    Assessing the effectiveness of business support services in England: evidence from a theory based evaluation

    In England, publicly supported advisory services for small firms are organised primarily through the Business Link (BL) network. Based on the programme theory underlying this business support services we develop four propositions and test these empirically using data from a new survey of over 3,000 English small firms. Our empirical results provide a broad validation of the programme theory underlying BL assistance for small firms in England during 2003, and more limited support for its effectiveness. More specifically, we find strong support for the value of BL operators maintaining a high profile as a way of boosting take-up. We also find some support for the approach to market segmentation adopted by BL allowing more intensive assistance to be targeted on younger firms and those with limited liability status. In terms of the outcomes of BL support, and allowing for issues of sample selection, we find no significant effects on growth from ‘other’ assistance but do find positive and significant employment growth effects from intensive assistance. This provides partial support for the programme theory assertion that BL support will lead to improvements in business growth performance and stronger support for the proposition that there would be differential outcomes from intensive and other assistance. The positive employment growth outcomes identified here from intensive assistance, even allowing for sample selection, suggest something of an improvement in the effectiveness of the BL network since the late 1990s

    THE IMPACT ON DENTAL STUDENTS’ KNOWLEDGE IN THREE EUROPEAN COUNTRIES THROUGH AN ONLINE MODULE ON ANTIBIOTIC PRESCRIBING: A PRELIMINARY STUDY

    In this preliminary study, the knowledge and impact on dental student understanding of antibiotic prescribing practice in the three EU countries was evaluated through the intervention of an online module. 39 respondents completed the pre and post questionnaires. Austria: n= 10; UK: n= 19, Italy: n= 10. Overall there was a 10% improvement between the pre and post questionnaire tests. In Italy there was a 50% increase in knowledge (Figure 2). On a mixed linear regression model fitted to the total score, the difference between the post and pre-intervention means was highly significant; the total mean score increased on average by 0.9 (95% c.i. 0.28 to 1.52; P=0.004) after the intervention in question. Understanding antibiotic prescribing across Europe has been supported by the intervention of an online module that was perceived as a helpful and effective learning tool. Although there are still differing recommendations for antibiotic prescribing across Europe, students and staff must be alerted so they can practice safely across borders

It documents the severe weakening of workplace union organization over the last 25 years, which is reflected in the declining number of reps, reduced bargaining power and the problem of bureaucratization. But it also provides evidence of the continuing resilience, and even combativity in certain areas of employment, of workplace union reps organization, and considers the future potential for a revival of fortunes

    FDG-PET Quantification of Lung Inflammation with Image-Derived Blood Input Function in Mice

    Dynamic FDG-PET imaging was used to study inflammation in lungs of mice following administration of a virulent strain of Klebsiella (K.) pneumoniae. Net whole-lung FDG influx constant (Ki) was determined in a compartment model using an image-derived blood input function. Methods. K. pneumoniae (~3 x 105 CFU) was intratracheally administered to six mice with 6 other mice serving as controls. Dynamic FDG-PET and X-Ray CT scans were acquired 24 hr after K. pneumoniae administration. The experimental lung time activity curves were fitted to a 3-compartment FDG model to obtain Ki. Following imaging, lungs were excised and immunohistochemistry analysis was done to assess the relative presence of neutrophils and macrophages. Results. Mean Ki for control and K. pneumoniae infected mice were (5.1 ± 1.2) ×10−3 versus (11.4 ± 2.0) ×10−3 min−1, respectively, revealing a 2.24 fold significant increase (P = 0.0003) in the rate of FDG uptake in the infected lung. Immunohistochemistry revealed that cellular lung infiltrate was almost exclusively neutrophils. Parametric Ki maps by Patlak analysis revealed heterogeneous inflammatory foci within infected lungs. Conclusion. The kinetics of FDG uptake in the lungs of mice can be noninvasively quantified by PET with a 3-compartment model approach based on an image-derived input function

    Abstract The characteristics of intra-host human immunodeficiency virus type 1 (HIV-1) env evolution were evaluated in untreated HIV-1-infected subjects with different patterns of disease progression, including 2 normal progressor [NP], and 5 Long term non-progressor [LTNP] patients. High-resolution phylogenetic analysis of the C2-C5 env gene sequences of the replicating HIV-1 was performed in sequential samples collected over a 3–5 year period; overall, 301 HIV-1 genomic RNA sequences were amplified from plasma samples, cloned, sequenced and analyzed. Firstly, the evolutionary rate was calculated separately in the 3 codon positions. In all LTNPs, the 3rd codon mutation rate was equal or even lower than that observed at the 1st and 2nd positions (p = 0.016), thus suggesting strong ongoing positive selection. A Bayesian approach and a maximum-likelihood (ML) method were used to estimate the rate of virus evolution within each subject and to detect positively selected sites respectively. A great number of N-linked glycosylation sites under positive selection were identified in both NP and LTNP subjects. Viral sequences from 4 of the 5 LTNPs showed extensive positive selective pressure on the CD4-binding site (CD4bs). In addition, localized pressure in the area of the IgG-b12 epitope, a broad neutralizing human monoclonal antibody targeting the CD4bs, was documented in one LTNP subject, using a graphic colour grade 3-dimensional visualization. Overall, the data shown here documenting high selective pressure on the HIV-1 CD4bs of a group of LTNP subjects offers important insights for planning novel strategies for the immune control of HIV-1 infection.</p

    Zero-one survival behavior of cyclically competing species

    Coexistence of competing species is, due to unavoidable fluctuations, always transient. In this Letter, we investigate the ultimate survival probabilities characterizing different species in cyclic competition. We show that they often obey a surprisingly simple, though non-trivial behavior. Within a model where coexistence is neutrally stable, we demonstrate a robust zero-one law: When the interactions between the three species are (generically) asymmetric, the `weakest' species survives at a probability that tends to one for large population sizes, while the other two are guaranteed to extinct. We rationalize our findings from stochastic simulations by an analytic approach.Comment: 4 pages, 3 figure

    Low-Dose Imaging in a New Preclinical Total-Body PET/CT Scanner.

    Ionizing radiation constitutes a health risk to imaging scientists and study animals. Both PET and CT produce ionizing radiation. CT doses in pre-clinical in vivo imaging typically range from 50 to 1,000 mGy and biological effects in mice at this dose range have been previously described. [ &lt;sup&gt;18&lt;/sup&gt; F]FDG body doses in mice have been estimated to be in the range of 100 mGy for [ &lt;sup&gt;18&lt;/sup&gt; F]FDG. Yearly, the average whole body doses due to handling of activity by PET technologists are reported to be 3-8 mSv. A preclinical PET/CT system is presented with design features which make it suitable for small animal low-dose imaging. The CT subsystem uses a X-source power that is optimized for small animal imaging. The system design incorporates a spatial beam shaper coupled with a highly sensitive flat-panel detector and very fast acquisition (&lt;10 s) which allows for whole body scans with doses as low as 3 mGy. The mouse total-body PET subsystem uses a detector architecture based on continuous crystals, coupled to SiPM arrays and a readout based in rows and columns. The PET field of view is 150 mm axial and 80 mm transaxial. The high solid-angle coverage of the sample and the use of continuous crystals achieve a sensitivity of 9% (NEMA) that can be leveraged for use of low tracer doses and/or performing rapid scans. The low-dose imaging capabilities of the total-body PET subsystem were tested with NEMA phantoms, in tumor models, a mouse bone metabolism scan and a rat heart dynamic scan. The CT imaging capabilities were tested in mice and in a low contrast phantom. The PET low-dose phantom and animal experiments provide evidence that image quality suitable for preclinical PET studies is achieved. Furthermore, CT image contrast using low dose scan settings was suitable as a reference for PET scans. Total-body mouse PET/CT studies could be completed with total doses of &lt;10 mGy
