandard Model branching fractions, the total W-pair cross sectionis measured to be 15.71 +- 0.34 (stat) +- 0.18 (syst) pb.Using also the W-pair data samples collected by ALEPH at lowercentre-of-mass energies, the decay branching fraction of the W bosoninto hadrons is measured to be BR (W hadrons) = 66.97+- 0.65 (stat) +- 0.32 (syst) %, allowing a determination of theCKM matrix element |V(cs)|= 0.951 +- 0.030 (stat) +- 0.015 (syst)

    Determination of sin2 θeff w using jet charge measurements in hadronic Z decays

    The electroweak mixing angle is determined with high precision from measurements of the mean difference between forward and backward hemisphere charges in hadronic decays of the Z. A data sample of 2.5 million hadronic Z decays recorded over the period 1990 to 1994 in the ALEPH detector at LEP is used. The mean charge separation between event hemispheres containing the original quark and antiquark is measured for bb̄ and cc̄ events in subsamples selected by their long lifetimes or using fast D*'s. The corresponding average charge separation for light quarks is measured in an inclusive sample from the anticorrelation between charges of opposite hemispheres and agrees with predictions of hadronisation models with a precision of 2%. It is shown that differences between light quark charge separations and the measured average can be determined using hadronisation models, with systematic uncertainties constrained by measurements of inclusive production of kaons, protons and A's. The separations are used to measure the electroweak mixing angle precisely as sin2 θeff w = 0.2322 ± 0.0008(exp. stat.) ±0.0007(exp. syst.) ± 0.0008(sep.). The first two errors are due to purely experimental sources whereas the third stems from uncertainties in the quark charge separations

    Measurement of the W mass by direct reconstruction in e+ee^+ e^- collisions at 172 GeV

    The mass of the W boson is obtained from reconstructed invariant mass distributions in W-pair events. The sample of W pairs is selected from 10.65~pb1^{-1} collected with the ALEPH detector at a mean centre-of-mass energy of 172.09 \GEV. The invariant mass distribution of simulated events are fitted to the experimental distributions and the following W masses are obtained: WWqqqqmW=81.30+0.47(stat.)+0.11(syst.)GeV/c2WW \to q\overline{q}q\overline{q } m_W = 81.30 +- 0.47(stat.) +- 0.11(syst.) GeV/c^2, WWlνqq(l=e,μ)mW=80.54+0.47(stat.)+0.11(syst.)GeV/c2WW \to l\nu q\overline{q}(l=e,\mu) m_W = 80.54 +- 0.47(stat.) +- 0.11(syst.) GeV/c^2, WWτνqqmW=79.56+1.08(stat.)+0.23(syst.)GeV/C62WW \to \tau\nu q\overline{q} m_W = 79.56 +- 1.08(stat.) +- 0.23(syst.) GeV/C62. The statistical errors are the expected errors for Monte Carlo samples of the same integrated luminosity as the data. The combination of these measurements gives: mW=80.80+0.11(syst.)+0.03(LEPenergy)GeV/2m_W = 80.80 +- 0.11(syst.) +- 0.03(LEP energy) GeV/^2
