    The objective of this mini-review is to discuss the role of real-world studies as a source of clinical evidence when experimental studies, such as randomised controlled trials (RCTs), are not available. Waiting for RCT evidence when the technology is diffusing could be anti-economical, inefficient from the policy perspective and methodologically questionable.We explain how real-world studies could provide relevant evidence to decision makers. Matching techniques are discussed as a viable solution for bias reduction.We describe a case study concerning a cost-effectiveness analysis based on real-world data of a technology already in use: Mitraclip combined with medical therapy versus medical therapy alone in patients with moderate-to-severe mitral regurgitation. The CEA has encountered the scepticism of most reviewers, due not to the statistical methodology but to the fact that the study was observational and not experimental. Editors and reviewers converged in considering real-world economic evaluations premature in the absence of a RCT, even if in the meantime the technology had been implanted >30 000 times. We believe there is a need to acknowledge the importance of real-world studies, and engage the scientific community in the promotion and use of clinical evidence produced through observational studies

    Unpacking hybridity: development and first validation of a multidimensional instrument to profile hybrid professionals

    Hybrid professionals, that is, professionals who have transitioned to managerial roles, have emerged in numerous public settings. Through in-depth qualitative methodologies, the literature has shown a good degree of heterogeneity in the way hybrid professionals perceive and manage their hybridity. In this study, we aim to develop a theory-based, multidimensional instrument able to capture such heterogeneity in a lean but sensitive way. In this instrument, we combine consolidated scales of identity centrality and integration and vignettes, on the one hand, to measure the perception of hybrid professionals of the relationship between their professional and managerial identities and, on the other hand, to elicit the practices they use to deal with the demands of the professional and managerial logics. We first validate the instrument on a sample of school principals in the Italian context and then describe the six profiles of hybrid professionals derived from the analysis. We suggest three avenues for applying such an instrument

    Evidence of the causal role of human papillomavirus type 58 in an oropharyngeal carcinoma

    Persistent human papillomavirus infection (HPV) is recognized as an important etiologic factor for a subset of head and neck squamous cell carcinomas (SCC), especially those arising from the oropharynx. Whereas HPV16 accounts for the majority of HPV DNA-positive oropharyngeal SCC, infections with other mucosal high-risk HPV types are quite rare and biological data demonstrating their causal involvement are insufficient. Here we present the first case of an oropharyngeal SCC driven by HPV type 58. A 69-year-old Caucasian woman presented with an enlarged and firm left tonsil. A computed tomography scan showed a left tonsillar mass, extending to the soft palate and the glossotonsillar sulcus. The patient underwent extended radical tonsillectomy and ipsilateral selective neck dissection. Pathology confirmed an infiltrating, poorly differentiated SCC of the left tonsil with node metastasis (pT2N1). Adjuvant external beam radiation therapy (60 Grays (Gy)) was administered. After 1 year of follow-up, the patient is well with no evidence of cancer recurrence. HPV analyses of the tumor tissue by BSGP5+/6+-PCR/MPG, targeting 51 mucosal HPV types, showed single positivity for HPV type 58. Presence of HPV58 E6*I RNA demonstrated biological activity of the virus in the tumor tissue, and presence of serum antibodies to HPV58 oncoproteins E6 and E7 indicated presence of an HPV58-driven cancer. Overexpression of cellular protein p16(INK4a) and reduced expression of pRb, two cellular markers for HPV-induced cell transformation, were observed. Exons 4-10 of TP53 showed no mutations or polymorphisms. The presence of HPV58 as single HPV infection in combination with a broad variety of direct and indirect markers of HPV transformation provides comprehensive evidence that this oropharyngeal SCC was driven by HPV58

    The predictive and prognostic potential of plasma telomerase reverse transcriptase (TERT) RNA in rectal cancer patients

    Background: Preoperative chemoradiotherapy (CRT) followed by surgery is the standard care for locally advanced rectal cancer, but tumour response to CRT and disease outcome are variable. The current study aimed to investigate the effectiveness of plasma telomerase reverse transcriptase (TERT) levels in predicting tumour response and clinical outcome. Methods: 176 rectal cancer patients were included. Plasma samples were collected at baseline (before CRT\ubcT0), 2 weeks after CRT was initiated (T1), post-CRT and before surgery (T2), and 4\u20138 months after surgery (T3) time points. Plasma TERT mRNA levels and total cell-free RNA were determined using real-time PCR. Results: Plasma levels of TERT were significantly lower at T2 (Po0.0001) in responders than in non-responders. Post-CRT TERT levels and the differences between pre- and post-CRT TERT levels independently predicted tumour response, and the prediction model had an area under curve of 0.80 (95% confidence interval (CI) 0.73\u20130.87). Multiple analysis demonstrated that patients with detectable TERT levels at T2 and T3 time points had a risk of disease progression 2.13 (95% CI 1.10\u20134.11)-fold and 4.55 (95% CI 1.48\u201313.95)-fold higher, respectively, than those with undetectable plasma TERT levels. Conclusions: Plasma TERT levels are independent markers of tumour response and are prognostic of disease progression in rectal cancer patients who undergo neoadjuvant therapy

    Clinical characteristics of coronavirus disease (COVID-19) early findings from a teaching hospital in Pavia, North Italy, 21 to 28 February 2020

    We describe clinical characteristics, treatments and outcomes of 44 Caucasian patients with coronavirus disease (COVID-19) at a single hospital in Pavia, Italy, from 21\u201328 February 2020, at the beginning of the outbreak in Europe. Seventeen patients developed severe disease, two died. After a median of 6 days, 14 patients were discharged from hospital. Predictors of lower odds of discharge were age>65 years, antiviral treatment and for severe disease, lactate dehydrogenase >300 mg/dL

    CLIVAR en la región del Atlántico: un esfuerzo conjunto hacia el mejor conocimiento de la variabilidad y predicción climática

    [EN] The CLIVAR (Climate Variability and Predictability) project, established in 1995 by the WCRP (World Climate Research Programme), aims at understanding the climate variability and improving models for climate predictions by coordinating the efforts of research groups in more than 70 countries around the world. CLIVAR activities consist of a balanced approach between observations, modelling and theory as well as diagnostic studies (http://www.clivar.org). CLIVAR objectives in the Atlantic sector focus on improving the understanding and assessing the predictability of three climate phenomena: the North Atlantic Oscillation, the tropical Atlantic variability and the variability of the Meridional Overturning Circulation. This paper attempts to give an overview of the state-of-the-art of these Atlantic modes of variability, the ongoing and planned CLIVAR activities related to these climatic phenomena and finally the recent advances in their predictability.[ES] El proyecto internacional CLIVAR (Climate Variability and Predictability), iniciado en 1995 dentro del WCRP(World Climate Research Programme), tiene como objetivo principal entender la variabilidad del clima y desarrollar mejores modelos para su predicción, a partir de estudios realizados por diferentes grupos de investigación pertenecientes a más de 70 países en el mundo.
