    Use Of The Vascularized Iliac-crest Flap In Musculoskeletal Lesions.

    Bone loss was in the past treated by several methods, such as bone distraction and the use of nonvascularized or tissue-bank bone grafts. With the advent of modern microsurgical techniques, the vascularized bone flap has been used with good results; it resolves local nutritional problems, repairs soft tissue that is often damaged by severe trauma, and treats bone loss due to tumors, pseudarthroses, and osteomyelitis. This paper reports the authors' experience with the use of vascularized iliac-crest flaps to treat orthopedic pathologies in five patients with traumatic bone loss (<10 cm), three with osteomyelitis, and three with atrophic nonunion. In all cases, the same surgeon obtained a vascularized iliac-crest flap with a pedicle based on the deep iliac circumflex artery. All flaps consolidated within a mean period of 3 months. These findings demonstrate that the use of an iliac-crest flap is a treatment option in cases of bone loss and that it is associated with good functional results and minimal donor-site morbidity.201323714

    Esporotricose é a micose subcutânea mais comum no mundo, causada por fungos dimórficos e geofílicos, das espécies Sporothrix spp. A forma linfo cutânea é a apresentação clinica mais comum e o acometimento facial é mais prevalente na faixa etária pediátrica, devido à baixa estatura e ao hábito de brincar com felinos próximo a face. Nesse relato de caso, uma paciente do sexo feminino, com diagnóstico prévio de rosácea, residente na cidade do Rio de Janeiro, área epidêmica da doença, apresentou lesões ulceradas no dorso nasal, lábio superior e glabela, diagnosticada tardiamente como esporotricose.Sporotrichosis is the most frequent subcutaneous mycosis worldwide, caused by a geophilic and dimorphic fungus of the species Sporothrix spp. Lymphocutaneous form is the most common clinical presentation and facial involvement is more prevalent in pediatric patients due to low stature and leisure habits of holding cats close to the face.     Biomechanical evaluation in runners with Achilles tendinopathy

    OBJECTIVES: To evaluate the clinical characteristics, ground reaction force (GRF), and function of the plantar muscles and dorsiflexors of the ankle in runners with and without Achilles tendinopathy (AT) and in nonrunners. METHODS: Seventy-two participants (42 men, 30 women; mean age: 37.3±9.9 years) were enrolled in this crosssectional study and divided into three groups: AT group (ATG, n=24), healthy runners’ group (HRG, n=24), and non-runners’ group (NRG, n=24). Both ankles were evaluated in each group. The American Orthopedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale was used for clinical and functional evaluation. GRF was evaluated using force plates and muscle strength was evaluated using an isokinetic dynamometer. RESULTS: The AOFAS scores were lower in the ATG. The strike impulse was higher in the ATG than in the HRG and NRG. However, GRF was similar among the groups. The ATG exhibited lower total work at 120o /s speed than the HRG. The peak torque in concentric dorsiflexion was lower in the NRG than in the ATG and HRG. The peak torque and total work in concentric plantar flexion were lower in the NRG than in the ATG. The peak torque and total work in eccentric plantar flexion were lower in the NRG than in the ATG and HRG. CONCLUSION: Runners with AT showed higher strike impulse, lower muscle strength of the plantar flexors, and higher clinical and functional damage

    VEGETALS AS NATURAL SOURCES OF ANTIOXIDANTS. Growing knowledge on the health-promoting impact of antioxidants in everyday foods, combined with the assumption that a number of common synthetic preservatives may have hazardous side effects has led to increased investigations in the field of natural antioxidants, principally those found in plants. Food industries normally discard plant residues that could benefit the human health and diminish undesirable environmental impact. Once estimated the content of antioxidants in these residues, advantageous economical and social alternatives to the discard are possible, for example, their use for preparation of nutraceuticals to be offered to low-income populations.     A cyclopalladated complex interacts with mitochondrial membrane thiol-groups and induces the apoptotic intrinsic pathway in murine and cisplatin-resistant human tumor cells

    <p>Abstract</p> <p>Background</p> <p>Systemic therapy for cancer metastatic lesions is difficult and generally renders a poor clinical response. Structural analogs of cisplatin, the most widely used synthetic metal complexes, show toxic side-effects and tumor cell resistance. Recently, palladium complexes with increased stability are being investigated to circumvent these limitations, and a biphosphinic cyclopalladated complex {Pd<sub>2 </sub>[<it>S<sub>(-)</sub></it>C<sup>2</sup>, N-dmpa]<sub>2 </sub>(μ-dppe)Cl<sub>2</sub>} named C7a efficiently controls the subcutaneous development of B16F10-Nex2 murine melanoma in syngeneic mice. Presently, we investigated the melanoma cell killing mechanism induced by C7a, and extended preclinical studies.</p> <p>Methods</p> <p>B16F10-Nex2 cells were treated <it>in vitro </it>with C7a in the presence/absence of DTT, and several parameters related to apoptosis induction were evaluated. Preclinical studies were performed, and mice were endovenously inoculated with B16F10-Nex2 cells, intraperitoneally treated with C7a, and lung metastatic nodules were counted. The cytotoxic effects and the respiratory metabolism were also determined in human tumor cell lines treated <it>in vitro </it>with C7a.</p> <p>Results</p> <p>Cyclopalladated complex interacts with thiol groups on the mitochondrial membrane proteins, causes dissipation of the mitochondrial membrane potential, and induces Bax translocation from the cytosol to mitochondria, colocalizing with a mitochondrial tracker. C7a also induced an increase in cytosolic calcium concentration, mainly from intracellular compartments, and a significant decrease in the ATP levels. Activation of effector caspases, chromatin condensation and DNA degradation, suggested that C7a activates the apoptotic intrinsic pathway in murine melanoma cells. In the preclinical studies, the C7a complex protected against murine metastatic melanoma and induced death in several human tumor cell lineages <it>in vitro</it>, including cisplatin-resistant ones. The mitochondria-dependent cell death was also induced by C7a in human tumor cells.</p> <p>Conclusions</p> <p>The cyclopalladated C7a complex is an effective chemotherapeutic anticancer compound against primary and metastatic murine and human tumors, including cisplatin-resistant cells, inducing apoptotic cell death via the intrinsic pathway.</p

    Orientador: Alberto Cliquet JuniorDissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências MédicasResumo: OBJETIVO O objetivo do presente estudo é avaliar a concordância entre os escores de Tokuhashi e Tomita, com o prognóstico dos pacientes acometidos por metástases vertebral por tumor de mama, assistidos no ambulatório da Universidade Estadual de Campinas (UNICAMP). MÉTODOS Foram avaliados, retrospectivamente, 29 pacientes com metástases por tumor de mama vertebral. Vinte pacientes submetidos a abordagem cirúrgica e a terapia adjuvante e 09 apenas com tratamento conservador (quimioterapia/ radioterapia) ou paliativo/suporte, dependendo da pontuação dos escores de Tokuhashi e Tomita . RESULTADOS No estudo, os pacientes selecionados são do sexo feminino acometidas por metástase vertebral por tumor de mama; idade média de 57,6 anos (DP = 11,8 anos). A acurácia da escala de Tokuhashi foi de 62,1% e a de Tomita 72,4%. Além disso, a escala de Tomita concentra a maioria das classificações dos pacientes no grupo > 12 meses (69%), indicando boa relação com o prognóstico a longo prazo (>12 meses). Nenhuma das características avaliadas- idade ou cirurgia- influenciou estatisticamente na sobrevida das pacientes com tumor primário de mama (p > 0,05). CONCLUSÃO Os escores de Tokuhashi e de Tomita apresentaram boa acurácia com o prognóstico dos pacientes acometidos por metástase de tumor de mama para a coluna vertebralAbstract: OBJECTIVE The aim of the present study was to evaluate the agreement between the Tokuhashi and Tomita scores, with the prognosis of patients with vertebral metastases due to breast tumor, assisted at the outpatient clinic of the Universidade Estadual de Campinas(UNICAMP). METHODS Twenty-nine patients with vertebral breast metastases were retrospectively evaluated. Twenty patients undergoing surgical approach and adjuvant therapy and 09 with conservative treatment (chemotherapy / radiotherapy) or palliative / support, depending on Tokuhashi and Tomita score scores. RESULTS In this study, all selected patients are female with vertebral metastasis due to breast tumor; Mean age of 57.6 years (SD = 11.8 years). The accuracy of the Tokuhashi scale was 62.1% and that of Tomita 72.4%. In addition, the Tomita scale concentrates the majority of patients scores in the group> 12 months (69%), indicating a good relation with the long-term prognosis (> 12 months). None of the evaluated characteristics - age or surgery - statistically influenced the survival of patients with primary breast tumor (p> 0.05).
