The original publication is available at http://www.biomedcentral.com/1471-244X/5/37Background: A substantial number of patients suffering from psychological problems or psychiatric disorders have turned to internet support groups for help. This paper reports on the perceived effectiveness of trichotillomania (TTM) internet support groups for people suffering from hair-pulling. Methods: A questionnaire was sent via e-mail to all subscribers of two mailing lists devoted to TTM, each of which takes a somewhat different approach to the condition. The questionnaire addressed the possible benefits and problems associated with belonging to a TTM virtual support group. Results: Subscribers had similar demographic features as clinical samples of trichotillomania patients. Subscribers to both internet lists found them helpful in terms of feeling supported and in obtaining information. The different approaches to TTM on the two lists were associated with differences in treatments attempted by participants. Conclusion: Internet support groups can potentially contribute to increasing awareness about and knowledge of psychiatric disorders such as TTM, as well as to their management. Nevertheless, additional effort is required to ensure that subscribers are able to make informed, evidence-based decisions

Ultrasound comparison of the effects of prehabilitation exercises and the scapular assistance test on the acromiohumeral distance

Abstract: Background: Prolonged participation in overhead sports creates shoulder muscle imbalances which eventually alter the efficacy of the shoulder stabiliser muscles and heighten injury risk, such as subacromial impingement syndrome. Objectives: The aim of this study was to determine if ultrasound is effective to measure the acromiohumeral distance (AHD) to compare the effect of the scapular assistance test (SAT) on the AHD with a prehabilitative exercise intervention programme in asymptomatic cricket players. Methods: Baseline testing on cricket players from the North- West University cricket squad (N=34) included AHD measurements performed by a sonographer at 0°, 30° and 60° humeral abduction angles respectively, with and without the SAT application. Players were then randomly assigned to an intervention and control group..

Recommendations for the use of endoscopic lung volume reduction in South Africa: Role in the treatment of emphysema

    Get PDF
Emphysema is a very common cause of morbidity and mortality in South Africa (SA). Therapeutic options in severe emphysema are limited. Endoscopic lung volume reduction (ELVR) is increasingly being used internationally for the treatment of advanced emphysema in a subset of patients with advanced disease, aiming to obtain the same functional advantages as surgical lung volume reduction while reducing risks and costs. In addition to endobronchial valves, ELVR using endobronchial coils is now available in SA. The high cost of these interventions underscores the need for careful patient selection to best identify those who may or may not benefit from ELVR-related procedures. The Assembly on Interventional Pulmonology of the South African Thoracic Society appointed a committee comprising both local and international experts to extensively review all relevant evidence and provide advice on the use of ELVR in SA based on published evidence, expert opinion and local access to the various devices

Antioxidant and anti-inflammatory activity of Ocimum labiatum extract and isolated labdane diterpenoid

BACKGROUND : Plants from the genus Ocimum are used as folk medicine for treating various diseases including inflammatory and immune-related diseases. Numerous reports have suggested plant extracts and their constituents as possible anti-inflammatory agents. Here, in vitro evidence of Ocimum labiatum’s immune-enhancing and antioxidant properties is presented for the first time. METHODS : The anti-inflammatory effect of O. labiatum ethanolic extract and an isolated diterpenoid was determined using a cytometric bead array (CBA) technique. The effect on phytohemagglutinin (PHA)-induced nitric oxide (NO) production in peripheral blood mononuclear cells (PBMCs) was also assessed. A battery of antioxidant assays were used for detecting antioxidant activity while the anti-inflammatory mechanism was evaluated using an ELISA-based activator protein (AP-1) (c-Jun) assay. Cytotoxicity was determined on TZM-bl and PBMCs using a tetrazolium dye and confirmed by a novel label-free real-time assay. RESULTS : A 25 μg/mL non-cytotoxic concentration of O. labiatum extract significantly (p < 0.05) inhibited the production of pro-inflammatory cytokines; IL-2, IL-4, IL-6 and IL-17A. Except for the dual acting pro- or anti-inflammatory cytokine, IL-6, which was upregulated, a non-cytotoxic 50 μM concentration of the isolated labdane diterpenoid compound significantly (p < 0.05) decreased the production of all the pro-inflammatory cytokines. In the anti-inflammatory pathway studies, the compound also inhibited AP-1 significantly (p < 0.05) at 50 μM. The extract demonstrated strong, dose dependent antioxidant activity with IC50 values ranging from 13 ± 0.8 to 54.86 ± 1.28 μg/mL while the terpene had no antioxidant property. The extract and diterpenoid decreased the production of the inflammatory mediator NO, at non-cytotoxic concentrations. The CC50 of the extract in TZM-bl and PBMCs was 62.6 ± 0.6 and 30.1 ± 0.4 μg/mL while that of the compound was 112.6 ± 0.2 and 70 ± 0.4 μM respectively. The real time studies confirmed tetrazolium dye assessed viability and also detected a unique growth pattern for the plant materials compared to untreated cells. CONCLUSIONS : O. labiatum extract demonstrated promising anti-inflammatory and antioxidant properties while the terpenoid showed anti-inflammatory but no antioxidant activity. The anti-inflammatory mechanism of the terpene was a result of inhibition of AP-1. These data represents promising first steps towards the development of naturally derived anti-inflammation drugs.Southern African Biochemistry and Informatics for Natural Products (SABINA), the Technology Innovation Agency (TIA, South Africa), Margaret McNamara Memorial Fund (MMMF), the Namibian Ministry of Education and the University of Pretoria.http://www.journal-inflammation.comhb2016Biochemistr

Tropical ancient DNA from bulk archaeological fish bone reveals the subsistence practices of a historic coastal community in southwest Madagascar

Taxonomic identification of archaeological fish bones provides important insights into the subsistence practices of ancient coastal peoples. However, it can be difficult to execute robust morphological identification of fish bones from species-rich fossil assemblages, especially from post-cranial material with few distinguishing features. Fragmentation, weathering and burning further impede taxonomic identification, resulting in large numbers of unidentifiable bones from archaeological sites. This limitation can be somewhat mitigated by taking an ancient DNA (aDNA) bulk-bone metabarcoding (BBM) approach to faunal identification, where DNA from non-diagnostic bone fragments is extracted and sequenced in parallel. However, a large proportion of fishing communities (both past and present) live in tropical regions that have sub-optimal conditions for long-term aDNA preservation. To date, the BBM method has never been applied to fish bones before, or to fossils excavated from an exposed context within a tropical climate. Here, we demonstrate that morphologically indistinct bulk fish bone from the tropics can be identified by sequencing aDNA extracted from 100 to 300 ya archaeological midden material in southwest Madagascar. Despite the biases of the approach, we rapidly obtained family, genus, and species-level assemblage information, and used this to describe a subset of the ichthyofauna exploited by an 18th century fishing community. We identified 23 families of fish, including benthic, pelagic, and coral-dwelling fishes, suggesting a reliance on a variety of marine and brackish habitats. When possible, BBM should be used alongside osteological approaches to address the limitations of both; however, this study highlights how genetic methods can nevertheless be a valuable tool for helping resolve faunal assemblages when morphological identification is hindered by taphonomic processes, lack of adequate comparative collections, and time constraints, and can provide a temporal perspective on fish biodiversity in the context of accelerated exploitation of the marine environment

Do satisfied cellar door visitors want to revisit? Linking past knowledge and consumption behaviors to satisfaction and intention to return

    Get PDF
This study evaluates the main determinants of wine tourists' intention to revisit the winery cellar door. The proposed tourist behavior model suggests that past wine-related knowledge and behaviors as well as motivation affect satisfaction with the cellar door visit. The model suggests that actual behavior at the cellar door (number of bottles bought and amount of money spent) is dependent on the previously mentioned factors. A survey of wine tourists in the Barossa Valley, Australia, led to 676 useable questionnaires. The results of a binary logistic model show that only monthly household expenditure on wine consumption and the motive of tasting wine predict satisfaction with the cellar door visit. A negative binomial model shows that the probability to buy more bottles at the winery increases if the visitor is from Australia, satisfied with the visit, has tasted wine at the cellar door, is younger, spends more on monthly household consumption of wine, and was primarily visiting to buy wine. However, intention to revisit is predicted only by satisfaction, awareness of the winery before the visit, motives of buying and tasting wine, and some sociodemographic characteristics. Implications for the management of visitor behavior and the cellar door experience are also discussed
