    The prognosis of allocentric and egocentric neglect : evidence from clinical scans

    We contrasted the neuroanatomical substrates of sub-acute and chronic visuospatial deficits associated with different aspects of unilateral neglect using computed tomography scans acquired as part of routine clinical diagnosis. Voxel-wise statistical analyses were conducted on a group of 160 stroke patients scanned at a sub-acute stage. Lesion-deficit relationships were assessed across the whole brain, separately for grey and white matter. We assessed lesions that were associated with behavioural performance (i) at a sub-acute stage (within 3 months of the stroke) and (ii) at a chronic stage (after 9 months post stroke). Allocentric and egocentric neglect symptoms at the sub-acute stage were associated with lesions to dissociated regions within the frontal lobe, amongst other regions. However the frontal lesions were not associated with neglect at the chronic stage. On the other hand, lesions in the angular gyrus were associated with persistent allocentric neglect. In contrast, lesions within the superior temporal gyrus extending into the supramarginal gyrus, as well as lesions within the basal ganglia and insula, were associated with persistent egocentric neglect. Damage within the temporo-parietal junction was associated with both types of neglect at the sub-acute stage and 9 months later. Furthermore, white matter disconnections resulting from damage along the superior longitudinal fasciculus were associated with both types of neglect and critically related to both sub-acute and chronic deficits. Finally, there was a significant difference in the lesion volume between patients who recovered from neglect and patients with chronic deficits. The findings presented provide evidence that (i) the lesion location and lesion size can be used to successfully predict the outcome of neglect based on clinical CT scans, (ii) lesion location alone can serve as a critical predictor for persistent neglect symptoms, (iii) wide spread lesions are associated with neglect symptoms at the sub-acute stage but only some of these are critical for predicting whether neglect will become a chronic disorder and (iv) the severity of behavioural symptoms can be a useful predictor of recovery in the absence of neuroimaging findings on clinical scans.     Ablation of Proliferating Cells in the CNS Exacerbates Motor Neuron Disease Caused by Mutant Superoxide Dismutase

    Proliferation of glia and immune cells is a common pathological feature of many neurodegenerative diseases including amyotrophic lateral sclerosis (ALS). Here, to investigate the role of proliferating cells in motor neuron disease, SOD1G93A transgenic mice were treated intracerebroventicularly (ICV) with the anti-mitotic drug cytosine arabinoside (Ara-C). ICV delivery of Ara-C accelerated disease progression in SOD1G93A mouse model of ALS. Ara-C treatment caused substantial decreases in the number of microglia, NG2+ progenitors, Olig2+ cells and CD3+ T cells in the lumbar spinal cord of symptomatic SOD1G93A transgenic mice. Exacerbation of disease was also associated with significant alterations in the expression inflammatory molecules IL-1β, IL-6, TGF-β and the growth factor IGF-1

    Design of the Dutch Obesity Intervention in Teenagers (NRG-DOiT): systematic development, implementation and evaluation of a school-based intervention aimed at the prevention of excessive weight gain in adolescents

    BACKGROUND: Only limited data are available on the development, implementation, and evaluation processes of weight gain prevention programs in adolescents. To be able to learn from successes and failures of such interventions, integral written and published reports are needed. METHODS: Applying the Intervention Mapping (IM) protocol, this paper describes the development, implementation, and evaluation of the Dutch Obesity Intervention in Teenagers (DOiT), a school-based intervention program aimed at the prevention of excessive weight gain. The intervention focussed on the following health behaviours: (1) reduction of the consumption of sugar-sweetened beverages, (2) reduction of energy intake derived from snacks, (3) decrease of levels of sedentary behaviour, and (4) increase of levels of physical activity (i.e. active transport behaviour and sports participation). The intervention program consisted of an individual classroom-based component (i.e. an educational program, covering 11 lessons of both biology and physical education classes), and an environmental component (i.e. encouraging and supporting changes at the school canteens, as well as offering additional physical education classes). We evaluated the effectiveness of the intervention program using a randomised controlled trial design. We assessed the effects of the intervention on body composition (primary outcome measure), as well as on behaviour, behavioural determinants, and aerobic fitness (secondary outcome measures). Furthermore, we conducted a process evaluation. DISCUSSION: The development of the DOiT-intervention resulted in a comprehensive school-based weight gain prevention program, tailored to the needs of Dutch adolescents from low socio-economic background

    Regional structuring of genetic variation in short-lived rock pool populations of Branchipodopsis wolfi (Crustacea : Anostraca)

    The genetic structure of three metapopulations of the southern African anostracan Branchipodopsis,sis wolfi was compared by analysing allozyme variation at four loci (PGM, GPI, APK, AAT). In total, 17 local populations from three sites (metapopulations) were analysed from rock pools in south-eastern Botswana ranging from 0.2 to 21 m(2) in surface area. In three populations we found significant deviations from Hardy-Weinberg (H-W) equilibrium at one or more loci due to heterozygote deficiencies. Genetic variability at one site was significantly lower than at the other sites, which may be linked to a greater incidence of extinction and recolonisation, as the basins at this site are shallower and have shorter hydrocycles. Across all local populations, a significant level of population differentiation was revealed. More than 90% of this variation was explained by differentiation among sites (metapopulations), although this differentiation did not correlate with geographic distance, or with environmental variables. Genetic differentiation among populations within metapopulations was low, but significant at ail sites. At only one of the sites was a significantly positive association measured between genetic and geographic distance among local populations. Our data suggest that persistent stochastic events and limited effective long-range dispersal appear to dominate genetic differentiation among populations of B. wolfi inhabiting desert rock pools. The lack of association between geographic distance and genetic or ecological differences between rock pool sites is indicative of historical stochastic events. Low heterozygosity, the significant deviations From H-W equilibrium, and the large inter- but low intra-site differentiation are suggestive of the importance of short-range dispersal.     Health-oriented physical activity in prevention of musculoskeletal disorders among young Polish musicians

    Objectives: Musicians represent a very specific professional group, which due to some occupational hazards is exposed to different health problems known as playing-related musculoskeletal disorders (PRMDs). The aim of this study was to assess correlations between the level of physical activity, and the occurrence of musculoskeletal pain among young instrumentalists. Materials and Methods: Total of 225 Polish musical school students were investigated. To assess the study participants' physical activity level, the moderate to vigorous physical activity (MVPA) index was used. To assess pain complaints on the side of the musculoskeletal system, Nordic Musculoskeletal Questionnaire (NMQ) was applied. The NMQ questionnaire was complemented by adding a visual-analog scale (VAS), which also allowed to assess pain intensity on a numerical scale 1-10. Results: The pain localized in neck, shoulders, upper and lower back was reported significantly more often by the participants who did not meet standard criteria for the recommended or minimal physical activity level. Conclusion: Performing the recommended health-oriented physical activity may reduce the prevalence of musculoskeletal pain in young musicians

    Causes for rarity in plants are poorly understood. Graptophyllum reticulatum is an endangered endemic species, and it has three close relatives with different conservation status: the vulnerable G. ilicifolium, the rare G. excelsum, and the common G. spinigerum. Applied to the chlorophyll a fluorescence transient of leaves, the JIP test provides a Performance Index (PI) which quantifies the main steps in photosystem II (PSII) photochemistry including light energy absorption, excitation energy trapping, and conversion of excitation energy into electron flow. The PI is calculated from three components which depend on the reaction center density, the trapping efficiency, and the electron transport efficiency. PI was measured in the natural habitats of the four species and under artificially imposed environmental stresses in the glasshouse to determine whether conservation status was related to stress resilience. The results showed that soil type is unlikely to restrict the endangered G. reticulatum, vulnerable G. ilicifolium, or rare G. excelsum because PI was similar in plants grown in diverse soils in the glasshouse. Photoinhibition is likely to restrict the endangered G. reticulatum to shade habitats because PI was significantly reduced when plants were exposed to more than 15% ambient light in controlled experiments. Water availability may determine the location and distribution of the vulnerable G. ilicifolium and common G. spinigerum because PI was reduced more than 60% when plants were exposed to water stress. While the characteristics of their natural habitats correspond to and explain the physiological responses, there was no obvious relationship between conservation status and environmental resilience. PI can be used to monitor vigor and health of populations of plants in the natural habitat.     Patient-centred communication intervention study to evaluate nurse-patient interactions in complex continuing care

    <p>Abstract</p> <p>Background</p> <p>Communication impairment is a frequent consequence of stroke. Patients who cannot articulate their needs respond with frustration and agitation, resulting in poor optimization of post-stroke functions. A key component of patient-centred care is the ability of staff to communicate in a way that allows them to understand the patient’s needs. We developed a patient-centred communication intervention targeting registered and unregulated nursing staff caring for complex continuing care patients with communication impairments post stroke. Research objectives include 1) examining the effects of the intervention on patients’ quality of life, depression, satisfaction with care, and agitation; and (2) examining the extent to which the intervention improves staff’s attitudes and knowledge in caring for patients with communication impairments. The intervention builds on a previous pilot study.</p> <p>Methods/design</p> <p>A quasi-experimental repeated measures non-equivalent control group design in a complex continuing care facility is being used. Patients with a communication impairment post-stroke admitted to the facility are eligible to participate. All staff nurses are eligible. Baseline data are collected from staff and patients. Follow-up will occur at 1 and 3 months post-intervention. Subject recruitment and data collection from 60 patients and 30 staff will take approximately 36 months. The Patient-Centred Communication Intervention consists of three components: (1) development of an individualized patient communication care plan; (2) a one-day workshop focused on communication and behavioural management strategies for nursing staff; and (3) a staff support system. The intervention takes comprehensive patient assessments into account to inform the development of communication and behavioural strategies specifically tailored to each patient.</p> <p>Discussion</p> <p>The Patient-Centred Communication Intervention will provide staff with strategies to facilitate interactions with patients and to minimize agitation associated with considerable stress. The improvement of these interactions will lead to a reduction of agitation, which has the additional significance of increasing patients’ well-being, quality of life, and satisfaction with care.</p> <p>Trial registration</p> <p>ClinicalTrials.gov Identifier NCT01654029</p
