    Impact of age and race on outcomes of a program to prevent excess weight gain and disordered eating in adolescent girls

    Interpersonal psychotherapy (IPT) prevents weight gain and reduces loss-of-control (LOC)-eating in adults. However, IPT was not superior to health-education (HE) for preventing excess weight gain and reducing LOC-eating over 1-year in adolescent girls at risk for excess weight gain and eating disorders. Limited data suggest that older and non-White youth may be especially responsive to IPT. In secondary analyses, we examined if age or race moderated weight and LOC-eating outcomes. The 113 participants (12–17 years; 56.6% White) from the original trial were re-contacted 3 years later for assessment. At baseline and follow-up visits through 3 years, we assessed BMI, adiposity by dual energy X-ray absorptiometry, and LOC-eating presence. In linear mixed models, baseline age moderated 3-year BMI outcome; older girls in IPT had the lowest 3-year BMI gain compared to younger girls in IPT and all girls in HE, p = 0.04. A similar pattern was observed for adiposity. Race moderated 3-year LOC-eating; non-White girls in IPT were most likely to abstain from LOC-eating at 3 years compared to all other girls, p = 0.04. This hypothesis-generating analysis suggests future studies should determine if IPT is especially efficacious at reducing LOC-eating in older, non-White adolescents

    Breast and Cervical Cancer Disparities in Alabama: Current Scenario, Ongoing Efforts to Reduce the Disparity Gaps, and What More We Could be Doing

    Over the years, we have made considerable progress in our understanding of the biology of various cancers leading to advancements in cancer management strategies. Consequently, we have witnessed steady improvement in survival rates of cancer patients post-diagnosis, although the progress has been slow for some cancer types. Moreover, the advances in cancer care have not equally benefited all the minority and ethnic populations residing in the United States. The state of Alabama has one of the most diverse demographics in the country and as a result, we witness significant health disparities among our populations. Breast and cervical cancers are two major cancer types that disparately affect the women in our state. Here, we discuss the extent of disparities in the diagnosis and death rates from these cancers in the state of Alabama and potential underlying causes affecting the health outcomes. We also discuss ongoing efforts undertaken to reduce the disparity gaps and provide a perspective for addressing these disparities more effectively. &nbsp

    Gestational age at delivery and special educational need: retrospective cohort study of 407,503 schoolchildren

    <STRONG>Background</STRONG> Previous studies have demonstrated an association between preterm delivery and increased risk of special educational need (SEN). The aim of our study was to examine the risk of SEN across the full range of gestation. <STRONG>Methods and Findings</STRONG> We conducted a population-based, retrospective study by linking school census data on the 407,503 eligible school-aged children resident in 19 Scottish Local Authority areas (total population 3.8 million) to their routine birth data. SEN was recorded in 17,784 (4.9%) children; 1,565 (8.4%) of those born preterm and 16,219 (4.7%) of those born at term. The risk of SEN increased across the whole range of gestation from 40 to 24 wk: 37–39 wk adjusted odds ratio (OR) 1.16, 95% confidence interval (CI) 1.12–1.20; 33–36 wk adjusted OR 1.53, 95% CI 1.43–1.63; 28–32 wk adjusted OR 2.66, 95% CI 2.38–2.97; 24–27 wk adjusted OR 6.92, 95% CI 5.58–8.58. There was no interaction between elective versus spontaneous delivery. Overall, gestation at delivery accounted for 10% of the adjusted population attributable fraction of SEN. Because of their high frequency, early term deliveries (37–39 wk) accounted for 5.5% of cases of SEN compared with preterm deliveries (<37 wk), which accounted for only 3.6% of cases. <STRONG>Conclusions</STRONG> Gestation at delivery had a strong, dose-dependent relationship with SEN that was apparent across the whole range of gestation. Because early term delivery is more common than preterm delivery, the former accounts for a higher percentage of SEN cases. Our findings have important implications for clinical practice in relation to the timing of elective delivery

    Predictability and Semiclassical Approximation at the onset of Black Hole formation

    We combine analytical and numerical techniques to study the collapse of conformally coupled massless scalar fields in semiclassical 2D dilaton gravity, with emphasis on solutions just below criticality when a black hole almost forms. We study classical information and quantum correlations. We show explicitly how recovery of information encoded in the classical initial data from the outgoing classical radiation becomes more difficult as criticality is approached. The outgoing quantum radiation consists of a positive-energy flux, which is essentially the standard Hawking radiation, followed by a negative-energy flux which ensures energy conservation and guarantees unitary evolution through strong correlations with the positive-energy Hawking radiation. As one reaches the critical solution there is a breakdown of unitarity. We show that this breakdown of predictability is intimately related to a breakdown of the semiclassical approximation.Comment: 26 pages RevTex + 8 figures in a separate postscript fil

    The Antibacterial Activity of Honey Derived from Australian Flora

    Chronic wound infections and antibiotic resistance are driving interest in antimicrobial treatments that have generally been considered complementary, including antimicrobially active honey. Australia has unique native flora and produces honey with a wide range of different physicochemical properties. In this study we surveyed 477 honey samples, derived from native and exotic plants from various regions of Australia, for their antibacterial activity using an established screening protocol. A level of activity considered potentially therapeutically useful was found in 274 (57%) of the honey samples, with exceptional activity seen in samples derived from marri (Corymbia calophylla), jarrah (Eucalyptus marginata) and jellybush (Leptospermum polygalifolium). In most cases the antibacterial activity was attributable to hydrogen peroxide produced by the bee-derived enzyme glucose oxidase. Non-hydrogen peroxide activity was detected in 80 (16.8%) samples, and was most consistently seen in honey produced from Leptospermum spp. Testing over time found the hydrogen peroxide-dependent activity in honey decreased, in some cases by 100%, and this activity was more stable at 4°C than at 25°C. In contrast, the non-hydrogen peroxide activity of Leptospermum honey samples increased, and this was greatest in samples stored at 25°C. The stability of non-peroxide activity from other honeys was more variable, suggesting this activity may have a different cause. We conclude that many Australian honeys have clinical potential, and that further studies into the composition and stability of their active constituents are warranted

    Nation-Building Through Compulsory Schooling During the Age of Mass Migration

    By the mid-19th century, America was the most educated nation on Earth: significant financial investments in education were being undertaken and the majority of children voluntarily attended public schools. So why did states across America start introducing compulsory schooling laws at this time in history? We provide qualitative and quantitative evidence that states adopted compulsory schooling laws as a nation-building tool to instill civic values to the tens of millions of culturally diverse migrants who arrived during the ‘Age of Mass Migration’ between 1850 and 1914. We show the adoption of state level compulsory schooling laws occurred significantly earlier in states that hosted a subgroup of European migrants with lower exposure to civic values in their home countries. We then use cross-county data to show the same subgroup of European migrants had significantly lower demand for American common schooling pre-compulsion, and so would have been less exposed to the kinds of civic value or discipline instilled by the American education system had compulsory schooling not been passed. By studying the link between mass migration and the endogenous policy responses of American-born voters in receiving states, our analysis provides new micro-foundations for compulsory schooling laws, the legislative bedrock on which all future developments of the American schooling system were built
