Tenascina e fibronectina em adenoma pleomórfico de glândula salivar
OBJECTIVES: To analyze the expression and distribution pattern of extracellular matrix components in pleomorphic adenomas of the major and minor salivary glands and to compare the morphological findings of these tumors with the immunohistochemical expression, considering the different types of stroma predominating in each case. METHODS AND RESULTS: The expression of tenascin (TN) and fibronectin (FN) was analyzed in 23 cases of pleomorphic adenomas, 11 major and 12 minor salivary gland tumors, by the streptavidin-biotin method using anti-tenascin and anti-fibronectin antibodies. In addition, the immunohistochemical results were correlated with the morphological findings of the lesions. All cases analyzed were immunoreactive for the antibodies used. Fibronectin showed strong labeling in fibrous and chondroid stroma, while labeling was weak in hyaline and myxoid stroma. Tenascin expression was more intense in fibrous and chondroid stroma and moderate in hyaline and myxoid stroma. CONCLUSIONS: No difference in the expression of these proteins was observed between major and minor salivary gland tumors.OBJETIVO: Analisar a expressão e o padrão de distribuição de componentes da matriz extracelular em adenomas pleomórficos de glândula salivar maior e menor e comparar os achados morfológicos destes tumores com a expressão imuno-histoquímica considerando os diferentes tipos de estromas presentes em cada caso. MÉTODOS E RESULTADOS: A expressão da tenascina (TN) e fibronectina (FN) foi analisada em 23 casos de adenomas pleomórficos, sendo 11 tumores em glândula salivar maior e 12 em glândula salivar menor, utilizando-se o método da estreptoavidina-biotina para os anticorpos anti-tenascina e anti-fibronectina. Os resultados imuno-histoquimicos foram correlacionados com os achados morfológicos das lesões. Todos os casos foram imunorreativos para a fibronectina mostrando forte expressão nos estromas fibrosos e condróides, embora fraca marcação tenha sido evidenciada nos estromas hialinos e mixóides. A expressão da tenascina foi mais intensa nos estromas condróides e fibrosos, sendo moderada nos estromas hialino e mixóide. CONCLUSÕES: Não houve diferença na expressão destas proteínas entre os tumores de glândula salivar maior e menor
Aplicabilidade da prototipagem rápida na odontologia – uma revisão de literatura
A prototipagem rápida é uma tecnologia de fabricação aditiva onde um modelo tridimensional é construído pela deposição de sucessivas camadas de material. O objetivo deste trabalho foi realizar uma revisão de literatura sobre a aplicabilidade da prototipagem rápida em especialidades da odontologia. Realizou-se uma abordagem indutiva com técnica de documentação indireta. Os dados obtidos foram submetidos a uma análise descritiva. Ao término da revisão, foi possível verificar que o desenvolvimento do estágio virtual ocorre a partir de softwares de modulação sólida (Computer-aided Design - CAD) ou da aquisição da imagem por scanner 3D, tomografia computadorizada ou ressonância magnética. Os processos mais usualmente aplicados são a estereolitografia, a modelagem por deposição fundida, a sinterização seletiva a laser, a impressão tridimensional, a thermojet e a polyjet. Essa técnica é utilizada na cirurgia bucomaxilofacial para o planejamento virtual de procedimentos cirúrgicos e personalização de placas de reconstrução; na implantodontia para a fabricação de biomodelos e guias cirúrgicas; na ortodontia para o diagnóstico e tratamento de caninos maxilares impactados e colagem de bráquetes; na prótese sua utilização consiste na confecção de próteses metálicas totais, próteses metalocerâmicas, coroas cerâmicas, próteses totais e próteses orbitárias, faciais e do septo nasal; e na endodontia para a documentação de casos não usuais. Conclui-se que, o crescente desenvolvimento da radiologia e imaginologia, em associação com a engenharia de softwares e processos de prototipagem rápida, tem possibilitado a introdução de procedimentos com mais segurança em odontologia e sua simplificação do ponto de vista operacional.
Oral health related quality of life and determinant factors in patients with head and neck cancer
The present study aimed to measure the impact of oral health on the quality of life of patients with head and neck cancer. A cross-sectional study was conducted with 130 patients diagnosed with head and neck cancer at two medical centers. Participants answered a sociodemographic questionnaire and the Oral Health Impact Profile - 14 (OHIP-14). Clinical aspects, cancer staging, and treatment approach were also investigated. Mann-Whitney and Kruskal-Wallis non-parametric tests were used for statistical analysis, followed by Poisson regression analysis (with robust error variance), to associate the OHIP-14 scores with independent variables. The OHIP-14 presented good internal consistency (Cronbach?s Alpha = 0.861). The mean score obtained was 19.52 (±11.79). Physical pain (3.70±2.44), physical disability (3.26±2.62) and functional limitation (3.24±2.45) were ranked as the main factors affecting the quality of life. Patients non-Caucasians (PR = 1.30; IC 95% = 1.07-1.58; p = 0.009), widowers (PR = 1.36; IC 95% = 1.13-1.64; p = 0.001), diagnosed with squamous cell carcinoma (PR = 1.28; IC 95% = 1.05-1.58; p = 0.017) and with temporomandibular pain (PR = 1.31; IC 95% = 1.08-1.60; p = 0.007) were more likely to exhibit lower rates of quality of life. The results showed a high impact of the oral health in the quality of life of patients with head and neck cancer was observed. Decrease in Mandibular Cortical in Patients With Type 1 Diabetes Mellitus Combined with Poor Glycemic Control
Análise da condição periodontal e da necessidade de tratamento em pacientes cardiopatas = Analysis of periodontal status and need for treatment in cardiac patients
Objetivo: avaliar a condição periodontal e a necessidade de tratamento em pacientes cardiopatas assistidos no Centro Hospitalar João XXIII em Campina Grande, estado da Paraíba. Métodos: o estudo utilizou uma amostra indutiva não probabilística, composta por 100 pacientes cardiopatas de ambos os gêneros, com idade superior a quarenta anos. Os instrumentos de pesquisa consistiram de um questionário e uma ficha clínica para registro do Índice de Necessidade de Tratamento Periodontal Comunitário da Organização Mundial da Saúde. Foram coletadas informações sociodemográficas, tipo de cardiopatia, presença de patologias sistêmicas e relativas à higiene bucal. O exame periodontal foi realizado sob adequada iluminação, por um único examinador previamente calibrado. Os dados foram organizados com o auxílio do Epi-Info 3. 3. 2 e apresentados por meio da estatística descritiva. Resultados: a maioria dos pacientes era do sexo masculino (53%), estado civil casado (74%), sem instrução formal (35%) e possuía entre 51 a 60 anos (32%). Verificou-se que 34% dos pacientes relatavam já ter sofrido infarto do miocárdio, enquanto 42,9% afirmaram ser hipertensos. Um percentual de 69% da amostra afirmou ter ido ao cirurgiãodentista pela última vez há mais de dois anos. Bolsa periodontal maior que 3,5 mm foi diagnosticada em 34,3% da amostra e 53,1% dos sextantes apresentavam cálculos dentários. Conclusões: a ocorrência de alterações clínicas sugestivas de doença periodontal avançada foi elevada nos pacientes cardiopatas estudado
Decrease in Mandibular Cortical in Patients With Type 1 Diabetes Mellitus Combined with Poor Glycemic Control
<div><p>Abstract In this study we analyzed the mandibular cortical bone of patients with type 1 diabetes mellitus (T1DM) and non-diabetic. Fifty patients with T1DM and 100 non-diabetic ones paired by age and gender were analyzed. Two double-blinded observers evaluated 150 digital panoramic images of both groups. The mandibular cortical bone was analyzed using the Mandibular Cortical Index (MCI), Mental Index (MI), Gonial Index (GI), Antegonial Index (AI) and Upper and Lower Panoramic Mandibular Indexes (UPMI and LPMI), with the aid of RADIOIMP® software. Influence of T1DM in the morphology of the mandibular cortical bone was studied based on obtaining data related to T1DM diagnosis time, blood glucose level, T1DM control and the presence of chronic complications. Collected data were submitted to descriptive (mean and standard deviation) and inferential analyzes (Pearson’s chi-squared test and Fisher’s exact test) (p≤0.05). According to the MCI, individuals with T1DM had higher frequencies of mandibular cortical alterations, in both sexes and all age groups (p<0.01). For quantitative indexes, MI, GI, AI, UPMI and LPMI, female patients showed statistically significant differences for GI and AI, while male patients had statistically significant differences for all indexes (p<0.05), presenting the individuals with TDM1 inferior measures. Individuals with poor T1DM control showed significantly higher frequency of mandibular cortical alteration (92.3%), with lower means for MI, GI, AI and LPMI (p≤0.05). In conclusion, patients with T1DM showed decrease in the mandibular cortical bone when compared to non-diabetic ones, indicating that poor disease control is associated with these alterations.</p></div
Radiographic evaluation of the dental condition of elderly people treated at a Brazilian public university
Aim: To assess the dental status of elderly patients examined in the
clinical dentistry course of a Brazilian public university. Methods: An
observational and descriptive study based on the analysis of panoramic
radiographs. The sample consisted of 60 elderly patients who met the
inclusion criteria. Two observers who had been trained in appropriate
conditions performed the radiographic analysis. Data were stored in a
specific form, recorded in a database and analyzed using descriptive
statistics (measures of central tendency and variability) and
inferential statistics (Mann-Whitney U test, Kruskal-Wallis test,
Chi-square analysis and calculation of the Cramer’s V
coefficient). Results: Most patients had at least one tooth in the oral
cavity (71.7%), while 28.3% were totally edentulous, an average of 10.5
teeth per individual. The average number of teeth was 11.36 in males
and 9.89 in females. The number of healthy teeth was 328 (5.47 per
patient). In this study, 88.3% of the subjects had periodontal bone
loss, with prevalence of moderate (35.0%) and severe (28.3%) bone loss.
Conclusions: Given the high incidence of edentulous individuals, the
high number of restored teeth and poor periodontal conditions, it is
concluded that the overall oral health status of the evaluated elderly
subjects is poor
LA PARTICIPACIÓN DE LAS CÉLULAS-TRONCO EN LA REGENERACIÓN PERIODONTAL
La enfermedad periodontal causa destrucción de los tejidos periodontales, incluyendo el ligamentoperiodontal, el hueso alveolar y el cemento, siendo considerada la mayor causa de pérdida dentaria enadultos. Una vez dañificado, el periodonto posee un limitado potencial de regeneración. La reconstruccióndel complejo periodontal destruido por la periodontitis es la mayor meta de la terapia periodontal. Paraeso varios tratamientos fueron propuestos, a pesar de basarse casi que completamente en laimplantación de substitutos estructurales con pequeño o ningún potencial reparador. Los progresosrecientes en la terapia con células-tronco y de nuevas técnicas basadas en la ingeniera tisular podrán serutilizados en las prácticas regenerativas. Las células-tronco pueden ser encontradas en muchos tejidosdentários adultos como pulpa y ligamento periodontal, y podrán ser utilizadas para regenerar los tejidosdel complejo periodontal. La reciente identificación de células-tronco dentro del ligamento periodontalrepresenta un desenvolvimiento significativo de nuevas terapéuticas clínicas direccionadas a laregeneración periodontal de manera previsible. Siendo así, este abordaje literario explora los conceptoscontemporáneos a cerca de la biología de las células-tronco, los recientes progresos en la identificaciónde estas, su potencial regenerador y sus implicaciones clínicas.ABSTRACT Periodontal diseases lead to the destruction of periodontal tissues, including periodontal ligament,cementum and alveolar bone, are a major cause of adult teeth loss. Once damaged, the periodontiumhas limited regeneration potential. The reconstruction of the complex destroyed by periodontitis is theperiodontal therapy top barrier. Aiming this, many treatments were proposed, almost all of them based on estructural substitute implantation with less on no repairing potential. The recent progress on stemcellstherapy and tissue enginnering based new technique will allow their use in regenerative practices.The stem-cells identification into the periodontal ligament and the cientific proofs of their applicability inperiodontal complex regeneration on predictable methods represent the periodontics revolution, providingtissue reconstruction never found on any other technique satisfactorily.
