    The 2010 UK Home Office ‘Sexualisation of Young People’ Review: a discursive policy analysis

    This paper offers a discursive policy analysis of the 2010 UK Home Office Sexualisation of Young People Review, authored by Linda Papadopoulos (2010a). It will scrutinise the narrative presented by the text of the danger posed by cultural representations to healthy development, and trace the way that the text links this danger to catastrophic outcomes: child sexual abuse, exploitation and trafficking. Examining this narrative, the article will propose that the UK Review deploys spatial metaphors to naturalise a gendered account of childhood, sexuality and danger, evoking the creeping influence of a corrupting culture on a girl's most private self. The article will also demonstrate that this spatial narrative underpins the epistemological structure of the text – its separation of the primary from the secondary, the real from the artificial

    Acknowledging the Relevance of Elephant Sensory Perception to Human–Elephant Conflict Mitigation

    Elephants are well known for their socio-cognitive abilities and capacity for multi-modal sensory perception and communication. Their highly developed olfactory and acoustic senses provide them with a unique non-visual perspective of their physical and social worlds. The use of these complex sensory signals is important not only for communication between conspecifics, but also for decisions about foraging and navigation. These decisions have grown increasingly risky given the exponential increase in unpredictable anthropogenic change in elephants’ natural habitats. Risk taking often develops from the overlap of human and elephant habitat in Asian and African range countries, where elephants forage for food in human habitat and crop fields, leading to conflict over high-quality resources. To mitigate this conflict, a better understanding of the elephants’ sensory world and its impact on their decision-making process should be considered seriously in the development of long-term strategies for promoting coexistence between humans and elephants. In this review, we explore the elephants’ sensory systems for audition and olfaction, their multi-modal capacities for communication, and the anthropogenic changes that are affecting their behavior, as well as the need for greater consideration of elephant behavior in elephant conservation efforts

    Increasing frailty is associated with higher prevalence and reduced recognition of delirium in older hospitalised inpatients: results of a multi-centre study

    Purpose: Delirium is a neuropsychiatric disorder delineated by an acute change in cognition, attention, and consciousness. It is common, particularly in older adults, but poorly recognised. Frailty is the accumulation of deficits conferring an increased risk of adverse outcomes. We set out to determine how severity of frailty, as measured using the CFS, affected delirium rates, and recognition in hospitalised older people in the United Kingdom. Methods: Adults over 65 years were included in an observational multi-centre audit across UK hospitals, two prospective rounds, and one retrospective note review. Clinical Frailty Scale (CFS), delirium status, and 30-day outcomes were recorded. Results: The overall prevalence of delirium was 16.3% (483). Patients with delirium were more frail than patients without delirium (median CFS 6 vs 4). The risk of delirium was greater with increasing frailty [OR 2.9 (1.8–4.6) in CFS 4 vs 1–3; OR 12.4 (6.2–24.5) in CFS 8 vs 1–3]. Higher CFS was associated with reduced recognition of delirium (OR of 0.7 (0.3–1.9) in CFS 4 compared to 0.2 (0.1–0.7) in CFS 8). These risks were both independent of age and dementia. Conclusion: We have demonstrated an incremental increase in risk of delirium with increasing frailty. This has important clinical implications, suggesting that frailty may provide a more nuanced measure of vulnerability to delirium and poor outcomes. However, the most frail patients are least likely to have their delirium diagnosed and there is a significant lack of research into the underlying pathophysiology of both of these common geriatric syndromes

    Identification of Adverse Drug Events from Free Text Electronic Patient Records and Information in a Large Mental Health Case Register

    Electronic healthcare records (EHRs) are a rich source of information, with huge potential for secondary research use. The aim of this study was to develop an application to identify instances of Adverse Drug Events (ADEs) from free text psychiatric EHRs.We used the GATE Natural Language Processing (NLP) software to mine instances of ADEs from free text content within the Clinical Record Interactive Search (CRIS) system, a de-identified psychiatric case register developed at the South London and Maudsley NHS Foundation Trust, UK. The tool was built around a set of four movement disorders (extrapyramidal side effects [EPSEs]) related to antipsychotic therapy and rules were then generalised such that the tool could be applied to additional ADEs. We report the frequencies of recorded EPSEs in patients diagnosed with a Severe Mental Illness (SMI) and then report performance in identifying eight other unrelated ADEs.The tool identified EPSEs with >0.85 precision and >0.86 recall during testing. Akathisia was found to be the most prevalent EPSE overall and occurred in the Asian ethnic group with a frequency of 8.13%. The tool performed well when applied to most of the non-EPSEs but least well when applied to rare conditions such as myocarditis, a condition that appears frequently in the text as a side effect warning to patients.The developed tool allows us to accurately identify instances of a potential ADE from psychiatric EHRs. As such, we were able to study the prevalence of ADEs within subgroups of patients stratified by SMI diagnosis, gender, age and ethnicity. In addition we demonstrated the generalisability of the application to other ADE types by producing a high precision rate on a non-EPSE related set of ADE containing documents.The application can be found at http://git.brc.iop.kcl.ac.uk/rmallah/dystoniaml
