BRIDGING A SUPPOSEDLY UNBRIDGEABLE GAP: ELABORATING SCIENTIFIC KNOWLEDGE FROM AND FOR PRACTICE
This article aims at advancing the still on-going conversations about the so-called research/practice gap. Some academics argue that it is not possible to develop knowledge that is both academically valuable and helpful for practice, while others hold the opposite view, justifying it on the basis of works published in top tier journals. The paper argues that the main reason scholars hold such contradictory views on this topic central to management science is the lack of explicitness of a number of founding assumptions which underlie their discourses, in particular the lack of explicitness of the epistemological framework in which the parties' arguments are anchored. The paper presents methodological guidelines for elaborating scientific knowledge both from and for practice, and illustrates how to use these guidelines on examples from a published longitudinal research project. In order to avoid the lack of explicitness pitfall, the paper specifies scientific and epistemological frameworks in which the knowledge elaborated in this methodological approach, when properly justified, can be considered as legitimate scientific knowledge.collaborative research ; constructivist epistemological paradigm ; sciences of the artificial ; organizational design science; rigor; actionability
Loyal after the End: The Endurance of Organizational Identification
This paper develops and tests a model of the role of organizational identification in membersâ propensity to express loyalty to a defunct organization through participation in activities that sustain its most valued elements. It also identifies four antecedent factors to organizational identification that can explain its persistence after formal organizational membership ends as well as the effects of loyalty behaviors. Survey results were analyzed using confirmatory factor analysis and structural equation modeling procedures. The resulting model demonstrated strong fit with the data according to several goodness-of-fit indices
Intracoronary EnalaPrilat to Reduce MICROvascular Damage During Percutaneous Coronary Intervention (ProMicro) study.
Intracoronary angiotensin-converting enzyme inhibitors have been shown to relieve myocardial ischemia in stable patients and to improve epicardial flow in patients with ST-segment elevation myocardial infarction. Yet, it is
still unclear whether these effects are mediated by a modulation of the coronary microcirculation.
Methods We randomly assigned 40 patients to receive either an intracoronary bolus of enalaprilat (50 g) or placebo before elective PCI. The index of microvascular resistance was measured at baseline, 10 minutes after study drug
administration, and after PCI. High-sensitivity cardiac troponin T was measured as a marker of myocardial injury.
Results Infusion of enalaprilat resulted in a significant reduction in index of microvascular resistance (27 11 at baseline vs. 19 9 after drug vs. 15 8 after PCI), whereas a significant post-procedural increase in index of microvascular resistance levels was observed in the placebo group (24 15 at baseline vs. 24 15 after drug vs.
33 19 after PCI). Index of microvascular resistance levels after PCI were significantly lower in the enalaprilat
group (p 0.001). Patients pre-treated with enalaprilat also showed lower peak values (mean: 21.7 ng/ml,
range: 8.2 to 34.8 ng/ml vs. mean: 32.3 ng/ml, range: 12.6 to 65.2 ng/ml, p 0.048) and peri-procedural increases of high-sensitivity cardiac troponin T (mean: 9.9 ng/ml, range: 2.7 to 19.0 ng/ml vs. mean: 26.6 ng/ml,
range: 6.3 to 60.5 ng/ml, p 0.025).
Conclusions Intracoronary enalaprilat improves coronary microvascular function and protects myocardium from procedurerelated injury in patients with coronary artery disease undergoing PCI. Larger studies are warranted to investigate whether these effects of enalaprilat could result into a significant clinical benefit. (J Am Coll Cardiol

Designing a complex intervention for dementia case management in primary care
Background: Community-based support will become increasingly important for people with dementia, but currently services are fragmented and the quality of care is variable. Case management is a popular approach to care co-ordination, but evidence to date on its effectiveness in dementia has been equivocal. Case management interventions need to be designed to overcome obstacles to care co-ordination and maximise benefit. A successful case management methodology was adapted from the United States (US) version for use in English primary care, with a view to a definitive trial. Medical Research Council guidance on the development of complex interventions was implemented in the adaptation process, to capture the skill sets, person characteristics and learning needs of primary care based case managers. Methods: Co-design of the case manager role in a single NHS provider organisation, with external peer review by professionals and carers, in an iterative technology development process. Results: The generic skills and personal attributes were described for practice nurses taking up the case manager role in their workplaces, and for social workers seconded to general practice teams, together with a method of assessing their learning needs. A manual of information material for people with dementia and their family carers was also created using the US intervention as its source. Conclusions: Co-design produces rich products that have face validity and map onto the complexities of dementia and of health and care services. The feasibility of the case manager role, as described and defined by this process, needs evaluation in âreal lifeâ settings
Implementing health research through academic and clinical partnerships : a realistic evaluation of the Collaborations for Leadership in Applied Health Research and Care (CLAHRC)
Background: The English National Health Service has made a major investment in nine partnerships between
higher education institutions and local health services called Collaborations for Leadership in Applied Health
Research and Care (CLAHRC). They have been funded to increase capacity and capability to produce and
implement research through sustained interactions between academics and health services. CLAHRCs provide a
natural âtest bedâ for exploring questions about research implementation within a partnership model of delivery.
This protocol describes an externally funded evaluation that focuses on implementation mechanisms and
processes within three CLAHRCs. It seeks to uncover what works, for whom, how, and in what circumstances.
Design and methods: This study is a longitudinal three-phase, multi-method realistic evaluation, which
deliberately aims to explore the boundaries around knowledge use in context. The evaluation funder wishes to see
it conducted for the process of learning, not for judging performance. The study is underpinned by a conceptual
framework that combines the Promoting Action on Research Implementation in Health Services and Knowledge to
Action frameworks to reflect the complexities of implementation. Three participating CLARHCS will provide indepth
comparative case studies of research implementation using multiple data collection methods including
interviews, observation, documents, and publicly available data to test and refine hypotheses over four rounds of
data collection. We will test the wider applicability of emerging findings with a wider community using an
interpretative forum.
Discussion: The idea that collaboration between academics and services might lead to more applicable health
research that is actually used in practice is theoretically and intuitively appealing; however the evidence for it is
limited. Our evaluation is designed to capture the processes and impacts of collaborative approaches for
implementing research, and therefore should contribute to the evidence base about an increasingly popular (e.g.,
Mode two, integrated knowledge transfer, interactive research), but poorly understood approach to knowledge
translation. Additionally we hope to develop approaches for evaluating implementation processes and impacts
particularly with respect to integrated stakeholder involvement

Contemporary Management of Stable Coronary Artery Disease.
Coronary artery disease (CAD) continues to be the leading cause of mortality and morbidity in developed countries. Assessment of pre-test probability (PTP) based on patient's characteristics, gender and symptoms, help to identify more accurate patient's clinical likelihood of coronary artery disease. Consequently, non-invasive imaging tests are performed more appropriately to rule in or rule out CAD rather than invasive coronary angiography (ICA). Coronary computed tomography angiography (CCTA) is the first-line non-invasive imaging technique in patients with suspected CAD and could be used to plan and guide coronary intervention. Invasive coronary angiography remains the gold-standard method for the identification and characterization of coronary artery stenosis. However, it is recommended in patients where the imaging tests are non-conclusive, and the clinical likelihood is very high, remembering that in clinical practice, approximately 30 to 70% of patients with symptoms and/or signs of ischemia, referred to coronary angiography, have non obstructive coronary artery disease (INOCA). In this contest, physiology and imaging-guided revascularization represent the cornerstone of contemporary management of chronic coronary syndromes (CCS) patients allowing us to focus specifically on ischemia-inducing stenoses. Finally, we also discuss contemporary medical therapeutic approach for secondary prevention. The aim of this review is to provide an updated diagnostic and therapeutic approach for the management of patients with stable coronary artery disease
Ambidexterity for corporate social performance
The literature on corporate social performance advocates that firms address social issues based on instrumental as well as moral rationales. While both rationales trigger initiatives to increase corporate social performance, these rest on fundamentally different and contradicting foundations. Building on the literature on organizational ambidexterity and paradox in management, we propose in this conceptual article that ambidexterity represents an important determinant of corporate social performance. We explain how firms achieve higher levels of corporate social performance through the ambidextrous ability to simultaneously pursue instrumentally and morally driven social initiatives. We distinguish between a balance dimension and a combined dimension of ambidexterity, which both enhance corporate social performance through distinct mechanisms. With the balance dimension, instrumental and moral initiatives compensate for each other â which increases the scope of corporate social performance. With the combined dimension, instrumental and moral initiatives supplement each other â which increases the scale of corporate social performance. The article identifies the most important determinants and moderators of the balance and the combined dimension to explain the conditions under which we expect firms to increase corporate social performance through ambidexterity. By focusing on the interplay and tensions between different types of social initiatives, an ambidextrous perspective contributes to a better understanding of corporate social performance. Regarding managerial practice, we highlight the role of structural and behavioral factors for achieving higher corporate social performance through the simultaneous pursuit of instrumental and moral initiatives
