    Mental health related determinants of parenting stress among urban mothers of young children – results from a birth-cohort study in Ghana and Cîte d’Ivoire

    Background There are limited data on the parenting stress (PS) levels in sub-Saharan African mothers and on the association between ante- and postnatal depression and anxiety on PS. Methods A longitudinal birth cohort of 577 women from Ghana and Cîte d’Ivoire was followed from the 3rd trimester in pregnancy to 2 years postpartum between 2010 and 2013. Depression and anxiety were assessed by the Patient Health Questionnaire depression module (PHQ-9) and the Generalized Anxiety Disorder (GAD-7) at baseline, 3 month, 12 month and 24 month postpartum. PS was measured using the Parenting Stress Index-Short Form (PSI-SF) at 3, 12 and 24 month. The mean total PS score and the subscale scores were compared among depressed vs. non-depressed and among anxious vs. non-anxious mothers at 3, 12 and 24 month postpartum. The proportions of clinical PS (PSI-SF raw score > 90) in depressed vs. non-depressed and anxious vs. non-anxious mothers were also compared. A generalized estimating equation (GEE) approach was used to estimate population-averaged associations between women’s depression/anxiety and PS adjusting for age, child sex, women’s anemia, education, occupation, spouse’s education, and number of sick child visits. Results A total of 577, 531 and 264 women completed the PS assessment at 3 month, 12 month and 24 month postpartum across the two sites and the prevalences of clinical PS at each time point was 33.1%, 24.4% and 14.9% in Ghana and 30.2%, 33.5% and 22.6% in Cîte d’Ivoire, respectively. At all three time points, the PS scores were significantly higher among depressed mothers vs. non-depressed mothers. In the multivariate regression analyses, antepartum and postpartum depression were consistently associated with PS after adjusting for other variables. Conclusions Parenting stress is frequent and levels are high compared with previous studies from high-income countries. Antepartum and postpartum depression were both associated with PS, while antepartum and postpartum anxiety were not after adjusting for confounders. More quantitative and qualitative data are needed in sub-Saharan African populations to assess the burden of PS and understand associated mechanisms. Should our findings be replicated, it appears prudent to design and subsequently evaluate intervention strategies

    Individual regional associations between AÎČ-, tau- and neurodegeneration (ATN) with microglial activation in patients with primary and secondary tauopathies.

    ÎČ-amyloid (AÎČ) and tau aggregation as well as neuronal injury and atrophy (ATN) are the major hallmarks of Alzheimer's disease (AD), and biomarkers for these hallmarks have been linked to neuroinflammation. However, the detailed regional associations of these biomarkers with microglial activation in individual patients remain to be elucidated. We investigated a cohort of 55 patients with AD and primary tauopathies and 10 healthy controls that underwent TSPO-, AÎČ-, tau-, and perfusion-surrogate-PET, as well as structural MRI. Z-score deviations for 246 brain regions were calculated and biomarker contributions of AÎČ (A), tau (T), perfusion (N1), and gray matter atrophy (N2) to microglial activation (TSPO, I) were calculated for each individual subject. Individual ATN-related microglial activation was correlated with clinical performance and CSF soluble TREM2 (sTREM2) concentrations. In typical and atypical AD, regional tau was stronger and more frequently associated with microglial activation when compared to regional AÎČ (AD: ÎČT = 0.412 ± 0.196 vs. ÎČA = 0.142 ± 0.123, p < 0.001; AD-CBS: ÎČT = 0.385 ± 0.176 vs. ÎČA = 0.131 ± 0.186, p = 0.031). The strong association between regional tau and microglia reproduced well in primary tauopathies (ÎČT = 0.418 ± 0.154). Stronger individual associations between tau and microglial activation were associated with poorer clinical performance. In patients with 4RT, sTREM2 levels showed a positive association with tau-related microglial activation. Tau pathology has strong regional associations with microglial activation in primary and secondary tauopathies. Tau and AÎČ related microglial response indices may serve as a two-dimensional in vivo assessment of neuroinflammation in neurodegenerative diseases

    Genetic screens identify a context-specific PI3K/p27Kip1 node driving extrahepatic biliary cancer

    Biliary tract cancer ranks among the most lethal human malignancies, representing an unmet clinical need. Its abysmal prognosis is tied to an increasing incidence and a fundamental lack of mechanistic knowledge regarding the molecular basis of the disease. Here, we show that the Pdx1-positive extrahepatic biliary epithelium is highly susceptible toward transformation by activated PIK3CAH1047R but refractory to oncogenic KrasG12D. Using genome-wide transposon screens and genetic loss-of-function experiments, we discover context-dependent genetic interactions that drive extrahepatic cholangiocarcinoma (ECC) and show that PI3K signaling output strength and repression of the tumor suppressor p27Kip1 are critical context-specific determinants of tumor formation. This contrasts with the pancreas, where oncogenic Kras in concert with p53 loss is a key cancer driver. Notably, inactivation of p27Kip1 permits KrasG12D-driven ECC development. These studies provide a mechanistic link between PI3K signaling, tissue-specific tumor suppressor barriers, and ECC pathogenesis, and present a novel genetic model of autochthonous ECC and genes driving this highly lethal tumor subtype

Die Literaturlage lĂ€sst dabei erkennen, welch großes Potenzial FĂŒhrung zur Leistungssteigerung von Organisationen zugeschrieben wird. Angesichts dieser Relevanz erstaunt, wie wenig FĂŒhrung als soziales PhĂ€nomen theoretisiert ist. Der vorliegende Beitrag schließt diese LĂŒcke mit einer theoretisch integrierten Begriffsbestimmung von FĂŒhrung. FĂŒhrung wird als situativ erfolgreiche Einflussnahme in kritischen Momenten konzipiert, die sich in einer Sequenz von drei Ereignissen realisiert: Das Auslöseereignis ist eine soziale Situation, in der ein kritischer Moment entsteht, der FĂŒhrung ĂŒberhaupt notwendig macht (I). Darauf folgt ein kommunikativ erhobener FĂŒhrungsanspruch, in dem Einflussmittel mobilisiert werden (II). Am Entstehen von Gefolgschaft (III) zeigt sich, ob FĂŒhrung stattgefunden hat. Das vorgeschlagene FĂŒhrungsverstĂ€ndnis nimmt einen dezidiert organisationssoziologischen Blickwinkel ein, indem es den Unterschied zwischen FĂŒhrung und Hierarchie hervorhebt und fĂŒr die Theoretisierung fruchtbar macht. Auf diese Weise erschließt sich ein problemsensibles, umfassendes, flexibles und zugleich trennscharfes FĂŒhrungsverstĂ€ndnis. Dieser Blickwinkel berĂŒcksichtigt, dass Organisationen sich nicht immer an ihre eigenen Regeln halten, also z. B. Hierarchien nicht unumstĂ¶ĂŸlich festlegen, wer in FĂŒhrung geht. Er ermöglicht außerdem, interaktionale Dynamiken mitzubeobachten, die sich einer planvollen Steuerung entziehen. So wird erklĂ€rbar, dass nicht nur von oben nach unten gefĂŒhrt werden kann, sondern ebenfalls unter Gleichen oder von unten nach oben.     Matrix metalloproteinases expression in spontaneous canine histiocytic sarcomas and its xenograft model

    Canine histiocytic sarcoma (HS) represents a malignant neoplastic disorder often with a rapid and progressive clinical course. A better understanding of the interaction between tumor cells and the local microenvironment may provide new insights into mechanisms of tumor growth and metastasis. The influence of matrix metalloproteinases (MMPs) and their inhibitors (TIMPs) on tumor angiogenesis, invasion and metastasis has been detailed in previous studies. In addition, inflammatory cells infiltrating neoplasms especially tumor associated macrophages (TAM) may contribute significantly to tumor progression. Due to the high variability of spontaneously occurring canine HS, standardized models are highly required to investigate tumor progression and interaction with its microenvironment. Therefore, the present study comparatively characterized the intratumoral macrophage infiltration as well as the expression of MMP-2, MMP-9, MMP-14 and TIMP-1 in spontaneous canine HS and its murine model. In spontaneous canine HS, scattered MAC 387-positive macrophages were randomly found in tumor center and periphery, whereas tumor cells were negative for this marker. Interestingly, quantitative analysis revealed that MMPs and TIMP-1 were mainly expressed at the invasive front while tumor centers exhibited significantly reduced immunoreactivity. Similar findings were obtained in xenotransplanted HS. Interestingly, murine tumor associated macrophages (TAM), characterized by Mac3 expression (CD107b/LAMP2), which was not present in xenotransplanted histiocytic sarcoma cells, strongly express MMPs and TIMP-1. In addition, MMPs are known to regulate angiogenesis and a positive correlation between MMP-14 expression and microvessel density was demonstrated in xenotransplanted histiocytic sarcomas. Summarized similar findings with respect to MMP and TIMP distribution and the role of macrophages in spontaneously-occurring and xenotransplanted HS indicate the high suitability of this murine model to further investigate HS under standardized conditions. Moreover results indicate that MMP expression contributes to tumor progression and invasion and TAMs seem to be major players in the interaction between neoplastic cells, the microenvironment and vessel formation indicating that therapeutic approaches modulating TAM associated molecules might represent promising future treatment options

    Training-related changes of brain activation for speech production in healthy speakers – a longitudinal fMRI study to mimic aphasia therapy

    Background: Re-learning of lexical entries is fundamental to rehabilitation of the common word finding deficits in language disorders after brain damage. Previous studies examined and compared neural correlates of speech production and word learning in aphasic and healthy speakers, but longitudinal control studies were rarely set out to mimic the lexical confusion and therapeutic remediation in aphasia.Aims: Thus, we aimed to examine functional brain organisation before (familiar word naming), during (learning phase) and after (consolidation phase) standardised training of speech production modulated by aphasia therapy.Methods & Procedures: During the first functional magnetic resonance imaging (fMRI) measurement, participants were asked to name pictures using familiar words; during learning and in the consolidation phase, they were asked to name pictures using newly acquired pseudowords. To examine differential involvement of brain areas dependent on learning and consolidation success, we followed up activations for finally correctly learned (CoL) items between fMRI measurements, and compared activation during naming of CoL versus finally not learned (NoL) items at each measurement.Outcomes & Results: Naming accuracy of participants improved significantly. Although performance increase until the second fMRI measurement was minor, brain activation was present for CoL in comparison to NoL items in right hemisphere homologues of fronto-temporal language-related areas in this phase. Comparing learning with consolidation phases for pseudowords, naming CoL items was accompanied by activation in areas related to monitoring and selection between multiple lexical competitors, and in the right posterior middle temporal gyrus. Conversely, activation specific to the consolidation phase, and also to CoL items in that phase, consisted of a widely distributed network involving areas associated with motor, language and consolidation processes. Activation in right supramarginal and left superior temporal gyrus was related to individual learning success.Conclusions: We were able to demonstrate phase- and performance-dependent activation differences in various areas of the speech production network, which were in part correlated with learning success. The observed similarities to therapy-induced activation changes in aphasia reveal that the novel paradigm is useful in mimicking therapy and may uncover compensatory mechanisms specific to aphasia

    Addison's disease - the traditional term for primary adrenal insufficiency (PAI) - is defined as the clinical manifestation of chronic glucocorticoid- and/or mineralocorticoid deficiency due to failure of the adrenal cortex which may result in an adrenal crisis with potentially life-threatening consequences. Even though efficient and safe pharmaceutical preparations for the substitution of endogenous gluco- and mineralocorticoids are established in therapy, the mortality in patients with PAI is still increased and the health-related quality of life (HRQoL) is often reduced.PAI is a rare disease but recent data report an increasing prevalence. In addition to the common "classical" causes of PAI like autoimmune, infectious, neoplastic and genetic disorders, other iatrogenic conditions - mostly pharmacological side effects (e. g., adrenal haemorrhage associated with anticoagulants, drugs affecting glucocorticoid synthesis, action or metabolism and some of the novel anti-cancer checkpoint inhibitors) are contributing factors to this phenomenon.Due to the rarity of the disease and often non-specific symptoms at least in the early stages, PAI is frequently not considered resulting in a delayed diagnosis. Successful therapy is mainly based on adequate patient education as a cornerstone in the prevention and management of adrenal crisis.     Trajectories of maternal ante- and postpartum depressive symptoms and their association with child- and mother-related characteristics in a West African birth cohort study

    <div><p>Background</p><p>The vast majority of research on mental health has been undertaken in high income countries. This study aimed at investigating the long-term course of maternal depressive symptoms and its association with various mother- and child-related characteristics in two West African lower middle income countries with focus on the relationship with long-term anxiety symptoms.</p><p>Methods</p><p>In the Child Development Study, a prospective birth cohort study in CĂŽte d’Ivoire and Ghana, the 9-item Patient Health Questionnaire (PHQ-9) was answered by <i>N</i> = 776 women 3 months antepartum, and 3, 12, and 24 months postpartum between April 2010 and March 2014. Growth mixture modeling was used to identify distinct trajectories of depressive symptoms. Several psychosocial, obstetric, and sociodemographic characteristics were assessed and multinomial regression analysis was performed to investigate the influence of these variables on the different depression trajectories.</p><p>Results</p><p>We found three distinct classes of depressive symptoms that were characterized by an asymptomatic trajectory (91.5%), by recurrent risk (4.3%) and by postnatal risk (4.3%). The longitudinal course of depressive symptoms was strongly associated with anxiety symptoms (<i>χ</i><sup>2</sup> = 258.54, df = 6, <i>p</i> < 0.001; <i>φ</i> = .577). Among other factors, higher levels of anxiety, new pregnancy 2 years after birth, economic stress, and family stress were associated with the risk classes.</p><p>Conclusions</p><p>A substantial proportion of West African women in our sample developed unfavorable patterns of depressive symptoms during the vulnerable phase of pregnancy and early motherhood. Psychosocial factors, especially antepartum anxiety symptoms, played a decisive role in this process. Perceived economic hardship further exaggerated the mental health burden.</p></div
