    The client satisfaction with device: a Rasch validation of the Arabic version in patients with upper and lower limb amputation

    BACKGROUND: The Client Satisfaction with Devices (CSD) module of the Orthotics and Prosthetics Users' Survey is an extensively used questionnaire that measures patients' satisfaction with orthosis and prosthesis. However, the validated version for Arabic speakers (CSD-Ar) is only applicable for orthosis users. OBJECTIVES: The aim of this study was to evaluate the psychometric proprieties of the CSD-Ar for prosthetics users. METHODS: The study used a convenience sample of prosthesis users from Saudi Arabia and Turkey (N\u2009=\u2009183), who completed the CSD-Ar. The collected data were analysed using Rasch analysis to evaluate item fit, reliability indices, item difficulty, local item dependency, and differential item functioning (DIF) using WINSTEPS version 4.6.1. RESULTS: Based on the analysis, the four-response Likert-scale was acceptable, as shown by the category functioning test, All eight items did achieve a fit to the Rasch Model [(infit) and (outfit) mean-square 0.75 to 1.3]. Person separation reliability was 0.76, and item separation reliability was 0.94. A principal component analysis (PCA) showed satisfactory unidimensionality and no local item dependency. The DIF analysis showed no notable dependency among items on participant characteristics in terms of age, gender, duration of use, country, and level of amputation. CONCLUSION: This study contributes to the confidence of using CSD-Ar to evaluate users' satisfaction with different prostheses, affirming the need for further refinement of the quality of the outcome measure

    Study of antioxidant activity and free radical scavenging power of Rheum Ribes flower extract

    In recent years, the use of synthetic antioxidants and the degradation products derived from them is limited, because they are toxic and carcinogenic. So, studying on the use of natural antioxidants to replace the synthetic antioxidants is very important. Natural antioxidants are healthier and have more benefits and fewer harmful side effects. In this study, first, the Rheum Ribes of flower was extracted by maceration method with methanol. The extract’s phenolic compounds and inhibitory activity of free radicals were evaluated in different concentrations (100, 150, 200, 250 and 300 ppm) by Folin-Ciocalteu method and DPPH test, respectively and then, compared with the synthetic antioxidant of BHT at the constant concentration of 200 ppm. The results showed that the highest antioxidant properties of the extract was related to the concentration of 300 ppm and also the concentration of 250 ppm of the extract of Rheum Ribes of flower was equivalent to a concentration of 200 ppm of BHT in terms of free radical scavenging properties.Keywords: Rheum Ribes flower, Free radical scavenging power, Natural antioxidants, Phenolic compound

    On the Mechanism of Drag Reduction in Fully-Developed Turbulent Channel Flow with a Streamwise Micro-featured Superhydrophobic Wall

    The superhydrophobic drag reduction changes the structures of turbulent flow. However, the underlying mechanism is not clear. The aim of this study is to determine the alternations of turbulent flow due to applying a streamwise micro-featured superhydrophobic wall. Large eddy simulations are performed to explore the effect of micro-features on near-wall behaviors. The results indicate that the outward motion of the lifted low-speed streaks is restricted to the lower wall layers, and the region of maximum production of streamwise vorticities is shifted toward the micro-featured wall. The quadrant analysis of Reynolds stress shows that there is a stronger increase in outward motion of high-speed fluid and inward motion of low-speed fluid than ejection and sweep

    Are welders more at risk of respiratory infections? Findings from a cross-sectional survey and analysis of medical records in shipyard workers: the WELSHIP project

    Background Exposure to welding fume increases the risk of pneumococcal infection; whether such susceptibility extends to other respiratory infections is unclear. We report findings from a survey and from medical consultation data for workers in a large shipyard in the Middle East. Methods Between January 2013 and December 2013, we collected cross-sectional information from 529 male workers variously exposed to welding fume. Adjusted ORs for respiratory symptoms (cough, phlegm, wheezing, shortness of breath and 'chest illness') were estimated using multivariable logistic regression. Subsequently, we examined consultation records from 2000 to 2011 for 15 954 workers who had 103 840 consultations for respiratory infections; the associations between respiratory infections and levels of welding exposure were estimated using a count regression model with a negative binomial distribution. Results 13% of surveyed workers reported respiratory symptoms with a higher prevalence in winter, particularly among welders. The adjusted OR in welders versus other manual labourers was 1.72 (95% CI 1.02 to 3.01) overall and 2.31 (1.05 to 5.10) in winter months; no effect was observed in summer. The risk of consultation for respiratory infections was higher in welders than in manual labourers, with an adjusted incidence rate ratio of 1.45 (1.59 to 1.83) overall, 1.47 (1.42 to 1.52) in winter and 1.33 (1.23 to 1.44) in summer (interaction, p<0.001). Conclusions The observation that respiratory symptoms and consultations for respiratory infection in welders are more common in winter may indicate an enhanced vulnerability to a broad range of infections. If confirmed, this would have important implications for the occupational healthcare of a very large, global workforce

    An overview of enhancing drought tolerance in cotton through manipulating stress resistance genes

    Drought stress affects the normal growth of plant by influencing Physiological, morphological molecular and biochemical traits at cellular level. It is a polygenic trait, controlled by multiple genes, which makes its manipulation difficult by genetic engineering. It seems drought could be major threat in future to high yield of cotton in Pakistan as well around the globe because it is spontaneous and cannot be controlled with manuring and skilled agricultural practices. Gene manipulation could be a solution of this threat by producing transgenic cotton plants. As it is polygenic trait, so, understanding about cellular mechanism of drought tolerance is crucial to impart tolerance by controlling gene expression under stressed conditions. Universal Stress Proteins (USP) genes have already been identified in drought stressed leaves of Gossypium arboreum which make this variety of cotton a rich source of stress tolerance genes. USP genes could be manipulated for drought tolerant transgenic cotton with high yielding as well and it is most important family of proteins in this regard. This family encompasses a conserved group of proteins that has been reported in different organisms which are activating under various abiotic stress conditions. USP is also a regulatory protein; its activity can be increased by manipulating its interactions

    Acidosis and Deafness in Patients with Recessive Mutations in FOXI1

    Maintenance of the composition of inner ear fluid and regulation of electrolytes and acid-base homeostasis in the collecting duct system of the kidney require an overlapping set of membrane transport proteins regulated by the forkhead transcription factor FOXI1. In two unrelated consanguineous families, we identified three patients with novel homozygous missense mutations in FOXI1 (p.L146F and p.R213P) predicted to affect the highly conserved DNA binding domain. Patients presented with early-onset sensorineural deafness and distal renal tubular acidosis. In cultured cells, the mutations reduced the DNA binding affinity of FOXI1, which hence, failed to adequately activate genes crucial for normal inner ear function and acidbase regulation in the kidney. A substantial proportion of patients with a clinical diagnosis of inherited distal renal tubular acidosis has no identified causative mutations in currently known disease genes. Our data suggest that recessive mutations in FOXI1 can explain the disease in a subset of these patients

    An InDel in Phospholipase-C-B-1 is linked with euthyroid multinodular goiter

    Background: Euthyroid multinodular goiter (MNG) is common, but little is known about the genetic variations conferring predisposition. Previously, a family with MNG of adolescent onset was reported in which some family members developed papillary thyroid carcinomas (PTC). Methods: Genome-wide linkage analysis and next-generation sequencing were conducted to identify genetic variants that may confer disease predisposition. A multipoint nonparametric LOD score of 3.01 was obtained, covering 19 cM on chromosome 20p. Haplotype analysis reduced the region of interest to 10 cM. Results: Analysis of copy number variation identified an intronic InDel (∼1000 bp) in the PLCB1 gene in all eight affected family members and carriers (an unaffected person who has inherited the genetic trait). This InDel is present in approximately 1% of “healthy” Caucasians. Next-generation sequencing of the region identified no additional disease-associated variant, suggesting a possible role of the InDel. Since PLCB1 contributes to thyrocyte growth regulation, the InDel was investigated in relevant Caucasian cohorts. It was detected in 0/70 PTC but 4/81 unrelated subjects with MNG (three females; age at thyroidectomy 27–59 years; no family history of MNG/PTC). The InDel frequency is significantly higher in MNG subjects compared to controls (χ2 = 5.076; p = 0.024. PLCB1 transcript levels were significantly higher in thyroids with the InDel than without (p < 0.02). Conclusions: The intronic PLCB1 InDel is the first variant found in familial multiple papilloid adenomata-type MNG and in a subset of patients with sporadic MNG. It may function through overexpression, and increased PLC activity has been reported in thyroid neoplasms. The potential role of the deletion as a biomarker to identify MNG patients more likely to progress to PTC merits exploration
