Voices in the Band: A Doctor, Her Patients, and How the Outlook on AIDS Care Changed from Doomed to Hopeful
[Excerpt] I wanted to write a book about my patients and my colleagues and how we made it through the roller-coaster last decade of the twentieth century, how we moved from helplessly watching our patients die to being able to offer them a treatment course and a fairly normal life expectancy. In that time we saw some patients literally return from deathâs door. This kind of dramatic success has not been seen in any other field of medicine, except perhaps following the introduction of penicillin many years ago. I wanted to try to answer the question âWhat was it like?â What was it like to care for patients with AIDS, a disease that didnât even exist when I was in college? How did we deal with dying patients for whom we had a diagnosis but no treatment? How did we care for patients that many in society rejected, patients that many even within the field of medicine rejected? What happened in those years, when the prognosis for a patient with HIV went from nearly hopeless to very hopeful
Evaluation of a Peer Group Model of Supervision for the Allied Health Workforce in Queensland: A descriptive overview
Purpose: An evaluation of a large scale implementation of a peer group model of professional supervision was conducted to inform service planning and guide policy and practice. A descriptive overview of the findings is presented. Method: Allied health staff trained in peer group supervision were surveyed about their experience of the model, its challenges and benefits. Interviews were also conducted with senior managers. Results: Analysis of 248 responses indicated that 72% of trained staff had participated in peer group supervision, and that these peer groups had continued for an average of 17.2 months. The majority of groups adhered to the guidelines presented at training, and found the model easy to implement and adaptable to a range of professions, settings and needs. Reported benefits included skill development as well as increased support and confidence. Improved relationships and team culture were also described. Management support and attendance at training were considered important to successful implementation. Conclusions: The evaluation demonstrated that a model of peer group supervision can be successfully implemented with a diverse and geographically dispersed allied health workforce. This model allowed professional supervision needs to be met in a group setting without the requirement for an expert supervisor. A number of benefits for individual clinicians and their teams were identified, and the need for further evaluation, in the context of widespread health reform is noted
Clumpy Galaxies in CANDELS. I. The Definition of UV Clumps and the Fraction of Clumpy Galaxies at 0.5<z<3
Although giant clumps of stars are crucial to galaxy formation and evolution,
the most basic demographics of clumps are still uncertain, mainly because the
definition of clumps has not been thoroughly discussed. In this paper, we study
the basic demographics of clumps in star-forming galaxies (SFGs) at 0.5<z<3,
using our proposed physical definition that UV-bright clumps are discrete
star-forming regions that individually contribute more than 8% of the
rest-frame UV light of their galaxies. Clumps defined this way are
significantly brighter than the HII regions of nearby large spiral galaxies,
either individually or blended, when physical spatial resolution and
cosmological dimming are considered. Under this definition, we measure the
fraction of SFGs that contain at least one off-center clump (Fclumpy) and the
contributions of clumps to the rest-frame UV light and star formation rate of
SFGs in the CANDELS/GOODS-S and UDS fields, where our mass-complete sample
consists of 3239 galaxies with axial ratio q>0.5. The redshift evolution of
Fclumpy changes with the stellar mass (M*) of the galaxies. Low-mass
(log(M*/Msun)<9.8) galaxies keep an almost constant Fclumpy of about 60% from
z~3.0 to z~0.5. Intermediate-mass and massive galaxies drop their Fclumpy from
55% at z~3.0 to 40% and 15%, respectively, at z~0.5. We find that (1) the trend
of disk stabilization predicted by violent disk instability matches the Fclumpy
trend of massive galaxies; (2) minor mergers are a viable explanation of the
Fclumpy trend of intermediate-mass galaxies at z<1.5, given a realistic
observability timescale; and (3) major mergers are unlikely responsible for the
Fclumpy trend in all masses at z<1.5. The clump contribution to the rest-frame
UV light of SFGs shows a broad peak around galaxies with log(M*/Msun)~10.5 at
all redshifts, possibly linked to the molecular gas fraction of the galaxies.

Emergence of Hemagglutinin Mutations during the Course of Influenza Infection
Influenza remains a significant cause of disease mortality. The ongoing threat of influenza infection is partly attributable to the emergence of new mutations in the influenza genome. Among the influenza viral gene products, the hemagglutinin (HA) glycoprotein plays a critical role in influenza pathogenesis, is the target for vaccines and accumulates new mutations that may alter the efficacy of immunization. To study the emergence of HA mutations during the course of infection, we employed a deep-targeted sequencing method. We used samples from 17 patients with active H1N1 or H3N2 influenza infections. These patients were not treated with antivirals. In addition, we had samples from five patients who were analyzed longitudinally. Thus, we determined the quantitative changes in the fractional representation of HA mutations during the course of infection. Across individuals in the study, a series of novel HA mutations directly altered the HA coding sequence were identified. Serial viral sampling revealed HA mutations that either were stable, expanded or were reduced in representation during the course of the infection. Overall, we demonstrated the emergence of unique mutations specific to an infected individual and temporal genetic variation during infection
Oxford SWIFT IFS and multi-wavelength observations of the Eagle galaxy at z=0.77
The `Eagle' galaxy at a redshift of 0.77 is studied with the Oxford Short
Wavelength Integral Field Spectrograph (SWIFT) and multi-wavelength data from
the All-wavelength Extended Groth strip International Survey (AEGIS). It was
chosen from AEGIS because of the bright and extended emission in its slit
spectrum. Three dimensional kinematic maps of the Eagle reveal a gradient in
velocity dispersion which spans 35-75 +/- 10 km/s and a rotation velocity of 25
+/- 5 km/s uncorrected for inclination. Hubble Space Telescope images suggest
it is close to face-on. In comparison with galaxies from AEGIS at similar
redshifts, the Eagle is extremely bright and blue in the rest-frame optical,
highly star-forming, dominated by unobscured star-formation, and has a low
metallicity for its size. This is consistent with its selection. The Eagle is
likely undergoing a major merger and is caught in the early stage of a
star-burst when it has not yet experienced metal enrichment or formed the mass
of dust typically found in star-forming galaxies.Comment: accepted for publication in MNRA
Understanding young people's transitions in university halls through space and time
This article contributes to the theoretical discussion about young people's transitions through space and time. Space and time are complex overarching concepts that have creative potential in deepening understanding of transition. The focus of this research is young people's experiences of communal living in university halls. It is argued that particular space-time concepts draw attention to different facets of experience and in combination deepen the understanding of young people's individual and collective transitions. The focus of the article is the uses of the space-time concepts 'routine', 'representation', 'rhythm' and 'ritual' to research young people's experiences. The article draws on research findings from two studies in the North of England. © 2010 SAGE Publications
Deficiency in origin licensing proteins impairs cilia formation: implications for the aetiology of meier-gorlin syndrome
Mutations in ORC1, ORC4, ORC6, CDT1, and CDC6, which encode proteins required for DNA replication origin licensing, cause Meier-Gorlin syndrome (MGS), a disorder conferring microcephaly, primordial dwarfism, underdeveloped ears, and skeletal abnormalities. Mutations in ATR, which also functions during replication, can cause Seckel syndrome, a clinically related disorder. These findings suggest that impaired DNA replication could underlie the developmental defects characteristic of these disorders. Here, we show that although origin licensing capacity is impaired in all patient cells with mutations in origin licensing component proteins, this does not correlate with the rate of progression through S phase. Thus, the replicative capacity in MGS patient cells does not correlate with clinical manifestation. However, ORC1-deficient cells from MGS patients and siRNA-mediated depletion of origin licensing proteins also have impaired centrosome and centriole copy number. As a novel and unexpected finding, we show that they also display a striking defect in the rate of formation of primary cilia. We demonstrate that this impacts sonic hedgehog signalling in ORC1-deficient primary fibroblasts. Additionally, reduced growth factor-dependent signaling via primary cilia affects the kinetics of cell cycle progression following cell cycle exit and re-entry, highlighting an unexpected mechanism whereby origin licensing components can influence cell cycle progression. Finally, using a cell-based model, we show that defects in cilia function impair chondroinduction. Our findings raise the possibility that a reduced efficiency in forming cilia could contribute to the clinical features of MGS, particularly the bone development abnormalities, and could provide a new dimension for considering developmental impacts of licensing deficiency
