    Towards a community of practice : evidence-based advocacy and policy change

    Supporting Colansa, the Community of Practice for healthy food systems, CLAS promoted evidence-based advocacy and policy change. In nine months over 2021-22, we set out to (1) position healthy and sustainable food systems among authorities and stakeholders; (2) support improved interaction between advocates and academics; (3) evaluate public awareness among stakeholders to improve communications; and (4) develop a strategy for reaching policy makers. Methods included group consensus, convening key actors, expert opinion, education for adults, and exploring social listening. To position the topic with authorities, Ministers of Health (MoH) and Finance were added to an electronic platform. MoH received a letter informing them about Colansa and its Manifesto. The “Regional NCD Agenda 2022-2030” included the collaboration with Colansa. To support interactions between advocates and academics, 2 videos and an interactive 2-day virtual session engaged them. The 51 participants discussed challenges of research for policy change, Mexico and Brazil experiences, and communicating research to non-academics. We convened and surveyed communication specialists and explored social listening in Central America showing there was a need for more frequent information and translating science into easy-to-understand concepts.     Prevalence of the Metabolic Syndrome in Latin America and its association with sub-clinical carotid atherosclerosis: the CARMELA cross sectional study

    <p>Abstract</p> <p>Background</p> <p>Metabolic syndrome increases cardiovascular risk. Limited information on its prevalence in Latin America is available. The Cardiovascular Risk Factor Multiple Evaluation in Latin America (CARMELA) study included assessment of metabolic syndrome in 7 urban Latin American populations.</p> <p>Methods</p> <p>CARMELA was a cross-sectional, population-based, observational study conducted in Barquisimeto, Venezuela; Bogota, Colombia; Buenos Aires, Argentina; Lima, Peru; Mexico City, Mexico; Quito, Ecuador; and Santiago, Chile. The prevalence of metabolic syndrome, defined according to the National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III), and associated carotid atherosclerosis were investigated in 11,502 participants aged 25 to 64 years.</p> <p>Results</p> <p>Across CARMELA cities, metabolic syndrome was most prevalent in Mexico City (27%) and Barquisimeto (26%), followed by Santiago (21%), Bogota (20%), Lima (18%), Buenos Aires (17%), and Quito (14%). In nondiabetic participants, prevalence was slightly lower but followed a comparable ranking. Overall, 59%, 59%, and 73% of women with high triglycerides, hypertension, or glucose abnormalities, respectively, and 64%, 48% and 71% of men with abdominal obesity, hypertension, or glucose abnormalities, respectively, had the full metabolic syndrome. Prevalence of metabolic syndrome increased with age, markedly so in women. Mean common carotid artery intima-media thickness (CCAIMT) and prevalence of carotid plaque increased steeply with increasing numbers of metabolic syndrome components; mean CCAIMT was higher and plaque more prevalent in participants with metabolic syndrome than without.</p> <p>Conclusion</p> <p>The prevalence of metabolic syndrome and its components by NCEP ATP III criteria was substantial across cities, ranging from 14% to 27%. CARMELA findings, including evidence of the association of metabolic syndrome and carotid atherosclerosis, should inform appropriate clinical and public health interventions.</p

    Analysis of a voluntary initiative to reduce sodium in processed and ultra-processed food products in Argentina : the views of public and private sector representatives

    Includes abstract in Spanish and PortugueseThis study is part of a comprehensive quantitative and qualitative research strategy to monitor sodium reduction policies in Argentina. The qualitative study design aims to contribute to the body of research in the public health sector regarding voluntary initiatives and public-private partnerships (PPPs) towards prevention of non-communicable diseases, and to understand the policy process of PPPs from stakeholders’ perspectives. Objectives include: (1) to identify the role and resources of the different major stakeholders; (2) analyze the main challenges and controversies perceived by stakeholders; and (3) analyze stakeholders’ perspectives concerning mandatory vs. voluntary initiatives. The article discusses the initiative’s challenges and controversies.El programa Menos Sal, Más Vida fue la primera iniciativa voluntaria para la reducción de la sal en Argentina. Este artículo analiza las perspectivas de los representantes del sector público y privado involucrados en este acuerdo voluntario, entre el Ministerio de Salud y la industria alimentaria, para reducir gradualmente el contenido de sodio en las comidas procesadas. Este estudio de caso se basó en una aproximación cualitativa, incluyendo 29 entrevistas en profundidad, con las partes interesadas del sector público y privado e identificó el papel de los mismos y sus percepciones respecto a los desafíos enfrentados durante el proceso, con el fin de contribuir al debate de las colaboraciones público-privadas en políticas de salud.     Institutional research on public opinion and participative devices. From polls to public debate

    Considering that the methods and techniques implemented in social research entail a certain definition of the concepts studied, it seems relevant to reflect on the possibilities of devices other than polls when studying “public opinion” as a sociological concept. Two arguments for such approach can be highlighted: the qualitative and discursive nature of public opinion, and polls’ difficulties for being collectively perceived as a reliable and interesting way to express points of view about public matters. The French CNDP (Commission Nationale du Débat Public) is conceived for encouraging national, regional or local debates about political decisions which involve an important governmental investment. Taking into account that &lt;i&gt;deliberative polling&lt;/i&gt; has probably been the most systematized among numerous research devices which have tried to implement the premises of deliberative/participative democracy, we will explore the possibilities of a different and wider representation of “public opinion” from the analysis of the dynamics generated by the institutional device of Public Debate.&lt;br&gt;&lt;br&gt;Sosteniendo la hipótesis de que los métodos y técnicas utilizadas para el estudio de los conceptos de interés sociológico tienen cierta influencia en la definición de los mismos, este artículo plantea una reflexión sobre las posibilidades e implicaciones de dispositivos diferentes a las encuestas para la investigación de la opinión pública. La aproximación a este enfoque se realizará desde dos argumentaciones: por un lado, sosteniendo la naturaleza cualitativa y discursiva de la opinión pública y, por otro, mostrando la dificultad de los sondeos para ser percibidos colectivamente como un medio confiable e interesante para expresar y registrar puntos de vista sobre los asuntos públicos. La CNDP (Comisión Nacional de Debate Público) es una institución pública francesa creada para animar debates nacionales, regionales o locales sobre grandes operaciones públicas. Teniendo en cuenta que la encuesta deliberativa ha sido probablemente el dispositivo que, con mayor grado de sistematización, ha intentado implementar las premisas de la democracia deliberativa/participativa, este artículo explorará las posibilidades de una forma más amplia de concebir la “opinión pública”, desde el análisis de las dinámicas generadas por el dispositivo institucional del Debate Público

    Successful up-scaled population interventions to reduce risk factors for non-communicable disease in adults: Results from the International Community Interventions for Health (CIH) project in China, India and Mexico

    Background: Non-communicable disease (NCD) is increasing rapidly in low and middle-income countries (LMIC), and is associated with tobacco use, unhealthy diet and physical inactivity. There is little evidence for up-scaled interventions at the population level to reduce risk in LMIC. Methods: The Community Interventions for Health (CIH) program was a population-scale community intervention study with comparator population group undertaken in communities in China, India, and Mexico, each with populations between 150,000-250,000. Culturally appropriate interventions were delivered over 18-24 months. Two independent cross-sectional surveys of a stratified sample of adults aged 18-64 years were conducted at baseline and follow-up. Results: A total of 6,194 adults completed surveys at baseline, and 6,022 at follow-up. The proportion meeting physical activity recommendations decreased significantly in the control group (C) (44.1 to 30.2%), but not in the intervention group (I) (38.0 to 36.1%), p<0.001. Those eating ≥5 portions of fruit and vegetables daily decreased significantly in C (19.2 to 17.2%), but did not change in I (20.0 to 19.6%,), p=0.013. The proportion adding salt to food was unchanged in C (24.9 to 25.3%) and decreased in I (25.9 to 19.6%), p<0.001. Prevalence of obesity increased in C (8.3 to 11.2%), with no change in I (8.6 to 9.7%,) p=0.092. Concerning tobacco, for men the difference-in-difference analysis showed that the reduction in use was significantly greater in I compared to C (p=0.014) Conclusions: Up-scaling known health promoting interventions designed to reduce the incidence of NCD in whole communities in LMIC is feasible, and has measurable beneficial outcomes on risk factors for NCD, namely tobacco use, diet, and physical inactivity

    Objective:Non-alcoholic fatty liver disease (NAFLD) is a complex pathology in which several dysfunctions, including alterations in metabolicpathways, mitochondrial functionality and unbalanced lipid import/export, lead to lipid accumulation and progression to inflammation andfibrosis.The enzyme glycine N-methyltransferase (GNMT), the most important enzyme implicated in S-adenosylmethionine catabolism in the liver, isdownregulated during NAFLD progression. We have studied the mechanism involved in GNMT downregulation by its repressor microRNA miR-873-5p and the metabolic pathways affected in NAFLD as well as the benefit of recovery GNMT expression.Methods:miR-873-5p and GNMT expression were evaluated in liver biopsies of NAFLD/NASH patients. Differentin vitroandin vivoNAFLD murinemodels were used to assess miR-873-5p/GNMT involvement in fatty liver progression through targeting of the miR-873-5p as NAFLD therapy.Results:We describe a new function of GNMT as an essential regulator of Complex II activity in the electron transport chain in the mitochondria.In NAFLD, GNMT expression is controlled by miR-873-5p in the hepatocytes, leading to disruptions in mitochondrial functionality in a preclinicalmurine non-alcoholic steatohepatitis (NASH) model. Upregulation of miR-873-5p is shown in the liver of NAFLD/NASH patients, correlating withhepatic GNMT depletion. Importantly, NASH therapies based on anti-miR-873-5p resolve lipid accumulation, inflammation andfibrosis byenhancing fatty acidb-oxidation in the mitochondria. Therefore, miR-873-5p inhibitor emerges as a potential tool for NASH treatment.Conclusion:GNMT participates in the regulation of metabolic pathways and mitochondrial functionality through the regulation of Complex II activityin the electron transport chain.     A Deep Insight into the Sialome of Rhodnius neglectus, a vector of chagas disease

    Background Triatomines are hematophagous insects that act as vectors of Chagas disease. Rhodnius neglectus is one of these kissing bugs found, contributing to the transmission of this American trypanosomiasis. The saliva of hematophagous arthropods contains bioactive molecules responsible for counteracting host haemostatic, inflammatory, and immuneresponses. Methods/Principal Findings Next generation sequencing and mass spectrometry-based protein identification were performed to investigate the content of triatomine R. neglectus saliva.We deposited 4,230 coding DNA sequences (CDS) in GenBank. A set of 636 CDS of proteins of putative secretory nature was extracted from the assembled reads, 73 of them confirmed by proteomic analysis. The sialome of R. neglectus was characterized and serine protease transcripts detected. The presence of ubiquitous protein families was revealed, including lipocalins, serine protease inhibitors, and antigen-5. Metalloproteases, disintegrins, and odorant binding protein families were less abundant. Conclusions/Significance The data presented improve our understanding of hematophagous arthropod sialomes, and aid in understanding hematophagy and the complex interplay among vectors and their vertebrate hosts

    Hypertension pharmacological treatment in adults : a world health organization guideline executive summary

    Hypertension is a major cause of cardiovascular disease and deaths worldwide especially in low- and middle-income countries. Despite the availability of safe, well-tolerated, and cost-effective blood pressure (BP)-lowering therapies, <14% of adults with hypertension have BP controlled to a systolic/diastolic BP <140/90 mm Hg. We report new hypertension treatment guidelines, developed in accordance with the World Health Organization Handbook for Guideline Development. Overviews of reviews of the evidence were conducted and summary tables were developed according to the Grading of Recommendations, Assessment, Development, and Evaluations approach. In these guidelines, the World Health Organization provides the most current and relevant evidence-based guidance for the pharmacological treatment of nonpregnant adults with hypertension. The recommendations pertain to adults with an accurate diagnosis of hypertension who have already received lifestyle modification counseling. The guidelines recommend BP threshold to initiate pharmacological therapy, BP treatment targets, intervals for follow-up visits, and best use of health care workers in the management of hypertension. The guidelines provide guidance for choice of monotherapy or dual therapy, treatment with single pill combination medications, and use of treatment algorithms for hypertension management. Strength of the recommendations was guided by the quality of the underlying evidence; the tradeoffs between desirable and undesirable effects; patient’s values, resource considerations and cost-effectiveness; health equity; acceptability, and feasibility consideration of different treatment options.
