    Eff ect of self-collection of HPV DNA off ered by community health workers at home visits on uptake of screening for cervical cancer (the EMA study): a population-based cluster-randomised trial

    Background Control of cervical cancer in developing countries has been hampered by a failure to achieve high screening uptake. HPV DNA self-collection could increase screening coverage, but implementation of this technology is diffi cult in countries of middle and low income. We investigated whether off ering HPV DNA self-collection during routine home visits by community health workers could increase cervical screening. Methods We did a population-based cluster-randomised trial in the province of Jujuy, Argentina, between July 1, 2012, and Dec 31, 2012. Community health workers were eligible for the study if they scored highly on a performance score, and women aged 30 years or older were eligible for enrolment by the community health worker. 200 community health workers were randomly allocated in a 1:1 ratio to either the intervention group (off ered women the chance to self-collect a sample for cervical screening during a home visit) or the control group (advised women to attend a health clinic for cervical screening). The primary outcome was screening uptake, measured as the proportion of women having any HPV screening test within 6 months of the community health worker visit. Analysis was by intention to treat. This trial is registered with ClinicalTrials.gov, number NCT02095561. Findings 100 community health workers were randomly allocated to the intervention group and 100 were assigned to the control group; nine did not take part. 191 participating community health workers (94 in the intervention group and 97 in the control group) initially contacted 7650 women; of 3632 women contacted by community health workers in the intervention group, 3049 agreed to participate; of 4018 women contacted by community health workers in the control group, 2964 agreed to participate. 2618 (86%) of 3049 women in the intervention group had any HPV test within 6 months of the community health worker visit, compared with 599 (20%) of 2964 in the control group (risk ratio 4·02, 95% CI 3·44–4·71). Interpretation Off ering self-collection of samples for HPV testing by community health workers during home visits resulted in a four-fold increase in screening uptake, showing that this strategy is eff ective to improve cervical screening coverage. This intervention reduces women’s barriers to screening and results in a substantial and rapid increase in coverage. Our fi ndings suggest that HPV testing could be extended throughout Argentina and in other countries to increase cervical screening coverage

    Entre otras opciones, la Dirección de Educación a Distancia de la UNLP pone al alcance de los profesores de dicha institución estrategias formativas para que se familiaricen con las propuestas de enseñanza mediadas por tecnologías considerando que la Universidad forma profesionales que se insertarán en un campo laboral atravesado por las TIC. En este contexto se considera imprescindible concentrarse en las competencias a desarrollar en los futuros profesionales y en los actuales profesores para estar a tono con las exigencias de una sociedad donde los conceptos de trabajo, empleo y tiempo libre están cambiando rápidamente. Entre las acciones formativas referidas a las propuestas de enseñanza mediadas por tecnologías describiremos y analizaremos: - Acciones de acercamiento y /o sensibilización. - Acciones de capacitación inicial - Programas de capacitación para destinatarios específicos Desarrollamos en la presente ponencia las estrategias abordadas, sus principales supuestos conceptuales, los aspectos metodológicos didácticos y algunas de las devoluciones realizadas por los profesores y profesoras destinatarios de la misma.     A construção da irrealidade na argumentação de arengas judiciárias da latinidade clássica

    -Tomando por corpus de análise os textos de arengas judiciárias da latinidade clássica, buscamos levantar na estruturação dos textos o emprego domodo subjuntivo na sua utilização para construir os diversos tipos de orações adverbiais, relativas e integrantes ou completivas. Particularmente ostextos do gênero judiciário oferecem uma rica variedade de estruturas subordinadas, fato que nos traz uma grande gama de exemplificação epossibilita um exame mais apropriado das questões relativas à sintaxe oracional. Neste sentido, nosso procedimento é verificar as recorrências doemprego do modo subjuntivo e refletir sobre o seu uso à luz dos fatos relativos à causa em questão, observando assim a questão semânticasubjacente aos textos examinados. As discrepâncias quanto ao que as gramáticas de língua latina apresentam e prescrevem quanto ao emprego dosmodos verbais e sua relação com a expressão da realidade, da potencialidade e da irrealidade, são tratadas como temas de reflexão e dereelaboração do tema discutido pelas gramáticas de língua latina. Como objetivo investigativo, pretendemos alargar a visão que as gramáticas delíngua latina nos propõem, encontrar explicações ou motivações para o uso do subjuntivo como lugar da expressão do potencial e do irreal, assuntopouco explorado pelos métodos de língua latina e, muitas vezes, pouco esmiuçado pelas gramáticas latinas. Como a gama de estruturas oracionais éampla, apresentaremos o que encontramos de novidade nas buscas realizadas nos textos do corpus de análise que constituiu o material levantadona temporada de pesquisa BIC e PIBIC 2007-2008

    Targeting Cattle-Borne Zoonoses and Cattle Pathogens Using a Novel Trypanosomatid-Based Delivery System

    Trypanosomatid parasites are notorious for the human diseases they cause throughout Africa and South America. However, non-pathogenic trypanosomatids are also found worldwide, infecting a wide range of hosts. One example is Trypanosoma (Megatrypanum) theileri, a ubiquitous protozoan commensal of bovids, which is distributed globally. Exploiting knowledge of pathogenic trypanosomatids, we have developed Trypanosoma theileri as a novel vehicle to deliver vaccine antigens and other proteins to cattle. Conditions for the growth and transfection of T. theileri have been optimised and expressed heterologous proteins targeted for secretion or specific localisation at the cell interior or surface using trafficking signals from Trypanosoma brucei. In cattle, the engineered vehicle could establish in the context of a pre-existing natural T. theileri population, was maintained long-term and generated specific immune responses to an expressed Babesia antigen at protective levels. Building on several decades of basic research into trypanosomatid pathogens, Trypanosoma theileri offers significant potential to target multiple infections, including major cattle-borne zoonoses such as Escherichia coli, Salmonella spp., Brucella abortus and Mycobacterium spp. It also has the potential to deliver therapeutics to cattle, including the lytic factor that protects humans from cattle trypanosomiasis. This could alleviate poverty by protecting indigenous African cattle from African trypanosomiasis

    Utilização de quitosana na alimentação de ruminantes

    O objetivo desta revisão foi avaliar os possíveis efeitos da utilização\ud da quitosana na nutrição de ruminantes e demonstrar os resultados obtidos\ud com sua aplicação na dieta de bovinos no Brasil. Foram utilizados neste\ud estudo 8 novilhos canulados da raça Nelore. Os animais foram submetidos\ud à 4 diferentes tratamentos, sendo além do controle, fomecidas as doses de\ud 50 mg, 100 mg ou 150 mg/kg de peso vivo (PV) de quitosana diariamente\ud e inseri da no rúmen. Na adição da concentração de 150 mg/kg de PV de\ud quitosana observou-se diminuição sobre o consumo de fibra detergente neutro\ud (FDN), expressos em kg/dia e em porcentagem de PV do animal. A inclusão\ud de quitosana na dieta proporcionou aumento da digestibilidade da matéria seca\ud (MS), matéria orgânica (MO), proteína bruta (PB),carboidratos totais (CT),\ud FDN e nutrientes digestíveis totais (NDT). Houve efeito quadrático sobre\ud o N-NH3 com a inclusão de quitosana na dieta. A inclusão de quitosana na\ud dieta alterou as proporções molares de AGCC individualmente. O aditivo\ud proporcionou aumento das concentrações de propionato (mmol/L) a medida\ud que se elevou as concentrações de quitosana e de forma semelhante houve\ud aumento de 7,47% para as porcentagens molares de propionato. Houve\ud diminuição da relação acetato:propionato, principalmente com a inclusão de\ud 150mg/Kg de peso vivo de quitosana diariamente.Foi observado também neste\ud trabalho, efeito linear decrescente para a proporção molar de butirato.Assim\ud como esperado as concentrações de glicose plasmática foram influenciadas\ud notadamente neste estudo, resultando num incremento de 18,58%, 26,35%,\ud 23,68% respectivamente para o controle versus as três concentrações\ud utilizadas. Estes dados são coesivos com o aumento também linear obtido\ud na participação do propionato no total de AGCC.     A survey of the clinicopathological and molecular characteristics of patients with suspected Lynch syndrome in Latin America

    Background: Genetic counselling and testing for Lynch syndrome (LS) have recently been introduced in several Latin America countries. We aimed to characterize the clinical, molecular and mismatch repair (MMR) variants spectrum of patients with suspected LS in Latin America. Methods: Eleven LS hereditary cancer registries and 34 published LS databases were used to identify unrelated families that fulfilled the Amsterdam II (AMSII) criteria and/or the Bethesda guidelines or suggestive of a dominant colorectal (CRC) inheritance syndrome. Results: We performed a thorough investigation of 15 countries and identified 6 countries where germline genetic testing for LS is available and 3 countries where tumor testing is used in the LS diagnosis. The spectrum of pathogenic MMR variants included MLH1 up to 54%, MSH2 up to 43%, MSH6 up to 10%, PMS2 up to 3% and EPCAM up to 0.8%. The Latin America MMR spectrum is broad with a total of 220 different variants which 80% were private and 20% were recurrent. Frequent regions included exons 11 of MLH1 (15%), exon 3 and 7 of MSH2 (17 and 15%, respectively), exon 4 of MSH6 (65%), exons 11 and 13 of PMS2 (31% and 23%, respectively). Sixteen international founder variants in MLH1, MSH2 and MSH6 were identified and 41 (19%) variants have not previously been reported, thus representing novel genetic variants in the MMR genes. The AMSII criteria was the most used clinical criteria to identify pathogenic MMR carriers although microsatellite instability, immunohistochemistry and family history are still the primary methods in several countries where no genetic testing for LS is available yet.
