    Cuture, identity and alternatives to the consumer culture

    This article explores questions of identity and culture in relation to the present systemic crises that confront human life on the planet, problematizing the pursuit of economic growth and consumerist culture. It uses the concept of Ideology-Action-Structure complexes to understand the saturating nature of social, political and economic domination, and then explores interventions in these complexes, which all have characteristics of informal education, to promote cultural growth, create new settings and establish a counter-hegemonic ideology and alliance. It is suggested that by joining up fragmented local interventions and movements, there is hope that society’s way of life can be shifted to one where cultural enrichment supports a less resource-exploitative economic and cultural model

    Theory and practice for a critical community psychology in the UK

    Over the last 30 years we have developed an approach to "Critical Community Psychology", that aims to be locally focussed but globally aware. Characteristics that distinguish it from other approaches in community and critical psychology include 1) the concept of prefigurative action, which relates work with local projects and initiatives to a wider project of principled social change, 2) an understanding of community that reflects its contested nature and lived diversity, 3) a priority for working with those most oppressed or excluded by dominant power systems, 4) ecological and systems thinking which includes our own distinctive use of boundary, edge and the ethic of stewardship, 5) use of a wide repertoire of methods and theories adequate to the variety of problem contexts community psychologists can encounter.     Changes in Climate and Land Use Over the Amazon Region: Current and Future Variability and Trends

Available on open access from Frontiers media via the DOI in this recordThis paper shows recent progress in our understanding of climate variability and trends in the Amazon region, and how these interact with land use change. The review includes an overview of up-to-date information on climate and hydrological variability, and on warming trends in Amazonia, which reached 0.6–0.7°C over the last 40 years, with 2016 as the warmest year since at least 1950 (0.9°C + 0.3°C). We focus on local and remote drivers of climate variability and change. We review the impacts of these drivers on the length of dry season, the role of the forest in climate and carbon cycles, the resilience of the forest, the risk of fires and biomass burning, and the potential “die back” of the Amazon forests if surpassing a “tipping point”. The role of the Amazon in moisture recycling and transport is also investigated, and a review of model development for climate change projections in the region is included. In sum, future sustainability of the Amazonian forests and its many services requires management strategies that consider the likelihood of multi-year droughts superimposed on a continued warming trend. Science has assembled enough knowledge to underline the global and regional importance of an intact Amazon region that can support policymaking and to keep this sensitive ecosystem functioning. This major challenge requires substantial resources and strategic cross-national planning, and a unique blend of expertise and capacities established in Amazon countries and from international collaboration.     Resolving structure of the disc around HD100546 at 7 mm with ATCA

    There is much evidence that planet formation is occurring in the disc around the Herbig Be star HD100546. To learn more about the processes occurring in this disc, we conducted high-resolution imaging at 43/45 GHz with the Australia Telescope Compact Array. Multiple array configurations were used, providing a best spatial resolution of ∼0.15 arcsec, or 15 au at HD100546's distance of ∼100 pc. Significant structure is revealed, but its precise form is dependent on the u − v plane sampling used for the image reconstruction. At a resolution of ≤30 au, we detected an inner gap in the disc with a radius of ∼25 au and a position angle approximately along the known disc major axis. With different weighting, and an achieved resolution of ∼15 au, emission appears at the centre and the disc takes on the shape of an incomplete ring, much like a horseshoe, again with a gap radius of ∼25 au. The position angle of the disc major axis and its inclination from face-on are determined to be 140° ± 5° and 40° ± 5°, respectively. The ∼25 au gap radius is confirmed by a null in the real part of the binned visibilities at 320 ± 10 kλ, whilst the non-axisymmetric nature is also confirmed through significant structure in the imaginary component. The emission mechanism at the central peak is most likely to be free–free emission from a stellar or disc wind. Overall our data support the picture of at least one, but probably several, giant planets orbiting HD100546 within 25 au

    The PD COMM trial: A protocol for the process evaluation of a randomised trial assessing the effectiveness of two types of SLT for people with Parkinson's disease

    BACKGROUND: The PD COMM trial is a phase III multi-centre randomised controlled trial whose aim is to evaluate the effectiveness and cost-effectiveness of two approaches to speech and language therapy (SLT) compared with no SLT intervention (control) for people with Parkinson's disease who have self-reported or carer-reported problems with their speech or voice. Our protocol describes the process evaluation embedded within the outcome evaluation whose aim is to evaluate what happened at the time of the PD COMM intervention implementation and to provide findings that will assist in the interpretation of the PD COMM trial results. Furthermore, the aim of the PD COMM process evaluation is to investigate intervention complexity within a theoretical model of how the trialled interventions might work best and why. METHODS/DESIGN: Drawing from the Normalization Process Theory and frameworks for implementation fidelity, a mixed method design will be used to address process evaluation research questions. Therapists' and participants' perceptions and experiences will be investigated via in-depth interviews. Critical incident reports, baseline survey data from therapists, treatment record forms and home practice diaries also will be collected at relevant time points throughout the running of the PD COMM trial. Process evaluation data will be analysed independently of the outcome evaluation before the two sets of data are then combined. DISCUSSION: To date, there are a limited number of published process evaluation protocols, and few are linked to trials investigating rehabilitation therapies. Providing a strong theoretical framework underpinning design choices and being tailored to meet the complex characteristics of the trialled interventions, our process evaluation has the potential to provide valuable insight into which components of the interventions being delivered in PD COMM worked best (and what did not), how they worked well and why

    Nodular melanoma presenting with rapid progression and widespread metastases: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Melanoma is responsible for 1% to 2% of all cancer deaths around the world. Nodular melanoma often carries a poor prognosis because of no prodromal radial growth phase, early distant metastasis and significant tumor volume.</p> <p>Case presentation</p> <p>We present a case of progressive melanoma. A 51-year-old man was admitted to our hospital with dyspnea and skin lesions. These were multiple, dark colored, firm, and nodular and varied in size. He was diagnosed with melanoma. Temozolomide was administered, but he died of respiratory failure within a week after diagnosis.</p> <p>Conclusion</p> <p>Nodular melanoma tends to spread rapidly and eventually metastasize to vital organs. It may be fatal within months of recognition.</p

    Correlation of psychomotor findings and the ability to partially weight bear

    <p>Abstract</p> <p>Background</p> <p>Partial weight bearing is thought to avoid excessive loading that may interfere with the healing process after surgery of the pelvis or the lower extremity. The object of this study was to investigate the relationship between the ability to partially weight bear and the patient's psychomotor skills and an additional evaluation of the possibility to predict this ability with a standardized psychomotor test.</p> <p>Methods</p> <p>50 patients with a prescribed partial weight bearing at a target load of 15 kg following surgery were verbally instructed by a physical therapist. After the instruction and sufficient training with the physical therapist vertical ground reaction forces using matrix insoles were measured while walking with forearm crutches. Additionally, psychomotor skills were tested with the Motorische Leistungsserie (MLS). To test for correlations Spearman's Rank correlation was used. For further comparison of the two groups a Mann-Withney test was performed using Bonferroni correction.</p> <p>Results</p> <p>The patient's age and body weight significantly correlated with the ability to partially weight bear at a 15 kg target load. There were significant correlations between several subtests of the MLS and ground reaction forces measured while walking with crutches. Patients that were able to correctly perform partial weight bearing showed significant better psychomotor skills especially for those subtests where both hands had to be coordinated simultaneously.</p> <p>Conclusions</p> <p>The ability to partially weight bear is associated with psychomotor skills. The MLS seems to be a tool that helps predicting the ability to keep within the prescribed load limits.</p

    Cellulose acetate phthalate, a common pharmaceutical excipient, inactivates HIV-1 and blocks the coreceptor binding site on the virus envelope glycoprotein gp120

    BACKGROUND: Cellulose acetate phthalate (CAP), a pharmaceutical excipient used for enteric film coating of capsules and tablets, was shown to inhibit infection by the human immunodeficiency virus type 1 (HIV-1) and several herpesviruses. CAP formulations inactivated HIV-1, herpesvirus types 1 (HSV-1) and 2 (HSV-2) and the major nonviral sexually transmitted disease (STD) pathogens and were effective in animal models for vaginal infection by HSV-2 and simian immunodeficiency virus. METHODS: Enzyme-linked immunoassays and flow cytometry were used to demonstrate CAP binding to HIV-1 and to define the binding site on the virus envelope. RESULTS: 1) CAP binds to HIV-1 virus particles and to the envelope glycoprotein gp120; 2) this leads to blockade of the gp120 V3 loop and other gp120 sites resulting in diminished reactivity with HIV-1 coreceptors CXCR4 and CCR5; 3) CAP binding to HIV-1 virions impairs their infectivity; 4) these findings apply to both HIV-1 IIIB, an X4 virus, and HIV-1 BaL, an R5 virus. CONCLUSIONS: These results provide support for consideration of CAP as a topical microbicide of choice for prevention of STDs, including HIV-1 infection
